1.Association between air pollutants and pediatric outpatient volumes in Baoshan District

Objective:
To explore the relationship between air pollutants and pediatric outpatient volumes in Baoshan District, Shanghai Municipality.
Methods:
Data of meteorological factors, air pollutants and pediatric outpatient volumes in four general hospitals were collected in Baoshan District from 2015 to 2019, and a generalized additive model was used to fit the Poisson-like distribution. The exposure-response relationship between fine particulate matter (PM2.5), sulfur dioxide (SO2), nitrogen dioxide (NO2), ozone (O3) and total pediatric outpatient volumes and pediatric respiratory outpatients.
Results:
The median of the average daily temperature and relative humidity were 18.7 (interquartile range, 14.4) ℃ and 74.5% (interquartile range, 18.0%) in Baoshan District from 2015 to 2019, respectively. The median of the average daily concentrations of PM2.5, SO2, NO2 and O3 were 35.0 (interquartile range, 35.0), 11.0 (interquartile range, 7.0), 45.0 (interquartile range, 31.8) and 84.5 (interquartile range, 50.0) μg/m3, respectively. The median of the average daily total and respiratory pediatric outpatient volumes were 680 (interquartile range, 246) and 392 (interquartile range, 253). Spearman rank correlation analysis showed that temperature, relative humidity, PM2.5, SO2, NO2 and O3 were associated with total and respiratory pediatric outpatient volume (all P<0.05). Under the single pollutant model, the excess risk of total and respiratory pediatric outpatient volume due to PM2.5 (ER=0.318, 0.257), SO2 (ER=1.610, 2.546), and NO2 (ER=0.808, 0.839) reached the maximum effect on the same day, and the effect of O3 (ER=0.102, 0.222) reached its maximum at the first day of lag. Under the multi-pollutant model, after O3, SO2, NO2 and PM2.5 were introduced, a exposure-response relationship between air pollutants and total pediatric outpatient volumes was the largest on the sixth day after the lag (ER=0.419). There was no exposure-response relationship between air pollutants and respiratory pediatric outpatient volumes.
Conclusion
PM2.5, SO2, NO2 and O3 are associated with total and respiratory pediatric outpatient volumes, and the lag effects due to different air pollutants are different.
2.Clinical effects of free superficial temporal fascia flap combined with split-thickness thin skin graft in repairing refractory wounds in the anterior tibia

Objective:To observe the clinical effect of free superficial temporal fascia flap combined with split thickness skin transplantation in repairing refractory wounds in the anterior tibia.Methods:Data on 19 patients with soft tissue defects in the anterior tibial region who were admitted to the First Affiliated Hospital of Air Force Medical University from September 2019 to October 2023 and met the inclusion criteria were collected and summarized. Among them, 11 were males and 8 were females, aged 19-70 years old. The wound area was 4.3 cm×5.0 cm-6.8 cm×9.5 cm, and all wounds were accompanied by tendon exposure. 5 patients also had local bone exposure, and 10 patients had varying degrees of local infection. All patients were treated with wound debridement and continuous closed negative pressure drainage to control infection. After controlling the wound infection, an equally large temporal superficial fascia tissue flap was designed and cut according to the size of the wound to repair the wound. At the same time, a scalp split thick skin was taken to cover the fascia flap.Results:All 19 patients with superficial temporal fascia flaps survived, while 2 patients had poor skin flap survival due to subcutaneous hematoma. After re-grafting, the wound healed. After follow-up for 6-24 months, all patients were satisfied with the appearance of the anterior tibial region and had good recovery of ankle joint function. The supply valve area was concealed, without obvious scars, hair loss, baldness and other complications.Conclusions:The use of free superficial temporal fascia flap combined with split thick skin transplantation for repairing anterior tibial wounds has the advantages of strong anti infection ability, thin fascia flap, concealed donor site, and reconstruction of supporting ligaments. It is an ideal repair method for repairing difficult to heal wounds in the anterior tibial area.
3.Phenylethanoid glycosides from Verbenae Herba

AIM To study the phenylethanoid glycosides from Verbenae Herba.METHODS The 80%ethanol extract from Verbenae Herba was isolated and purified by silica gel,Sephadex LH-20,TLC and semi-preparative HPLC,then the structures of obtained compounds were identified by physicochemical properties and spectral data.RESULTS Nine compounds were isolated and identified as verbofficoside A(1),cistanoside D(2),epimeredinoside A(3),verbascoside(4),isoverbascoside(5),cistanoside C(6),cistanoside F(7),decaffeoylacteoside(8),jionoside C(9).CONCLUSION Compound 1 is a new compound.Compounds 3 and 6-9 are isolated from this plant for the first time.

RESULTS:We included 57 studies in this review.Chlorfenapyr can be degraded into tralopyril,which is more toxic and reduces energy production by inhibiting the conversion of adenosine diphosphate to adenosine triphosphate.High fever and altered mental status are characteristic clinical presentations of chlorfenapyr poisoning.Once it occurs,respiratory failure occurs immediately,ultimately leading to cardiac arrest and death.Chlorfenapyr poisoning is difficult to treat,and there is no specific antidote. CONCLUSION:Chlorfenapyr is a new pyrrole pesticide.Although it has been identified as a moderately toxic pesticide by the World Health Organization(WHO),the mortality rate of poisoned patients is extremely high.There is no specific antidote for chlorfenapyr poisoning.Therefore,based on the literature review,future efforts to explore rapid and effective detoxification methods,reconstitute intracellular oxidative phosphorylation couplings,identify early biomarkers of chlorfenapyr poisoning,and block the conversion of chlorfenapyr to tralopyril may be helpful for emergency physicians in the diagnosis and treatment of this disease.
