1.Effect of flow rehabilitation nursing on functional recovery after replantation of severed finger

Objective To explore the effect of process rehabilitation nursing on the joint range of motion,grip strength,replantation survival rate and functional recovery of severed fingers after replantation.Methods A total of 70 patients undergoing replantation of severed fingers who were admitted to Linping District,the Second Affiliated Hospital of Zhejiang University School of Medicine from February 2019 to December 2022 were selected.The patients were divided into the control group(n=35)with routine nursing,and the research group(n=35)with routine rehabilitation nursing,and the nursing effects of the two groups were compared.Results After nursing,the excellent and good rate of the research group was 97.14%(34/35),which was significantly higher than that of the control group was 82.86%(29/35)(P<0.05);After nursing,the range of motion,grip strength,replantation survival rate and functional score of the finger joint in the study group were higher than those in the control group(P<0.05);After 3 months of nursing,the excellent and good rate of joint range of motion in the study group was 94.29%(33/35),which was significantly higher than that in the control group was 74.29%(26/35)(P<0.05).Conclusion The application of process rehabilitation nursing in patients with replantation of severed fingers has significantly improved joint activity and restored the function of severed fingers,which can be popularized.
2.Comparative analysis on the identification of global Salmonella based on average nucleotide identity and 16S rDNA technology

Objective To evaluate the ability of average nucleotide identity(ANI)and 16S rDNA technology on the identification of Salmonella.Methods The genomes and corresponding serovars of global Salmonella were downloaded in batch from the GenBank database.The classical strains of Salmonella were used as typing strains.The ANI analysis was conducted by the fastANI software according to the silent parameters.The species and serovars of Salmonella were identified by their 16S rDNA using the online software SpeciesFinder.Results Among the downloaded 2 306 genomes,1 767 strains of Salmonella had 178 serovars,with 323 strains(18.3%)of Salmonella Typhimurium and 300 strains(17.0%)of Salmonella Enteritidis being the most common.The ANI analysis showed that with a 95%threshold,only 30 strains(1.3%)of Salmonella were assigned to a specific subspecies,while the remaining 2 276 strains(98.7%)of Salmonella could be assigned to 2-5 subspecies.When the threshold was 97%,all 2 306 strains(100%)of Salmonella could be assigned to a specific subspecies.Based on the analysis of 16S rDNA,only 1 072 strains(46.5%)of Salmonella were identified,of which 95.2%(1 021/1 072)of Salmonella subspecies were completely consistent with the results of ANI(≥97%)analysis.Only 2.4%(19/784)of Salmonella strains showed consistent results with known serovars.Conclusion ANI is more suitable for the identification of Salmonella species and subspecies,and ANI≥97%can be used as the identification standard for Salmonella subspecies.The sensitivity of 16S rDNA for the identification of Salmonella still needs to be improved.
3.Characteristics of epidemiology and distribution of Salmonella Typhimurium worldwide based on NCBI database

Objective To analyze the prevalence and molecular characteristics of global Salmonella Typhimurium.Method The ge-nome and the corresponding serovars as well as meta-information of global Salmonella were downloaded in batch from the National Cen-ter for Biotechnology Information(NCBI)database using the online softwares,ResFinder,PlasmidFinder,Mobile Element Finder and MLST,to analyze the distribution of antibiotic resistant genes(ARGs),plasmids,mobile genetic elements(MGEs)and the sequence types(STs)among S.Typhimurium strains.Results A total of 101 ARGs were detected in 323 strains of S.Typhimurium,among which the most common was aac(6')-Iaa(322/323,99.69%),followed by sul2(155/323,47.99%),aph(3")-Ib(128/323,39.63%),aph(6)-Id(127/323,39.32%),tet(B)(109/323,33.75%),and blaTEM-1B(106/323,32.82%).Totally,36 plasmids were identified,among which IncFⅡ(S)(110/323,34.06%)and IncFⅠB(S)(108/323,33.44%)were the most common.Moreo-ver,376 MGEs were found,including 367 insertion sequences and nine transposons,among which MITEEc1(322/323,99.69%)and ISSen1(308/323,95.36%)were the most popular.Furthermore,323 strains of S.Typhimurium were assigned into 32 different STs,a-mong which,ST19(157/323,48.61%)and ST34(105/323,32.51%)were the most common accounting for over 82%.Among 115 strains of S.Typhimurium from human,18 STs were identified including ST19(52/115,45.22%)and ST34(39/115,33.91%)which were the most common.Conclusion S.Typhimurium carried multiple types of ARGs,in addition to the wide distribution of a large number of insertion sequences and plasmids,whch provide favorable conditions for the spread of drug resistance.Therefore,the meas-ures of prevention and control against the infection from this bacterium should be strengthened.
4.Effects of wrist hand thumb joint fixtures in infants to prevent the unplanned extubation of radial artery manometric tube in children with congenital heart disease

Objective:To understand clinical effect of a homemade infants wrist hand thumb joint fixtures to prevent the unplanned extubation (UEX) in radial arterial puncture tube in children with congenital heart disease.Methods:From October 2016 to October 2019, 150 cases infants patients with simple congenital heart disease in cardiac surgery of the First Affiliated Hospital of University of Science and Technology of China were selected as the research objects, and the radial artery invasive manometric tube was routinely indwelling after operation. Subjects were randomly divided into observation group and control group, with 75 cases in each group. In the observation group, the wrist joint of children was fixed with homemade fixation device. The control group used conventional fixation methods to fix the wrist joint of infants. The incidence of pressure injury and unplanned extubation were compared between the two groups.Results:There was 1 case (1.33%) pressure injury in the observation group and 2 cases (2.67%) in the control group, and there was no statistical significance in the two groups ( P>0.05). The unplanned extubation rate of radial artery pressure tube occurred in 5 cases (6.67%) in the observation group, and lower than that in the control group (17/75, 22.67%) ( χ2=7.67, P<0.01). When children in the state of agitation and very agitation, the incidence of unplanned extubation rate in the observation group was 0 and 4.35%(1/23), lower than that in the control group 15.79%(6/38) and 35.00% (7/20), respectively ( χ2=6.19, 6.64, both P<0.05). With the extension of ICU time, the incidence of unplanned extubation rate was increased. The incidence of unplanned extubation rate was 4.76% (2/42) and 9.09% (3/33) in the observation group and 18.18%(8/44) and 29.03% (9/31) in the control group when ICU time was ≤24 h and>24 h, respectively. The difference was statistically significant ( χ2=5.51, 4.17, both P<0.05). Conclusions:The homemade wrist thumb joint fixtures device for infants does not increase the pressure injury of the fixed part, and can reduce the incidence of unplanned extubation of radial artery manometric tube in children with congenital heart disease.

The impact of parental obesity on offspring health attracts increasing attention with the rising obese population worldwide. Much more studies have concentrated on the influences of maternal obesity, while studies regarding paternal obesity were relatively few and mainly focused on glucose and insulin regulation, adiposity, etc. Several large cohort studies and animal experiments have shown that paternal obesity can damage the neuropsychological development in offspring, which has become a risk factor for autistic spectrum disorders, cognitive disorders, and lower intelligence quotient, and epigenetic studies have explored the related mechanisms. This review summarizes the progress in this field, aiming to provide a reference for basic and clinical research in the future.
6.Fasting Blood Glucose, Cholesterol, and Risk of Primary Liver Cancer: The Kailuan Study

Purpose:
The influence of fasting blood glucose (FBG) and cholesterolemia primary liver cancer (PLC) in china was analyzed via a large prospective cohort study based on a community population, and the combined effects between them were investigated.
Materials and Methods:
Overall, 98,936 staff from the Kailuan Group who participated in and finished physical examinations between 2006 and 2007 were included in the cohort study. Their medical information was collected and they were followed up after examination. The correlations of serum FBG or TC with PLC were analyzed. Then, we categorized all staff into four groups: normal FBG/ non-hypocholesterolemia, normal FBG/hypocholesterolemia, elevated FBGon-hypocholesterolemia, elevated FBG/hypocholesterolemia and normal FBG/ non-hypocholesterolemia was used as a control group. The combined effects of elevated FBG and hypocholesterolemia with PLC were analyzed using the Age-scale Cox proportional hazard regression model.
Results:
During 1,134,843.68 person*years follow up, a total of 388 PLC cases occured. We found the elevated FBG and hypocholesterolemia increases the risk for PLC, respectively. Compared with the non-hypocholesterolemiaormal FBG group, the risk of PLC was significantly increased in the non-hypocholesterolemia/elevated FBG group (HR=1.19,95%CI 0.88–1.62) and hypocholesterolemiaormal FBG group (HR=1.53,95%CI 1.19–1.97), and in the hypocholesterolemia/elevated FBG group (HR=3.16 95%CI2.13-4.69). And, a significant interaction effect was found of FBG and TC on PLC. All results were independent from the influence of liver disease.
Conclusion
Elevated serum FBG and hypocholesterolemia are risk factors for PLC, especially when combined. Thus, for the prevention and treatment of PLC, serum FBG and TC levels should be investigated.
7.Establishment of HPLC Fingerprint of Chinese Gall Leaven and Content Determination of 5 Components

OBJECTIVE:To establis h HPLC fingerprint of Chinese gall leaven ,simultaneous determination of 5 components and optimization of distillers ’grains. METHODS :HPLC method was adopted. The determination was carried out on Waters Symmetry Shield TM RP18 column with mobile phase consisted of acetonitrile- 0.1% trifluoroacetic acid aqueous (gradient elution )at the flow rate of 0.8 mL/min. The column temperature was 30 ℃,and detection wavelength was set at 280 nm. The sample size was 5 µL. Using gallic acid as reference ,HPLC fingerprints of 14 batches of sample were drawn. The similarity evaluation was performed by using Similarity Evaluation System of Chromatogram Fingerprint of TCM (2012 edition)to determine common peaks;SPSS 20.0 software was used for cluster analysis. RESULTS :There were 11 common peaks in 14 batches of samples ,and the similarity range of 14 batches of samples was 0.424-0.998,A1-A5 was less than 0.850;5 components were identified in B 1-B9 sample,i.e. gallic acid ,(-)-epigallocatechin,methyl gallate ,2,4,6-tri-O-galloyl-α-D-glucose and 2,4,6-tri-O-galloyl- β-D-glucose. The results of cluster analysis showed that 14 batches of samples could be grouped into 3 categories,i.e. A1-A5 into one category,B1 and B 6 into one category ,B2-B5 and B 7-B9 into one category. The linear ranges of above 5 components were 29.96-599.2 μg/mL(r=0.999 6),0.832-416 μg/mL(r=0.999 6),0.102-51 μg/mL(r=0.999 8),0.286 4-143.2 μg/mL(r=0.999 8), 0.286 4- 143.2 μg/mL(r=0.999 8),respectively. The limits of quantitation were 0.060,0.104,0.017,0.029,0.057 μg/mL;the limits of detection were 0.018,0.031,0.005,0.009,0.017 μg/mL,respectively. RSDs of precision ,stability,reproducibility and durability tests were all lower than 3%. The recoveries were 97.16%-101.88%(RSD=1.60%,n=6),96.98%-99.24%(RSD= 0.85%,n=6),97.7%-101.64%(RSD=1.54%,n=6),97.77%-103.08%(RSD=1.82%,n=6),98.16%-101.88%(RSD=1.24%, n=6),respectively. CONCLUSIONS:The established HPLC fingerprint and cluster analysis can be used to evaluate the quality of Chinese gall leaven. 8.Comparative Study on the Fingerprints and Content of 5 Kinds of Nucleosides in Raw Products and Different Processed Products of Pinellia pedatisecta

OBJECTIVE:To establish the fingerprint of raw produc ts and different processed products of Pinellia pedatisecta , to determine the contents of 5 kinds of nucleosides ,and to compare the differences of components between the raw products and processed products. METHODS :P. pedatisecta raw products ,processed products by Processing Standard of Chinese Medicine in Henan Province (called“Standard processed product ”for short )and processed products by new integrated processing technology in the production area (called“new integrated processed product ”for short )were collected as investigation objects (12 batches of each). The determination was performed on SymmetryShield RP 18 column with mobile phase consisted of acetonitrile (A)-0.1% acetic acid aqueous water solution (B)(gradient elution )at the flow rate of 0.8 mL/min,with the column temperature of 30 ℃, the detection wavelength of 270 nm,and the injection volume of 15 µL. HPLC fingerprints of 3 kinds of P. pedatisecta samples were established by using Similarity Evaluation System of TCM Chromatographic Fingerprints (2004 A version ) ,and the similarity of fingerprints was evaluated. The chromatographic peaks were identified by comparing with the reference chromatogram. Five nucleosides (adenine,hypoxanthine,uridine,xanthine,inosine)were quantitatively analyzed. SPSS 21.0 software was used for cluster analysis of 36 batches of samples. RESULTS :The results of fingerprint and content determination met the relevant requirements. The similarity of 3 kinds of sample with their control fingerprint were all greater than 0.990. There were 22 common peaks in the raw products of P. pedatisecta ,and 16 common peaks were identified in the 2 kinds of processed products (the same 6 peaks disappeared from 2 kinds of processed products ). Fivecomponents were identified in 3 kinds of samples ,such as adenine(peak 3),hypoxanthine(peak 7),uridine(peak 8), 1064056472@qq.com xanthine(peak 9)and inosine (peak 11). Results of content determination showed that total contents of 5 kinds of nucleosides in 2 kinds of proc essed products were all· decreased;the contents of them in descending order w as raw product >new i ntegrated processed products >Standard processed products. Results of cluster analysis showed that 36 batches of samples could be clustered into 2 categories,i.e. raw product was clustered into one category and 2 kinds of processed products into other one . CONCLUSIONS :Established method is stable , feasible and suitable for the quality evaluation of raw products and different processed products of P. pedatisecta . Fingerprints have changed significantly and the total content of 5 kinds of nucleosides in P. pedatisecta are all decreased after processing ,but that of new integrated processed products is slightly higher than that of Standard processed products .
9.Coverage rate and adverse reactions of National Immunization Program vaccines in children with spinal muscular atrophy: a cross-sectional retrospective cohort study

Objective:To investigate the coverage rate and the adverse reactions of National Immunization Program vaccines in children with spinal muscular atrophy (SMA).Methods:A cross-sectional retrospective cohort study was carried out from July 2016 to June 2019, 192 children (116 boys and 76 girls) with SMA registered by Capital Institute of Pediatrics and 191 healthy children (115 boys and 76 girls) vaccinated in Chaoyang Olympic Village Community Health Service Center from July 2016 to December 2018 were included. Questionnaire survey was designed to investigate the vaccination coverage rate and associated adverse events. The t-test and χ 2 test were used to compare the difference between SMA patients and healthy children. Results:The coverage rate of age-appropriate immunization in SMA children was 62.0% (119/192) in general, and were 52.2% (12/23), 55.7% (68/122), and 83.0% (39/47) for SMA type 1-3 patients, respectively (χ 2=12.23, P=0.002). The vaccination coverage rates of Bacillus Calmette-Guerin (BCG) vaccine, the 3 rd dose of hepatitis B, the 3 rd dose of polio, the 3 rd dose of diphtheria-pertussis-tetanus, the 1 st dose of meningococcal polysaccharide group A, the 1 st dose of measles or measles and rubella vaccine, the 1 st dose of Japanese encephalitis vaccine, hepatitis A, measles-mumps-rubella, and group A+C meningococcal polysaccharide vaccine were 100.0% (192 cases), 94.3% (181 cases), 81.8% (157 cases), 88.5% (170 cases), 83.9% (161 cases), 76.6% (147 cases), 80.2% (154 cases), 68.2% (131 cases), 69.8% (134 cases), 54.7% (105 cases), respectively. Among the 73 patients who did not have their planned immunization completed, 57 cases (78.1%) gave up the vaccination due to parents′ concern of potential aggravation of their disease, and 16 cases (21.9%) had the plan discontinued by the immunization department because of the disease. Fever, local redness and swelling were the most common side-effects after vaccination both in SMA patients and healthy children (19.8% (38/192) vs. 18.8% (36/191) , χ 2=0.055, P=0.815). The main abnormal reactions of vaccination were rash and neurovascular edema, without significant difference between these two groups (2.6% (5/192) vs. 3.7% (7/191), χ 2=0.355, P=0.551). The coverage rate of Influenza and pneumococcal vaccine in SMA patients were 22.4% (43 cases) and 31.8% (61 cases), respectively. The incidence of pneumonia in the SMA patients decreased from 59.0% (23/39) to 41.0% (16/39) after vaccination. And none of the Influenza vaccinated patients had the flu in the year of vaccination. Conclusions:The coverage rate of National Immunization Program vaccines in the SMA children is low, especially in type 1 SMA patients, which is mainly due to their guardians′ concern of potential adverse events, even though the incidence of adverse reactions is similar in SMA patients and healthy children. Influenza and pneumococcal vaccine can reduce the risk of pneumonia and flu in children with SMA effectively.
10.Comparative analysis of lncRNA-mRNA co-expression between Keshan disease and dilated cardiomyopathy

Objective By constructing the differential expression profile of lncRNA/mRNA in peripheral blood plasma of patients with Keshan disease (KSD) and dilated cardiomyopathy (DCM),to explore the commonality and characteristics of the two diseases in molecular mechanism.Methods Ten patients with chronic KSD were selected in the severe disease area of KSD in Shandong Province,and 10 cases of DCM and 10 healthy subjects (control group) were selected in non-KSD area.Blood of elbow vein was collected and plasma was separated.RNA-seq technology was used to construct the differential lncRNA/mRNA expression profile between KSD and control group,DCM and control group,and co-expression and specific expression of partial genes in KSD and DCM were analyzed through Wien analysis.The lncRNA-mRNA co-expression network maps of specific part of KSD,specific part of DCM and common part of the two diseases were constructed,and Gene Ontology (GO) analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis were applied to distinguish the biological function of the two diseases.Results Compared with control group,102 dysregulated mRNAs and 22 dysregulated lncRNAs showed the same trend in KSD and DCM.And 3 606 mRNAs and 451 lncRNAs were only differentially expressed in KSD group,217 mRNAs and 137 lncRNAs were only differentially expressed in DCM group.The differentially expressed lncRNA/mRNA shared between the KSD and DCM groups were mainly about viral transcription,immuno-inflammatory response,oxidative stress signaling pathways.The KSD specific lncRNA/mRNA mainly participated in cell membrane damage and viral myocarditis.The DCM specific lncRNA/mRNA mainly regulated mitochondrial structure and oxidative phosphorylation related enzymes.Conclusion The differentially expressed lncRNA/mRNA shared in KSD and DCM groups are mainly involved in viral transcription,oxidative stress signaling pathways;KSD specific lncRNA/mRNA are mainly related to cell membrane damage and viral myocarditis;DCM specific lncRNA/mRNA mainly regulate mitochondrial structure.
