1.Traditional Chinese Medicine Regulates JAK/STAT Signaling Pathway to Treat Colorectal Cancer: A Review

Colorectal cancer (CRC) ranks as the second leading cause of cancer death worldwide. Although preventive colonoscopy screening has improved the survival rate of CRC patients in the past few years, there are still many patients diagnosed after symptoms appear. The surgery for CRC carries high risks and high recurrence, and ideal therapies remain to be developed. The Janus kinase/signal transducer and activator of transcription (JAK/STAT) signaling pathway has become a focus of research due to its central role in cellular activities. As a classic oncogenic pathway, the JAK/STAT signaling pathway offers new possibilities for diagnosing and treating various malignancies, and it paves a new way for developing therapies for CRC. This pathway not only participates in basic cellular processes, such as proliferation, differentiation, and apoptosis but also plays a crucial role in immune responses and inflammation. Abnormal activation of the JAK/STAT signaling pathway is closely related to the occurrence and development of CRC. Studies have shown that the active components and compound prescriptions of traditional Chinese medicine (TCM) can inhibit the proliferation, invasion, migration, and angiogenesis while promoting the apoptosis and autophagy of CRC cells by interfering with the JAK/STAT signaling pathway. Furthermore, this pathway may also play a role in regulating the sensitivity of tumor cells to chemotherapy and radiotherapy, thus influencing the effectiveness of tumor treatment and impeding the progression of CRC. In recent years, research results have been updated rapidly, and previous literature summaries have failed to incorporate the latest findings, creating obstacles to accessing current literature. Therefore, this article supplements and summarizes information from the definition of the JAK/STAT pathway, association of this pathway with CRC, and TCM intervention of CRC. This review aims to provide references for future development of molecular biology regarding CRC and the research and development of new drugs.
2.Development of DUS Test Guidelines for New Pinellia ternata

Pinellia ternata, belonging to the Pinellia genus within the Araceae family, is a medicinal plant due to its tubers. There are severe issues with unclear germplasm and mixed varieties in its cultivation, necessitating urgent new variety protection efforts. The distinctness, uniformity, and stability (DUS) testing of the plant variety is the basis for protecting new plant varieties, and the DUS test guidelines are the technical basis for DUS testing. To develop the DUS test guidelines for P. ternata, agronomic traits of 229 germplasm of P. ternata were observed and measured during its two growth stages over the years, and each character was graded and described. A total of 38 traits were selected as the test traits of the DUS test guideline for P. ternata. There were three plant traits, 19 leaf traits, six flower traits, two fruit traits, two tuber traits, five bulbil traits, and one ploidy trait. These traits could be divided into 22 quality characters, 12 quantitative characters, and four pseudo-quantitative characters, as well as seven groups, including plants, leaves, flowers, fruit, tubers, bulbils, and ploidy. By searching for standard traits, 10 standard varieties were ultimately determined. Preparing these guidelines will have great significance for reviewing and protecting P. ternata varieties, safeguarding breeders' rights, and promoting the development of the P. ternata industry.
3.Effects of Zhimu (Anemarrhena asphodeloides)-Huangbai (Phellodendron amurense) Medicinal Pair on Femoral Microstructure and Osteogenic-Adipogenic Differentiation in Ovariectomized Osteoporosis Model Rats

ObjectiveTo investigate the potential mechanisms of Zhimu (Anemarrhena asphodeloides)-Huangbai (Phellodendron amurense) medicinal pair in alleviating postmenopausal osteoporosis (PMOP). MethodsSixty unpregnant female SD rats were randomly divided into five groups, blank group, model group, low-dose Zhimu-Huangbai group, high-dose Zhimu-Huangbai group, and estradiol group, with 12 rats in each group. Except for the blank group, all other groups had their ovaries removed to create PMOP rat models, while the blank group only had the fat tissue around the ovaries removed. One week after the ovarian removal, the low-dose and high-dose Zhimu-Huangbai groups received concentrated solution of Zhimu and Huangbai with 1.8, 7.2 g/(kg·d) via gavage, the estradiol group received estradiol solution 0.09 mg/(kg·d) via gavage, and the blank group and the model group received 10 ml/(kg·d) of normal saline via gavage, once daily for 12 weeks. Before sampling, the body mass of the rats was recorded, and uterine tissue was taken to calculate the uterine index. The levels of interleukin-1β (IL-1β), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) in serum were detected by ELISA; micro CT was used to examine the parameters of femoral microstructure, including bone volume/tissue volume (BV/TV), trabecular number (Tb.N), trabecular thickness (Tb.Th), bone mineral density (BMD), trabecular separation (Tb.Sp), and cortical bone area (Ct.Ar). HE staining was used to observe pathological changes in the femur; RT-qPCR was used to detect the mRNA expression of osteogenic-adipogenic differentiation-related factors in femoral tissue, including Runt-related transcription factor 2 (Runx2), bone morphogenetic protein 2 (BMP-2), peroxisome proliferator-activated receptor γ (PPARγ), chemerin and chemokine-like receptor 1 (CMKLR1). ResultsCompared with the blank group, the model group showed a significant increase in body mass, a significant decrease in the uterine index, BV/TV, Tb.N, Tb.Th and BMD, a significant increase in Tb.Sp, and serum IL-1β, IL-6, and TNF-α levels, a significant reduction of mRNA expression of Runx2 and BMP-2 in bone tissue, and a significant increased mRNA expression of PPARγ, chemerin, and CMKLR1 (P<0.01). HE staining revealed that the femoral tissue showed a reduction and sparsity of trabeculae, a significant enlargement of the medullary cavity, and a large number of fat cells. Compared to the model group, the low-dose, high-dose Zhimu-Huangbai groups, and estradiol group showed significant improvements in all the above-mentioned indicators (P<0.05 or P<0.01). HE staining revealed a significant increase in trabeculae, more organized arrangement, and a marked reduction in fat cells. Compared to low-dose Zhimu-Huangbai group, the high-dose Zhimu-Huangbai group exhibited a significant increase in the uterine index and BMD, and a significant reduction in body mass and PPARγ and Chemerin mRNA expression (P<0.05 or P<0.01). Compared to high-dose Zhimu-Huangbai group, the estradiol group showed a decrease in uterine index, BV/TV, Tb.N, Tb.Th, BMD, and BMP-2 mRNA expression, while the levels of IL-1β, TNF-α, and IL-6, as well as Tb.Sp and the mRNA expressions of PPARγ, chemerin, and CMKLR1 increased (P<0.05 or P<0.01). ConclusionThe Zhimu-Huangbai medicinal pair can alleviate PMOP bone loss, and its mechanism of action is related to reducing the levels of inflammatory factors, correcting the disorder of osteogenic-adipogenic differentiation of bone marrow mesenchymal stem cells (BMSCs), and promoting the differentiation of BMSCs into osteoblasts.
4.Application of Colorectal Cancer Early Screening Based on Multitarget Fecal FIT-DNA Joint Detection Technology

Objective To evaluate the efficacy and further analyze the application prospects of the combined multitarget fecal FIT-DNA assay in the early screening of colorectal cancer.Methods Subjects were selected from a population attending the Inner Mongolia Medical University Hospital.Each subject underwent a combined multi-target fecal FIT-DNA test(experimental group),a serum tumor marker test and enteroscopy(control group).The pathological results were used as the gold standard to evaluate the efficacy of novel fecal molecular testing techniques for colorectal cancer screening with timely intervention given to screen positive individuals.Results The data of 115 individuals were analyzed.Serum tumor markers test had a sensitivity of 63.2%(43/68)and a specificity of 74.5%(35/47).The enteroscopy had a sensitivity of 97.1%(66/68)and a specificity of 80.7%(38/47);the combined multitarget fecal FIT-DNA test had a sensitivity of 89.7%(61/68)and a specificity of 87.2%(41/47).Conclusion The sensitivity and specificity of multitarget fecal FIT-DNA combined detection are better than those of serum tumor marker detection.Although its sensitivity is lower than enteroscopy,its operation is simpler and can be tested at home.
5.Discussion on Key Technologies of Health Care Big Data Processing Based on the Process Perspective

Purpose/Significance The processing links and key technologies of health care big data are deeply discussed to provide in-formation support and guarantee for further promoting the application of health care big data.Method/Process According to the scientific research paradigm driven by big data,and based on the process perspective,the paper introduces the sources and characteristics of health care big data,analyzes the processing links and key technologies of big data,and discusses the current challenges and future development directions of health care big data processing.Result/Conclusion Both China and foreign countries have achieved innovative development in technologies for health care big data processing.However,there are still some challenges in multi-dimensional data collection,multi-mo-dal data integration,large model data analysis,and data security.In the future,through building a comprehensive platform of big data pro-cessing,and developing or integrating targeted functional modules,the existing problems can be effectively addressed.
6.Advances in the diagnostic and monitoring value of glycocalyx injury in inflammatory bowel disease

Inflammatory bowel disease(IBD)is no cure at present,and only drugs and surgery can relieve symptoms,delay and reduce recurrence.Therefore,it is very important to monitor the disease activity.Glycocalyx,a polyglycoprotein complex covering luminal side of vascular endothelial cells,is supportive in maintaining the intestinal mucosal barrier,and inflammation leads to its structural damage.This review introduces currently available non-invasive biomarkers of clinical importance for IBD in laboratory testing,and analyzes the value of serum glycocalyx injury markers(syndecan-1,HS and HA)in the diagnosis or monitoring of IBD.
7.Effectiveness of 5G-based robot-assisted remote thoracolumbar internal fixation surgery

Objective:To assess the effectiveness and viability of 5G-enabled robot-assisted thoracolumbar internal fixation surgery, scrutinizing its benefits and potential limitations in a clinical setting.Methods:This retrospective case series study analyzed 61 patients who underwent robot-assisted remote thoracolumbar internal fixation surgery between April and December 2021. The orthopaedic robotic telesurgery system, centered at Beijing Jishuitan Hospital and utilizing 5G technology for network transmission, was used to assist in screw placement. The surgeries, distributed across ten hospitals and executed over an average distance of 1 617 km (maximum exceeding 3 000 km), were appraised by examining the accuracy of screw placement, intraoperative communication efficacy, complication rates, among other metrics, and benchmarked against traditional local procedures.Results:Telesurgery was completed successfully in all patients, averaging 92±32 minutes per operation. Out of 354 screws placed, 89.8% were positioned perfectly and 97.5% were deemed clinically acceptable. The 61 patients, followed-up for an average of 18.9±1.9 months, exhibited significant postoperative improvement in pain. The preoperative visual analogue scale (VAS) pain score was 5.74±1.30 and improved to 2.30±0.90 at discharge, a statistically significant difference ( t=11.002, P<0.001). 6 patients with thoracolumbar degenerative spine disease showed significant relief of claudication and pain symptoms postoperatively. 55 patients with thoracolumbar spine fractures had a preoperative height ratio of 62.78%±12.60% at the anterior margin of the injured vertebrae, which recovered to 85.36%±8.35% postoperatively, a statistically significant difference ( t=16.977, P<0.001). All fractures were healed at final follow-up. No perioperative complications, such as vascular or nerve injuries or infections, occurred. Additionally, there were no complications related to the use of the robot. The 5G network ensured stable transmission of robotic control signals, images, and audio signals during the procedure, with no signal delays or communication failures observed. The perfect rate (90.2% vs. 93.8%, χ 2=0.169, P=0.681) and clinical acceptability rate (99.0% vs. 100.0%, P=1.000) of remote versus local robotic-assisted screw placement in participating hospital 1 (Shantou Xin Sheng Orthopaedic Hospital) were similar. The perfect rate (87.0% vs. 85.4%, χ 2=0.075, P=0.784) and clinical acceptability rate (100.0% vs. 96.6%, χ 2=0.580, P=0.446) of remote versus local robotic-assisted screw placement in participating hospital 2 (Guizhou Provincial People's Hospital) were similar. Conclusion:5G technology enhances the remote application of robotic thoracolumbar surgery without compromising patient safety or outcome quality. It underscores the potential for broader adoption of surgical robotics, aiding in the standardization of medical practices across China, boosting healthcare efficiency, and aligning with the Healthy China strategy.
8.Research advances on treatment of recurrent IgA nephropathy after kidney transplantation

IgA nephropathy (IgAN) is one common type of glomerulonephritis caused by a deposition of immune complexes in mesangium and partial capillary loops. It is also an important risk factor for end-stage renal disease (ESRD). Kidney transplantation (KT) has been an ultimate treatment for IgAN patients progressing into ESRD. However, there is still a high risk of recurrence after transplantation. Currently no effective treatment is available for recurrent IgAN. This review summarized the latest researches of managing IgAN recurrence after KT, such as optimal treatment, immunosuppression, complement therapy and surgery.
9.Cost-Effectiveness Analysis of Roxadustat and Darbepoetin Alfa for Treating Renal Anemia in Dialysis-Dependent Chronic Kidney Disease Patients

Objective To evaluate the cost-effectiveness of roxadustat and darbepoetin alfa on treating renal anemia in dialysis-dependent chronic kidney disease(DD-CKD)patients,thus providing health economics reference for treatment of renal anemia.Methods A Markov model simulating the development and treatment of anemia in DD-CKD patients in a lifetime horizon(20 years)was constructed.Total costs of roxadustat and darbepoetin alfa injection were estimated from the perspective of Chinese healthcare system,with health outcomes converted into quality-adjusted life year(QALY).The incremental cost-effectiveness ratio(ICER)was used to describe the results.The willingness-to-pay(WTP)threshold was set at 257 094 yuan,which was three times China's gross domestic product(GDP)per capita in 2023.Sensitivity analyses were performed to test the uncertainties of the results.Results The total treatment costs of roxadustat and darbepoetin alfa injection were 111 902.41 yuan and 52 927.92 yuan respectively,corresponding to QALY values of 4.76 and 4.74 life-years.The incremental cost-effectiveness(ICER)was 2 654 912.45 yuan/QALY,which exceeded 3 times GDP per capita.Therefore,compared with darbepoetin alfa injection,roxadustat has no cost-effectiveness for patients with DD-CKD.Conclusion In the context of current economic development in China,darbepoetin alfa injection is more cost-effective than roxadustatin for treating anemia in DD-CKD patients.

Objective:To evaluate the diagnostic efficacy and practicality of the Jaundice color card (JCard) as a screening tool for neonatal jaundice.Methods:Following the standards for reporting of diagnostic accuracy studies (STARD) statement, a multicenter prospective study was conducted in 9 hospitals in China from October 2019 to September 2021. A total of 845 newborns who were admitted to the hospital or outpatient department for liver function testing due to their own diseases. The inclusion criteria were a gestational age of ≥35 weeks, a birth weight of ≥2 000 g, and an age of ≤28 days. The neonate′s parents used the JCard to measure jaundice at the neonate′s cheek. Within 2 hours of the JCard measurement, transcutaneous bilirubin (TcB) was measured with a JH20-1B device and total serum bilirubin (TSB) was detected. The Pearson′s correlation analysis, Bland-Altman plots and the receiver operating characteristic (ROC) curve were used for statistic analysis.Results:Out of the 854 newborns, 445 were male and 409 were female; 46 were born at 35-36 weeks of gestational age and 808 were born at ≥37 weeks of gestational age. Additionally, 432 cases were aged 0-3 days, 236 cases were aged 4-7 days, and 186 cases were aged 8-28 days. The TSB level was (227.4±89.6) μmol/L, with a range of 23.7-717.0 μmol/L. The JCard level was (221.4±77.0) μmol/L and the TcB level was (252.5±76.0) μmol/L. Both the JCard and TcB values showed good correlation ( r=0.77 and 0.80, respectively) and agreements (96.0% (820/854) and 95.2% (813/854) of samples fell within the 95% limits of agreement, respectively) with TSB. The JCard value of 12 had a sensitivity of 0.93 and specificity of 0.75 for identifying a TSB ≥205.2?μmol/L, and a sensitivity of 1.00 and specificity of 0.35 for identifying a TSB ≥342.0?μmol/L. The TcB value of 205.2?μmol/L had a sensitivity of 0.97 and specificity of 0.60 for identifying TSB levels of 205.2 μmol/L, and a sensitivity of 1.00 and specificity of 0.26 for identifying TSB levels of 342.0 μmol/L. The areas under the ROC curve (AUC) of JCard for identifying TSB levels of 153.9, 205.2, 256.5, and 342.0 μmol/L were 0.96, 0.92, 0.83, and 0.83, respectively. The AUC of TcB were 0.94, 0.91, 0.86, and 0.87, respectively. There were both no significant differences between the AUC of JCard and TcB in identifying TSB levels of 153.9 and 205.2 μmol/L (both P>0.05). However, the AUC of JCard were both lower than those of TcB in identifying TSB levels of 256.5 and 342.0 μmol/L (both P<0.05). Conclusions:JCard can be used to classify different levels of bilirubin, but its diagnostic efficacy decreases with increasing bilirubin levels. When TSB level are ≤205.2 μmol/L, its diagnostic efficacy is equivalent to that of the JH20-1B. To prevent the misdiagnosis of severe jaundice, it is recommended that parents use a low JCard score, such as 12, to identify severe hyperbilirubinemia (TSB ≥342.0 μmol/L).
