1.Clinical research progress of orthostatic hypotension in patients with hypertension

Hypertension and orthostatic hypotension (OH) often coexist in the range of cardiovascular autonomic dysfunction, especially in the elderly population, where antihypertensive therapy may be more likely to cause OH. The incidence rate of hypertension combined with OH is increasing year by year, which brings clinicians a clinical dilemma as to what measures to provide to hypertensive patients to prevent the occurrence of OH in the future. However, current clinical studies have shown that the management strategies based on pathophysiology are effective for most patients. This paper mainly elaborates on the complex relationship between OH and hypertension, and discusses the risk factors and prevention strategies of hypertension with OH, in order to provide theoretical support for the prevention of OH in patients with hypertension.
2.NR4A1 suppresses cisplatin-induced ferroptosis in renal proximal tubular epithelial cells by up-regulating the expression of NRF2

Objective:To explore the role and mechanism of nuclear receptor subfamily 4 group A member 1 ( NR4A1) in suppressing cisplatin nephrotoxicity. Methods:The expression of NR4A1 gene in renal cell subpopulations was analyzed using the "Tabula-muris" single cell transcriptome sequencing database. NR4A1 gene was over-expressed by lentivirus infection in HK-2 cell line and primary renal proximal tubular epithelial cells. Cell counting kit-8 was used to detect the cytotoxicity of cisplatin. The cell death ratio was analyzed using propidium iodide (PI) staining by flow cytometry. The expression of NR4A1 and nuclear factor erythroid 2-related factor 2 ( NRF2) was detected by real-time fluorescent quantitative PCR and Western blotting. Ferroptosis was analyzed by detecting the contents of malondialdehyde (MDA), oxidized glutathione (GSSG) and lipid reactive oxygen species (ROS). Results:The single cell transcriptome sequencing database showed that NR4A1 gene was the lowest expression in renal proximal tubular epithelial cell subsets. Cisplatin (50 μmol/L or 100 μmol/L) could significantly induce MDA, GSSG and lipid ROS production in renal proximal tubular epithelial cells (all P<0.01), and higher cisplatin concentration accompanied with a more increase of MDA, GSSG and lipid ROS. Compared with the control HK-2 cells, the lipid ROS content and iron ion content of HK-2 cells over-expressing NR4A1 were significantly lower (all P<0.01), and the over-expression of NR4A1 inhibited cisplatin-induced cytotoxicity and ferroptosis in renal proximal tubular epithelial cells. Mechanistically, NR4A1 up-regulated the expression of anti-ferroptosis gene NRF2 in proximal renal tubular epithelial cells ( P<0.01). Furthermore, single cell data analysis showed that, similar to the expression of NR4A1 in renal tissue subsets, NRF2 was also the lowest in renal proximal tubular epithelial cells. Conclusions:Cisplatin can induce ferroptosis of renal proximal tubular epithelial cells in a dose-dependent manner. NR4A1 can inhibit cisplatin-induced ferroptosis by up-regulating NRF2 in renal proximal tubular epithelial cells, thereby alleviating the cytotoxicity of cisplatin.
3.Motivations for hospice care volunteerism and its influencing factors among undergraduate nursing students in Xinjiang

Objective:To investigate the current situation of motivations for hospice care volunteerism among undergraduate nursing students, and to analyze its influencing factors.Methods:Using the convenient sampling method, a total of 575 nursing students from Nursing Department of Xinjiang Medical University were selected as the research objects in August 2022. They were investigated using general information questionnaire, Chinese version of Inventory of Motivations for Hospice Palliative Care Volunteerism Scale, The Palliative Care Quiz for Nursing, the Organizational Climate Scale and Prosocial Tendencies Measure Scale.Results:The score of Motivations for Hospice Palliative Care Volunteerism Scale for 575 nursing students was (87.32±20.54) , score of Palliative Care Quiz for Nursing was (7.89±3.44) , score of Organizational Climate Scale was (49.35±10.33) and the score of Prosocial Tendencies Measure scale was (96.71±18.25) . Qualities and abilities that college student volunteers should possess, access to hospice care, pro-social tendencies, and knowledge of palliative care were influential factors in nursing students' motivation to volunteerism for hospice care ( P<0.05) , which could explain 19.9% of the total variation. Conclusions:The motivations for hospice care volunteerism of undergraduate nursing students is in a medium and above level. Nursing educators should strengthen education and training related to nursing students' participation in hospice care, so that nursing students will participate in voluntary service activities through their own professional advantages, strengthen the voluntary team of hospice care, and promote the development of voluntary hospice care services.
4.Construction and application of mCherry red fluorescent protein fusion expression system in lactic acid bacteria.

In order to enrich the library of domestic research about new red fluorescent marker in lactic acid bacteria (LAB), we described a new fusion expression system in Lactobacillus plantarum WCFS1 based on the pSIP vector. This system contained red fluorescent protein mCherry as a marker and bile salt hydrolase gene (bsh) as a reporter gene. Moreover, in this study, four different promoters (PsppA, PldhL, P32 and PslpA) were used to regulate the expression of the fusion protein mCherry-BSH, completing the inducible and constitutive expression in lactic acid bacteria. The recombinant protein mCherry-BSH presented activity of red fluorescence and bile salt hydrolase (BSH). The successful construction of the fusion expression system in LAB using a red fluorescent protein mCherry provides favorable conditions for the distribution, intestinal colonization and survival rate of lactic acid bacteria, so as to reveal the function mechanism of its probiotic characteristics; and the system also could lay the foundation for researches on protein expression, cellular localization and properties identification of active protein in lactic acid bacteria.

5.Therapeutic effect of concurrent chemotherapy and intensity-modulated radiotherapy following induction chemotherapy in patients with locally advanced nasopharyngeal carcinoma

Objective To investigate the therapeutic effect and side effects of concurrent chemotherapy and intensity-modulated radiotherapy (IMRT) following induction chemotherapy (IC) in patients with locally advanced nasopharyngeal carcinoma (NPC).Methods From January 2005 to January 2009,62 cases of locally advanced NPC confirmed by pathological and cytological examination received IC with vinorelbine (25 mg/m2) plus cisplatin (25 mg/m2) for 2-4 cycles and then concurrent chemotherapy and IMRT.Conventional fractionated radiotherapy was adopted in IMRT.The radiotherapy for the nasopharyngeal region was performed a dose of 72-76 Gy/36-38 fractions,and additional 5-Gy gammaknife treatment was carried out in case of local tumor residue.Prophylactic irradiation to the cervical lymph nodes was performed at a dose of 50 Gy,and the dose was increased to 60-70 Gy in case of lymph node enlargement.Results The follow-up rate was 100%.The patients showed a response rate (RR) of 89% in the nasopharyngeal region and an RR of 90% in the cervical lymph nodes.The 1-,2-,and 3-year overall survival rates,disease-free survival rates,local relapse-free survival rates,and distant metastasis-free survival rates were 97%,92%,and 82%,94%,73%,and 65%,97%,89%,and 87%,and 97%,84%,and 77%,respectively.The incidence rates of grade 3-4 acute reactions were 37% for leucopenia,18% for thrombocytopenia,and 6% for mucositis.No grade 3-4 long-term temporomandibular joint injury and xerostomia were observed.Conclusions Concurrent chemotherapy and IMRT following IC with vinorelbine (25 mg/m2) plus cisplatin (25 mg/m2) have tolerable adverse effects and can achieve high survival rate in the patients with locally advanced NPC.
6.Effect of rehabilitation nursing on patients with vertebral body tumor after operation

ObjectiveTo observe the effect of rehabilitation nursing on patients with vertebral body tumor after operation.Methods12 patients with vertebral body tumor (5 cases of vertebral hemangiomas and 7 cases of vertebral malignant tumors) were operated with percutaneous vertebroplasty combined with psychological nursing,backing nursing,post-procedure function exercises.ResultsThe pain index and motive ability of patients was improved after treatment.ConclusionPercutanous vertebroplasty is effective in the treatment of vertebral benign and malignant tumors and rehabilitation nursing is also playing a important role.
