1.Comparative study of therapeutic effect of rubber ring and dental floss combined with hemoclipping assisted endoscopic submucosal dissection on early gastrointestinal tumor

Objective To compare the effect of rubber ring versus dental floss combined with hemoclipping assisted en-doscopic submucosal dissection(ESD)in the treatment of early gastrointestinal tumor.Methods A total of 103 patients with early gastrointestinal cancer were divided into the rubber band traction group(rubber band group,n=51)and the dental floss traction assist group(dental floss group,n=52)according to different assisted traction techniques during ESD.The basic con-ditions of the two groups were compared before operation,as well as the amount of submucosal injection,installation time of the traction device,the amount of bleeding during operation,the time of operation,the delayed bleeding after operation,one-time complete resection rate of tumor,days in hospital after operation and complication rate at 72 hours after operation.The degree of postoperative pain(VAS pain Score),follow-up quality of life(SF-36 Score)and complications 12 months after op-eration were also recorded.Results The intraoperative submucosal injection volume,intraoperative bleeding volume,num-ber of hemostatic clamp detachment,surgical time,complication rate 72 hour after operation and postoperative hospital stay were lower in the rubber band group than those in the dental floss group(P<0.05).There was no significant difference in the number of delayed postoperative bleeding between the two groups(P>0.05).After 12 months of follow-up,patients in the rubber band group had mild postoperative pain and good quality of life.The incidence of complications decreased within 12 months after surgery in the rubber band group(P<0.05).Conclusion Compared with floss traction aid,rubber ring traction aid ESD can shorten the operation time and reduce postoperative complications,which is worth popularizing.
2.Application of the teaching model of massive open online course combined with small private online course in the teaching of Fundamentals of Surgery

Objective:To investigate the application effect of the teaching model of massive open online course (MOOC)+small private online course (SPOC) in the undergraduate clinical teaching of Fundamentals of Surgery.Methods:A total of 30 undergraduate medical trainees in the five-year program were randomly divided into experimental group and control group, with 15 trainees in each group. With the chapter of basic surgical procedures as an example, the trainees in the control group received traditional teaching, and those in the experimental group received MOOC+SPOC teaching. Theoretical assessment and surgery simulation were performed at one week after the course to evaluate the teaching effect. SPSS 17.0 was used for the t-test and the chi-square test. Results:The experimental group had a theoretical assessment score of (92.5±6.4) points and a basic skill operation score of (91.7±2.2) points, while the control group had a theoretical assessment score of (85.3±7.3) points and a basic skill operation score of (83.6±6.4) points. The experimental group had an excellent rate of 60% (9/15) in theoretical assessment and 66.7% (10/15) in basic skill operation test, while the control group had an excellent rate of 40% (6/15) in theoretical assessment and 33.3% (5/15) in basic skill operation test. The experimental group had significantly higher scores and grades than the control group ( P<0.05). Conclusion:MOOC+SPOC teaching can significantly enhance the understanding of theoretical knowledge and the mastery of operational skills among trainees and improve teaching effect, and therefore, it has a good application prospect.
3.Efficacy of postural restriction in preventing recurrence of Benign Paroxysmal Positional Vertigo:a randomized controlled clinical study

Objective To evaluate the effect of postural restriction in preventing recurrence of Benign Paroxysmal Positional Vertigo(BPPV). Methods A total of 217 unilateral posterior semicircular canal BPPV patients were diagnosed clearly and divided into two groups randomly:104 caes(observation group) were demand postural restriction(head up 45 degrees for 72 hours,but did not restricted sleeping posture or departure from bed) after Epley maneuver,while 114 case(control group) could freely sleep. Demographic data,concomitant diseases and betahistine mesylate treatment were recorded. All patients were follow-uped at 72 hours after the Epley maneuver at outpatients of our hospital. A induced vertigo by position change or positional nystagmus were defined as recurrence of BPPV. Results There was no statics defferrence between two groups for base line data. After 72 hours of Epley maneuver,BPPV recurrence rates was 21.2% and 41.6% for observation group and control group,respectively(P<0.01). Odds ratio of postural restriction for BPPV recurrence was 0.377(95%CI 0.207~0.687). Conclusion Head up 45 degrees for 72 hours after Epley maneuver was helpful for preventing BPPV early recurrence.
4.Prognostic analysis of patients with T1 stage high grade of bladder urothelial carcinoma and glandular differentiation

Objective:To evaluate the recurrence and progression of patients with pT1 high grade urothelial carcinoma of bladder (UCB) and glandular differentiation.Methods:We retrospectively analyzed the clinical and pathological information of 208 patients diagnosed as pT1 high grade urothelial carcinoma in the Fifth Central Hospital of Tianjin from January 2006 to February 2019.Among them, 78 cases were diagnosed as glandular differentiation (UCGD), the other 130 patients without histologic variants were served as control. The UCGD group included 62 male and 16 female, whose median age was 67 years old (range 38-81 years old). The control group contained 105 male and 25 female, whose median age was 66 years old (range 40-82 years old). Kaplan-Meier and Cox proportional hazard regression analyses were used to evaluate the predictors of oncologic outcomes.Results:The disease recurrence rate and progression rate in UCGD group were 65.4% (51/78) and 28.2% (22/78), higher than 38.5%(50/130) and 14.6%(19/130) of control group ( P<0.05). The median recurrence time in UCGD group was 41 months while 55 months in the control group. The median progression time in UCGD group was 39 months while 54 months in the control group. According to the univariate analysis, largest tumor size ( P=0.030), UCGD ( P=0.003) and lymphovascular invasion (LVI) ( P=0.032) were associated with disease recurrence. UCGD ( P=0.036) and LVI ( P=0.011) were associated with progression. Additionally, Cox multivariate analysis revealed that UCGD ( P=0.001), LVI ( P=0.038) were the independent factors of disease recurrence. UCGD ( P=0.007) and LVI ( P=0.037) were also found to be the independent factors of disease progression. Conclusions:Patients with T1 stage UCB and UCGD are at higher risk of disease recurrence and progression. Therefore, these patients should be followed up closely after being diagnosed and undergo individual treatment according to the situation.

6.Pim-1 Protects Retinal Ganglion Cells by Enhancing Their Regenerative Ability Following Optic Nerve Crush

Provirus integration site Moloney murine leukemia virus (Pim-1) is a proto-oncogene reported to be associated with cell proliferation, differentiation and survival. This study was to explore the neuroprotective role of Pim-1 in a rat model subjected to optic nerve crush (ONC), and discuss its related molecules in improving the intrinsic regeneration ability of retinal ganglion cells (RGCs). Immunofluorescence staining showed that AAV2- Pim-1 infected 71% RGCs and some amacrine cells in the retina. Real-time PCR and Western blotting showed that retina infection with AAV2- Pim-1 up-regulated the Pim-1 mRNA and protein expressions compared with AAV2-GFP group. Hematoxylin-Eosin (HE) staining, γ-synuclein immunohistochemistry, Cholera toxin B (CTB) tracing and TUNEL showed that RGCs transduction with AAV2-Pim-1 prior to ONC promoted the survival of damaged RGCs and decreased cell apoptosis. RITC anterograde labeling showed that Pim-1 overexpression increased axon regeneration and promoted the recovery of visual function by pupillary light reflex and flash visual evoked potential. Western blotting showed that Pim- 1 overexpression up-regulated the expression of Stat3, p-Stat3, Akt1, p-Akt1, Akt2 and p-Akt2, as well as βIII-tubulin, GAP-43 and 4E-BP1, and downregulated the expression of SOCS1 and SOCS3, Cleaved caspase 3, Bad and Bax. These results demonstrate that Pim-1 exerted a neuroprotective effect by promoting nerve regeneration and functional recovery of RGCs. In addition, it enhanced the intrinsic regeneration capacity of RGCs after ONC by activating Stat3, Akt1 and Akt2 pathways, and inhibiting the mitochondrial apoptosis pathways. These findings suggest that Pim-1 may prove to be a potential therapeutic target for the clinical treatment of optic nerve injury.
7.Cytotoxic effect and mechanism of Aflatoxins B1 on Vero E6 cells

Objective To explore the cytotoxic effects and mechanisms of Aflatoxins B1(AFB1) on the African green monkey kidney cells (Vero E6).Methods After incubation of Vero E6 cells with AFB1,cellular toxicity was assessed by CCK-8 assay and LDH release rate.Annexin V/PI double staining and DAPI staining was used to detected cell apoptosis.Results AFB1 has significant toxic effects on Vero E6 cells.Cell viability was less than 10% when treated by 200 μmol/L AFB1 for 48 h.LDH release rate was increased significantly with the increase of AFB1 concentration.Externalization of cell membrane phosphatidylserine was observed by Annexin V/PI double staining.Nucleus fragmentation was detected by DAPI staining.Conclusion AFB1 exhibits significantly cytotoxic effects on Vero E6 cells by inducing cell apoptosis.
8.Nursing care of patients with malignant tumor treated by 3D printing individualized template and 125I seed implantation

This paper summarized nursing experience of 23 patients with malignant tumor treated by 3D printing individualized template and 125I seed implantation.Nursing points included:preoperative assessment and preparation,reviewing the process of template conduction,assisting the physician to simulate the position of patients,making treatment plans,preparing templates before operation;resetting and maintaining position of patients,performing template alignment,seed implantation,monitoring vital signs and complications during operation;observation of complications,providing radiation protection and discharge guidance after operation.All 23 patients completed 125I seed implantation and no serious complication was observed.All patients recovered well and were discharged after treatment.
9.Monosaccharide Composition Analysis of Arabinoxylan by High Performance Anion Exchange Chromatography with Pulsed Amperometric Detection

A method of complete acid hydrolysis combined with high performance anion exchange chromatography and pulsed amperometric detection was developed for the monosaccharide composition analysis of arabinoxylan from the seeds of Plantago asiatica L. The parameters including hydrolysis methods, acid types, acid concentration, hydrolysis temperature, hydrolysis time and placement time, which would affect the hydrolysis process, were optimized. The results showed that it would have a better hydrolysis effect for polysaccharide from the seeds of Plantago asiatica L. with 2 mol/L H2 SO4 in an atmospheric oil bath at 120℃for 2 hours. However, the placement time for diluted solution of the hydrolyzed polysaccharide should be less than 6 hours. The polysaccharide was mainly composed of Arabinose (8. 89%) and Xylose (41. 52%) and Galacturonic acid (0. 73%). Glcuronic acid (3. 44%) was detected simultaneously, and there were also trace amounts of Galatose and Glucose. The results were reproducible. Other arabinoxylans from Panicummiliaceum L. shell, Avena sativa L. bran and Hordeum vulgare L. were taken for monosaccharide compositions analysis under the optimal hydrolysis conditions and the analysis results were good. This study would provide a good reference for monosaccharides composition analysis of arabinoxylans from various sources.
10.Prevalence of anxiety and depression before idiopathic benign paroxysmal positional vertigo episode

Objective To investigate the prevalence of anxiety and depression one month before idiopathic benign paroxysmal positional vertigo (BPPV) episode. Methods Patients from Vertigo Clinic of Department of Neurology, West Campus, Chaoyang Hospital, were enrolled from January 2014 to June 2016. For patients with first visiting idiopathic BPPV, Hamilton Anxiety Scale and Hamilton Depression Scale were used at the consulting room by an experienced neurologist to reflect the emotional state one month before idiopathic BPPV episode. The occurrence rate of mood disorders was compared with usual model. Results One hundred and eighty five patients with idiopathic BPPV, aged 28 to 82 (57.6±12.0) years, 19.5% (36/185) being male, 80.5% (149/185) being female, were enrolled into this research. No anxiety, probable anxiety, definite anxiety and severe anxiety accounted for 15.1% (28/185), 33.0% (61/185), 47.6% (88/185) and 4.3% (8/185), respectively;no depression, probable depression, definite depression and severe depression accounted for 18.4% (34/185), 48.6% (90/185), 29.2% (54/185) and 3.8% (7/185), respectively;no mood disorders, probable mood disorders, definite mood disorders and severe mood disorders accounted for 11.4% (21/185), 33.5% (62/185), 49.2% (91/185) and 5.9% (11/185), respectively;total definite mood disorders and severe mood disorders accounted for 55.1% (102/185), higher than usual model (14.4%)Conclusion Anxiety and depression is common before idiopathic BPPV episode.
