Based on the strategy of metabolomics combined with bioinformatics, this study analyzed the potential allergens and mechanism of pseudo-allergic reactions (PARs) induced by the combined use of Reduning injection and penicillin G injection. All animal experiments and welfare are in accordance with the requirements of the First Affiliated Experimental Animal Ethics and Animal Welfare Committee of Henan University of Chinese Medicine (approval number: YFYDW2020002). Based on UPLC-Q-TOF/MS technology combined with UNIFI software, a total of 21 compounds were identified in Reduning and penicillin G mixed injection. Based on molecular docking technology, 10 potential allergens with strong binding activity to MrgprX2 agonist sites were further screened. Metabolomics analysis using UPLC-Q-TOF/MS technology revealed that 34 differential metabolites such as arachidonic acid, phosphatidylcholine, phosphatidylserine, prostaglandins, and leukotrienes were endogenous differential metabolites of PARs caused by combined use of Reduning injection and penicillin G injection. Objective To evaluate the efficacy and safety of brexpiprazole in treating acute schizophrenia.Methods Patients with schizophrenia were randomly divided into treatment group and control group.The treatment group was given brexpiprozole 2-4 mg·d-1 orally and the control group was given aripiprazole 10-20 mg·d-1orally,both were treated for 6 weeks.Clinical efficacy of the two groups,the response rate at endpoint,the changes from baseline to endpoint of Positive and Negative Syndrome Scale(PANSS),Clinical Global Impression-Improvement(CGI-S),Personal and Social Performance scale(PSP),PANSS Positive syndrome subscale,PANSS negative syndrome subscale were compared.The incidence of treatment-related adverse events in two groups were compared.Results There were 184 patients in treatment group and 186 patients in control group.After treatment,the response rates of treatment group and control group were 79.50%(140 cases/184 cases)and 82.40%(150 cases/186 cases),the scores of CGI-I of treatment group and control group were(2.00±1.20)and(1.90±1.01),with no significant difference(all P>0.05).From baseline to Week 6,the mean change of PANSS total score wese(-30.70±16.96)points in treatment group and(-32.20±17.00)points in control group,with no significant difference(P>0.05).The changes of CGI-S scores in treatment group and control group were(-2.00±1.27)and(-1.90±1.22)points,PSP scores were(18.80±14.77)and(19.20±14.55)points,PANSS positive syndrome scores were(-10.30±5.93)and(-10.80±5.81)points,PANSS negative syndrome scores were(-6.80±5.98)and(-7.30±5.15)points,with no significant difference(P>0.05).There was no significant difference in the incidence of treatment-related adverse events between the two group(69.00%vs.64.50%,P>0.05).Conclusion The non-inferiority of Brexpiprazole to aripiprazole was established,with comparable efficacy and acceptability.
6.Mechanism of icariin regulating the NLRP3 inflammasome against cerebral ischemia reperfusion

Objective To investigate the mechanism of icariin regulating the NLRP3 inflammasome in the treatment of cerebral ischemia-reperfusion injury in rats.Methods A rat model of focal cerebral ischemia-reperfusion was induced using the thread embolism method.At 24 hours post-operation,the rats were randomly allocated into a sham operation group,model group,butylphthalide group(70 mg/kg),ICA-low dose(20 mg/kg),ICA-middle dose(40 mg/kg),and ICA-high dose(80 mg/kg)groups.The corresponding drugs were administered by gavage at 10 mL/kg once a day for 13 consecutive days.One hour after the last administration,neurological function was scored.The cerebral cortex was observed by hematoxylin-eosin(HE)staining.Expression of interleukin(IL)-1β and IL-18 in the cerebral cortex was determined by immunohistochemistry.Expression of NLRP3,ASC,and Caspase-1 in the cerebral cortex was determined by Western Blot.Results In contrast to the sham operation group,there was a notable increase in neural function scores within the model group.The ischemic area around the visible cerebral cortex showed neuron necrosis at various level or glial cell proliferation,and the number of intact neurons was significantly reduced.IL-1β and IL-18 positive cells were significantly increased.Expression of NLRP3,ASC,and Caspase-1 was significantly increased(P<0.01,P<0.05).After treatment with icariin,the neural function score was decreased significantly.The degree of neuronal necrosis in the peri-ischemic area was significantly reduced,and the number of intact neurons was significantly increased.IL-1 β and IL-18-positive cells were decreased significantly.Expressions of NLRP3,ASC,and Caspase-1 were significantly decreased(P<0.01,P<0.05).Conclusions Treatment of cerebral ischemia-reperfusion injury by icariin may be related to regulation of the NLRP3 inflammasome.

Exploring the risk "time interval window" of sequential medication of Reduning injection (RDN) and penicillin G injection (PG) by detecting the correlation between serum biochemical indexes and plasma metabonomic characteristics, in order to reduce the risk of adverse reactions caused by the combination of RDN and PG. All animal experiments and welfare are in accordance with the requirements of the First Affiliated Experimental Animal Ethics and Animal Welfare Committee of Henan University of Chinese Medicine (approval number: YFYDW2020002). The changes of biochemical indexes in serum of rats were detected by enzyme-linked immunosorbent assay. It was determined that RDN combined with PG could cause pseudo-allergic reactions (PARs) activated by complement pathway. Further investigation was carried out at different time intervals (1.5, 2, 3.5, 4, 6, and 8 h PG+RDN). It was found that sequential administration within 3.5 h could cause significant PARs. However, PARs were significantly reduced after administration interval of more than 4 h. LC-MS was used for plasma metabolomics analysis, and the levels of serum biochemical indicators and plasma metabolic profile characteristics were compared in parallel. 22 differential metabolites showed similar or opposite trends to biochemical indicators before and after 3.5 h. And enriched to 10 PARs-related pathways such as arachidonic acid metabolism, steroid hormone biosynthesis, linoleic acid metabolism, glycerophospholipid metabolism, and tryptophan metabolism. In conclusion, there is a risk "time interval window" phenomenon in the adverse drug reactions caused by the sequential use of RDN and PG, and the interval medication after the "time interval window" can significantly reduce the risk of adverse reactions.
8.Deep learning-based Gaussian and pepper noise removal method for visual images of surgical instruments

Objective To propose a deep learning-based method for removing Gaussian and pepper noises of the surgical instrument visual images so as to recover the detailed features of the images.Methods A lightweight multi-task progressive network was constructed involving in a multi-feature fusion encoder-decoder network,an attention-guided network and a detail-recovery progressive network,which used the multi-feature fusion encoder-decoder network to predict and eliminate the noise information in the visual images,the attention-guided network to remove the residual noise and the detail-recovery progressive network to restore the underlying detail features of the denoised images.Some of the regular convolutions in the detail recovery progressive network were replaced with depth separable convolutions to realize lightweight design of the network constructed.Denoising experiments were conducted on the publicly available CBSD68 and Kodak24 datasets and the self-constructed surgical instrument noise dataset so as to compare the denoising effects of the network constructed and the traditional methods and the classification accuracies of ResNet-18 model and ResNet-34 model for the denosied images by the network and to analyze computing power and memory usage before and after the lightweight design.Results The network constructed gained better denoising effect than the classical methods for publicly available datasets,and ResNet-18 model and ResNet-34 model had higher accuracies when used to classify the images denoised by the network for the self-constructed surgical instrument noise dataset.Lightweight design had the parameter number and floating point operations(FLOPs)decreased by approximately 27.27%and 29.81%,respectively.Conclusion The proposed lightweight multi-task progressive network behaves well in denoising surgical instrument visual images with reduced computating power consump-tion and memory usage.[Chinese Medical Equipment Journal,2024,45(2):1-7]

Background::Although overnight fasting is recommended prior to endoscopic retrograde cholangiopancreatography (ERCP), the benefits and safety of high-carbohydrate fluid diet (CFD) intake 2 h before ERCP remain unclear. This study aimed to analyze whether high-CFD intake 2 h before ERCP can be safe and accelerate patients’ recovery.Methods::This prospective, multicenter, randomized controlled trial involved 15 tertiary ERCP centers. A total of 1330 patients were randomized into CFD group ( n = 665) and fasting group ( n = 665). The CFD group received 400 mL of maltodextrin orally 2 h before ERCP, while the control group abstained from food/water overnight (>6 h) before ERCP. All ERCP procedures were performed using deep sedation with intravenous propofol. The investigators were blinded but not the patients. The primary outcomes included postoperative fatigue and abdominal pain score, and the secondary outcomes included complications and changes in metabolic indicators. The outcomes were analyzed according to a modified intention-to-treat principle. Results::The post-ERCP fatigue scores were significantly lower at 4 h (4.1 ± 2.6 vs. 4.8 ± 2.8, t = 4.23, P <0.001) and 20 h (2.4 ± 2.1 vs. 3.4 ± 2.4, t= 7.94, P <0.001) in the CFD group, with least-squares mean differences of 0.48 (95% confidence interval [CI]: 0.26–0.71, P <0.001) and 0.76 (95% CI: 0.57–0.95, P <0.001), respectively. The 4-h pain scores (2.1 ± 1.7 vs. 2.2 ± 1.7, t = 2.60, P = 0.009, with a least-squares mean difference of 0.21 [95% CI: 0.05–0.37]) and positive urine ketone levels (7.7% [39/509] vs. 15.4% [82/533], χ2 = 15.13, P <0.001) were lower in the CFD group. The CFD group had significantly less cholangitis (2.1% [13/634] vs. 4.0% [26/658], χ2 = 3.99, P = 0.046) but not pancreatitis (5.5% [35/634] vs. 6.5% [43/658], χ2 = 0.59, P = 0.444). Subgroup analysis revealed that CFD reduced the incidence of complications in patients with native papilla (odds ratio [OR]: 0.61, 95% CI: 0.39–0.95, P = 0.028) in the multivariable models. Conclusion::Ingesting 400 mL of CFD 2 h before ERCP is safe, with a reduction in post-ERCP fatigue, abdominal pain, and cholangitis during recovery.Trail Registration::ClinicalTrials.gov, No. NCT03075280.
10.Identification Methods of Tumor Tissue Origin Based on Different STR Typing Kits

Objective To establish the identification method of tumor tissue origin based on commonly used STR typing kits.Methods ForenSeqTM DNA Signature Prep kit was used to detect the typing of 27 autosomal STR loci in 55 paired tumor tissue samples(tumor tissue paired with normal tissue of the same individual)and 75 unrelated individual whole blood samples.The genotyping data of full sib-ling pairs and parent-child pairs of 55 tumor tissues were simulated.The number of total identical al-leles(An)and identity by state(IBS)scores were calculated within the paired carcinoma(PC),the tumor-unrelated individual(UI),the tumor-simulated full sibling(FS)and the tumor-simulated parent-offspring(PO)groups.The tumor tissue origin identification and prediction models of 8 commonly used STR typing kits were established based on the statistical results obtained above,and an attempt was made to establish a specific model for tumor tissue origin identification.The accuracy,sensitivity and specificity were verified and evaluated using the additional 23 paired tumor tissue samples.Re-sults(1)In any kit,there was no statistically significant difference in the number of loci shared with 0 identical allele(A0)between the PC and PO groups.The number of loci shared with 1 identical al-lele(A1),2 identical alleles(A2),and IBS scores were statistically significant different between the PC group and the UI,FS and PO groups.(2)The An and IBS scores of different STRs varied in dif-ferent groups.The A2 levels of 13 STRs(CSF1PO,D12S391,D19S433,D20S482,D2S1338,D3S1358,D4S2408,D7S820,D8S1179,FGA,TH01,TPOX,vWA)in PC group were higher than those of other STR loci.The A2 levels of two STRs(D6S1043,Penta E)in UI group were significantly lower than those of other STR loci.(3)The tumor tissue origin identification and prediction models of 8 com-monly used STR typing kits and the identification model of tumor tissue origin with 15 STR loci(15-STRs)were successfully established,with sensitivity of 100%,specificity of 97.56%-99.88%,and accu-racy of 97.59%-99.89%.Among them,the 15-STRs model had 100%sensitivity,99.88%specificity,and 99.89%accuracy,which were higher than those of commonly used commercial kits.Conclusion This study successfully establishes the tumor tissue origin identification methods with 8 commonly used STR typing kits,which expands the application of tumor tissue origin identification.In addition,the differences of different loci in the identification of tumor tissue origin were compared,and 15 STR loci which were particularly suitable for the identification of tumor tissue origin were selected,providing the data basis for the establishment of tumor origin tracing kits in future.
