OBJECTIVE: To observe the skin surface microcirculation of acupoints of conception vessel, governor vessel and thoroughfare vessel in patients with primary dysmenorrhea using laser speckle contrast imaging (LSCI), and provide acupoint selection basis of acupuncture-moxibustion for primary dysmenorrhea. METHODS: Ninety-nine healthy female college students with regular menstrual cycles (normal group) and 94 female college students with primary dysmenorrhea (dysmenorrhea group) were recruited. Before menstrual period, on the first day of menstruation, and on the third day after menstruation, LSCI was used to observe the surface microcirculation at the abdominal acupoints of conception vessel, i. e. Yinjiao (CV 7), Qihai (CV 6), Shimen (CV 5), Guanyuan (CV 4), Zhongji (CV 3) and Qugou (CV 2), acupoints of thoroughfare vessel, i. e. Huangshu (KI 16), Zhongzhu (KI 15), Siman (KI 14), Qixue (KI 13), Dahe (KI 12), Henggu (KI 11) and acupoints of lumbosacral region of governor vessel, i. e. Xuanshu (GV 5), Mingmen (GV 4), Yaoyangguan (GV 3), Yaoshu (GV 2) as well as two non-acupoints. RESULTS: Before menstrual period, there was no significant difference in the surface blood perfusion of the acupoints between the dysmenorrhea group and the normal group (P>0.05). On the first day of menstruation, the surface blood perfusion of Xuanshu (GV 5), Mingmen (GV 4), Yaoyangguan (GV 3) and right Huangshu (KI 16) in the dysmenorrhea group was higher than that in the normal group (P<0.05, P<0.01). On the third day after menstruation, the surface blood perfusion of the right Henggu (KI 11) in the dysmenorrhea group was lower than that in the normal group (P<0.05). CONCLUSION In patients with primary dysmenorrhea, on the first day of menstruation, the surface blood perfusion of Xuanshu (GV 5), Mingmen (GV 4), Yaoyangguan (GV 3) of governor vessel, and the right Huangshu (KI 16) of thoroughfare vessel is increased, while on the third day after menstruation, the surface blood perfusion of the right Henggu (KI 11) of thoroughfare vessel is decreased. These findings might provide a basis for acupoint selection in the acupuncture-moxibustion treatment of primary dysmenorrhea.

OBJECTIVE: To observe the clinical efficacy of different treatment frequency of auricular bloodletting combined with auricular point sticking for acne vulgaris. METHODS: A total of 90 patients with acne vulgaris were randomized into a treatment group 1 (30 cases, 2 cases dropped off), a treatment group 2 (30 cases, 4 cases dropped off) and a treatment group 3 (30 cases, 5 cases dropped off). Combination therapy of auricular bloodletting and auricular point sticking at Fei (CO₁₄), Shenmen (TF₄), Neifenmi (CO₁₈) and Shenshangxian (TG₂p) were given once a week, twice a week and 3 times a week in the treatment group 1, the treatment group 2 and the treatment group 3 respectively, 4 weeks were as one course and totally 3 courses were required in the 3 groups. Before treatment and after 1, 2, 3 courses of treatment, the scores of global acne grading system (GAGS), skin lesion and quality of life-acne (QoL-Acne) were observed, the clinical efficacy was evaluated after 3 courses of treatment and the recurrence rate was evaluated in follow-up of 1 month after treatment in the 3 groups. RESULTS: Compared before treatment, the scores of GAGS and skin lesion were decreased at each time point in the 3 groups (P<0.05), the QoL-Acne scores were increased after 1 course of treatment in the treatment group 3 and after 2, 3 courses of treatment in the 3 groups (P<0.05). There were no statistical differences in scores of GAGS, skin lesion and QoL-Acne among the 3 groups (P>0.05). There were no statistical differences in effective rate and recurrence rate among the 3 groups (P>0.05). CONCLUSION Different frequency of auricular bloodletting combined with auricular point sticking have similar efficacy in treating acne vulgaris, all can improve the skin lesion and quality of life in patients with acne vulgaris, and have a cumulative effect and good long-term curative effect.

Traditional Chinese medicine （TCM） and western medicine have their respective advantages and limitations in the diagnosis and treatment of common otorhinolaryngology head and neck diseases. Although the integrated TCM and western medicine exhibits definite curative effects， there is no consensus on the otorhinolaryngology head and neck diseases responding specifically to TCM or integrated TCM and western medicine， as well as the diagnosis and treatment schemes. The China Association of Chinese Medicine （CACM） thus organized the otorhinolaryngology head and neck specialists of both TCM and western medicine to discuss the etiology， pathogenesis， and clinical diagnosis and treatment methods of common otorhinolaryngology head and neck diseases with the results of multiple clinical trials taken into account. The acute pharyngitis， chronic pharyngolaryngitis， paraesthesia pharyngis， hysterical aphasia， allergic rhinitis， subjective tinnitus， and otogenic vertigo were confirmed to respond specifically to TCM or integrated TCM and western medicine. Then a mutually agreed diagnosis and treatment scheme and recommendation with integrated TCM and western medicine was formulated as a reference for clinical practice， thus benefiting more patients.

BACKGROUNDProlonged gonadal hormone deficiency in patients with idiopathic hypogonadotropic hypogonadism (IHH) may produce adverse effects on the endocrine homeostasis and metabolism. This study aimed to compare basal serum adrenocorticotropic hormone (ACTH) and cortisol levels between male IHH patients and healthy controls. Moreover, this study compared the basal hypothalamic-pituitary-adrenal (HPA) axis in patients with and without nonalcoholic fatty liver disease (NAFLD), and also evaluated the relationship between basal HPA axis and NAFLD in male IHH patients.

METHODSThis was a retrospective case-control study involving 75 Chinese male IHH patients (mean age 21.4 ± 3.8 years, range 17-30 years) and 135 healthy controls after matching for gender and age. All subjects underwent physical examination and blood testing for serum testosterone, luteinizing hormone, follicle-stimulating hormone, ACTH, and cortisol and biochemical tests.

RESULTSHigher basal serum ACTH levels (8.25 ± 3.78 pmol/L vs. 6.97 ± 2.81 pmol/L) and lower cortisol levels (366.70 ± 142.48 nmol/L vs. 452.82 ± 141.53 nmol/L) were observed in male IHH patients than healthy subjects (all p<0.05). IHH patients also showed higher metabolism parameters and higher prevalence rate of NAFLD (34.9% vs. 4.4%) than the controls (all P < 0.05). Basal serum ACTH (9.91 ± 4.98 pmol/L vs. 7.60 ± 2.96 pmol/L) and dehydroepiandrosterone sulfate (2123.7 ± 925.8 μg/L vs. 1417.1 ± 498.4 μg/L) levels were significantly higher in IHH patients with NAFLD than those without NAFLD (all P < 0.05). We also found that basal serum ACTH levels were positively correlated with NAFLD (r = 0.289,p<0.05) and triglyceride levels (r = 0.268, P< 0.05) in male IHH patients. Furthermore, NAFLD was independently associated with ACTH levels in male IHH patients by multiple linear regression analysis.

CONCLUSIONSThe male IHH patients showed higher basal serum ACTH levels and lower cortisol levels than matched healthy controls. NAFLD was an independent associated factor for ACTH levels in male IHH patients. These preliminary findings provided evidence of the relationship between basal serum ACTH and NAFLD in male IHH patients.

Objective To evaluate the clinical efficacy and safety of oxaliplatin combined with 5 -fluorouracil ( 5 -FU ) in the treatment of advanced esophageal cancer.Methods Sixty -five patients with advanced esophageal cancer were randomly divided into control group (n=32) and treatment group(n=33).The patients in two groups were treated with conventional fractionated radiotherapy at 2.0 Gy, with a total dose of 56-66 Gy.Chemotherapy was delivered on the first day of radio-therapy.Control group was received 52.5 mg · m-2 · d -1 cisplatin on day 1 +500 mg · m-2 · d -1 5 -Fu on day 1 -5 through intravenous drip.Treatment group was received 85-100 mg · m-2 · d-1 oxaliplatin through intravenous infusion two hours on day 1 +500 mg · m -2 · d -1 5-FU on day 1 -5 through intravenous drip.Every 4 weeks was a cycle, all received two cycles of chemotherapy.The clinical efficacy and incidence of adverse drug reactions between two groups were compared.Results After treatment, total effective rates were 81.80% and 75.00%in treatment group and control group, disease control rates were 96.97% and 93.75% in treatment group and control group, one year survival rates were 77.30%and 68.20%in treatment group and control group, with no statistical difference between the two groups( P>0.05).After treatment, the occurrence rate of nausea and vomiting in treatment group was signifi-cantly lower than that in control group(34.60%vs 62.50%, P<0.05) .The occurrence rate of sensory neuropathy in treatment group was significantly higher than that in control group(24.00%vs 0, P<0.05).Conclusion Concurrent chemoradiation with oxaliplatin and 5-FU for the treatment of advanced esophageal cancer exhibited a definitive clini-cal efficacy with clinical manageable adverse drug reactions.

Objective To explore the relationship and interaction of elevated fasting glucose and hypertension on cardiocerebral vascular disease.Methods 10 054 males and females were recruited for our cross-sectional study during May 2007 to August 2007.Unconditional logistic regression was used to analysis the relationship between fasting glucose and hypertension on cardiocerebral vascular disease.A product of fasting glucose and hypertension was added to the logistic regression model to evaluate the multiplicative interaction and relative excess risk of interaction(RERI),attributable proportion(AP)of interaction and synergy index(S)was applied to evaluate the additive interaction of the two factors.Bootstrap was used to calculate 95％ confidence intervals(CI)of RERI,AP and S.Results After adjusting age,gender,smoking,drinking,body mass index(BMI)and region,the product of fasting glucose and hypertension was not statistically significant,which means there was no multiplicative interaction between the two.But the additive indexes RERI,AP and S with 95％ CI of diabetes and hypertension were 0.64(0.03,1.25),0.27(0.01,O.47)and 1.83(1.02,5.13)respectively,which means significant additive interaction was shown between the two on cardiovascular disease but not no stroke.And there were no additive interaction bmtween impaired fasting glucose on cardiovascular disease or stroke.Conclusions Hypertension was independently related to cardiovascular disease and stroke in Beijing citizens,and diabetes were independently related to stroke.There was additive interaction between diabetes and hypertension on cardiovascular disease.

Objective To observe the prevalence, awareness, treatment and control rate of hypertension and to evaluate the 10-year absolute risk of coronary heart disease (CHD) and ischemic cardiovascular disease (ICVD) in Chinese cardiovascular physicians. Methods A total of 4032cardiovascular physicians (28 to 79 years old) from 386 hospitals in 31 provinces, autonomous regions and municipalities were randomly selected and received an epidemiologic survey of prevalence, awareness, and control of hypertension and evaluations of CHD and ICVD risk. Results The prevalence of hypertension in Chinese cardiovascular physicians was 13.1%. The awareness rate of hypertension in Chinese cardiovascular physicians was 81.7%. Hypertension treatment rate was 69. 6% and blood pressure control rate was 44. 6%. The prevalence of hypertension was higher in male physicians than in female physicians before the age of 55 years old. Ten-year absolute risk of CHD and ICVD was 0. 08 and 0. 03 in hypertensive physicians compared to 0. 03 and 0. 01 in non-hypertensive physicians. Conclusions The results show suboptimal awareness, treatment and control rate in Chinese cardiovascular physicians for their own hypertension status. Physicians suffering from hypertension face higher risk for cardiovascular disease. It is therefore necessary to improve the self-monitoring of blood pressure in Chinese cardiovascular physicians.

Objective To investigate the relationship between marital status and prevalence of peripheral arterial disease (PAD) among Beijing residents.Methods From May to August 2007,10 054 residents were interviewed to collect demographic and biochemistry information.Generalized linear mixed model was used to determine the relationship between marital status and prevalence of PAD in terms of OR and corresponding 95％ confidence intervals.Results After adjusting the traditional risk factors including age,area,lipid,serum glucose,blood pressure,central obesity,smoking,drinking and physical activities for PAD,relationship between marital status and prevalence of PAD was found both at the female and male groups of those with 45 years or older.Compared with married men,OR and 95％ confidence interval of PAD for unmarried men were 1.56 (0.39 - 6.35 ) and 1.61 (0.77 - 3.35 ) for those with 45 years younger and older respectively,while compared with married women,OR and 95％ confidence interval of PAD for unmarried women were 0.75 ( 0.22 - 2.57 ) and 1.78 ( 1.23 - 2.58 ) for those with 45 years younger and older respectively.Significant differences ( all P ＜ 0.01 ) were found between married and unmarried women of 45 years or older in terms of age,waist circumstances,systolic blood pressure,fasting glucose,total cholesterol,low-density lipoprotein and smoking.Conclusions Peripheral arterial disease was related to marital status for women of 45 years or older among Beijing residents.Estrogen level and the distribution of other PAD risk factors might contribute to the observed finding.

OBJECTIVETo study the changes of platelet in May-Hegglin anomaly (MHA) and the molecular pathogenesis mechanism.

METHODSPeripheral blood was drawn from the MHA proband, her father and her uncle. Platelet count and morphology were examined by automatic blood cell counter and microscopy, respectively. The platelet membrane protein was examined by flow cytometry. Membrane antibodies were determined by ELISA. PCR was used to amplify the exons 25, 31 approximately 32, 38 and 40 of the MYH 9 gene in the MHA patient and her diseased father. Furthermore, PCR products were sequenced, a specific point mutation was identified and inclusions (Dohle's body) in the neutrophil was detected by indirect immunofluorescence technique.

RESULTSIt was proved that in MHA patients, platelet count was higher by cell counter than by microscope (P < 0.01). Giant platelet was 94% but platelet membrane proteins (CD41, CD61, CD42A, CD42b) were in normal range. Membrane antibodies was undetectable. An A5521G mutation (GAG-->AAG) in the exon 38 was found in the proband and her diseased father, resulting in a characteristic change of NMMHC-A1841 (Glutamic acid-->Arginine), which was not found in other members of the family and in normal controls. Spindle-like inclusions with fluorescence were clearly displayed in neutrophil cytoplasm.

CONCLUSIONThe molecular pathogenesis mechanism of May-Hegglin anomaly is the mutation in MYH 9 gene.
