Unusual presentation of localized gingival enlargement associated with a subjacent tumoural pathology is reported. The patient was a 55-year-old black male, whose chief complaint was a progressive gingival overgrowth for more than ten years, in the buccal area of the anterior left mandible. According to the clinical features and the radiological diagnosis of odontogenic keratocyst, a conservative surgery with enucleation and curettage was performed. Tissue submitted for histopathological analysis rendered the diagnosis of odontogenic myxoma. After 12-month of follow-up, no evidence of recurrence was found. Clinicians should be cautious when facing any gingival enlargement to avoid diagnostic pitfalls and to indicate the appropriate treatment.

Aggressive angiomyxoma is an uncommon mesenchymal myxoid tumor that is characterized by slow growth and frequent local recurrence. It is currently regarded as a nonmetastasizing tumor. We describe a case of recurrent aggressive angiomyxoma with invasion into the veins including the inferior vena cava and the right atrium and with pulmonary metastases. Our case, together with those unusual cases documented in previous reports, may lead to a reappraisal of the nature of aggressive angiomyxoma.

OBJECTIVETo study the clinicopathologic features, immunophenotype and differential diagnosis of plexiform angiomyxoid myofibroblastic tumor (PAMT) of the stomach.

METHODSThe clinical and pathologic findings of 3 cases of PAMT in the gastric antrum were retrospectively analyzed. Immunohistochemical study was carried out and the literature was reviewed.

RESULTSThe age of patients ranged from 31 to 47 years. The male-to-female ratio was 1:2. The clinical presentation included epigastric pain and distension. Endoscopically, the tumor mass protruded into the gastric cavity at the antrum and ranged from 4.5 cm to 8.0 cm in greatest dimension. One of the tumors studied was associated with surface ulceration. Histologically, the tumors were located in the gastric wall. They were composed of bland spindle cells and small vessels arranged in a plexiform or nodular pattern within a myxoid stroma. Immunohistochemical study showed that the spindle cells were consistently positive for smooth muscle actin and muscle-specific actin. There was focal staining for h-caldesmon, desmin in case 3 and focal positive for epithelial membrane antigen, CAM5.2 in case 1. Further, CD10 and progesterone receptor were positive in case 3.

CONCLUSIONSPAMT represents a rare novel mesenchymal tumor of the stomach, with a propensity of gastric antral involvement. The distinctive pathologic features help to differentiate this entity from other benign and malignant tumors.

Plexiform angiomyxoid myofibroblastic tumor (PAMT) is a recently described mesenchymal tumor of the stomach. We report the first case of PAMT in Korea. A 52-yr-old man underwent esophagogastroduodenoscopy due to dyspepsia for 2 yr. There was a submucosal mass with small mucosal ulceration in the gastric antrum. The tumor measured 3.5 x 2.3 cm in size and showed multinodular plexiform growth pattern of bland-looking spindle cells separated by an abundant myxoid or fibromyxoid matrix rich in small thin-walled blood vessels. The tumor cells were negative for CD117 (c-KIT), CD34 and S-100 protein, but diffusely positive for smooth muscle actin consistent with predominant myofibroblastic differentiation. The patient is doing well without recurrence or metastasis for 5 months after surgery. Although there have been limited follow-up data, PAMT is regarded as a benign gastric neoplasm with histological and immunohistochemical charateristics distinguished from gastrointestinal stromal tumor and other mesenchymal tumors of the stomach.

OBJECTIVETo study the clinicopathological features, immnophenotype and differential diagnosis of prepubertal-type vulva fibroma (PVF).

METHODClinical, radiological and pathological features of 2 cases of PVF were analyzed along with a literature review.

RESULTSBoth patients were females of 8.5 and 54 years of age, presented with recurrent unilateral mass at the labium major respectively. Grossly, cut surface of the tumor appeared as the gray fibrous tissue without any definited lump detected. Histologically, the ill-circumscribed lesion located predominantly in the deep dermis with an extension into the subcutaneous tissue. They had a low cell density consisting of scattered spindle shaped fibroblast-like cells and a large amount of collagen fiber matrix, nuclear atypia not seen and mitotic figures scanty. The tumor cells extended downward under the epithelium and infiltrated between the fat tissue, nerve fibers as well as the capillaries making a lesion looked somewhat like a harmatoma. Immunohistochemically, the spindle cells were uniformly positive for vimentin, weakly positive for CD34, and negative for alpha-smooth muscle actin (SMA), muscle specific actin (HHF35), desmin, h-caldesmon, CD99, S-100 protein, bcl-2, beta-catenin, estrogen and progesterone receptors.

CONCLUSIONSPVF is a benign mesenchymal lesion with a predilection of involving the vulva of prepubertal girls or adults in rare cases. PVF may represent an overgrowth of normal stromal tissue of vulva. Approximately one third of the tumors develop local recurrence due to incomplete excision, however, there is also occasionally spontaneous regression.
