1.Dynamic Monitoring and Correlation Analysis of General Body Indicators, Blood Glucose, and Blood Lipid in Obese Cynomolgus Monkeys

ObjectiveThis study aims to investigate the dynamic changes in general body parameters, blood glucose, and blood lipid profiles in obese cynomolgus monkeys, exploring the correlations among these parameters and providing a reference for research on the obese cynomolgus monkey model. Methods30 normal male cynomolgus monkeys aged 5 - 17 years old (with body mass index < 35 kg/m² and glycated hemoglobin content < 4.50%) and 99 spontaneously obese male cynomolgus monkeys (with body mass index ≥35 kg/m² and glycated hemoglobin content < 4.50%) were selected. Over a period of three years, their abdominal circumference, skinfold thickness, body weight, body mass index, fasting blood glucose, glycated hemoglobin, and four blood lipid indicators were monitored. The correlations between each indicator were analyzed using repeated measurement ANOVA, simple linear regression, and multiple linear regression correlation analysis method. Results Compared to the control group, the obese group exhibited significantly higher levels of abdominal circumference, skinfold thickness, body weight, body mass index, and triglyceride (P<0.05). In the control group, skinfold thickness increased annually, while other indicators remained stable. Compared with the first year, the obese group showed significantly increased abdominal circumference, skinfold thickness, body weight, body mass index, triglyceride, and fasting blood glucose in the second year(P<0.05), with this increasing trend persisting in the third year (P<0.05). In the control group, the obesity incidence rates in the second and third years were 16.67% and 23.33%, respectively, while the prevalence of diabetes remained at 16.67%. In the obese group, the diabetes incidence rates were 29.29% and 44.44% in years 2 and 3, respectively. Among the 11-13 year age group, the incidence rates were 36.36% and 44.68%, while for the group older than 13 years, the rates were 28.13% and 51.35%. Correlation analysis revealed significant associations (P<0.05) between fasting blood glucose and age, abdominal circumference, skinfold thickness, body weight, and triglyceride in the diabetic monkeys. Conclusion Long-term obesity can lead to the increases in general physical indicators and fasting blood glucose levels in cynomolgus monkeys, and an increase in the incidence of diabetes. In diabetic cynomolgus monkeys caused by obesity, there is a high correlation between their fasting blood glucose and age, weight, abdominal circumference, skinfold thickness, and triglyceride levels, which is of some significance for predicting the occurrence of spontaneous diabetes.
2.Rapid health technology assessment of ulinastatin in the treatment of acute pancreatitis

OBJECTIVE To conduct rapid health technology assessment (HTA) of ulinastatin (UTI), and to evaluate the efficacy, safety and cost-effectiveness of UTI in the treatment of acute pancreatitis (AP). METHODS Retrieved from PubMed, Embase, the Cochrane Library, CNKI, Wanfang database, CBM and official websites of HTA institutions, the systematic review (SR)/meta-analysis, economic evaluation and HTA reports of UTI in the treatment of AP were collected from the inception to Apr. 2024. Two researchers independently conducted screening, quality evaluation and data extraction according to the admission and exclusion criteria, and descriptive analysis was adopted to analyze and summarize the data. RESULTS A total of 19 studies were included, involving 15 SR/meta-analysis and 4 economic studies, and no HTA report was retrieved. In the treatment of AP, UTI showed clear advantages over conventional treatment alone in terms of improving the overall effective rate, shortening the recovery time of amylase, reducing the time required to relieve abdominal pain and distension, lowering the mortality rate, and decreasing the average hospital stay. Compared to other positive drugs (carbendate mesylate, octreotide, somatostatin, etc.), its efficacy is similar, with a favorable safety profile. As far as the current research was concerned, UTI had obvious economic advantages over other positive drugs. CONCLUSIONS UTI is safe and effective in the treatment of AP, and has economic advantages.
3.Factors influencing malnutrition in tuberculosis patients based on analysis of nutritional status in different populations

Objective To investigate the nutritional status and dietary structure of tuberculosis patients among dif-ferent populations, analyze the factors influencing the nutritional status of tuberculosis patients, and provide theo-retical basis for improving clinical nutrition and related issues in tuberculosis patients.Methods Tuberculosis pa-tients, non-tuberculosis patients, and healthy individuals were randomly selected for a questionnaire survey.De-scriptive analysis was conducted using SPSS 20.0 software.Statistical description was performed using rates and composition ratios, and qualitative data were described using relative numbers.Chi-square test was used to compare overall rates and composition ratios among different health conditions groups, with a significance level of α=0.05.Independent factors analysis of nutritional status body mass index (BMI) was conducted using multiple Logistic re-gression analysis for variables with statistically significant differences in the univariate analysis.Results There were differences in the nutritional status (x2 =62.184, P<0.05) and dietary diversity score (x2 =64.049, P<0.05) among tuberculosis patients, non-tuberculosis patients, and healthy individuals.Univariate analysis of nutri-tional status BMI showed statistically significant differences in gender, smoking, meat-based diet, vegetable-based diet, moderate diet diversity score, and 6 other variables for tuberculosis patients (P <0.05) , and in gender, age, ethnicity, marital status, occupation, education level, smoking, drinking white wine, drinking beer, meat-based diet, moderate diet, and 11 other variables for healthy individuals (P<0.05) .The variables with statisti-cally significant differences in the univariate analysis were included in the multiple ordinal logistic regression analy-sis model for both tuberculosis patients and healthy individuals.The results showed that the level of education, veg-etable intake, moderate food diversity score (DDS) of 4-6 were independent influencing factors of nutritional sta-tus BMI among tuberculosis patients (P<0.05);marital status was an independent influencing factor of nutritional status BMI among non-tuberculosis patients (P<0.05);while gender and occupation were independent influencing factors of nutritional status BMI among healthy individuals (P<0.05).Conclusion The dietary nutritional status of the three population groups varied.Targeted health education should be conducted, especially for tuberculosis patients, to address the issue of uneven dietary intake and promote good dietary habits among local tuberculosis pa-tients.
4.Expression of Ppp3cb and Ppm1g in the hippocampus of NHE1 gene knockout rats based on proteomics

Objective To investigate and validate the expression profiles of Ppp3cb and Ppm1g through differential proteomic analysis of hippocampal tissue in NHE1 gene knockout mice with proteomic analysis.Method ① Six 2-week-old NHE1 knockout mice were selected as the model group,and 6 wild-type mice of the same age served as the control group,and their genotypes were detected by agar-gel electrophoresis.Open field test and forced swimming test were used to evaluate the behaviors of mice in the model group and control group,and epileptic seizure was graded according to Racine scoring.② Tandem mass spectrometry was employed to screen the differential proteins in the hippocampus tissues from the model group and the control group.Then the obtained differential proteins were annotated and enriched in the Gene Ontology(GO)database.Search tool for the retrieval of interesting genes(STRING)database was used to analyze protein-protein interaction(PPI)among different proteins.③ The transcriptional and translational levels of Ppp3cb and Ppm1g were detected by qPCR and Western blotting,respectively,and their expression levels in the tissues were observed with immunohistochemistry.Results ① NHE1 was not expressed in the model group.The mice of the model group had shorter total movement distance(P=0.007 3)and less crossing cells(P<0.000 1)in open field test,and longer period of immobility in forced swimming test(P<0.000 1)when compared with those from the control group.② When fold change ≥1.2 times and P<0.05 were set as the significant threshold for differential expression,845 differentially expressed protein sites were detected in the hippocampus,among which 9 proteins(including Ppm1g)were up-regulated and 7 ones(including Ppp3cb)were down-regulated.Gene Ontology(GO)functional analysis showed that after NHE1 knockout,the most significant differences were observed in the concentration of molecular function(MF)related to protein serine/threonine phosphatase activity,concentration of cellular component(CC)related to the plasma membrane,and concentration of biological process(BP)related to negative regulation of biological processes and immune system processes.STRING analysis indicated that the differential proteins Ppp3cb and Slc9a1 directly acted,Ppm1g indirectly acted through Ppp3cb and Slc9a1,and Ppp3cb and Ppm1g interacted.③The transcriptional and translational levels of Ppp3cb were decreased,and its expression level was reduced in the tissues,while those of Ppm1g were increased,and its expression was elevated in the tissues(P<0.05).Conclusion In the hippocampus of NHE1 gene knockout mice,the expression of differential protein Ppp3cb is down-regulated and that of Ppm1g is up-regulated,which provide a basis for further study on their involvement in the pathogenesis of epilepsy.
5.Short-term outcomes of the TRIANGLE operation after neoadjuvant chemotherapy in locally advanced pancreatic cancer

Objective:To investigate the safety and efficacy of the TRIANGLE operation after neoadjuvant chemotherapy in locally advanced pancreatic cancer(LAPC).Methods:This study is a retrospective case series analysis. Between January 2020 and December 2022, a total of 103 patients were diagnosed as LAPC who underwent neoadjuvant chemotherapy at the Pancreas Center, the First Affiliated Hospital of Nanjing Medical University. Among them, 26 patients (25.2%) underwent the TRIANGLE operation. There were 15 males and 11 females,with a age of (59±7) years (range: 49 to 74 years). The pre-treatment serum CA19-9( M(IQR)) was 248.8(391.6)U/ml (range: 0 to 1 428 U/ml),and the serum carcinoembryonic antigen was 4.1(3.8)μg/L(range: 1.4 to 13.4 μg/L). The neoadjuvant chemotherapy regimens included: mFOLFIRINOX regimen in 6 cases(23.1%), GnP regimen in 14 cases(53.8%), and mFOLFIRINOX+GnP regimen in 6 cases(23.1%). The follow-up duration extended until June 2023 or until the occurrence of the patient′s death or loss to follow-up. The Kaplan-Meier method was employed to estimate the 1-year and 3-year overall survival rates. Results:After neoadjuvant chemotherapy,CA19-9 levels decreased by 92.3(40.1)%(range:2.1% to 97.7%). Evaluation of the response to treatment revealed 13 cases(50.0%) of stable disease,11 cases(42.3%) of partial response,and 2 cases(7.7%) of complete response. The surgical operation consisted of 12 cases(46.2%) of pancreaticoduodenectomy,12 cases(46.2%) of distal pancreatectomy,and 2 cases(7.7%) of total pancreatectomy. Margin determination was based on the “standardised pathology protocol” and the “1 mm” principle. No R2 and R1(direct) resections were observed,while the R0 resection rate was 61.5%(16/26), and the R1(1 mm) resection rate was 38.5%(10/26).The R1(1 mm) resection rates for the anterior margin,posterior margin,transected margin,portal vein groove margin,and uncinate margin were 23.1%(6/26),19.2%(5/26),12.5%(3/24),2/14, and 1/12, respectively. The overall postoperative complication rate was 57.8%(15/26),with major complications including grade B/C pancreatic fistula 25.0%(6/24,excluding 2 cases of total pancreatectomy),delayed gastric emptying in 23.1%(6/26),wound complications 11.5%(3/26),postoperative hemorrhage 7.7%(2/26), chylous fistula 7.7%(2/26) and bile fistula 3.8%(1/26). No reoperation was performed during the perioperative period(<90 days). One patient died on the 32 nd day postoperatively due to a ruptured pseudoaneurysm. A total of 25 patients were followed up,with a follow-up time of 21(24)months(range: 8 to 42 months). During the follow-up period,8 cases(32.0%) died due to tumor recurrence and metastasis,while 17 patients(68.0%) remained alive,including 11 cases of disease-free survival,5 cases of distant metastasis,and 1 case of local recurrence. The overall survival rates at 1- and 3-year after the initiation of neoadjuvant chemotherapy were 95.8% and 58.9%, respectively. The overall survival rates at 1- and 3-year after surgery were 77.7% and 57.8%, respectively. Conclusion:Performing pancreatoduodenectomy according to the Heidelberg triangle protocol in LAPC patients after neoadjuvant chemotherapy might increase the R0 resection rate without increasing perioperative mortality or the incidence of major postoperative complications.

7.The Development and Equivalence Analysis of Mandarin Tracking of Noise Tolerance Test Materials

Objective To develop and evaluate the equivalence of the Mandarin test material for tracking of noise tolerance(TNT)test.Methods Six different speech materials were developed(themes including daily life,entertainment,family,festivals,outdoors,and school).Four-minute TNT tests were measured in 21 normal hear-ing subjects using six different test materials.For each session,the tolerable noise level(TNL)and TNT scores were acquired and calculated for 3 time windows(31~240 s,31~120 s,151~240 s).Results Statistic analysis showed significant differences in the TNL(F=43.611,P<0.05)among the normal hearing listeners.There were statistically significant differences in standardize z-scored TNT scores of the six different materials in the three time windows(P<0.05).Post-hoc comparisons revealed that all significant differences involved the family and daily life themes.Conclusion Entertainment,festival,outdoors and school themed test materials can serve as the materials of Mandarin tracking of noise tolerance test and can be appied in research and clinical testing.
8.Expressions of β-catenin/transcription factor 4 signaling pathway-related proteins in colon cancer tissues and their relationship with postoperative recurrence and metastasis

Objective To investigate the expressions of the β-catenin/transcription factor 4(TCF4)signaling pathway-related proteins in colon cancer tissues and their relationship with postoperative recurrence and metastasis of colon cancer.Methods A total of 86 patients with colon cancer admitted to Zhumadian Central Hospital from January 2020 to January 2022 were selected as the research subjects,all of whom underwent surgical resection of the colon.Real-time fluorescence quantitative polymerase chain reaction was used to detect the expression levels of Wnt-1,β-catenin and TCF4 mRNA in cancer tissues and adjacent tissues of patients,and immunohistochemistry was used to detect the protein expression levels of Wnt-1,β-catenin and TCF4.The Cox proportional hazards regression model was used to analyze the risk factors of postoperative recurrence and metastasis of colon cancer.Results The relative mRNA expression levels and protein expression positive rates of Wnt-1,β-catenin and TCF4 in cancer tissues were significantly higher than those in adjacent tissues of colon cancer patients(P<0.05).The expression positive rates of Wnt-1,β-catenin and TCF4 protein were correlated with the degree of tumor invasion,tumor stage and lymph node metastasis(P<0.05),but not correlated with age,sex,tumor size and differentiation degree(P>0.05).Cox regression analysis showed that lymph node metastasis,and positive protein expressions of Wnt-1,β-catenin and TCF4 were risk factors for postoperative recurrence and metastasis of colon cancer(P<0.05).Conclusion The mRNA expression levels and protein expression positive rates of Wnt-1,β-catenin and TCF4 are higher in colon cancer tissues.The protein expression positive rates of Wnt-1,β-catenin and TCF4 are significantly correlated with the degree of tumor invasion,tumor stage and lymph node metastasis,and are risk factors for postoperative recurrence and metastasis of colon cancer.
9.Construction and Validation of Prognostic Risk Scoring Model for Uterine Corpus Endometrial Carcinoma Based on ADME-related Gene Markers

Objective To construct a prognostic model for uterine corpus endometrial carcinoma(UCEC)using the prognosis-associated ADME genes involved in controlling drug absorption,distribution,metabolism,and excretion(ADME),providing a reference for predicting the prognosis of UCEC and tumor treatment.Methods The gene expression and clinical information data of UCEC were obtained from the TCGA and ICGC databases.Prognosis-related ADME genes were screened using the univariate Cox regression analysis.Least absolute shrinkage and selection operator(LASSO)regression was used to identify optimal prognostic genes,and then a risk score model was constructed.Kaplan-Meier curve and receiver operating characteristic(ROC)curve were constructed to assess predictive capability.R software was used to perform differentially expressed genes anal-ysis,functional enrichment analysis.Results A nine-gene signature(DHRS7B,CYP46A1,SLCO4C1,NR1I2,SLC16A1,SLCO3A1,ARSA,ABCC5,MGST2)was used for constructing a risk score model.Survival analysis showed that the survival time in high-risk patients was significantly shorter than in low-risk patients(the train set:P<0.001;the test set:P=0.032).The are-as under the ROC curve of the train set and the test set for ROC at 1,3,5 years were 0.792,0.724,0.712,and 0.651,0.620,0.677,respectively.Univariate and multivariate Cox regression analysis showed that the risk score was an independent risk factor for UCEC(HR=1.77,P=0.035).In addition,there was a significant difference in the miRNA-mediated silencing and regulation of sprouting angiogenesis pathways between the high-risk and low-risk score groups.Conclusion The prognosis model con-structed with nine key ADME genes may have the potential to be used for predicting the survival prognosis of patients with UCEC.
10.Imaging manifestations of alveolar soft part sarcoma

Objective To observe the imaging manifestations of alveolar soft part sarcoma(ASPS).Methods CT,MRI and PET/CT data of 21 patients with pathologically confirmed ASPS were retrospective reviewed,and the imaging findings were analyzed.Results All 21 cases of ASPS presented as single primary lesion,mostly located in lower limbs(9/21,42.86%)and pelvis(5/21,23.81%),with the average maximum diameter of(72.10±42.23)mm.On non-contrast CT imaging,primary lesions appeared as iso-low inhomogeneous density masses(9/9,100%),which obviously heterogeneously enhanced in 6 cases(6/6,100%).Enlarged blood vessel shadows were noticed in 3 cases(3/6,50.00%),especially upper/lower pole of the tumors.The primary lesions presented as iso-slight high signals on non-contrast T1WI,mixed high signals on both T2WI and diffusion weighted imaging(DWI),as well as low signals on apparent diffusion coefficient(ADC)images(18/18,100%),with necrosis and cystic areas within the tumor and empty blood vessel shadows in 15 cases(15/18,83.33%).Heterogeneous enhancement of primary lesions were detected in 9 cases(9/9,100%),while necrosis and cystic areas did not enhance,but empty blood vessel shadows obviously enhanced.PET/CT demonstrated increased FDG uptake in 4 cases(4/4,100%),with the maximum standard uptake value ranged from 4.2 to 7.9.Among 21 cases,distant metastases developed in 19(19/21,90.48%),most frequently to lungs(17/21,80.95%),bones(11/21,52.38%)and lymph nodes(6/21,28.57%).Conclusion ASPS usually occurred in lower limbs and pelvis,which was prone to distant metastasis.The imaging manifestations of primary ASPS mostly presented as large tumors with uneven density,and enlarged blood vessel above and below the tumor were the relative characteristic performances.
