There are various complicated wired connections in traditional endoscopic systems, which have disadvantages such as prolonging the preoperative setup time, restricting the movement of laparoscope, hindering the intraoperative manipulation, potentially contaminating the operation area and causing safety hazards.Our team has developed a wireless intelligent ultra-high-definition endoscopic system for the first time, which has been widely applied in many urological surgeries, such as tumors, stones, transurethral prostate enucleation, laparoscopic pyeloureteroplasty, laparoscopic renal cyst top decompression and so on.This essay reviews the applications and advantages of this system in urology, and forecasts its prospects.
2.Study on Compatibility and Efficacy of Blood-activating Herb Pairs Based on Graph Convolution Network

ObjectiveThis study aims to develop a prediction model for the compatibility of Chinese medicinal pairs based on Graph Convolutional Networks (GCN), named HC-GCN. The model integrates the properties of herbs with modern pharmacological mechanisms to predict pairs with specific therapeutic effects. It serves as a demonstration by applying the model to predict and validate the efficacy of blood-activating herb pairs. MethodsThe training dataset for herb pair prediction was constructed by systematically collecting commonly used herb pairs along with their characteristic data, including Qi, flavor, meridian tropism, and target genes. Integrating traditional characteristics of herb with modern bioinformatics, we developed an efficacy-oriented herb pair compatibility prediction model (HC-GCN) using graph convolutional networks (GCN). This model leverages machine learning to capture the complex relationships in herb pair compatibility, weighted by efficacy features. The performance of the HC-GCN model was evaluated using accuracy (ACC), recall, precision, F1 score (F1), and area under the ROC curve (AUC). Its predictive effectiveness was then compared to five other machine learning models: eXtreme Gradient Boosting (XGBoost), logistic regression (LR), Naive Bayes, K-nearest neighbor (KNN), and support vector machine (SVM). ResultsUsing herb pairs with blood-activating effects as a demonstration, a prediction model was constructed based on a foundational dataset of 46 blood-activating herb pairs, incorporating their Qi, flavor, meridian tropism, and target gene characteristics. The HC-GCN model outperforms other commonly used machine learning models in key performance metrics, including ACC, recall, precision, F1 score, and AUC. Through the predictive analysis of the HC-GCN model, 60 herb pairs with blood-activating effects were successfully identified. Among of these potential herb pairs, 44 include at least one herb with blood-activating effects. ConclusionIn this study, we established an efficacy-oriented compatibility prediction model for herb pairs based on GCN by integrating the unique characteristics of traditional herbs with modern pharmacological mechanisms. This model demonstrated high predictive performance, offering a novel approach for the intelligent screening and optimization of traditional Chinese medicine prescriptions, as well as their clinical applications.

As new evidence emerges, treatment strategies toward the functional cure of chronic hepatitis B are evolving. In 2019, a panel of national hepatologists published a Consensus Statement on the functional cure of chronic hepatitis B. Currently, an international group of hepatologists has been assembled to evaluate research since the publication of the original consensus, and to collaboratively develop the updated statements. The 2.0 Consensus was aimed to update the original consensus with the latest available studies, and provide a comprehensive overview of the current relevant scientific literatures regarding functional cure of hepatitis B, with a particular focus on issues that are not yet fully clarified. These cover the definition of functional cure of hepatitis B, its mechanisms and barriers, the effective strategies and treatment roadmap to achieve this endpoint, in particular new surrogate biomarkers used to measure efficacy or to predict response, and the appropriate approach to pursuing a functional cure in special populations, the development of emerging antivirals and immunomodulators with potential for curing hepatitis B. The statements are primarily intended to offer international guidance for clinicians in their practice to enhance the functional cure rate of chronic hepatitis B.

6.Reported incidence of adverse reactions in plateletpheresis donors in China: a meta-analysis

【Objective】 To analyze the overall situation and main influencing factors of adverse reactions of blood donation of apheresis platelet donors in China by systematic review, and to provide basis for preventing and controlling adverse reactions of platelet donation and improving the quality of blood donation service. 【Methods】 CNKI, VIP, Wanfang Data, Pubmed and Embase databases were searched to collect cross-section studies on adverse reactions of platelet donation in China. The observation period was ≥6 months, and the retrieval time was from 1998 to March 2023. The studies were independently screened by 2 evaluators according to the inclusion and exclusion criteria. Data extraction and literature quality evaluation were carried out, and meta analysis was performed by Stata 14.0 software. 【Results】 A total of 91 literature involving 585 769 apheresis platelet donors were included, and 9 102 of them had adverse reactions. Random effect model meta-analysis showed that the total incidence of adverse reactions of platelet donation was 2.65% [95% CI(2.04, 3.45), P<0.000 1]. Subgroup analysis showed that the incidence of platelet donation adverse reactions in the eastern, central and western regions were 2.63% [95% CI(1.94, 3.58)], 2.32% [95% CI(1.29, 4.16)] and 4.10% [95% CI(1.90, 8.83)], respectively, with regional differences (P<0. 000 1). The incidence of adverse reactions of different degrees was 3.62% [95% CI (2.69, 4.86)] for non-severe and 0.28% [95% CI(0.18, 0.45)] for severe. The incidence of adverse reactions of blood donation was also different among different apheresis instruments (P<0. 000 1): Amicus 2.73% [95% CI(1.23, 6.06)], CS3000 series 3.59% [95% CI(2.64, 4.89)], MCS+ series 3.42% [95% CI(2.23, 5.25)], Trima 4.86% [95% CI(3.08, 7.66)], other groups 1.71% [95%CI (0.15, 20.02)]. In addition, the incidence of adverse reactions was 4.80% [95% CI(3.05, 7.55)] in females and 1.39% [95% CI(0.90, 2.15)] in males, with the incidence in females higher than that in males (P<0.000 1). The adverse reactions of platelet donors under 30 years old and over 30 years old were 2.31% [95% CI(1.34, 3.98)] and 1.95% [95% CI(1.19, 3.20)], respectively, and there was no statistically significant difference (P>0.01). The incidence of adverse reactions of blood donation for first-time donors and repeat donors was 2.16% [(95%CI (1.18, 3.95)] and 1.12% [(95% CI(0.35, 3.58)], with the former higher than the latter(P<0.000 1). The incidence of adverse reactions for single and double platelet collection was 1.67% [95% CI(0.57, 4.89)] and 3.13% [95% CI(1.11, 8.86)] respectively, with no significant difference(P>0.01). 【Conclusion】 It′s suggested to unify the monitoring standards of adverse reactions of apheresis platelet donation, understand and master the factors that affect the adverse reactions of apheresis platelet donors, so as to improve the quality of apheresis platelet donation service and ensure the safety of donations.

A domestically produced self-expanding transcatheter aortic valve controllable bending delivery system(VitaFlow? Ⅲcontrollable bending retrievable delivery system)was first used to perform transcatheter aortic valve replacement(TAVR)in a symptomatic severe aortic valve stenosis patient with severe heart failure and high risk of surgery in China on September 22,2023.The patient successfully completed TAVR under general anesthesia,with good valve position and function after the operation.Before discharge and at one month of follow-up,the patient's symptoms and degree of heart failure were significantly improved.The follow-up results of this case showed that the VitaFlow? Ⅲ controllable bending retrievable delivery system for TAVR is safe and feasible,and future prospective,multicenter clinical trials are expected to evaluate its efficacy.
8.Laparoscopic ventral mesh rectopexy and anal sphincter plasty for complete rectal prolapse

Objective:To evaluate the safety and effectiveness of laparoscopic ventral mesh rectopexy (LVMR) +anal sphincter plasty for complete rectal prolapse.Methods:From Jan 1, 2018 to Dec 31, 2022, 45 patients with complete rectal prolapse diagnosed in Beijing Erlong Road Hospital received laparoscopic ventral mesh rectopexy +anal sphincter plasty were included in this study.Result:There was no mortality rate associated with LVMR in this study cohort. The follow-up period was 8-76 months, with a median follow-up period of 59 months. There were 2 cases of recurrence, with a recurrence rate of 4%. Patients with concomitant fecal incontinence symptoms had a preoperative Jorge Wexner fecal incontinence score of 13.8±2.0, and postoperative Jorge Wexner fecal incontinence scores of 7.5±1.5, 5.3±1.3, 4.3±1.1, 2.8±1.0, and 1.8±0.5 at 3, 6, 12, 36, and 60 months, respectively, all P<0.001. Patients with concomitant constipation had a preoperative Wexner constipation score of 15.7 ± 1.5, and a postoperative Wexner constipation score of 9.0±1.8, 6.8±1.5, 5.2±1.4, 4.1±0.7, 2.0±0.0 at 3, 6, 12, 36, and 60 months, respectively, all P<0.001. Conclusions:LVMR +anal Sphincter plasty is safe and effective for the treatment of complete rectal prolapse, and there are few complications related to biological patches. Anal sphincter plasty can effectively improve anal function.
9.Interpretation of Shanghai’s Guidelines for Healthy Industrial Park Construction

Various types of industrial parks in Shanghai play an important role in promoting industrial upgrading and technological progress, yet they are also fraught with occupational health hazards. To effectively promote workplace health and enable various industrial parks to play a positive role, the Shanghai Municipal Government has proposed accelerating the construction of healthy industrial parks. To meet the requirements for the scientific and standardized construction of healthy parks, the Shanghai Municipal Center for Disease Control and Prevention has compiled the Guidelines for Healthy Industrial Park Construction(Guidelines). Adhering to the overall principles of scientific, feasibility, advancement, and standardization, the Guidelines address three construction levels: industrial parks, employers, and employees. They set clear requirements for organizational management, healthy environments, health services, health activities, and occupational health in the workplaces, respectively. The Shanghai healthy industrial park evaluation form was provided as an informative appendix for the Guidelines. The Guidelines offer a scientific basis for standardizing and guiding the construction of healthy industrial parks, providing new solutions and technical support for urban occupational health management.
10.Efficacy-driving Mechanism of Danhong Injection for Stable Angina Pectoris Based on Composition-activity Relationship of Target Modules

ObjectiveTo explore the efficacy-driving mechanism of Danhong injection (DHI) in the treatment of stable angina pectoris (SAP) based on the composition-activity relationship of target modules and clarify the pharmacological effects of DHI. MethodAccording to the angina frequency (AF) in the Seattle Angina Questionnaire (SAQ) that was obtained in the previous clinical trial, the patients before and after DHI treatment were grouped based on efficacy. The transcriptomic data of the patients before treatment and in the best efficacy group 30 days post-treatment were selected as the data source, and then weighted gene co-expression network analysis (WGCNA) was employed to construct the co-expression network. Relevant modules in the network were identified and associated with clinical features. In addition, the On-modules (Z value below 0) were identified by Zsummary. The topological indicators such as density, centrality, and clustering coefficient were adopted to explore the dynamics of DHI efficacy at the network level and module level, respectively. In addition, the driver genes were screened by the personalized network control (PNC) algorithm. Finally, rat H9C2 cells were used to establish the model of hypoxia/reoxygenation (H/R), which was used to confirm the potential therapeutic target of DHI for SAP and provide a scientific basis for revealing the therapeutic mechanism of DHI. ResultWe identified 19 modules in the best efficacy group of DHI for SAP, and the comparison between day 0 and day 30 revealed 12 On-modules. The changes of network topological indicators at the network and module levels confirmed the correlation between the best efficacy of DHI treatment and topological dynamics. Finally, the driver genes, Klotho and fibroblast growth factor 22 (FGF22), in DHI treatment of SAP were verified by the H9C2 cell model of H/R. ConclusionBased on clinical transcriptome data, this study determined the composition-activity relationship of target modules of DHI for SAP, which provided a scientific basis for deciphering the efficacy-driven mechanism of DHI for SAP.
