Objective:To investigate the efficacy of arthroscopic anatomical reconstruction of the ligament with autologous semitendinosus tendon in the treatment of chronic lateral ankle instability (CLAI) complicated with multiple ligament laxity.Methods:A retrospective study was conducted to analyze the 34 patients with CLAI plus multiple ligament laxity who had been treated at Foot and Ankle Surgery Center, The Fourth Hospital of Wuhan from March 2014 to December 2021. They were 8 males and 26 females with an age of (32.2±5.6) years. The patients were divided into 2 groups based on their treatment methods. A reconstruction group of 20 cases were treated by arthroscopic reconstruction of the ligament with autologous semitendinosus tendon while a repair group of 14 cases treated by arthroscopic repair of the ligament with the modified Brostr?m procedure. The 2 groups were compared in terms of surgical time, and the American Orthopaedic Foot & Ankle Society (AOFAS) ankle-hindfoot score, visual analog scale (VAS) pain score, talar tilt (TT), anterior translation of the talus (ATT), and complications at the last follow-up.Results:The 2 groups were comparable because there were no statistically significant differences in the general data, AOFAS ankle-hindfoot score, VAS pain score, TT, or ATT before surgery between the 2 groups ( P > 0.05). The surgical time for the reconstruction group [(97.5±11.4) min] was significantly longer than that for the repair group [(53.6±10.7) min] ( P < 0.05). All the 34 patients were followed up for (35.4±3.5) months. The TT, ATT, AOFAS ankle-hindfoot score, and VAS pain score at the last follow-up were all significantly improved compared with the preoperative values in both groups ( P < 0.05). The AOFAS ankle-hindfoot score [(90.6±3.6) points], TT (6.0°±1.5°), and ATT [(3.6±1.4) mm] at the last follow-up in the reconstruction group were all significantly better than those in the repair group [(84.1±11.0) points, 8.6°±4.3°, and (6.6±4.1) mm] ( P < 0.05). There was no statistically significant difference in the VAS pain score between the 2 groups at the last follow-up ( P > 0.05). All incisions healed at one stage without such complications as nerve or vascular injury. CLAI recurrence occurred in 5 cases in the repair group, significant worse than that in the reconstruction group (no recurrence) ( P=0.015). Conclusion:In the treatment of CLAI complicated with multiple ligament laxity, arthroscopic anatomical reconstruction of the ligament with autologous semitendinosus tendon can effectively improve ankle function, enhance ankle stability, and reduce recurrence of the condition.

OBJECTIVE: To analyze the influence of noise measuring skills by continuous inter-laboratory comparison among occupational hygiene technical service organizations. METHODS: The results of 68, 55 and 50 occupational hygiene technical service organizations(reference organizations) participating in the inter-laboratory comparison of workplace noise measurement in 2014, 2017 and 2018 respectively were collected. The measurement operation and the calculation of equivalent sound level were evaluated according to the GBZ/T 189.8-2007 Measurement of Physical Agents in Workplace--Part 8: Noise.The measured results were analyzed by z score evaluation method. RESULTS: The results of noise measurement field operation of the reference organizations in the year 2014, 2017 and 2018 show that the accuracy rates of instrument setting was 89.7%, 98.2% and 100.0%, the accuracy rates of measuring position were 52.9%, 81.8% and 94.0%, the accuracy rates of sound level meter holding were 54.4%, 80.0% and 92.0%, the accuracy rates of microphone pointing were 98.5%, 98.2% and 94.0%, and the accuracy rates of noise type recognition were 83.8%, 92.7% and 84.0%, respectively. Among them, the accuracy of instrument setting, measurement position and sound level meter holding showed an increasing trend year by year(P<0.01). The pass rates of measurement results were 91.2%, 94.5% and 88.0% in 2014, 2017 and 2018 respectively. The pass rates of equivalent sound level calculation were 73.5%, 92.7% and 88.0%. The pass rates of comprehensive evaluation were 70.6%, 89.1% and 80.0% respectively. Among them, the pass rates of equivalent sound level calculation and comprehensive evaluation in 2017 were higher than that in 2014(P<0.017). There was no significant difference in the three-year comprehensive assessment total pass rate between private organizations and non-private organizations(84.0 % vs 75.5%, P>0.05). There was no significant difference in the total pass rate of three-year comprehensive evaluation between non-Pearl-River-Delta organizations and Pearl-River-Delta organizations(70.3% vs 81.6%, P>0.05). CONCLUSION: Continuous inter-laboratory comparison in occupational hygiene technical service organizations is beneficial to improve on-site noise measure capability.

Objective To investigate the effect of small interfering RNA(siRNA)-mediated inhibition of tumor necrosis factor receptor associated factor 6(TRAF6) gene on murine odontoblast-like cell line,MDPC-23 cell and the effect of TRAF6 on MDPC-23 cell proliferation.Methods The vectors expressing siRNA against TRAF6 were constructed and introduced into MDPC-23 cell with lipofectin,and the cell line with stable expression of siRNA of TRAF6 was obtained by G418 screening and colony culture.Reverse transcription-PCR(RT-PCR) and Western blotting were performed to detect the expression of TRAF6.The proliferation of transfected MDPC-23 cell was investigated through methabenzthiazuron(MTF)and flow cytometry(FCM) assay.Results The positive single colony was screened out,and was found to express siRNA against TRAF6 effectively because both TRAF6 mRNA and protein relative expression were significantly decreased in the experimental group(pSUPER-TRAF6siRNA:mRNA 0.163 ± 0.008,protein 0.215±0.006) compared with controls(pSUPER:mRNA 0.778±0.017,protein 0.964±0.007 (P ＜ 0.001).The A value of treated pSUPER-TRAF6siRNA cells (3 d:0.46±0.03,5 d:1.35±0.06) was increased compared with controls(P＜0.01).The result of proliferation index(PrI) was also increased compared with controls [pSUPER-TRAF6siRNA:(24.1 ± 2.2)％ ; pSUPER(11.2 ± 1.0)％ ; control(10.5 ± 0.7)％,P＜0.01].Conclusions The transcription and expression of TRAF6 gene were inhibited.The proliferation ability was increased in MDPC-23 cells by the constructed pSUPER-TRAF6siRNA vector.It may further influence the formation and repair of dentin,and may be involved in the regulation of normal tooth eruption and process of dentin repair after injury.

Objective To investigate the changes and clinical significance of serum total and high molecular weight adiponectin (HMW APN) levels in type 2 diabetic patients with non-alcoholic fatty liver disease (NAFLD).Methods Serum levels of total adiponectin and HMW APN were measured by ELISA,in 58 type 2 diabetic patients with nonalcoholic fatty liver disease (T2DM with NAFLD group),59 type 2 diabetic patients without nonalcoholic fatty liver disease (T2DM group),and 55 control subjects with normal glucose tolerance and without nonalcoholic fatty liver disease (NC group).Results (1) Alanine transaminase and total cholesterol levels were significantly higher in T2DM with NAFLD group than those in NC group(P＜0.01),while body mass index (BMI),aspartate aminotransferase,triglyceride (TG),and fasting insulin were also significantly increased (P＜0.05 or P＜ 0.01),and high density lipoprotein-cholesterol (HDL-C) were significantly decreased compared with those in NC and T2DM groups (P＜0.01).(2) Total adiponectin,HMW APN,and the ratio of HMW APN to total adiponectin in T2DM group were lower than those in NC group.Total adiponectin and HMW APN levels in T2DM with NAFLD group were significantly lower than those in T2DM group(P＜0.01).(3) Regression analysis showed that homeostasis model assessment insulin resistance index (HOMA-IR),TG,and HDL-C levels were independent predicting factors for total adiponectin and HMW APN levels.TG and BMI were independent risk factors for T2DM complicated with NAFLD,while total adiponectin and HMW APN levels were the protective ones (OR =0.701,0.489,respectively).Conclusions Hypoadiponectinemia may partially play an important role in the development and progression of NAFLD in T2DM.

OBJECTIVETo investigate more deeply the function and mechanism of DSPP during tooth development.

METHODSExplants of tooth germs from embryonic 17th day mice were divided into two groups. In the control experiment, explants were cultured in agarose semi-solid medium under serum-free and chemically defined conditions, while explants in the other group were cultured with 30 mumol/L, 15 bp antisense oligodeoxynucleotide targeted to DSPP mRNA. After 10 ds, the explants were examined by transmission electron microscope. The width of dentin matrix at the tip of the cusps were then measured and statistically analyzed with Student t-test.

RESULTSUltrastructure analyses showed that large cisternae of the rough endoplasmic reticulum (RER) existed in the odontoblasts at the tip of the cusps of antisense-treated explants and the average thickness of dentin matrix (2.5 microns) was thinner compared to the control ones (3 microns, P < 0.001). In addition, the collagen fibers in extracellular matrix were disorganized.

CONCLUSIONThese findings indicated that DSPP played an important role in keeping tooth normal development, as well as in dentin mineralization by maintaining odontoblasts' secreting ability and controlling fiber structure and orientation.

Objective:To study the function of tumor necrosis factor r eceptor-associated factor 6(TRAF6) in the development of rat dental germ. Methods:After specimen preparation of every stage of rat developing d ental germs, immunohistochemical staining of TRAF6 was carried out. Resu lts:Positive expression of TRAF 6 was observed in thickening stage of de ntal lamina, peripheral dental lamina of bud stage, outer and inner enamal epith elium, stellate reticulum,dental papilla cells and dentinoblasts in early bell s tage, ameloblasts, dentinoblast,intermediate layer, stellate reticulum and denta l papilla cells in late bell stage, and dentinoblasts and ameloblasts in the ger m 7 days after birth. Conclusions:TRAF6 may be a signal transduc tion factor which modulates the proliferation and differentiation of developing epithelial and mesenchymal cells in the development of the dental germ.

Objective:To determine whether the immortalized human odo nt oblast-like cell line hTERT-hOd-l has transformed phenotype. Methods: The tumorigenicity, anchorage-independent growth, serum dependence and contact inhibition of the immortalized human odontoblast-like cell line hTERT- hOd-l were observed during continuous culture in vitro. Results: The cells showed to be nontumorigenic in nude mice, and to have no anchorag e-independent growth. The cells maintained the normal serum dependence and cont act inhibition. Conclusion:hTERT-hOd-l is a normal cell line w ithout obvious transformed phenotype.

Objective:To study the effects of parathyroid hormone (PTH) on BMP3 expression of cultured human dental papilla mesenchymal cells (DPMCs). Methods:DPMCs were obtained by cell culture, BMP3 mRNA expression was studied by in situ hybridization. BMP3 gene expression of human DPMCs after exposure to 33.3 nmol/L PTH for 5 days were measured. The cells cultured in the medium without PTH served as control.Results: Stronger BMP3 positive signals were observed in PTH treated cells than that in the control cells.Conclusion:PTH stimulats BMP3 systhesis of cultured DPMCs.

Objective: To study the effects of parathyroid hormone(PTH) on the calcification characters of cultured human dental papilla mesenchymal(DPM) cells.Methods: DPM cells were cultured up to 35 days in two groups. The control group was cultured in DMEM supplemented with 15% FCS,50 ?g/ml ascorbic acid and 10 mmol/L ?-glycerophosphate. The cells in experimental group were cultured in above mentionned medium containing 33.3 nmol/L of PTH. The medium was changed every 3 or 4 days.
