1.Clinicopathological and prognostic characteristics of intestinal inflammatory myofibroblastic tumor in middle-aged and elderly patients

Objective To investigate the clinicopathological and prognostic characteristics of intestinal inflammatory myofibroblastic tumours(IMT)in middle-aged and elderly patients.Methods The clinical,pathologi-cal morphology,immunophenotype and follow-up results of 5 cases of intestinal IMT in middle-aged and elderly patients were retrospectively analyzed.Results 4 cases of IMT occurred in the right half colon and 1 in the ileum.Most patients(3/5)had a history of intestinal injury,starting the digestive tract symptoms and increased leukocytes.The tumor tissue was composed of fusiform myofibroblasts and fibroblasts arranged in storiform pattern,with an infiltrative growth pattern,accompanied by a large number of lymphocytes and plasma cells infiltration,collagen formation and myxedema.One case was atypically large and deformed.Immunophenotype:vimentin(5cases),SMA(5 cases),desmin(3 cases),ALK(3 cases),CK(2 cases)were positive.Caldesmon,CD34,β-catenin,MC,CD117,DOG1,S-100,BCL-2,CD99,CD68 were negative,and Ki-67 proliferation index was 1.28%to 10.01%.All the 5 cases underwent complete tumor resection and were followed up for 48.5 to 133 months.Among them,1 patient aged 83 was considered to have tumor recurrence 27 months after surgery.The other patient survived 122 months without tumor and died of other causes.All the others survived without tumor and were in good condition.Conclusion(1)Intestinal IMT in the middle-aged and elderly people in this group was more common in the right half colon,and most of them had a history of intestinal injury,first gastrointestinal symptoms and elevated white blood cells;(2)Vimentin and SMA were positive at the same time,and ALK was more positive;(3)4/5 patients had good surgical resection,and 1/5 patients could relapse 2~3 years after surgery;old age,ALK-positive,Ki67 up to 10%,atypia may be an important risk factor for intestinal IMT recurrence in the elderly,of which ALK-positive patients may have a recurrence risk of 1/3.
2.Regularity and mechanism of traditional Chinese medicine compound prescriptions in the treatment of primary osteoporosis

BACKGROUND:Traditional Chinese medicine compound prescription has a long history in the treatment of primary osteoporosis,and the curative effect is definite,but the medication rule and mechanism are not clear. OBJECTIVE:Using the methodology of data mining and network pharmacology,to explore and verify the law of drug use and molecular mechanism of modern traditional Chinese medicine in the treatment of primary osteoporosis. METHODS:The relevant documents included in CNKI,WanFang,VIP and PubMed were used as data sources,and the relevant data were statistically counted and extracted by Microsoft EXCEL2019,IBMSPSS25.0 and other software.The high-frequency drugs obtained from the data statistics were analyzed by association rules analysis and cluster analysis,and the core drug combination of traditional Chinese medicine compound prescription in the treatment of primary osteoporosis was obtained by combining the two results.The therapeutic mechanism of this combination was explained by network pharmacology and verified by molecular docking. RESULTS AND CONCLUSION:Finally,151 articles were included and 207 prescriptions were selected,involving 285 flavors of Chinese herbs.(1)Ten groups of important drug combinations were obtained through the above two analyses,among which the core drug combination with the highest confidence and improvement was"Drynaria-Eucommia-Angelica."The key components of the combination in the treatment of primary osteoporosis were quercetin,kaempferol,naringenin and so on.The core targets were SRC proto-oncogene,phosphoinositide-3-Kinase regulatory subunit 1 and RELA proto-oncogene.The main pathways were cancer signaling pathway,JAK-STAT signaling pathway,VEGF signaling pathway,and NF-κB signaling pathway.(2)The key active components were docked with the core targets,and the two showed a good combination.To conclude,Chinese herbal compound therapy in the treatment of primary osteoporosis can use a variety of active components to exert its efficacy through multiple signal pathways and acting on multiple targets,which can provide a theoretical basis for the research and development of new drugs for the follow-up treatment of primary osteoporosis.
3.Research progress on community health management models for elderly hypertensive patients

Objective Summarize domestic and international community health management models and characteristics for elderly hypertensive patients,providing references for the development of community health management in China.Methods Employ literature research to retrieve relevant documents on community health management models for elderly hypertensive pa-tients,and analyze the characteristics and applicability of different models through summarization and comparative analysis.Results In China,community health management models for elderly hypertensive patients include family doctor contracting serv-ices,Hospital-Community-Home health management model,traditional Chinese medicine health management model,"Internet+"health management model,PDCA cycle model,PRECEDE-PROCEED model,and comprehensive community management model.Foreign studies can be categorized into self-management model,Health Rise model,Community Health2(CH2)model,and community pharmacy management model.Conclusion Conducting hypertension health management for elderly patients at the community level is effective,serving as a widely applicable strategy for chronic disease prevention and control.Continued ex-ploration of the scientific and effective aspects of different management models,improving the efficiency and effectiveness of com-munity health management,can contribute more evidence for the formulation of scientific and effective strategies for chronic dis-ease prevention and control.
4.Effects of health education based on reinforcement theory combined with empowerment theory in patients with chronic obstructive pulmonary disease

Objective:To explore the effects of health education based on reinforcement theory combined with empowerment theory in patients with chronic obstructive pulmonary disease (COPD) .Methods:From January 2018 to January 2021, convenience sampling was used to select 80 COPD patients admitted to the Respiratory Department of Lishui Hospital of Traditional Chinese Medicine as the subject. Patients were randomly divided into a control group and an observation group with 40 cases each in a 1∶1 ratio using the random number table. The control group received routine health education and psychological intervention, while the observation group was treated with health education based on reinforcement theory combined with empowerment theory. The pulmonary rehabilitation compliance, Short Form of the Profile of Mood States (POMS-SF) and Health-Promoting Lifestyle Profile (HPLP) scores before and after intervention were compared between two groups.Results:After intervention, the total score and all dimension scores of pulmonary rehabilitation compliance in both groups were higher than those before intervention, and the observation group scored higher than the control group, with statistically significant differences ( P<0.05). After intervention, the energy-vitality scores of the two groups were higher than those before intervention, the observation group scored higher than the control group, and the scores of other dimensions were lower than those before intervention, the observation group scored lower than the control group, and the differences were statistically significant ( P<0.05). After intervention, the scores of HPLP in all dimensions in both groups were higher than those before intervention, and the observation group scored higher than the control group, with statistically significant differences ( P<0.05) . Conclusions:Health education based on reinforcement theory combined with empowerment theory for COPD patients can enhance their rehabilitation compliance, effectively improve their mental state, and promote the development of healthy behaviors.
5.Drug use patterns in the treatment of lung cancer based on data mining

Objective To explore drug use patterns in treatment of lung cancer based on traditional Chinese medicine inheritance assistance platform.Methods A total of 200 effective prescriptions for the treatment of lung cancer patients in the outpatient department of Lishui Hospital of Traditional Chinese Medicine Affiliated to Zhejiang Chinese Medical University from January 2022 to September 2023 were collected and input into the traditional Chinese medicine inheritance assistance platform to conduct association rules and cluster analysis of drugs.Results A total of 200 effective prescriptions were included,involving 146 traditional Chinese medicines.The medicinal properties were mainly cold,warm and flat,and the medicinal flavours were mainly sweet,bitter and pungent,which mainly belong to lung,spleen,stomach and heart meridian.The most frequently used Chinese medicines were Radix Codonopsis Lanceolatae,Radix et Rhizoma Glycyrrhizae,Radix et Rhizoma Salviae Miltiorrhizae,Rhizoma Pinelliae,Actinidia valvata Dunn,Fritillariae Thunbergii Bulbus,etc.27 groups of core drug combinations were obtained,and 5 new prescriptions were obtained.Conclusion Clinical diagnosis and treatment of lung cancer is closely related to the core pathogenesis of Yin deficiency and stasis toxicity,and at the same time,it takes care of the acquired spleen and stomach,mind-body harmony,reflecting the academic thought of traditional Chinese medicine tumor,which reinforce healthy qi to anti-cancer and the holistic view of traditional Chinese medicine.
6.Epidemiological status and clinical characteristics of 225 cases of non-tuberculous mycobacterial pulmonary disease in Hainan Province

Abstract: Objective To explore the epidemiological and clinical characteristics of non-tuberculous mycobacterial(NTM) pulmonary disease in Hainan in recent years, and to provide data support for the prevention and treatment of NTM pulmonary disease. Methods Medical records of patients diagnosed with NTM pulmonary disease who treated at the Second Affiliated Hospital of Hainan Medical University (Hainan Provincial Tuberculosis Hospital) from January 2018 to December 2022 were collected. The demographics, regional distribution, temporal distribution, distribution of mycobacterial species, clinical symptoms, and radiological imaging changes of these patients were analyzed. Results A total of 225 confirmed cases of NMT pulmonary disease were collected in this study, with 133(59.1%) female patients outnumbering 92(40.9%) male patients. The disease predominantly affected people over 50 years old, with 192 cases (85.3%), and the major onset age range was 61-<81 years old, with a median age of 63 and an average age of 62. Farmers comprised the majority of the patients, with 112 cases (49.8%). More patients were from the western region (90 cases, 40.0%) than from the central region (76 cases, 33.8%), followed by the eastern region (59 cases, 26.2%). A total of 10 strains were detected from 225 samples, with the most common strains being Mycobacterium intracellulare (92 cases, 40.9%), Mycobacterium chelonae/abscessus(80 cases, 35.6%), and Mycobacterium avium (15 cases, 6.7%), followed by Mycobacterium kansasii (5 cases, 2.2%), Mycobacterium fortuitum (4 cases, 1.8%), and Mycobacterium parascrofulaceum (3 cases, 1.3%). There were 10 cases (4.5%) of mixed infections, including Mycobacterium avium and Mycobacterium intracellulare (8 cases, 3.6%), Mycobacterium intracellulare and Mycobacterium chelonae/abscessus (1 case, 0.4%), and Mycobacterium fortuitum and Mycobacterium chelonae/abscessus (1 case, 0.4%). The primary clinical manifestations of NTM pulmonary disease included cough and sputum (219 cases, 97.3%), hemoptysis (92 cases, 40.9%), fever (61 cases, 27.1%), dyspnea (57 cases, 25.3%), night sweats (52 cases, 23.1%), weight loss (50 cases, 22.2%), and thoracodynia (35 cases, 15.6%). There was no significant difference in symptoms between male and female patients. Pleural thickening (188 cases, 83.6%) and bronchiectasis (151 cases, 67.1%) were the most common imaging changes in NTM pulmonary disease, followed by cavities (93 cases, 41.3%), emphysema (41 cases, 18.2%), and lung damage (28 cases, 12.4%). Male patients were more likely to have lung damage and emphysema, while female patients were more likely to have bronchiectasis. Conclusions The distribution of NTM species is diverse in Hainan area, with Mycobacterium intracellulare, Mycobacterium chelonae/abscessus, and Mycobacterium avium are dominant species. NTM pulmonary disease and pulmonary tuberculosis have similar clinical features, which requires attention for differentiation in clinical practice.
7.Study on the effects and mechanism of luteolin on osteogenic repair of bone defects

OBJECTIVE To investigate the effects and mechanism of luteolin on osteogenic repair of bone defects. METHODS The targets and potential pathways of luteolin in the treatment of bone defects were screened by network pharmacology method, and then the top 2 targets were selected by Hub gene screening for molecular docking verification, with binding energy as the evaluation standard. In vitro experiments were conducted on rat bone mesenchymal stem cells (BMSC) and rat umbilical vein endothelial cells (RUVEC). Phenotypic validation was performed using alkaline phosphatase staining, alizarin red S staining, and in vitro angiogenesis experiments. Western blot assay was used to detect the protein expressions of phosphatidylinositol 3 kinase (PI3K) and protein kinase 1 (Akt1), so as to validate the mechanism of luteolin on osteogenic differentiation of BMSC and angiogenesis of RUVEC in vitro. RESULTS The results of network pharmacology showed that the effects of luteolin on vascular formation and bone repair in bone defects were mainly related to Akt1, SRC, estrogen receptor 1, epidermal growth factor receptor, cyclooxygenase 2, matrix metalloproteinase 9 targets, and were closely related to PI3K-Akt signaling pathway. The results of molecular docking showed that luteolin binding to Akt1 and SRC proteins was stable. The results of in vitro experiments showed that luteolin could significantly improve the expressions and activities of alkaline phosphatase in BMSC, increased the number of calcium salt deposits and calcified nodules, and promoted calcification of BMSC. Compared with luteolin 0 μmol/L group, the angiogenesis ability of RUVEC was enhanced significantly in luteolin 1, 10 μmol/L groups, the length of blood vessels and the protein expressions of PI3K and Akt1 were significantly increased (P<0.05 or P<0.01); the higherthe concentration, the better the effect. CONCLUSIONS Luteolin may play a role in promoting angiogenesis and bone repair at the fracture site by activating PI3K/Akt signaling pathway and promoting the protein expressions of PI3K and Akt1.
8.Implementation effect and thought of the basic essential surgical training course of laparoscopic skills

Objective:To analyze and summarize the implementation effect of basic essential surgical training (BEST) course of laparoscopic skills over the past 10 years and the practical experience in updating course content and models.Methods:The pre-class assessment questionnaires, basic laparoscopic operation assessment results, and post-class assessment questionnaires of the students who participated in the BEST course of laparoscopic skills were collected. According to the period of the course construction, the students were divided into two groups, namely students who used the course of single training system in the early stage (traditional group) and students who used the course integrating a variety of training systems after the course model was updated in the later stage (test group). The two groups were compared for the scores of track circle moving, tunnel crossing, and high and low columns, as well as their subjective evaluation of course setting and implementation effect. The t-test, Wilcoxon test, or chi-square test was conducted according to the data type using SPSS 13.0. Results:The time for 150 traditional group students to complete track circle moving, tunnel crossing, and high and low columns was 1.08 min (0.81 min, 1.60 min), 2.20 min (1.60 min, 3.27 min), and 4.86 min (3.28 min, 6.36 min), respectively, while the time for 75 test group students to complete the three operations was 1.27 min (0.87 min, 1.83 min), 2.57 min (1.58 min, 4.07 min), and 4.35 min (2.90 min, 6.42 min), respectively, with no significant difference between the two groups ( P>0.05). In terms of students' subjective evaluation of the course, a higher percentage of the test group students were satisfied with classroom environment, teaching method arrangement, training equipment, training opportunities, helping clinical work, and meeting pre-class expectations than those in the traditional group. Conclusion:The constantly updated BEST course can ensure the training quality of trainees and obtain their higher satisfaction. The benefits of this course in clinical practice can be further verified through long-term follow-up of these trainees.
9.Pharmaceutical practice of clinical pharmacist participation in a patient with Epstein-Barr virus encephalitis

OBJECTIVE To investigate the role of clinical pharmacists in the treatment of a patient with Epstein-Barr (EB) virus encephalitis. METHODS Clinical pharmacist participated in drug diagnosis and therapy for a patient with EB virus encephalitis. According to the physiological characteristics of the disease and the pharmacokinetic-pharmacodynamic characteristics of antibiotics, clinical pharmacists suggested that the dose should be adjusted as ceftriaxone 2 g, q12 h+meropenem 2 g, q8 h. Based on the uncontrolled infection of the patient, pharmacists suggested that ceftriaxone should be stopped and vancomycin 1 million U and q12 h should be used as alternative therapy. According to the results of etiology, pharmacists suggested that acyclovir should be discontinued and replaced with ganciclovir 5 mg/kg, q12 h. The electrolyte disturbance of the patient may be adverse drug reactions caused by Mannitol injection, it was recommended to stop the drug. RESULTS The clinician followed the advice of the clinical pharmacists. After treatment, the patient improved and was discharged. CONCLUSIONS Clinical pharmacists can carry out pharmaceutical care for patients with EB virus encephalitis, assist physicians in optimizing the treatment plan of patients, and ensure the effectiveness and safety of drug treatment.
10.The research status and prospects of microRNA-glial regulatory network in radiation-induced brain injury

Radiation-induced brain injury (RBI) is the most serious complication of head and neck tumor after radiotherapy. The pathogenesis of RBI is complicated, and the clinical course is irreversible, while no effective treatment available. The activation of glial cells is one of the main theories of RBI, and the prevention and treatment of RBI by targeting glial cells is the focus of current research. As a post-transcriptional regulatory factor, microRNA (miRNA) has been confirmed to be involved in regulatingglial cell radiosensitivity, inflammation type transformation, autophagy, exosomatic, long non-coding RNA (lncRNA), circular RNA (circRNA) and other related pathways, thereby mediating the occurrence and development of cascade reaction of inflammatory injury and neurological function repair of central nervous system (CNS) disease. Therefore, the establishment of miRNA - glial regulatory network may provide a new strategy for the prevention and treatment of RBI.
