1.Low-Frequency of Midnight-Noon Ebb-Flow Acupuncture-Opening Method in Treating Low Myopia in Children:A Retrospective Study

Objective To analyse the clinical efficacy of low-frequency of midnight-noon ebb-flow acupuncture-opening method(also called Ziwu Liuzhu Acupuncture,referring to the needling method based on acupoints selection according to qi and blood movement following heavenly-stems and earthly-branches cycle)in treating low myopia in children.Methods A retrospective study was conducted to collect the clinical data of 84 cases(168 eyes)of children with low myopia who received treatment at the Outpatient Department of Guangdong Provincial Hospital of Chinese Medicine from January 2021 to December 2022.The children were divided into two groups according to the different treatment protocols:42 cases(84 eyes)treated with acupuncture combined with conventional optometry as the control group,and 42 cases(84 eyes)treated with low-frequency of midnight-noon ebb-flow acupuncture-opening method combined with conventional optometry as the observation group,and the treatment course of both groups was 24 weeks.The changes of refractive error and axial length before and after treatment were observed,and the clinical efficacy in the two groups of children were evaluated.Results(1)After treatment,there being statistically significant differences in the refractive error between the two groups of the children(P<0.05),and the differential value of the refractive error of the children between the two groups before and after treatment was statistically significant(P<0.05).(2)After treatment,there being statistically significant differences in the axial length of the children in both groups(P<0.05).The differential value of axial lengths of the children between the two groups before and after treatment was statistically significant(P<0.05).(3)The total effective rate of the observation group was 84.52%(71/84),and which of the control group was 63.10%(53/84),the efficacy of the observation group was significantly superior to that of the control group,and the difference was statistically significant(P<0.05).Conclusion The low-frequency of midnight-noon ebb-flow acupuncture-opening method in treating low myopia in children can effectively postpone the degree of myopia,and decrease the growth of the axis,with exact therapeutic effect.
2.Therapeutic effect and safety observation of dexamethasone combined with ulinastatin in patients with acute severe viral myocarditis

Objective:To explore the therapeutic effect and safety of dexamethasone combined with ulinastatin in pa-tients with acute severe viral myocarditis(ASVMC).Methods:Clinical data of 92 ASVMC patients treated in our hospital from August 2018 to November 2020 were retrospectively collected,and divided into dexamethasone group(n=46)and combined treatment group(n=46,dexamethasone combined with ulinastatin)according to different treatment program.Each group received corresponding medication based on routine therapy for four weeks.Thera-peutic effect,levels of myocardial enzyme spectrum indexes[cardiac troponin Ⅰ(cTnⅠ),creatine kinase isoenzyme MB(CK-MB),lactate dehydrogenase(LDH)]and related indexes of cardiomyocyte apoptosis[soluble factor-related apoptosis(sFas),soluble factor-related apoptosis ligand(sFasL)]before and after treatment,and inci-dence of adverse reactions were compared between two groups.Results:The total effective rate(91.30%vs.73.91%)of combined treatment group was significantly higher than that of dexamethasone group(P=0.028).Compared with dexamethasone group,there were significant reductions in serum levels of cTnⅠ[(4.38±1.02)ng/ml vs.(1.76±0.24)ng/ml],CK-MB[(29.85±5.30)U/L vs.(20.26±4.18)U/L],LDH[(202.84±16.70)U/L vs.(184.21±12.63)U/L],sFas[(2.82±0.37)ng/L vs.(2.39±0.24)ng/L]and sFasL[(0.16±0.04)ng/L vs.(0.08±0.02)ng/L]in combined treatment group after 4-week treatment(P<0.001 all).There was no sig-nificant difference in incidence rate of adverse reactions during treatment between two groups(P=0.711).Conclusion:Ulinastatin combined with dexamethasone can further inhibit myocardial cell apoptosis,and promote the improvement of myocardial function with significant therapeutic effect in patients with acute severe viral myocarditis and it's safe.

COVID-19 has been prevalent for three years. The virulence of SARS-CoV-2 is weaken as it mutates continuously. However, elderly patients, especially those with underlying diseases, are still at high risk of developing severe infections. With the continuous study of the molecular structure and pathogenic mechanism of SARS-CoV-2, antiviral drugs for COVID-19 have been successively marketed, and these anti-SARS-CoV-2 drugs can effectively reduce the severe rate and mortality of elderly patients. This article reviews the mechanism, clinical medication regimens, drug interactions and adverse reactions of five small molecule antiviral drugs currently approved for marketing in China, so as to provide advice for the clinical rational use of anti-SARS-CoV-2 in the elderly.

In the outbreak of COVID-19,triage procedures based on epidemiology were implemented in a local hospital in Changsha to control the transmission of SARS-CoV-2 and avoid healthcare-associated infection.This re-trospective study analyzed the data collected during the triage period and found that COVID-19 patients were en-riched 7 folds into the Section A designated for patients with obvious epidemiological history.On the other side,nearly triple amounts of visits were received at the Section B for patients without obvious epidemiological history.8 COVID-19 cases were spotted out of 247 suspected patients.More than 50%of the suspected patients were submi-tted to multiple rounds of nucleic acid analysis for SARS-CoV-2 infection.Of the 239 patients who were diagnosed as negative of the virus infection,188 were successfully revisited and none was reported as COVID-19 case.Of the 8 COVID-19 patients,3 were confirmed only after multiple rounds of nucleic acid analysis.Besides comorbidities,delayed sharing of epidemiological history added complexity to the diagnosis in practice.The triaging experience and strategy will be helpful for the control of infectious diseases in the future.

Gouty arthritis is a type of metabolic rheumatic disease caused by autoimmune abnormalities. Currently, the use of Western medicine in the clinical treatment of gouty arthritis has been associated with a high risk of adverse reactions. Therefore, there is a growing interest in exploring therapeutic drugs from traditional Chinese medicine as a potential alternative. According to the theory of traditional Chinese medicine, gouty arthritis has been classified as damp-heat arthralgia syndrome. Shirebi granules has been found to have good clinical efficacy in treating gouty arthritis. However, its underlying pharmacological mechanisms remain unclear. To address this problem, the study first established the interaction network of candidate targets for Shirebi granules, which is used to treat damp-heat syndrome of gouty arthritis. Then, the key candidate targets of Shirebi granules for treating gouty arthritis with damp-heat syndrome were screened by calculating the topological features of the network nodes. Then, the functional mining of the key candidate targets revealed that the candidate targets of Shirebi granules may intervene in the biological process of inflammatory response and lipid metabolism through the crosstalk of Wnt/

8.Baicalin inhibits LPS/IFN-γ-induced inflammation via TREM2/TLR4/NF-κB pathway in BV2 cells.

This study investigated the mechanism of baicalin on lipopolysaccharide(LPS)/interferon γ(IFN-γ)-induced inflammatory microglia based on the triggering receptor expressed on myeloid cells 2(TREM2)/Toll-like receptor 4(TLR4)/nuclear factor kappaB(NF-κB) pathway. Specifically, LPS and IFN-γ were used to induce inflammation in mouse microglia BV2 cells. Then the normal group, model group, low-dose(5 μmol·L~(-1)) baicalin group, medium-dose(10 μmol·L~(-1)) baicalin group, high-dose(20 μmol·L~(-1)) baicalin group, and minocycline(10 μmol·L~(-1)) group were designed. Cell viability was detected by CCK-8 assay and cell morphology was observed under bright field. The expression of interleukin-1β(IL-1β), interleukin-4(IL-4), inducible nitric oxide synthase(iNOS), interleukin-6(IL-6), interleukin-10(IL-10), and arginase-1(Arg-1) mRNA was detected by real-time quantitative PCR, the protein expression of tumor necrosis factor-α(TNF-α), IL-1β, TREM2, TLR4, inhibitor kappaB-alpha(IκBα), p-IκBα, NF-κB p65 and p-NF-κB p65 by Western blot, and transfer of NF-κB p65 from cytoplasm to nucleus by cellular immunofluorescence. Compared with the normal group, most of the BV2 cells in the model group tended to demonstrate the pro-inflammatory M1 amoeba morphology, and the model group showed significant increase in the mRNA levels of IL-1β, IL-6, and iNOS, decrease in the mRNA levels of IL-4, IL-10, and Arg-1(P<0.01), rise of the protein expression of TNF-α, IL-1β, TLR4, p-IκBα, and p-NF-κB p65(P<0.01), reduction in TREM2 protein expression, and increase in the expression of NF-κB p65 in nucleus. Compared with the model group, baicalin groups and minocycline group showed the recovery of BV2 cell morphology, significant decrease in the mRNA levels of IL-1β, IL-6 and iNOS, increase in the mRNA levels of IL-4, IL-10, and Arg-1(P<0.01), reduction in the protein expression of TNF-α, IL-1β, TLR4, p-IκBα, and p-NF-κB p65(P<0.05), rise of TREM2 protein expression, and decrease in the expression of NF-κB p65 in nucleus. In summary, these results suggest that baicalin can regulate the imbalance between TREM2 and TLR4 of microglia and inhibit the activation of downstream NF-κB, thus promoting the polarization of microglia from pro-inflammatory phenotype to anti-inflammatory phenotype.

9.Application of the Mathieu combined tunnel technique for repairing glans dehiscence after failed hypospadias repair.

Repairing glans dehiscence after failed hypospadias repair is challenging for pediatric surgeons. Here, we introduced and evaluated a newly modified Mathieu technique, Mathieu combined tunnel (MCT), which involves multiple custom-designed flaps for the shortage of flap source material after repeated operations; we also constructed a tunnel to avoid the glans incision that may carry new risks of dehiscence. This retrospective study included 26 patients who were consecutively admitted to the First Affiliated Hospital of Sun Yat-Sen University (Guangzhou, China) for glans dehiscence repair after failed hypospadias repair from October 2014 to October 2020; sixteen patients underwent surgery using the MCT (MCT group) and ten patients underwent surgery using the tubularized incised plate (TIP) technique (TIP group). The operative time, blood loss, postoperative complications, normal urethral meatus rate, success rate, and Hypospadias Objective Penile Evaluation (HOPE) score were compared between the two groups. The MCT group achieved an overall satisfactory penile appearance and voiding function, with a higher rate of normal urethral meatus (15/16, 93.8%) and a lower rate of glans dehiscence (1/16, 6.2%), compared with the TIP group (70.0% and 30.0%, respectively). However, these differences were not statistically significant, possibly because of the limited number of patients (all P > 0.05). Mean postoperative HOPE scores were similar in the MCT group (mean ± standard deviation: 8.83 ± 0. 89) and TIP group (8.94 ± 0.57) (P > 0.05). No significant differences were found between the two groups in terms of blood loss and success rate, nor in the rates of various complications (e.g., fistula, urethral stricture, and glans dehiscence). In conclusion, the MCT technique appears to be feasible and reliable for repairing glans dehiscence after failed hypospadias repair.

10.Effects of Gelsemium elegans Combined with Mussaenda pubescens on Transporter BCRP and Drug-metabolizing Enzyme CYP3A11

ObjectiveTo explore the effect of Gelsemium elegans combined with Mussaenda pubescens on efflux transporter breast cancer resistance protein (BCRP) and cytochrome P450 3A11 (CYP3A11) and their attenuation mechanism, and to investigate whether the nuclear receptors were involved in such regulation by intervening it with nuclear receptor activators. MethodC57BL/6 mice were divided into the blank group, G. elegans (GE, 0.25 g·kg-1)group, GE + M. pubescens (MP) (0.25 g·kg-1+10 g·kg-1) group, GE + pregnane X receptor (PXR) activator (rifampicin)(GE + Rif,0.25 g·kg-1+50 mg·kg-1) group, GE + MP + Rif (0.25 g·kg-1+10 g·kg-1+50 mg·kg-1) group, GE + constitutive androstane receptor (CAR) activator (1,4-Bis [2-(3,5-Dichloropyridyloxy)] benzene, TCPOBOP)(GE + TCP, 0.25 g·kg-1+0.5 mg·kg-1) group, and GE + MP + TCP (0.25 g·kg-1+10 g·kg-1+0.5 mg·kg-1) group. The medication lasted for 14 successive days. One hour after the last administration, the mice were sacrificed by cervical dislocation and the liver tissue was harvested. The left liver tissue was stained with hematoxylin- eosin (HE) for observing the pathological changes. The right liver tissue was used for BCRP and CYP3A11 mRNA and protein expression detection by real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) and Western blot. ResultThe survival rates of mice in the GE + Rif group, GE group, and GE + MP group were 25% (the lowest), 40%, and 80%, respectively, and no death was observed in the other groups. Compared with the obvious lesions in the liver cells of the GE group, the pathological changes in liver cells of the GE + MP group were alleviated, while those in the GE + Rif group were worsened. Compared with the GE + Rif group, the GE + MP + Rif group exhibited relieved pathological changes in liver cells. Both the GE + TCP group and the GE + MP + TCP group showed mild liver lesions. The comparison with the GE + MP group revealed that the pathological changes in the GE + MP + TCP group were slightly relieved. Compared with the blank group, the expression of BCRP protein and mRNA in GE group were significantly decreased (P<0.05,P<0.01).The expression of CYP3A11 protein in GE group were significantly decreased (P<0.01). Compared with the GE group, the GE + MP group displayed remarkably up-regulated BCRP protein and mRNA expression (P<0.05,P<0.01) and CYP3A11 protein expression (P<0.05), but slightly up-regulated CYP3A11 mRNA expression. Compared with the GE group, the GE + Rif group exhibited down-regulated BCRP protein expression (P < 0.05). The protein and mRNA expression levels of BCRP were lower in the GE + MP + Rif group than in the GE + MP group (P<0.05,P<0.01). The PXR activator rifampicin regulated BCRP before and after the combination of G. elegans with M. pubescens. The CYP3A11 protein and mRNA expression levels in the GE + TCP group were higher than those in the GE group (P<0.05,P<0.01). Compared with the GE + MP group, the GE + MP + TCP group showed up-regulated CYP3A11 protein and mRNA expression (P<0.05,P<0.01). CAR activator TCPOBOP also had a regulatory effect on CYP3A11 before and after the compatibility of G. elegans with M. pubescens. ConclusionThe attenuated toxin after the combination of G. elegans with M. pubescens is closely related to the efflux transporter BCRP and the drug-metabolizing enzyme CYP3A11.
