1.Effects of electroacupuncture with"Zhi San Zhen"on Notch signaling pathway and synaptic plasticity in 5xFAD mice

BACKGROUND:Alzheimer's disease is a degenerative neurological disorder characterized primarily by cognitive impairment.Acupuncture is a kind of traditional Chinese medicine therapy for treating Alzheimer's disease,but its mechanism is not yet clear. OBJECTIVE:To observe the effects of electroacupuncture with"Zhi San Zhen"on the Notch signaling pathway,β-amyloid protein(Aβ)and synaptic plasticity in 5xFAD mice. METHODS:Sixteen male,6-month-old 5xFAD mice,SPF-grade,were randomly divided into the electroacupuncture with"Zhi San Zhen"group(electroacupuncture group)and the model group,with eight mice in each group.Eight SPF-grade,male,6-month-old C57BL/6 mice were used as the wild control(wild)group.The electroacupuncture group received electroacupuncture with"Zhi San Zhen"intervention,5 times a week for 4 consecutive weeks.The model group and the wild group did not receive electroacupuncture intervention.The Morris water maze was used to preliminarily assess their learning and memory abilities.Thioflavin S staining was performed to detect Aβ plaque deposition.Western blot and real-time quantitative polymerase chain reaction(RT-qPCR)were used to measure the expression levels of transmembrane receptor protein Notch-1,Notch 1 intracellular domain(NICD),hairy and enhancer of split 1(Hes 1),hairy and enhancer of split 5(Hes 5),synaptophysin(SYN),postsynaptic density protein-95(PSD-95),and Aβ. RESULTS AND CONCLUSION:Compared with the model group,the wild group and the electroacupuncture group showed shortened escape latency,increased platform crossing times,and longer target quadrant dwell time(P<0.05).Compared with the wild group,the model group had significantly increased deposition of Aβ plaques,while electroacupuncture with"Zhi San Zhen"inhibited the deposition of Aβ plaques in the hippocampus of 5xFAD mice(P<0.05).Compared with the wild group,the model group had decreased mRNA levels of SYN,PSD-95,Notch 1,NICD,Hes 1,and Hes 5 in the hippocampal tissue of mice,and increased mRNA levels of Aβ(P<0.05).Electroacupuncture with"Zhi San Zhen"increased the mRNA levels of SYN,PSD-95,Notch 1,NICD,Hes 1,and Hes 5 in the hippocampal tissue,and decreased the mRNA level of Aβ(P<0.05).Compared with the Wild group,the model group had decreased protein expression levels of SYN,PSD-95,Notch 1,NICD,Hes 1,and Hes 5 in the hippocampal tissue of mice,and increased protein expression levels of Aβ(P<0.05).Electroacupuncture with"Zhi San Zhen"upregulated the protein expression levels of SYN,PSD-95,Notch 1,NICD,Hes 1,and Hes 5,and inhibited the protein expression of Aβ(P<0.05).To conclude,electroacupuncture with"Zhi San Zhen"can improve the learning and memory abilities of 5xFAD mice,possibly by inhibiting the deposition of Aβ protein and activating the Notch signaling pathway in the hippocampus to enhance synaptic plasticity.
2.Establishment and Initial Application of YOLO-V5 Deep Learning Prediction Model for Accurate Identification of Drug Packaging in Outpatient and Emergency Pharmacies

Objective To develop an accurate deep learning prediction model of YOLO-V5 capable of accurately iden-tifying medication packaging boxes in outpatient and emergency pharmacies,aiming to assist pharmacists in achieving"zero dis-pensing error".Methods A total of 2 560 images of packaging boxes from 136 different drugs were collected and labeled to form the deep learning dataset.The dataset was split into training and validation sets at a ratio of 4∶1.YOLO-V5 deep-learning algorithm was employed for training the data using images from our dataset(train epochs:500,batch size:4,learning rate:0.01).The values of the precision(Pr)and mean average precision(mAP)were used as measures for model performance evaluation.Results The Pr of the four sub-models of YOLO-V5 in the training set all reached 1.00.The mAP_0.5 of YOLO-V5x was 0.95,which was higher than those of YOLO-V5s(0.94),YOLO-V5l(0.94),and YOLO-V5m(0.94).The mAP_0.5:0.95 of YOLO-V5l and YOLO-V5x were 0.85 which were higher than those of YOLO-V5s(0.84)and YOLO-V5m(0.84).Training time and model size were 82.56 hours and 166.00MB for YOLO-V5x which were the highest among the four models.The speed of detection in one im-age was 11ms for YOLO-V5s which was the fastest among the four models.Conclusion YOLO-V5 can accurately identify the packaging of drugs in outpatient and emergency pharmacies.Implementing an artificial-intelligence-assisted drug dispensation sys-tem is feasible for pharmacists to achieve"zero dispensing error".

The correspondence between prescription and syndrome is the advantage and characteristic of traditional Chinese medicine(TCM)treatment.However,the pathogenesis of clinical diseases is complex and the condition is changeable,and the clinical application is difficult to achieve the maximum effect under the existing cognition of the correspondence between prescription and syndrome.In this paper,the five categories of physiological characteristics,pathological characteristics,constitution,syndrome,and disease of the longitudinal classification of"TCM state"are introduced into the correspondence of prescription and syndrome.Under the vertical perspective of"TCM state",the theoretical connotation of the correspondence between prescription and syndrome is interpreted as"correspondence between prescription and state",namely correspondence of"prescription-physiological characteristics",correspondence of"prescription-pathological characteristics",correspondence of"prescription-constitution",correspondence of"prescription-syndrome",and correspondence of"prescription-disease".It is hoped to accurately grasp the corresponding connotation of the correspondence between prescription and syndrome,in order to deepen the clinical thinking mode of TCM.
4.Identification of in Vitro and in vivo Chemical Constituents of Ruyi Zhenbaowan Based on UHPLC-Q Exactive Orbitrap HRMS

ObjectiveTo identify the chemical constituents of Ruyi Zhenbaowan in vitro and in vivo. MethodThe chemical constituents of Ruyi Zhenbaowan were identified based on UHPLC-Q Exactive Orbitrap HRMS. A total of 12 male SD rats were randomized into two groups: control (pure water) and Ruyi Zhenbaowan (1.8 g·kg-1). The rats were administrated with the suspension of Ruyi Zhenbaowan or pure water by gavage. After 1.5 h, the plasma and cerebrospinal fluid were collected. Chromatographic separation was performed on a Waters ACQUITY UPLC BEH C18 column (2.1 mm × 150 mm, 1.7 μm) with a mixture of 0.1% formic acid aqueous solution (A) and acetonitrile (B) as the mobile phase. Gradient elution was carried out according to the procedure of 0~15 min,97%~80%A;15~30 min ,80%~60%A;30~40 min,60%~30%A;40~45 min,30%~5%A. The ion source was electrospray ionization, and scan range was m/z 100-1 500. The prototype components and the components in the plasma and cerebrospinal fluid were analyzed qualitatively by scanning in positive and negative ion modes and identified by comparison with the data in published literature and the information of standard substances. ResultA total of 126 chemical constituents were identified from the 80% methanol solution of Ruyi Zhenbaowan, and 14 and 7 prototype constituents were detected in the plasma and the cerebrospinal fluid, respectively. In addition, the fragmentation rules of apigenin, apigenin-7-O-glucuronide, galangin, liquiritin, piperine, glycyrrhizic acid, eugenol, gallic acid, and cholic acid were deduced. ConclusionThis study achieved rapid multicomponent characterization and identification of Ruyi Zhenbaowan in vitro and in vivo, providing theoretical support for exploring active substances and performing quality control.l.
5.Percutaneous mechanical thrombectomy for treating different traditional Chinese medicine syndrome type lower extremity arterial thromboses

Objective To observe the efficacy of percutaneous mechanical thrombectomy(PMT)for treating different traditional Chinese medicine(TCM)syndrome type lower extremity arterial thromboses.Methods Forty patients with lower extremity arterial thromboses who underwent PMT were retrospectively enrolled and divided into dampness-heat syndrome group(n=18)and blood stasis syndrome group(n=22)according to TCM syndrome types.The technical success rate,ankle-brachial index(ABI),Rutherford grade and vascular patency rate 12 months after PMT were compared between groups.Perioperative complications and adverse events during follow-up were recorded.Results The technical success rate of PMT in dampness-heat syndrome group and blood stasis syndrome group was 94.44%(17/18)and 100%(22/22),respectively.Twelve months after PMT,ABI was 0.45±0.11 and 0.52±0.14,and vascular patency rate was 94.44%(17/18)and 81.82%(18/22)in dampness-heat syndrome group and blood stasis syndrome group,respectively,both not significantly different between groups(all P>0.05).No significant difference of Rutherford grade before treatment was found between groups(P>0.05),while 12 months after PMT,Rutherford grade in dampness-heat syndrome group was higher than in blood stasis syndrome group(P<0.05).During perioperative period,false aneurysm of brachial artery occurred in 1 case in dampness-heat syndrome group,while osteofascial compartment syndrome and atrial fibrillation occurred each in 1 case in blood stasis syndrome group,both relieved after treatments.No serious adverse event such as amputation nor death occurred during follow-up.Conclusion PMT was effective and safe for treating different TCM syndrome type lower extremity arterial thromboses.The prognosis of patients with blood stasis syndrome type lower extremity arterial thromboses was better than that of those with dampness-heat syndrome.
6.Relationship between GTSE1 and Cell Cycle and Potential Regulatory Mechanisms in Lung Cancer Cells

The regulation of the cell cycle is essential for maintaining normal cellular function,especially in the development of diseases such as lung cancer.The cell cycle consists of four major phases(G1,S,G2 and M phases),which are characterized by a series of precise molecular events to ensure proper cell proliferation and division.In lung cancer cells,cell cycle dysregulation can lead to disordered proliferation and increased invasiveness of cancer cells.G2 and S-phase expressed 1(GTSE1)is a regulatory protein found in the cytoplasm of the cell,which plays a key role in the cell cycle distribution of a wide range of cancer cells and is involved in life processes such as cell proliferation and apoptosis.GTSE1 affects cell cycle progres-sion by interacting with cyclin-dependent kinase inhibitor 1A(p21)and maintaining the stability of p21,which in turn inhibits the activity of cyclin-dependent kinase 1/2(CDK1/2).In addition,GTSE1 is also involved in the regulation of tumor protein 53(p53)signaling pathway.With the assistance of mouse double minute 2 homolog(MDM2),GTSE1 is able to transport p53 from the nucleus to the cytoplasm and promote its ubiquitination and degradation,thus affecting cell cycle and cell death-related signaling pathways.This paper reviews the expression of GTSE1 in lung cancer cells and its effects on lung cancer,as well as its potential mechanisms involved in cell cycle regulation.
7.Repair methods and effects of refractory wounds in patients after spinal internal fixation operation

Objective:To explore the repair methods and effects of refractory wounds in patients after spinal internal fixation operation .Methods:The study was a retrospective observational study. From November 2020 to October 2023, 10 patients with refractory wounds after spinal internal fixation operation were admitted to the Department of Burns and Plastic Surgery of Changde Hospital of Xiangya School of Medicine of Central South University. They were 3 males and 7 females, aged 35 to 68 years. There were 6 cases of thoracolumbar tuberculosis, 3 cases of thoracolumbar fracture, and 1 case of recurrent sacrococcygeal chordoma with skin, soft tissue, and bone defects after radical resection. The wound areas after debridement were 6.0 cm×1.5 cm to 27.0 cm×6.5 cm. The wound repair operation was decided to perform in the primary stage or in the secondary stage according to the wound situation. Two patients with type Ⅰ wounds were treated with debridement, direct suture, and continuous irrigation and drainage with catheter after operation. Eight patients with type Ⅱ wounds were repaired with local flaps (including rotation flap with dermis-fat flap at the end), muscle flaps, or muscle flaps combined with local flaps. The flap sizes were 10.0 cm×5.0 cm to 27.0 cm×14.0 cm, and the sizes of muscle flap were 8.0 cm×5.0 cm×4.0 cm to 17.0 cm×9.5 cm×2.0 cm. The wounds in flap donor areas were sutured directly. The wound treatment methods of patients with type Ⅱ wounds were recorded. The wound healing was observed after operation. The infection and recurrence of wounds, the retention of internal fixation materials, and spinal motor function were observed during follow-up.Results:Among patients with type Ⅱ wounds, there were 3 cases applied with local flaps (including 1 case with rotation flap with dermis-fat flap at the end), 3 cases with muscle flaps (including 1 case with latissimus dorsi muscle flap and 2 cases with erector spinal muscle flaps), and 2 cases with muscle flaps (1 case with latissimus dorsi muscle flap and 1 case with erector spinal muscle flap) combined with local flaps. Only 1 case with secondary defects after radical surgery of sacrococcygeal chordoma had poor wound healing which healed after dressing change, and the wounds of the remaining 9 cases all healed well. During the follow-up of 4 to 18 months, no infection or recurrence of local wounds developed in 10 patients, the internal fixation materials were not loosening, and there was no significant limitation in spinal motor function.Conclusions:For refractory wounds after spinal internal fixation operation, according to the wound type of patients, debridement, suture, irrigation, and drainage in the primary stage, or transplantation of local flaps, muscle flaps, muscle flaps combined with local flaps are performed in the primary stage or in the secondary stage. These methods are proved to have reliable therapeutic effects, not only repairing the wounds, but also retaining the internal fixation materials.
8.Association Between Normal-weight Central Obesity With New-onset Cardiovascular Disease and All-cause Mortality

Objectives:To investigate the association between normal-weight central obesity with new-onset cardiovascular disease and all-cause mortality risk. Methods:A prospective cohort study was conducted,selecting a total of 93885 participants from the Kailuan Study who had their first physical examination in 2006-2007.According to waist circumference (central obesity:male waist circumference ≥90 cm,female waist circumference ≥85 cm;no central obesity:male waist circumference<90 cm,female waist circumference<85 cm) and body mass index (BMI,normal weight:18.5 kg/m2≤BMI<24.0 kg/m2;overweight/obesity:BMI ≥24.0 kg/m2),the participants were divided into 4 groups:normal weight no central obesity group (G1 group),normal weight central obesity group (G2 group),overweight/obesity no central obesity group (G3 group) and overweight/central obesity group (G4 group);Using the Kaplan-Meier method,the cumulative incidence of new-onset cardiovascular diseases (including hemorrhagic stroke,ischemic stroke and myocardial infarction) and all-cause mortality in different groups was calculated,and the Log-rank test was used for intergroup comparisons.Furthermore,the associations between the different groups and the risk of new-onset cardiovascular diseases and all-cause mortality were analyzed using the multivariate Cox proportional hazard regression model. Results:After a median follow-up of 14.97 (14.55,15.17) years,the cumulative incidence of new-onset cardiovascular diseases in G1 group,G2 group,G3 group and G4 group was 7.62%,10.84%,8.67%,12.91% respectively (log-rank P<0.05) and the cumulative incidence of all-cause mortality was 12.83%,19.72%,10.65%,16.33% respectively (log-rank P<0.01).After adjusting for confounding factors,Cox regression analysis showed that the HR (95%CI) of new-onset cardiovascular diseases in G2 group,G3 group and G4 group were 1.14 (1.04-1.25),1.07 (1.01-1.14),1.27 (1.21-1.34),respectively compared with G1 group (all P<0.05).The HR (95%CI) of all-cause mortality were 1.06 (1.00-1.14),0.90 (0.85-0.95),0.97 (0.93-1.01) compared with G1 group,and P values were 0.07,<0.01,0.15,respectively.The results of sensitivity analysis were consistent with the above major studies after excluding overweight/obesity and cancer participants during follow-up. Conclusions:Normal-weight central obesity increases the risk of new-onset cardiovascular diseases and all-cause mortality.
9.A Whole-Course Nursing Quality Evaluation System for Liver Transplantation in Children Based on Donabedian Theory

Objective To build a whole-course nursing quality evaluation system for liver transplanta-tion in children,so as to provide a basis for nursing quality evaluation and management.Methods With Donabedian's"structure-process-outcome"model as the theoretical framework,we employed literature analysis,Delphi method,and hierarchical analysis to determine the contents and weights of indexes in the whole-course nursing quality evaluation system for liver transplantation in children.Results The three rounds of survey based on questionnaires showed the questionnaire recovery rate of 100% ,the expert authority coefficients of 0.95,0.96,and 0.98,and the Kendall's coefficients of concordance of 0.165,0.209,and 0.220,respectively(all P<0.001).The established nursing quality evaluation system included 3 first-level indexes,15 second-level inde-xes,and 67 third-level indexes.Conclusion The whole-course nursing quality evaluation system for liver trans-plantation in children that was built in this study can provide a basis for the evaluation of the nursing quality.
10.Modified YOLO-V5 model for identifying inflammatory bowel disease on CT enterography

Objective To investigate the value of modified YOLO-V5 model for identifying inflammatory bowel disease(IBD)displayed on CT enterography(CTE).Methods Totally 192 patients with IBD(103 cases of Crohn disease[CD subgroup]and 89 cases of ulcerative colitis[UC subgroup])and 103 patients with clinically suspected IBD but CTE showed no abnormality(no abnormality subgroup)were retrospectively collected as study group,while 5 patients with CD and 3 with UC were collected as test group.CTE images with diseased intestinal tubes present as thickened intestinal wall or no abnormality intestinal tubes were selected as data set(n=3 511).CTE in study group were divided into training set(n=3 160,including 1 063 from CD subgroup,931 from UC subgroup and 1 166 from no abnormality subgroup)and verification set(n=351,including 118 from CD subgroup,103 from UC subgroup and 130 from no abnormality subgroup)at the ratio of 9∶1,while 25 CET images(17 from 5 cases of CD and 8 from 3 cases of UC)in test group were used as test set.Diseased tubes of CD,UC and no abnormality tubes were labeled.Then 5 sub-models,including YOLO-V5n,YOLO-V5s,YOLO-V5m,YOLO-V5l and YOLO-V5x were constructed and trained with modified YOLO-V5,and their efficacy were verified in test set.Precision(Pr),recall(Rc)and mean average precision(mAP)were used to evaluate the efficacy of each sub-model for identifying IBD lesions displayed on CTE.Results The complexity of the above 5 sub-models increased successively.YOLO-V5l and YOLO-V5x sub-model had better diagnostic efficacy,the overall Pr,Rc,mAP_0.5 and mAP_0.5.0.95 of the former for identifying IBD lesions in training and validation sets was 0.97,0.93,0.96 and 0.91,while of the latter was 0.97,0.95,0.96 and 0.92,respectively.In test set,the efficacy of YOLO-V5n sub-model for identifying IBD lesions was low,with mAP_0.5∶0.95 of 0.66 and AUC of 0.82,whereas mAP_0.5∶0.95 of YOLO-V5x sub-model for identifying CD was as high as 0.92,and of YOLO-V5l sub-model for identifying UC was as high as 0.91.Conclusion YOLO-V5l and YOLO-V5x sub-models based on modified YOLO-V5 could effectively identify IBD lesions displayed on CTE.
