1.Study on toxicity-reducing and efficacy-enhancing effects of Polygala tenuifolia compatibility on sand-ironing Strychnos nux-vomica

OBJECTIVE To explore the effects of Polygala tenuifolia compatibility on toxicity, anti-inflammatory and analgesic efficacy of sand-ironing Strychnos nux-vomica (SS). METHODS The preparation of SS single decoction, SS-P. tenuifolia core-removed (PC) (1∶2.5) or (1∶5) combined decoction, and SS-PC (1∶5) mixture were carried out to investigate their median lethal dose (LD50). Using aspirin as positive control, the number of writhing movements, analgesic rate, pain latency, ear swelling degree and inflammation inhibition rate induced by the above-mentioned medicinal liquids in mice were compared. The contents of the active and toxic components, strychnine and brucine, in the above-mentioned medicinal liquids were also determined. RESULTS The LD50 values of SS single decoction, SS-PC (1∶2.5) combined decoction, SS-PC (1∶5) combined decoction and SS- PC (1∶5) mixture were 302.00, 614.47, 1 445.44 and 1 778.28 mg/kg, respectively. Compared with control group, the number of writhing movements and ear swelling degree in the mice of the above-mentioned medicinal liquid groups were reduced or decreased significantly (P<0.05 or P<0.01); pain latency [at 90 and 120 minutes in the SS single decoction group, at 60 and 90 minutes in the SS-PC (1∶2.5) combined decoction group, and at 60,90, 120 minutes in the SS-PC (1∶5) combined decoction group and SS-PC (1∶5) mixture group] was significantly prolonged (P<0.05 or P<0.01); analgesic rates of the respective medicinal liquids were 39.30%, 70.87%, 80.00% and 82.46%, and inflammation inhibition rates were 38.08%,TD 57.89%, 76.47% and 50.46%; analgesic and anti-inflammatory effects of combined decoction and mixture were generally better than those of the single decoction (P<0.05 or P<0.01). In the above-mentioned four medicinal liquids, the total contents of strychnine were 0.71%, 0.42%, 0.47% and 0.64%, and the total contents of brucine were 0.88%, 0.63%, 0.57% and 0.88%, respectively. CONCLUSIONS The combination of P. tenuifolia can reduce the toxicity of SS and enhance its anti-inflammatory and analgesic effects. Moreover, there is a tendency for the toxicity-reducing and efficacy-enhancing effects to increase with the increasing dosage of P. tenuifolia. Additionally, the combined decoction of SS and P. tenuifolia can reduce the contents of the active and toxic components, strychnine and brucine, in SS.
2.Exploring the Value Evaluation Framework for High-Value Medical Consumables Access Management in China

Objective:To construct a value evaluation framework for high-value medical consumables,providing a guidance for medical insurance access and hospital access management scenarios in China.Methods:It conducted literature review,qualitative in-terviews and quantitative surveys.A total of 12 experts were invited for qualitative interviews,while 100 experts from four fields of health technology assessment,medical insurance,hospital management,and clinical practice participated in the quantitative survey.Through those process,it generated the composition of the value framework and the scoring of each item.Differences in ratings be-tween different scenarios and experts were analyzed through chi-square tests.The recommendation level for each item was graded.Re-sults:A comprehensive value evaluation framework for high-value medical consumables was established,which included 6 core dimen-sions,comprised 16 items for secondary dimensions and 50 items for tertiary dimensions.It showed significant differences between the medical insurance access and hospital access scenarios,as well as among different fields of experts in the same scenario.furthermore,grading the items in two scenarios.The medical insurance access scenario had 8 highly recommended items,and the hospital access scenario had 24 highly recommended items.Conclusion:Value evaluation should encourage multi-dimensional assessments and inter-disciplinary participation,continually improving the management of high-value medical consumables in medical insurance and hospital access.
3.Effect of extracorporeal shock wave combined with meloxicam tablets on joint function and serum levels of MMP-9 and resistin in elderly patients with osteoarthritis

Objective:To explore the effect of extracorporeal shock wave combined with meloxicam tablets on joint function and serum matrix metalloproteinase-9 (MMP-9) and resistin levels in elderly patients with osteoarthritis.Methods:80 cases of elderly patients with osteoarthritis in Nanchong Central Hospital from May 2016 to March 2019 were selected as the research objects, and they were randomly divided into two groups, 40 cases in each group. The control group was given conventional oral medicine (meloxicam + potassium glucosamine sulfate tablets), and the observation group was given meloxicam tablets combined with extracorporeal shock wave therapy. The total effective rate, incidence of adverse reactions, and the scores of osteoarthritis symptoms (WOMCA), joint function (Lysholm), serum MMP-9 and resistin before and after 8 weeks' treatment were compared between the two groups.Results:After treatment, the WOMCA score of the observation group was lower than that of the control group, and Lysholm score was higher than that of the control group ( P<0.05); the total effective rate of the observation group (95.00%) was higher than of the control group (77.50%) ( P<0.05); the serum resistin and MMP-9 levels of the observation group were lower than those of the control group after treatment ( P<0.05); there was no significant difference in the incidence of adverse reactions between the observation group (15.00%) and the control group (17.50%) ( P>0.05). Conclusions:Extracorporeal shock wave combined with meloxicam tablets in the treatment of elderly patients with osteoarthritis can significantly improve the clinical symptoms, promote the recovery of joint function, reduce the levels of serum resistin and MMP-9, and further improve the treatment effect with high safety.
4.Relationship between SUVmax in 18F-FDG PET/CT and PD-L1 expression in invasive lung adenocarcinoma

Objective To investigate the relationship between the expression of programmed cell death ligand-1 (PD-L1) and the maximal standardized uptake value (SUVmax) in 18F-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) and the correlation of clinical factors between SUVmax values and PD-L1. Methods The clinical data of 84 patients with invasive lung adenocarcinoma diagnosed pathologically in West China Hospital, Sichuan University from August 2016 to November 2018 were analyzed retrospectively, including 38 males and 46 females, aged 60 (32-85) years. The tumor was acinar-predominant in 37 patients, papillary in 20, lepidic in 19, solid in 5 and micropapillary in 3. Multivariate analysis of the relationship between SUVmax value and other clinicopathological features was performed by linear regression. Logistic regression analysis was used to analyze the relationship between PD-L1 protein expression and other pathological features. Results The SUVmax of the PD-L1 expression group was significantly higher than that of the non-PD-L1 expression group in the whole invasive lung adenocarcinoma group (P=0.002) and intermediate-grade histologic subtype (P=0.016). The SUVmax cut-off value of PD-L1 expression in the whole invasive lung adenocarcinoma group and intermediate-grade histologic subtype was 5.34 (AUC: 0.732, P=0.002) and 5.34 (AUC: 0.720, P=0.017), respectively. Multivariate analysis showed that pleura involvement, vascular tumor thrombus and the increase of tumor diameter could cause the increase of the SUVmax value, while the SUVmax value decreased in the moderately differentiated tumor compared with the poorly differentiated tumor. The SUVmax cut-off value between low-grade histologic subtype and intermediate-grade histologic subtype, intermediate-grade histologic subtype and high-grade histologic subtypes was 1.54 (AUC: 0.854, P<0.001) and 5.79 (AUC: 0.889, P<0.001), respectively. Multivariate analysis of PD-L1 expression showed pleura involvement (P=0.021, OR=0.022, 95%CI 0.001 to 0.558) and moderate differentiation (opposite to poor differentiation) (P=0.004, OR=0.053, 95%CI 0.007 to 0.042) decreased the expression of PD-L1. Conclusion The SUVmax of the PD-L1 expression group is significantly higher than that of the non-PD-L1 expression group in the whole invasive lung adenocarcinoma group and intermediate-grade histologic subtype. The level of SUVmax and the expression of PD-L1 in invasive lung adenocarcinoma are related to many clinical factors.
5.Safety assessment of mammary gland stem cells from normal tissues in breast cancer patients

BACKGROUND:The cultivation of mammary gland stem cel s is of great significance for the development of mammary gland and breast cancer.
OBJECTIVE:To seek an easy method to isolate and culture mammary gland stem cel in vitro, and verify the safety of cel s.
METHODS:Mammary epithelial cel s were isolated from normal tissues surrounding breast cancer, and CD49f-and EPCAM-positive cel s were sorted using flow cytometry fol owed by surface marker analysis and cel colony formation ability analysis. Afterwards, real-time fluorescent quantitative PCR was used to detect C-erbB-2 and Maspin mRNA expression in mammary gland stem cel s, breast cancer tissues and normal tissues surrounding breast cancer.
RESULTS AND CONCLUSION:Human mammary gland stem cel s were successful y cultured and highly expressed CD49f and EPCAM, with the presence of mixed colony, pleural epithelial cel colony, and myoepithelial cel colony. c-erbB-2 was lowly expressed while Maspin highly expressed in mammary gland stem cel s. Our experimental findings indicate that the mammary gland stem cel s derived from normal tissue surrounding breast cancer have biological safety.
6.A diagnostic meta-analysis:fiberoptic ductoscope versus galactography for the diagnostic value of nipple discharge

Objective To evaluate the diagnostic efficiency of fiberoptic ductoscope versus galactography for nipple discharge. Methods Literatures on comparing the diagnostic value of galactography with fiberoptic ductoseope (FDS) on the nipple discharge were searched from the Cochrane library, Evidence based medicine (EBM ),PubMed,China Vip database, and 8 studies were intaked. Then the data was analyzed by software Metadisc1.4 and Review Manager5.1. Results There are 1170 patients in the 8 studies. The pooled estimates for sensitivity of FDS and galactography was 82%, 78%, and the pooled estimates for specificity was 51%, 50%, respectively. The weighted AUC (area under curve) was 0.776, 0.705, Q*values was 0.715, 0.657, respectively. The sensitivities and specificity of the 2 methods were not significantly different. The overall diagnostic efficacy of FDS was better than that of galactography. Conclusion FDS is better than galaetography in the diagnostic value of nipple discharge.
7.Development and application of practical testing system for diagnostic imaging based on PACS

Objective To develop a practical testing system on internet,to offer a modern pattern of practicing testing for diagnostic imaging and to evaluate the effects.Methods Practical testing system for diagnostic imaging based on PACS was set up and its effect was evaluated by students.Results The developed practical testing system combines question bank,image reading tool and answering interface together and has functions of controlling and managing both the questions and students participating in the examination.In the satisfaction survey of students,practicing testing system of diagnostic imaging got a score of 96.75 (full mark 100) and was highly prized.Conclusions This pattern of online testing offers more imaging information and makes practicing examination of diagnostic imaging more vivid and scientific.
8.Experiment study about effect of Yinqiao detoxifcation oral liquid on natural killer cells and TNF-alpha, TGF-beta1 of BALB/C nude mouse infected by influenza virus.

OBJECTIVETo explore the effect of Yinqiao detoxifcation oral liquid on activity of natural killer cells (NK) and serum content of TNF-alpha, TGF-beta1 of BALB/C nude mouse infected by influenza virus.
METHODTo establish infected mice model by FM1 followed by intragastric administration of Yinqiao detoxifcation oral liquid for treatment. LDH method was used to observe NK cells. ELISA method was used to determine the levels of TNF-alpha, TGF-beta1, in serum on 1st, 3rd, 5th, 7th days after infection.
RESULTComparing to the normal group, the NK activity of the model group was significantly increased on 1 dpi (day post infection), and significantly decreased on 3, 5, 7 dpi. The NK activity of three dosage groups (5, 10, 20 g x kg(-1)) of Yinqiao detoxifcation oral liquid were respectively higher than that of the model on 3, 5, 7 dpi, especially with high dose (P < 0.01). The serum level of TNF-alpha and TGF-beta1 of model group is higher than that of normal group on 1, 3, 5, 7 d. Compared with model group, the serum level of Yinqiao detoxifcation oral liquid groups (5, 10, 20 g x kg(-1)) were decreased in different degree on every time point, especially the serum level of the higher dose of Yinqiao detoxifcation oral liquid decreased on 3 dpi (P < 0.05), Yinqiao detoxifcation oral liquid inhibit the serum level of TGF-beta1 in a dose-dependent manner.
CONCLUSIONYinqiao detoxifcation oral liquid could enhance the activity of NK cell and decrease the serum level of TNF-alpha and TGF-beta1 of the mice infected by influenza virus.

9.Effects of Yinqiao detoxifcation oral liquid on NK cell viability and serum concentration of IFN-gamma in SCID mouse infected by influenza virus FM1.

OBJECTIVETo explore the dynamic effects of Yinqiao detoxifcation oral liquid (YQD) on NK cell lysis in spleen and serum concentration of IFN-gamma in SCID mouse infected by influenza A virus FM1.
METHODThe mice were divided into six different groups randomly. Except normal control, other mice were intranasally instilled with 15 TCID of virus. Three dosage groups of YQD (5.0, 10.0, 20.0 g x kg(-1)) were respectively fed with YQD. Positive control group was administrated orally with 0.07 g x kg(-1) of ribavirin. Normal control and model group were fed with physiological saline. After 1, 3, 5 and 7 days' infection, spleens and serum were collected. Then the NK cell lysis was detected by LDH release kit and the concentration of IFN-gamma was examined by ELISA assay.
RESULTContents of IFN-gamma reached to peak value on the 3th day and until the 5th day. Later, the level of IFN-gamma returned to normal level. The variation tendency of activity of NK cell in spleen was according with that of IFN-gamma. But it reached the maximum value until the 5th day. The activity of NK cell lysis in three groups of YQD was well above that i n model group. In addition, therapeutic action of both 10.0 g x kg(-1) and 20.0 g x kg(-1) of YQD treatment groups was better than that of 5.0 g x kg(-1).
CONCLUSIONThe data showed that serum concentration of IFN-gamma and NK cell lysis were improved by YQD at different time, which was demonstrated YQD could perform well in immune system.

10.Roles of Mitogen Activated Protein Kinases in Cerebral Ischemic Preconditioning: Comparison among ERK, JNK and p38

Objective To explore initially the roles of the 3 major signaling pathways of mitogen activated protein kinases (MAPK) in cerebral ischemia preconditioning. Methods Healthy adult SD rats were randomly divided into 3 groups: normal control group; sham-operated group and ischemic preconditioning or ischemic tolerance group (n=6). SDS-PAGE, Western blot and Gel Doc imagine systems were applied to determine the phosphorylation and protein expression of ERK, JNK and p38 in somatosensory cortex of rat. Results The phosphorylation level of ERK1 and JNK46KD in somatosensory cortex increased significantly (P<0.05) after ischemia preconditioning. Conclusion The increased ERK1 and JNK46KD phosphorylation in somatosensory cortex may be involved in the development of cerebral ischemia preconditioning and can not be ruled out in which the role of p38.
