There are many factors to consider regarding the orthodontic treatment of an individual with cleft lip and palate in relation to the alveolar bone graft procedure. Some of these are: the sequence and timing of treatment, indications for pre-graft orthodontics, the appropriate appliances that should be used, and considerations in post-graft orthodontics. A review of some of the current concepts, management, and protocols are described.

2.Mandibular advancement with clear aligners and functional appliances in the treatment of skeletal ClassⅡmalocclusion: a systematic review and meta-analysis.

OBJECTIVES:
This study aimed to conduct a meta-analysis of the efficacy of mandibular advance clear alig-ners with traditional functional appliances as the control group.
METHODS:
PubMed, Web of Science, Embase, Cochrane Library, China Biomedical Abstracts Database, China Knowledge Network Database, Wanfang Database, and Weipu Database were used in this study. The two groups of researchers screened the literature and extracted data based on the inclusion and exclusion criteria established by PICOS entries, and used the ROBINS-I scale for quality evaluation. Revman 5.4 and Stata 17.0 software were used for meta-analysis.
RESULTS:
Nine clinical controlled trials were included in this study with a total sample size of 283 cases. No significant difference was found in SNA, SNB, ANB, Go-Pog, U1-SN, Overjet, and other aspects between the invisible group and the traditional group in the treatment of skeletal class Ⅱ ma-locclusion patients; there was a 0.90° difference in mandibular plane angle between the two groups; the growth of the mandibular ramus (Co-Go) in the traditional group was 1.10 mm more than that in the invisible group; the lip inclination of the lower teeth in the invisible group was better controlled, 1.94° less than that in the control group.
CONCLUSIONS
The invisible group can better control the lip inclination of the mandibular anterior teeth when guiding the mandible. Furthermore, the mandibular plane angle (MP-SN) can remain unchanged, but the growth of the mandibular ramus is not as good as the traditional group, and auxiliary measures should be taken to improve it in clinical practice.

Over the past decade, dramatic progress has been made in dental research areas involving laser therapy. The photobiomodulatory effect of laser light regulates the behavior of periodontal tissues and promotes damaged tissues to heal faster. Additionally, photobiomodulation therapy (PBMT), a non-invasive treatment, when applied in orthodontics, contributes to alleviating pain and reducing inflammation induced by orthodontic forces, along with improving tissue healing processes. Moreover, PBMT is attracting more attention as a possible approach to prevent the incidence of orthodontically induced inflammatory root resorption (OIIRR) during orthodontic treatment (OT) due to its capacity to modulate inflammatory, apoptotic, and anti-antioxidant responses. However, a systematic review revealed that PBMT has only a moderate grade of evidence-based effectiveness during orthodontic tooth movement (OTM) in relation to OIIRR, casting doubt on its beneficial effects. In PBMT-assisted orthodontics, delivering sufficient energy to the tooth root to achieve optimal stimulation is challenging due to the exponential attenuation of light penetration in periodontal tissues. The penetration of light to the root surface is another crucial unknown factor. Both the penetration depth and distribution of light in periodontal tissues are unknown. Thus, advanced approaches specific to orthodontic application of PBMT need to be established to overcome these limitations. This review explores possibilities for improving the application and effectiveness of PBMT during OTM. The aim was to investigate the current evidence related to the underlying mechanisms of action of PBMT on various periodontal tissues and cells, with a special focus on immunomodulatory effects during OTM.

4.Ability and inability of artificial intelligence in orthodontics.

With the development of artificial intelligence (AI) technology, it has a wide range of explorations in orthodontics. AI has greater application prospects in precise measurement, multidimensional diagnosis, treatment planning and efficacy prediction. At the same time, there are certain limitations in the application of AI, such as risks caused by individual variability, black box properties and unclear delineation of medical responsibilities. This paper summarized the history and current status of AI applications in orthodontics and discussed future development trends, to provide reference for clinical orthodontics.

5.‘FAKE BRACES’: A CONTENT ANALYSIS OF INSTAGRAM POSTS BY UNLICENSED PROVIDERS

“Fake braces” is a term used to describe fixed orthodontic appliances that are fitted by unlicensed providers. This trend has raised public health concerns in certain countries with marketing typically undertaken on social media. We aimed to investigate the frequency and nature of Instagram posts by fake braces providers in Malaysia. Initial screening of Instagram accounts was performed using relevant hashtags (#fakebraces, #cheapbraces). Public Instagram accounts that fulfilled the inclusion criteria were assigned a code with posts evaluated over a one-month period. Data were coded and analysed using thematic analysis. Frequency distribution was calculated using Microsoft Excel. Thirty-eight Instagram accounts were analysed. 2831 images/videos were posted, receiving 7165 likes with videos being viewed 21, 918 times. The majority of posts were related to fake braces services (36.7%), followed by non-relevant posts (29.4%) and other services (14.5%). Services were provided mainly at home (44.7%) or offered both home and door-to-door services (23.7%). Although the posts on promotions were less frequent (5.4%), the displayed prices were low (RM150 to RM500), offering additional packages and discounts. Unlicensed providers have a variety of content to engage with social media audiences. Efforts should be made to increase public awareness, directing them towards reliable information about orthodontic services.

6.Timely and appropriately performing early orthodontic treatment.

In recent years, the early orthodontic treatment of malocclusion has become a hot issue in the field of orthodontics. Some new phenomena and new problems that have emerged require in-depth analysis and discussion, and need to be given correct guidance and norms. Malocclusion refers to the deformities of teeth, jaws, and craniofacial bones caused by genetic and environmental factors during growth and development. Based on the accurate judgment of the etiology and diagnosis, choosing the right time and suitable indications, with the advantage of growth and development, the most suitable appliance can be selected in the early stage of malocclusion to fix malocclusion, and to effectively prevent and block the formation and development of malocclusion. This article will discuss how to timely and appropriately carry out the early prevention and treatment of malocclusion with six aspects, including methods of performing early orthodontic treatment, the basis and guarantee of early orthodontic treatment, correctly understanding early orthodontic treatment, the issues that should be paid attention to in carrying out early orthodontic treatment, actively advocating multidisciplinary cooperation during early orthodontic treatment and understanding face management in a serious way.

8.Construction and application of virtual patients in prosthodontics.

In recent years, with the rapid development of digital technology, the application of oral virtual patients in prosthodontics, orthodontics, oral and maxillofacial surgery, and other disciplines has gradually increased. Although the focus on oral virtual patients varies in different disciplines, the application of oral virtual patients in assisting the prediction of treatment effects and the formulation of treatment plans will have good prospects. The construction accuracy and presentation effects of oral virtual patients can be influenced by the source of three-dimensional (3D) image, and methods of registration. Based on the studies and clinical experiences of our team, researches of other teams, the source of 3D images, the construction methods, and the clinical applications of virtual patients in prosthodontics will be presented, so as to provide a reference for normalized application and development of oral virtual patients and to offer a future development direction of oral virtual patients.

In recent years, the application of artificial intelligence technology in the field of orthodontics has gradually increased, and deep learning, as a hot direction, has also been rapidly applied in the detection, evaluation, diagnosis, prediction and effect evaluation. At present, deep learning research has the advantages of high efficiency and accuracy, but it also has limitations such as weak interpretability and insufficient data volume. This paper reviewed the proposal and development of deep learning, the application in orthodontic diagnosis and treatment, as well as the limitations and countermeasures of the popularization, and prospect of the future research.

10.Social Media Use and Marketing within the Orthodontic Practice in Malaysia

ABSTRACT
This study investigated personal and professional social media use among orthodontists in Malaysia, to
determine marketing strategies and to identify potential determinants associated with their behaviours.
A cross sectional study using an online questionnaire distributed to members of the Malaysian
Association of Orthodontists (MAO). Data were analysed using SPSS software to derive descriptive
statistics and analysis of variance was applied to compare responses between age groups and working
sector. Responses to open ended questions were analysed using thematic analysis. A response rate of
41% was obtained (n = 72). Almost all respondents were social media users with Facebook being the
most common online platform. Majority relied on traditional methods of marketing such as good service
and image practice (94.4%), word of mouth (94.4%) and referrals (93.1%). Only 9.7% of respondents
had adopted social media marketing but 23.6% had future plans to adopt it as their marketing strategy.
Difference in knowledge of social media marketing was significantly different between age groups
(p = 0.024). Concerns over patient confidentiality (p = 0.016) and risk of breaching online professional
behaviour (p = 0.025), as reasons discouraging social media marketing was statistically significant
between work sector groups. Most orthodontists in Malaysia use social media for personal use but only
a minority incorporate it into their marketing strategies. Majority see its potential and predict its use will
increase in the future.
