1.PACS Implementation Challenges in a Public Healthcare Institution: A South African Vendor Perspective

OBJECTIVES: Conventional radiological processes have been replaced by digital images and information technology systems within South Africa and other developing countries. Picture Archiving and Communication Systems (PACS) technology offers many benefits to institutions, medical personnel and patients; however, the implementation of such systems can be a challenging task. It has been documented that South Africa has been using PACS for more than a decade in public hospitals with moderate success. The aim of this study was to identify and describe the PACS challenges endured by PACS vendors during implementation in the South African public healthcare sector. METHODS: This was achieved by engaging in a methodological approach that was qualitative in nature collecting data through semi structured interviews from 10 PACS experts/participants which were later analysed qualitatively. RESULTS: The findings show that PACS vendors have countless challenges, some of which include space, insufficient infrastructure, image storage capacity, system maturity and vendor related concerns. It was clear that the PACS experts readily offered contextually appropriate descriptions of their encounters during PACS implementations in South African public healthcare institutions. CONCLUSIONS: PACS vendors anticipate these challenges when facing a public healthcare institution and it is recommended that the hospital management and potential PACS stakeholders be made aware of these challenges to mitigate their effects and aid in a successful implementation.

2.Blogs as Channels for Disseminating Health Technology Innovations.

OBJECTIVES: The objective of this study was to describe the features of health informatics blogs on the Internet. METHODS: A search was conducted in August, 2016 using the search engine, Google, and key words: ‘mobile health blog,’‘telehealth/telemedicine blog,’‘Electronic Health Record blog,’‘personalized health record blog,’‘population health decision support system blog,’ and ‘public/population health dashboard blog.’ The first 24 blogs resulting from each key word search were recorded, generating 144 blogs. A total of 109 unique blogs resulted after removing duplicates and non-functional sites. RESULTS: Blogs with ‘.com’ extensions were most prevalent (72%, n = 79). More than half of the blogs (79%, n = 86) were created by industries. Mobile health (88%, n = 96), telehealth (82%, n = 89), and health IT (78%, n = 85) were the predominant topics covered. Health providers (44%, n = 48), industries (33%, n = 36), patients/consumers (25%, n = 27) and payers/insurance providers (19%, n = 21) constituted the most common target audience. Blogs catering to payers commonly used ‘.org’ extension (n = 10 out of 21), compared to ‘.com’ (n = 7) or ‘.gov’ (n = 2) (p < 0.0001). Significant differences were also observed by topics covered health IT (p = 0.007), subscription (p = 0.048) and LinkedIn social media (p = 0.019) across the website extensions. CONCLUSIONS: Further research is needed to examine the use of blogs as channels of communication of best evidence in health informatics research among diverse stakeholders. The role of blogs as policy informatics tools need to be evaluated in order for stakeholders to collaborate, coordinate and share opportunities and challenges of various public health programs and policies.

3.New Evaluation Vector through the Stanford Mobile Inquiry-Based Learning Environment (SMILE) for Participatory Action Research.

OBJECTIVES: This article reviews an evaluation vector model driven from a participatory action research leveraging a collective inquiry system named SMILE (Stanford Mobile Inquiry-based Learning Environment). METHODS: SMILE has been implemented in a diverse set of collective inquiry generation and analysis scenarios including community health care-specific professional development sessions and community-based participatory action research projects. In each scenario, participants are given opportunities to construct inquiries around physical and emotional health-related phenomena in their own community. RESULTS: Participants formulated inquiries as well as potential clinical treatments and hypothetical scenarios to address health concerns or clarify misunderstandings or misdiagnoses often found in their community practices. From medical universities to rural village health promotion organizations, all participatory inquiries and potential solutions can be collected and analyzed. The inquiry and solution sets represent an evaluation vector which helps educators better understand community health issues at a much deeper level. CONCLUSIONS: SMILE helps collect problems that are most important and central to their community health concerns. The evaluation vector, consisting participatory and collective inquiries and potential solutions, helps the researchers assess the participants' level of understanding on issues around health concerns and practices while helping the community adequately formulate follow-up action plans. The method used in SMILE requires much further enhancement with machine learning and advanced data visualization.

5.Review of Brucellosis in Nepal.

OBJECTIVES: The aim of this paper is to evaluate the current status of the disease, the mechanism of infection, and pathogenesis, its zoonotic potential, diagnostic advances, treatment regimens, and the preventive measures that can be adopted in managing human brucellosis in under-developed countries such as Nepal. METHODS: We performed a systematic review of all the available literture through Google Scholar, PubMed, Gideon Informatics, World Health Organization and other legitimate sources. Other secondary informations were collected from the government agencies such as department of livestock services and Ministry of Health. The obtained information was then re-analysed and summarized. RESULTS: Few publications have addressed brucellosis in Nepal and most of those publications have focused on bovine brucellosis with sparse information available on brucellosis in humans and small ruminants. Brucella abortus is the most predominant causative agent followed by B. suis. B. abortus is predominant in cattle accounting for a substantial portion of bovine abortion in the country. Lack of awareness, unhealthy food habit, traditional husbandry practices, and a lack of surveillance and immunization have been the major factors in maintaining a vicious cycle of propagation of the disease in human and animals. Unfortunately, nothing has been done to identify the species of Brucella at the biovar level. CONCLUSIONS: Although brucellosis has been reported to be endemic in Nepal, neither the distribution nor the economic and public health impact of this disease is well characterized. Robust and well-designed nationwide survey is warranted to assess the prevalence and distribution of disease in livestock and humans. Such data would facilitate the design of appropriate control programmes.

6.Health and Social Media: Perfect Storm of Information.

OBJECTIVES: The use of Internet in the health domain is becoming a major worldwide trend. Millions of citizens are searching online health information and also publishing content about their health. Patients are engaging with other patients in online communities using different types of social media. The boundaries between mobile health, social media, wearable, games, and big data are becoming blurrier due the integration of all those technologies. In this paper we provide an overview of the major research challenges with the area of health social media. METHODS: We use several study cases to exemplify the current trends and highlight future research challenges. RESULTS: Internet is exploding and is being used for health purposes by a great deal of the population. Social networks have a powerful influence in health decisions. Given the lack of knowledge on the use of health social media, there is a need for complex multidisciplinary research to help us understand how to use social networks in favour of public health. A bigger understanding of social media will give health authorities new tools to help decision-making at global, national, local, and corporate level. CONCLUSIONS: There is an unprecedented amount of data that can be used in public health due the potential combination of data acquired from mobile phones, Electronic Health Records, social media, and other sources. To identify meaningful information from those data sources it is not trial. Moreover, new analytics tools will need to be developed to analyse those sources of data in a way that it can benefit healthcare professionals and authorities.

7.Health Informatics in Developing Countries: Going beyond Pilot Practices to Sustainable Implementations: A Review of the Current Challenges.

OBJECTIVES: Information technology is an essential tool to improve patient safety and the quality of care, and to reduce healthcare costs. There is a scarcity of large sustainable implementations in developing countries. The objective of this paper is to review the challenges faced by developing countries to achieve sustainable implementations in health informatics and possible ways to address them. METHODS: In this non-systematic review of the literature, articles were searched using the keywords medical informatics, developing countries, implementation, and challenges in PubMed, LILACS, CINAHL, Scopus, and EMBASE. The authors, after reading the literature, reached a consensus to classify the challenges into six broad categories. RESULTS: The authors describe the problems faced by developing countries arising from the lack of adequate infrastructure and the ways these can be bypassed; the fundamental need to develop nationwide e-Health agendas to achieve sustainable implementations; ways to overcome public uncertainty with respect to privacy and security; the difficulties shared with developed countries in achieving interoperability; the need for a trained workforce in health informatics and existing initiatives for its development; and strategies to achieve regional integration. CONCLUSIONS: Central to the success of any implementation in health informatics is knowledge of the challenges to be faced. This is even more important in developing countries, where uncertainty and instability are common. The authors hope this article will assist policy makers, healthcare managers, and project leaders to successfully plan their implementations and make them sustainable, avoiding unexpected barriers and making better use of their resources.

8.Nursing Informatics Competencies of Public Health Nurses in Chungcheongnam-do.

PURPOSE: The aim of the study was to identify nursing informatics competencies of nurses working for public health centers in Chungcheongnam-do. METHODS: Data were collected from June 10 to July 25, 2012 using the Nursing Informatics Competencies Questionnaire (NICQ). Data were analyzed by descriptive statistics, t-test, chi2-test, ANOVA and Pearson's correlation coefficient. RESULTS: Nursing informatics competencies of the subjects showed a mean score of 3.3+/-1.0 out of 5. As for scores of individual categories, the score for computer skills competencies was 3.3+/-1.0, informatics knowledge competencies 3.4+/-0.9, and informatics skills competencies 3.0+/-0.9. Nursing informatics competencies were positively correlated with the subjects' ages (r=.65, p<.001), computer usage hours (r=.23, p = .015), levels of demand for informatics knowledge (r=.51, p<.001), and informatics skills education (r=.78, p<.001). CONCLUSION: Nursing informatics is required to be connected with job training or in-service education on account of its growing necessity for public health nurses. It is also essential to develop programs for strengthening informatics competencies reflecting sub-categories of educational needs.

9.Web-Based Integrated Public Healthcare Information System of Korea: Development and Performance.

OBJECTIVES: The Web-based integrated public healthcare information system (PHIS) of Korea was planned and developed from 2005 to 2010, and it is being used in 3,501 regional health organizations. This paper introduces and discusses development and performance of the system. METHODS: We reviewed and examined documents about the development process and performance of the newly integrated PHIS. The resources we analyzed the national plan for public healthcare, information strategy for PHIS, usage and performance reports of the system. RESULTS: The integrated PHIS included 19 functional business areas, 47 detailed health programs, and 48 inter-organizational tasks. The new PHIS improved the efficiency and effectiveness of the business process and inter-organizational business, and enhanced user satisfaction. Economic benefits were obtained from five categories: labor, health education and monitoring, clinical information management, administration and civil service, and system maintenance. The system was certified by a patent from the Korean Intellectual Property Office and accredited as an ISO 9001. It was also reviewed and received preliminary comments about its originality, advancement, and business applicability from the Patent Cooperation Treaty. It has been found to enhance the quality of policy decision-making about regional healthcare at the self-governing local government level. CONCLUSIONS: PHIS, a Web-based integrated system, has contributed to the improvement of regional healthcare services of Korea. However, when it comes to an appropriate evolution, the needs and changing environments of community-level healthcare service and IT infrastructure should be analyzed properly in advance.

10.Geographical Information Systems and Health: Current State and Future Directions.

This paper provides an introduction to Geographical Information Systems (GIS) and how they can be used. It reviews the current state of GIS use in health care before identifying the barriers to more pervasive use of GIS in health. Finally, it makes recommendations for the direction of health GIS research over the next decade and concludes with a call to action to health informatics researchers to stop ignoring a tool and methodology that has such immense potential for improving the health of our communities.
