1.A retrospective cohort study on the impact of hemoglobin A1c variability on the incidence of malignant tumors in patients with type 2 diabetes mellitus in Minhang District, Shanghai

		                        			Objective  To investigate the effect of hemoglobin A1c variability on the incidence of malignant tumors in type 2 diabetes mellitus (T2DM) patients, and to provide a reference for the prevention and treatment of diabetes.  Methods  A retrospective cohort study was used to integrate Shanghai malignant tumor registration information and T2DM patient management information of Minhang Districts in Shanghai. A record linkage was carried out using ID numbers for patients who had been diagnosed through the Cancer Registry System. Average Real Variability(ARV)was used to assess the variability for hemoglobin A1c results, with no less than 3 measurements and no more than 1 follow-up visit per year. Cox proportional hazards regression models and Restricted Cubic Splines (RCS) were used for statistical analysis. ARV was grouped using quartile classification, represented by Q1, Q2, Q3, and Q4, respectively. Cox regression was performed using unadjusted model I and model II adjusted for gender, age, and group.  Results  A total of 2 762 research subjects were included in the present study, with 45.84% males and 54.16% females. The average age was (63.58±10.11) years, and an average follow-up time was (9.96±3.66) years. Malignant tumor incidence rate was 345.45/100 000 person years. There was statistical significance among different ARV groups (P=0.01). COX regression analysis of model I and model II showed that compared with Q1, the risk of Q4 group was significantly increased, with HRs of 2.72 (1.56-4.74) and 2.68 (1.53-4.68), respectively. The RCS graph's analysis showed that except for the ≥65-year-old group, the risk of tumor incidence gradually increased with the increase of ARV coefficient.  Conclusion  The variability of hemoglobin A1c is positively associated with the risk of occurrence of malignant tumors in type 2 diabetes patients. 3.Genomic characteristics of Streptococcus pyogenes isolated from children with respiratory tract infections in a tertiary hospital in Jinshan District of Shanghai, 2013‒2024

		                        			ObjectiveTo analyze the genomic characteristics of Streptococcus pyogenes (GAS) isolated from children with respiratory tract infections in a tertiary hospital in Jinshan District of Shanghai during 2013‒2024, to compare the changes in trend for genomic characteristics before and after 2000, and to provide scientific data for the prevention and control of GAS infections. MethodsGAS strains isolated from children with respiratory tract infections in this hospital were collected from 2013 to 2024. Antimicrobial susceptibility of the isolated strains to 12 antibiotics, including penicillin, cefotaxime, cefepime, linezolid, vancomycin, meropenem, chloramphenicol, ofloxacin, levofloxacin, erythromycin, clindamycin, and tetracycline, was determined using broth microdilution plate method. Besides, whole genome sequencing (WGS) was used to analyze multilocus sequence type (MLST), emm typing, carriage of superantigen genes, mobile genetic element (MGE), carriage of virulence gene, and genomic phylogenetic tree of the isolated strains. ResultsA total of 50 GAS strains were collected and identified from children with respiratory tract infections aged 4‒14 years old, and the resistance rates of those isolates to erythromycin, clindamycin, and tetracycline were 100.00%, 100.00%, and 86.00%, respectively. There were two emm types in the GAS isolates; the emm12 type accounted for 76.00% (38/50), corresponding to ST36 type, and the emm1 type accounted for 24.00% (12/50), corresponding to ST28, ST1274, and new-1 types. There was a statistically significant difference in the constitution of the MLST before and after 2020 (P=0.015). All the isolates carried the superantigen genes speC, speG, ssa, and smeZ. The predominant emm12 isolates belonged to the Clade Ⅱ, carrying the mobile elements ICE-emm12 (harboring erythromycin-resistance gene ermB and tetracycline-resistance gene tetM) and ΦHKU.vir (carrying virulence genes speC and ssa). The emm1 isolates carried the mobile elements ICE-HKU488 (harboring erythromycin-resistance gene ermB and tetracycline-resistance gene tetM) and ΦHKU488.vir (carrying virulence genes speC and ssa), and had close phylogenetical relationships with isolates from Hong Kong, China. No M1UK new clone strains were found. The ST1274 isolates of emm1 were newly discovered in 2020‒2024, and belonged to a separate phylogenetic clade. ConclusionGAS strains isolated from children with respiratory tract infections in a tertiary hospital in Jinshan District of Shanghai exhibit a high resistance to erythromycin, clindamycin, and tetracycline. It is recommended that the clinical treatments change to use other antimicrobial drugs, such as penicillin, third-generation cephalosporins, and fluoroquinolones. During 2020‒2024, a new ST1274 clone strain is discovered in emm1 GAS isolates, without M1UK new clone strains being found. It is essential to continuously concern locally prevalent GAS strains and perform early identification of MLST types to promptly monitor the internal changes of the bacterial population and potential prevalence of new clones. 
		                        		
		                        		
		                        		
		                        	
4.Impact of eye-use behaviors on the onset and progression of myopia among primary school students in Minhang district under the background of “double reduction”

		                        			 AIM: To observe related influence of eye-use behaviors on the onset and progression of myopia among primary school student in Minhang district, Shanghai under the background of “double reduction”, and evaluate the impact of eye-use habit and behavior on the myopia development in children and adolescents.METHODS:A stratified cluster sampling method was adopted to select study subjects for longitudinal observation. Random sampling was conducted among grade 1-4 students from three primary schools in Minhang District, Shanghai from September to December 2021 after the implementation of the “double reduction” policy, with a 2-year observation period. Annual vision screenings were performed to obtain uncorrected visual acuity, non-cycloplegic refraction data, and spherical equivalent(SE). Biometric measurements were taken to acquire axial length(AL)and corneal curvature(CR). Questionnaires were administered to investigate the eye-use behaviors of participants. Taking myopia ≤-0.50 D as the diagnostic criterion, myopia incidence were calculated. SE and AL changes were used to evaluate myopia progression. Logistic regression analysis was employed to identify risk factors for myopia, and analyze the relationship between eye-use behaviors and myopia development or progression.RESULTS:A total of 2 986 children complete a follow-up for 2 a. The baseline prevalence of poor vision was 32.74%, increasing to 38.92% and 43.73% at 1 and 2 a follow-up, respectively, demonstrating a yearly upward trend. Taking uncorrected visual acuity combined with non-cycloplegic refraction as clinical screening criteria for myopia, the incidence was 24.72%, 27.60%, and 29.64% at baseline, 1, and 2 a follow-ups, respectively. These showed no statistical significant difference compared to the diagnostic standard of cycloplegic refraction SE(23.98%, 27.03%, and 29.17%; all P>0.05). Logistic regression analysis found that daily outdoor and sleep duration were protective factors against myopia(OR=0.54, 0.84, all P<0.05), while age, parental myopia, duration of offline education, online education, electronic device usage, and poor visual posture were identified as risk factors(OR=2.72, 1.54, 1.72, 1.12, 1.47, 1.26, all P<0.001). Further analysis of myopia progression in baseline myopic children over 2 a revealed that age, parental myopia, baseline SE, duration of offline/online education, electronic device usage, and poor visual posture may contribute to rapid myopia progression.CONCLUSION:Under the background of “double reduction”, the eye-use behavior among primary school students in Minhang District, Shanghai has an important impact on the occurrence and development of myopia. 5. Analysis of the effect of fudosteine on induced sputum components in patients with stable neutrophil-dominated COPD

		                        			 AIM: To observe the effect of fudosteine on induced sputum cell components and lung function in patients with stable neutrophil-dominated COPD. METHODS: From October 2019 to October 2022, 53 patients with stable COPD were selected and divided into fudosteine group and placebo group. The placebo group was treated with routine treatment, and the fudosteine group was treated with fudosteine on the basis of routine treatment. The two groups were treated for 6 months. The clinical symptoms [Saint George's Respiratory Questionnaire (SGRQ), COPD Assessment Test (CAT) and Modified British Medical Research Council Dyspnea scale (MMRC), Breathlessness, Cough, and Sputum Scale (BCSS)], lung function index, induced sputum cytology analysis and other related examination results were recorded in detail before and after treatment. RESULTS: (1) Compared with the baseline, the forced expiratory volume in one second (FEV1), forced vital capacity (FVC), and the ratio of FEV1 to FVC (FEV1/FVC) of the two groups were improved after treatment, and the differences were statistically significant (P<0.05). However, after treatment, there was no significant difference in pulmonary function between the two groups except for the percentage of carbon monoxide diffusion in the predicted value (DLCO%pre) (DLCO%pre in the fudosteine group was higher than that in the placebo group). (2) After treatment, the total number of induced sputum cells and neutrophil counts in the fudosteine group were lower than those in the placebo group. Compared with the number of cells in each component at baseline, the total number of induced sputum cells and neutrophil count in the fudosteine group were significantly lower (P< 0.05). CONCLUSION: Fudosteine treatment in patients with stable neutrophil-dominated COPD can improve lung function, reduce the total number of induced sputum cells and the total number of neutrophils, thereby improving airway inflammation. 
		                        		
		                        		
		                        		
		                        	
6.Risk analysis of hypertension among community-dwellers at risk for high blood pressure in Minhang District of Shanghai

		                        			ObjectiveTo investigate the incidence of hypertension and its influencing factors in community-dwellers at risk for high blood pressure in Minhang District of Shanghai, and to provide scientific evidence for the community management. MethodsA retrospective cohort study was conducted using the electronic health records of community-dwellers at risk for high blood pressure in Minhang District, Shanghai from January 1, 2011 to December 31, 2017. The study end-point was the occurrence of hypertension,and the followup was finished in December 2021. A total of 17 265 community-dwellers at risk for high blood pressure were enrolled in our study. Log-rank test and Cox regression analysis were used to determine the influencing factors. ResultsAfter 6.04 years of follow-up, the hypertension incidence among community-dwellers at risk for high blood pressure in Minhang District of Shanghai was 25.5%. Family history of hypertension (HR=1.250, 95%CI: 1.168‒1.338), family history of stroke (HR=1.295, 95%CI: 1.080‒1.553), history of diabetes (HR=1.203, 95%CI: 1.076‒1.345), daily smoking (HR=1.187, 95%CI: 1.087‒1.296), overweight (HR=1.393, 95%CI:1.308‒1.484), obesity(HR=1.903, 95%CI: 1.719‒2.106), high values of normal blood pressure (HR=1.275, 95%CI: 1.195‒1.359) and advanced age (HR=1.033, 95%CI: 1.030‒1.036) were all risk factors. Emaciation (HR=0.649, 95%CI: 0.500‒0.840) was a protective factors. ConclusionBlood pressure monitoring should be strengthened for people elderly, with family history of hypertension, family history of stroke, diabetes or high values of normal blood pressure, so as to diagnose hypertension early. 8.Status quo of screen exposure and its determinants in 3- to 6-year-old children in Minhang district, Shanghai

		                        			【Objective】 To investigate the screen exposure status in 3- to 6-year-old children in Minhang district and to analyze its determinants, so as to provide a basis for scientific intervention of screen exposure in children. 【Methods】 A total of 1 035 children aged 3 to 6 years were selected from 30 classes in 10 kindergartens in Minhang district by multi-stage cluster sampling in October 2022. An online questionnaire survey was conducted by their parents. Screen exposure was difined as spending more than one hour on video products per day. Multivariate Logistic regression model was adopted to analyze the determinants of children screen exposure. 【Results】 Among 1 035 children, 730 experienced screen exposure (70.53%). The average time of daily screen exposure was (1.67±0.98)h. The daily screen exposure time of children aged 3, 4, 5 and 6 years at weekends was (1.77±1.13), (1.76±1.04), (1.98±1.10)h and(2.08±1.22)h, respectively, and the difference was statistically significant (F=3.98, P<0.01). Multivariate Logistic analysis showed that age group of 5 years old (OR=1.79, 95%CI:1.19 - 2.68), the caregivers using video products for 1 - 2hours/day (OR=2.61, 95%CI:1.90 - 3.60) and >2hours/day (OR=2.10, 95%CI:1.35 - 3.27) when accompanying children, parents supporting children using video products(OR=1.59, 95%CI:1.17 - 2.15), children using video products before bedtime(OR=1.94, 95%CI:1.32 - 2.86), and unsupervised use of video products for children(OR=1.94, 95%CI:1.36 - 2.77) were independent risk factors for children′s screen exposure(P<0.05). Father′s education level of bachelor(OR=0.61,95%CI:0.43 - 0.89), master and above(OR=0.49, 95%CI:0.34 - 0.73) was a protective factor for children′s screen exposure(P<0.05). 【Conclusions】 Attention should be paid to the problem of screen exposure of 3 -to 6 - year-old children. Targeted efforts should be made to popularize the knowledge of the harm of early screen exposure to children among caregivers, caregivers are advocated for the rule formulation of screen behavior, scientifically regulate children′s screen exposure behaviors, and increase parent-child communication, so as to reduce the adverse effects of screen exposure on children′s health.
		                        		
		                        		
		                        		
		                        	
9.Factors related to medication adherence in patients with hypertension based on cross-sectional study strategy

		                        			Objective  To evaluate the relationship between perceived risk of complications of hypertension and medication adherence among patients with known hypertension.  Methods  Data were collected using a structured administrative questionnaire that mainly included information on basic demographic information, smoking history, access to healthcare services, duration of antihypertensive treatment, number of concurrent medications, and their perceived risk of complications related to hypertension. Morisk Medication Adherence Scale (MMAS-8) was used to measure the patients' antihypertensive medication adherence, and multivariate logistic regression analysis was used to identify independent factors related to adherence.  Results  Nearly half (48.8%) of the participants had uncontrolled hypertension, and more than one fifth (22.3%) of the patients had high adherence to antihypertensive medication. Independent factors highly associated with antihypertensive medication adherence were higher education level (OR:1.71, 95% CI: 1.06-2.75), never smoking (OR: 1.62 , 95% CI: 1.06-2.46), easy access to health care (OR: 1.91, 95% CI : 1.10-3.35), lower average treatment duration (OR: 0.96, 95% CI: 0.92-0.99), and higher perceived risk of hypertension-related complications (OR: 2.34 , 95% CI: 1.52-3.60).
