Objective:To construct a health education program for patients with permanent enterostomy based on the information motivation behavioral skills model, and to test its effectiveness in improving patient readiness for discharge and reducing the incidence of early ostomy complications after discharge. Methods:From February 2021 to January 2022, convenience sampling was used to select 68 patients with low rectal cancer who were diagnosed by the Colorectal Surgery Department at the First Affiliated Hospital of Wenzhou Medical University and underwent abdominal perineal combined radical resection of rectal cancer (Miles) as research subjects. Randomized number series were generated using the digital table method, and the subjects were randomly divided into control group (33 cases) and intervention group (35 cases) using the envelope method. The control group was housed in the first ward, while the intervention group was housed in the second ward. The control group implemented the conventional health education, while the intervention group carried out the health education based on the information motivation behavioral skills model. The discharge readiness scores, ostomy self-efficacy and ostomy complications were compared between the two groups on the day of discharge and one month after discharge. Results:Patients in the intervention group had higher discharge readiness scores and higher discharge readiness levels than those in the control group, with statistical differences ( P<0.01) . The intervention group had higher levels of ostomy self-efficacy than the control group one month after discharge, with statistically significant difference ( P<0.01) . At the time of discharge for one month, the overall incidence of ostomy complications in the intervention group was 11.43% (4/35) , lower than 42.42% (14/33) in the control group, with a statistically significant difference ( P<0.01) . Conclusions:The health education program based on the information motivation behavioral skills model can effectively increase the readiness of patients with permanent enterostomy for discharge, reduce the incidence of complications of the ostomy and surrounding skin, improve the ostomy self-efficacy and promote patient rehabilitation.

Objective:To investigate the clinical and pathological features of pediatric NTRK-rearranged tumors. Methods:Four NTRK-rearranged soft tissue tumors and one renal tumor at Shanghai Children′s Medical Center, Shanghai Jiaotong University and Singapore KK Women′s and Children′s Hospital from January 2017 to September 2019 were identified. Pan-TRK immunohistochemistry, and the ALK and ETV6 gene break-apart fluorescence in situ hybridizations (FISH) were performed. NTRK gene rearrangement was detected using sequencing-based methods. Results:There were 3 males and 2 females in this study. The patients were between 3 months and 13 years of age. Histologically, the tumors were infiltrative spindle cell tumors with variable accompanying inflammatory cells. Immunohistochemistry showed positive reactivity for pan-TRK in all tumors, with nuclear staining for NTRK3 fusion, and cytoplasmic staining for NTRK1 fusion. The molecular testing revealed NTRK gene fusions (one each of TPM3-NTRK1, ETV6-NTRK3 and DCTN1-NTRK1, and two cases of LMNA-NTRK1). Two patients were receiving larotrectinib. The others were are well without disease, with follow-up durations of 9 to 29 months.Conclusions:NTRK-rearranged mesenchymal tumors from soft tissue sites and kidney are identified. A novel DCTN1-NTRK1 fusion is described. Pan-TRK immunohistochemistry is useful for diagnosis. NTRK-targeted therapy may be an option for unresectable, recurrent or metastatic cases.

Objective To investigate the relationship between vascular endothelial cell nitric oxide synthase (eNOS) gene polymorphism and the pathogenesis of avascular femoral head necrosis (ANFH).Methods The eNOS full-length CDS fragments, containing 894G or 894T separately, was subcloned into lentiviral expression vector, and then infect the human umbilical vein endothelial cells (HUVEC).The contents of nitric oxide NO and cyclic guanosine monophosphate (cGMP) in cell culture supernatant were detected in a time-dependent manner. The luciferase-labeled reporter plasmid pNF-κB-luc was co-transfected with the reference control plasmid pRL-TK into HUVEC cells infected with LV-eNOS-894G,LV-eNOS-894T,and control lentivirus (LV-NC) for 48 h.The luciferase activity of each group was detected. The expression of nuclear factor (NF)-κB protein and eNOS protein in HUVEC cells were detected by Western blot assay. The HUVEC cells in each group were co-cultured with hFOB1.19 cells, and the concentration of alkaline phosphatase (ALP) or osteocalcin (OCN) in the supernatant were detected at different time points. Results The contents of NO and cGMP in the cell culture supernatant of the full-length lentivirus expressing eNOS gene (containing 894G or 894T) were significantly higher than that in the empty cell group and the empty vector group, and the contents of NO and cGMP in the cell culture supernatant of the 894G group were significantly higher than that of 894T group (P < 0.01).Compared with blank cells, the expression levels of NF-κB/p65 protein and eNOS protein were significantly increased in cells expressing eNOS,and the expression of NF-κB/p65 protein in 894G group was significantly increased than 894T group, but there was no difference in eNOS protein expression between the 894G and 894T groups;each group of HUVEC cells were co-cultured with hFOB1.19 osteoblasts, and at each of the same time points, the concentrations of ALP and OCN in the cell culture supernatant expressing lentivirus were significantly higher than that in the empty cell group and the empty vector group, and the ALP and OCN concentrations in the cell culture supernatant of the 894G group were significantly higher than those in the 894T group. Conclusions The eNOS gene exon G894T mutation reduces the levels of nitric oxide and cGMP produced by vascular endothelial cells through the eNOS-NO pathway, affecting the expression levels of NF-κB/p65 protein and eNOS protein, and reducing osteoblast activity, and blood supply to blood vessels. It may be one of the pathogenic mechanisms leading to femoral head necrosis.

Objective To observe the effects of dual-task motor training on motor and balance function for stroke patients. Methods From January, 2016 to June, 2017, 30 patients with stroke were assigned randomly into control group (n = 15) and treatment group (n = 15). Both groups accepted routine physical therapy, the control group accepted anti-gravity treadmill training, while the treatment group accepted dual-task training with anti-gravity treadmill, for four weeks. They were measured with balance training and evaluation system, and assessed with Fugl-Meyer Assessment-lower extremity (FMA-L), Berg Balance Scale (BBS) and modified Barthel Index (MBI), before and after treatment. Results The range of swaying on X axial (Rx) and Y axial (Ry), as well as rectangle area (RecArea) decreased after treatment in the treatment group (t > 4.719, P < 0.001), while RecArea decreased in the control group (t = 5.069, P < 0.001). Ry and RecArea were less in the treatment group than in the control group (t > 2.288, P < 0.05). The scores of FMA-L, BBS and MBI improved after treatment in both groups (t > 7.316, P < 0.001), and improved more in the treatment group than in the control group (t > 2.322, P < 0.05). Conclusion The dual-task training via anti-gravity treadmill may improve motor and balance function in stroke patients, and promote the activities of daily living.

Objective To identify primary gout biomarkers. Methods Isobaric tags for relative and absolute quantitation (iTRAQ) technique combined with liquid chromatography-tandem mass spectrometry (LC-MS/MS) was used to screen differentially expressed proteins, and to identify potential biomarkers by analysis of the biological process, cellular components, molecular functions, KEGG pathways and protein-protein interactions. Difference between two groups were measured byt test. Results We identified 95 differentially expressed proteins (50 up-regulated proteins and 45 down-regulated proteins, respectively), and 20 significant KEGG pathways. Among them, glyceraldehyde-3-phosphate dehydrogenase (GAPDH), α-enolase (ENOA), phosphoglycerate kinase (PGK1), glucose-6-phosphate isomerase (G6PI) and moesin might play a role in the pathogenesis of primary gout. Conclusion iTRAQ technology can detect differentially expressed proteins from proteome, provides a strong theoretical basis for the study of biomarkers and evidence for the mechanisms in primary gout. However, further studies are needed.
