1.Traditional Chinese medicine phenomics research on glycolipid metabolism disorder: a review

Abstract
Traditional Chinese medicine (TCM) has demonstrated unique advantages in the prevention and treatment of chronic diseases such as glycolipid metabolism disorder. However, its widespread application has been hindered by the unclear biological essence of TCM syndromes and therapeutic mechanisms. As an emerging interdisciplinary field, phenomics integrates multi-dimensional data including genome, transcriptome, proteome, metabolome, and microbiome. When combined with TCM's holistic philosophy, it forms TCM phenomics, providing novel approaches to reveal the biological connotation of TCM syndromes and the mechanisms of herbal medicine. Taking glycolipid metabolism disorder as an example, this paper explores the application of TCM phenomics in glycolipid metabolism disorder. By analyzing molecular characteristics of related syndromes, TCM phenomics identifies differentially expressed genes, metabolites, and gut microbiota biomarkers to elucidate the dynamic evolution patterns of syndromes. Simultaneously, it deciphers the multi-target regulatory networks of herbal formulas, demonstrating their therapeutic effects through mechanisms including modulation of insulin signaling pathways, improvement of gut microbiota imbalance, and suppression of inflammatory responses. Current challenges include the subjective nature of syndrome diagnosis, insufficient standardization of animal models, and lack of integrated multi-omics analysis. Future research should employ machine learning, multimodal data integration, and cross-omics longitudinal studies to establish quantitative diagnostic systems for syndromes, promote the integration of precision medicine in TCM and western medicine, and accelerate the modernization of TCM.
2.Research advances in neutron shielding materials

With the extensive application of nuclear technology in industry, agriculture, and medicine, the safety issues associated with neutron radiation have become increasingly prominent. Due to their high penetrability and strong ionization effect, neutrons can cause serious health risks by directly damaging DNA or inducing secondary γ radiation. Therefore, the neutron radiation protection has become a core challenge in radiation protection, especially the research and development of neutron shielding materials. To ensure the safe development of nuclear technology, neutron shielding materials are indispensable and constitute a fundamental core technology for radiation protection. This paper reviews the theory of neutron radiation protection and the research progress of neutron shielding materials, with a focus on the current application status and existing problems of neutron shielding materials. This article also discusses the future development trends. This review aims to provide theoretical support and technical references for the safe application and development of nuclear technology.
3.Calcium phosphate combined with recombinant human bone morphogenetic protein-2 in repair and reconstruction of tibial infectious bone defects

BACKGROUND:Although the clinical application of Masquelet technology has achieved extensive success,the research on optimizing all aspects of Masquelet technology is still being carried out.The focus of doctors is to speed up bone healing and shorten bone healing time after bone grafting. OBJECTIVE:To observe the effect of calcium phosphate combined with recombinant human bone morphogenetic protein-2 in repairing tibial infectious bone defects. METHODS:Thirty-one patients with tibial infectious bone defects were selected from The People's Hospital of Jianyang City from June 2017 to June 2022.They were treated with the Masquelet membrane induction technique.During the second stage of operation,they were divided into a control group(n=15)and a study group(n=16)according to different bone graft materials.Patients in the control group were implanted with autologous bone/allogeneic bone particles,and those in the study group were implanted with calcium phosphate combined with recombinant human bone morphogenetic protein-2/autologous bone particles.Six months after the second stage operation,peripheral blood inflammatory indexes such as white blood cell count,C-reactive protein,and erythrocyte sedimentation rate were detected.Imaging bone healing time,bone healing X-ray score,bone defect healing classification,and adjacent joint function were recorded.The presence of nail track infection,implant absorption,pain,and infection in the bone extraction area were observed. RESULTS AND CONCLUSION:(1)White blood cell count,erythrocyte sedimentation rate,and C-reactive protein levels of the two groups 6 months after the second stage operation were significantly lower than those before the first stage operation(P<0.05).There was no significant difference in each index between the two groups(P>0.05).(2)Bone healing time in the study group was shorter than that in the control group(P<0.05).(3)The Samantha X-ray score of the study group 6 months after the second stage operation was higher than that of the control group(P<0.05).The excellent and good rate of bone defect healing and adjacent joint function of the study group was higher than that of the control group(P<0.05).There was no significant difference in the recurrence rate and complication rate between the two groups(P>0.05).(4)These findings indicate that the effect of calcium phosphate combined with recombinant human bone morphogenetic protein-2 during the second stage operation of the Masquelet membrane induction technique in the treatment of tibial infectious bone defect is good and safe.
4.Curative Effect and Mechanism of Traditional Chinese Medicine in Treatment of Diabetic Gastroenteropathy: A Review

Diabetic gastroenteropathy is a serious chronic complication that accompanies the progression of diabetes mellitus, severely impacting patients' quality of life and overall health. Nearly half of diabetic patients experience symptoms such as nausea, vomiting, early satiety, abdominal distension, and abdominal pain, which increases their anxiety and depression, prompting frequent medical visits and further burdening the healthcare system. In-depth research into the pathogenesis of diabetic gastroenteropathy has identified several core mechanisms, including hyperglycemia, autonomic and enteric nervous system dysfunction, abnormal secretion of gastrointestinal hormones, macrophage polarization, brain-gut axis dysregulation, microRNA deficiency, and oxidative stress-induced damage and apoptosis of interstitial cells of Cajal (ICC). Current clinical treatments mainly rely on prokinetic and antiemetic drugs. However, their notable adverse effects and diminishing efficacy with long-term use remain pressing issues. Traditional Chinese medicine (TCM), with its unique theoretical framework and extensive practical experience, potent in prescription formulation and acupoint selection guided by holistic concepts and syndrome differentiation, has gradually become an important option for treating diabetic gastroenteropathy. Numerous studies have confirmed that mechanisms include improving gastrointestinal hormone secretion, repairing ICC damage, regulating the nervous system, reducing oxidative stress, and modulating the brain-gut axis. These findings provide new insights into the treatment of diabetic gastroenteropathy. This article summarized the pathogenesis of diabetic gastroenteropathy and reviewed recent research on Chinese medicine and acupuncture-moxibustion therapy in improving gastrointestinal motility for diabetic gastroenteropathy treatment, aiming to offer clinical treatment insights and highlight the need for further research to explore comprehensive and individualized treatment approaches, providing better strategies for managing diabetic gastroenteropathy.
5.Deep learning model based on integrated 18F-FDG PET/MRI for evaluating cerebral metabolism around cerebral infarction

Objective To investigate the value of deep learning(DL)model based on integrated 18F-FDG PET/MRI for evaluating cerebral metabolic status around cerebral infarction.Methods A total of 46 patients with cerebral infarction caused by unilateral internal carotid artery(ICA)or middle cerebral artery(MCA)steno-occlusion were retrospectively collected.Based on integrated 18F-FDG PET/MRI,DL model was used to automatically segment cerebral infarction area.Asymmetry index(AI)was used to evaluate the volume of reduced metabolic areas in the segmented affected frontal lobe,temporal lobe,parietal lobe,occipital lobe and cerebral hemisphere of cerebral infarction area as well as their proportions,while their correlations with National Institutes of Health stroke scale(NIHSS)score of neurological function were analyzed.Results Among 46 patients,the volume of decreased metabolism in the affected temporal lobe,parietal lobe and cerebral hemisphere was(41.35±10.52)ml,(65.58±14.82)ml and(178.89±34.23)ml,respectively,all positively correlated with NIHSS scores(rs=0.359,0.343,0.362,all P<0.05).The proportion of the reduced metabolic volume in the affected frontal lobe,temporal lobe,parietal lobe and cerebral hemisphere was(45.68±10.35)%,(42.32±10.19)%,(45.05±9.44)%and(44.11±8.63)%,respectively,all positively correlated with NIHSS scores(rs=0.344,0.340,0.439,0.393,all P<0.05).Conclusion DL model based on integrated 18F-FDG PET/MRI was of important clinical value for evaluating cerebral metabolic state around cerebral infarction.
6.Hederagenin alleviates neuroinflammation response by regulating glutamate-induced ferroptosis in HT22 cells

Objective To explore the regulatory role of hederagenin(HG)on glutamate(Glu)-in-duced ferroptosis and corresponding inflammatory responses in mouse hippocampal neuron HT22 cells and investigate its potential mechanisms.Methods HT22 cells were randomly divided into control,Glu and HG groups(n=3).The cells of the control group received no treatment,the cells of the Glu group were treated with 35 mmol/L Glu for 24 h to establish a cellular model of ferroptosis in Alzheimer's disease,and the cells of the HG group were treated with 0.5 μmol/L HG and 35 mmol/L Glu for 24 h simultaneously.FerroOrange fluorescent probe was used to de-tect intracellular Fe2+.The production of reactive oxygen species(ROS),mitochondrial membrane potential,and levels of inflammatory factors TNF-α,IL-1β and IL-6 in the cells were assessed.Finally,the expression of the key regulator of iron death,glutathione peroxidase 4(GPX4)was measured.Results Compared to the control group,the levels of intracellular Fe2+,ROS,TNF-α,IL-1β,and IL-6 were significantly elevated,while the mitochondrial membrane potential was obvi-ously reduced in the Glu group(P<0.05,P<0.01).The HG group had significantly decreased Fe2+,ROS,TNF-α,IL-1β,and IL-6 and enhanced mitochondrial membrane potential than the Glu group(P<0.05,P<0.01).The GPX4 expression was significantly lower in the Glu group than the control group(1.00±0.02 vs 0.46±0.04,P<0.01),and was notably higher in the 0.5 and 1.0 μmol/L HG groups when compared to the Glu group(0.64±0.03 and 0.59±0.05 vs 0.46±0.04,P<0.01).Conclusion HG inhibits ferroptosis by regulating GPX4 expression,and thereby effec-tively alleviates the inflammatory response.
7.Analysis of control status and related influencing factors of plaque in patients with periodontal disease

Objective To investigate the status of plaque control in patients with periodontal disease and its related in-fluencing factors.Methods A total of 350 periodontal disease patients admitted to the Hospital of Stomatological Xi'an Jiao-tong University from May 2018 to May 2022 were selected as the research subjects by the random sampling method.All patients underwent total supragingival oral prophylaxis.The total oral plaque assessment and detection,general oral examination,and a questionnaire survey on oral hygiene behavior will be conducted when patients came back for return visit at 7-10 days after operation.Single factor and multivariate logistic regression analysis were used to analyze the factors affecting the effectiveness of plaque control.Results A total of 350 questionnaires were distributed in this study,and 340 valid questionnaires(97.14%)were collected.The total plaque index of the patient was(68.42±16.93)%;the plaque index in maxillary was significantly lower than that in mandibular(P<0.05);the plaque index in anterior tooth area was significantly lower than that in the poste-rior tooth area(P<0.05);there was no significant difference in plaque index between the left and right halves of the mouth(P>0.05);there was no significant difference in plaque index between zone 1 and zone 2(P>0.05);there was no signifi-cant difference in plaque index between zone 3 and zone 4(P>0.05);the plaque index in zone 3 and zone 4 was significantly higher than that in zone 1 and zone 2,respectively(P<0.05).There was no significance difference in the plaque index be-tween distal and lingual surfaces(P>0.05),while there was significance difference in the plaque index between the other dental surfaces(P<0.05).The results of univariate analysis showed that there were significant difference in plaque index among patients with different genders,educational levels,disease severity,brushing duration,and brushing method(P<0.05);the results of multivariate logistic regression analysis showed that the severity of the disease and brushing method were the factors affecting the plaque index(P<0.05).Conclusion The overall control situation of plaque control in patients with periodontal disease is not ideal.The severity of the disease and the method of brushing teeth are factors affecting the effectiveness of plaque index control.Oral health education should be strengthened to promote plaque control in patients with periodontal disease.
8.Clinical observation of acupuncture with host-guest combination using Yuan-Primordial and Luo-Connecting points plus Tuina for the third lumbar transverse process syndrome

Objective: To observe the effects of acupuncture at Yuan-Primordial and Luo-Connecting points by host-guest combination plus Tuina (Chinese therapeutic massage) on the third lumbar transverse process syndrome. Methods: A total of 88 patients with the third lumbar transverse process syndrome were selected and divided into an observation group and a control group according to the random number table method, with 44 cases in each group. The control group was treated with Tuina, and the observation group was treated with additional acupuncture at Yuan-Primordial and Luo-Connecting points by host-guest combination. The clinical efficacy of the two groups was compared after treatment. The changes in the scores of physical signs, Roland-Morris disability questionnaire (RMDQ), Oswestry disability index (ODI), Quebec back pain disability scale (QBPDS), and pain factors [including serum prostaglandin (PG) E2, neuropeptide Y (NPY), and matrix metalloproteinase-3 (MMP-3)] were observed. Results: After treatment, the total effective rate in the observation group was 93.2%, higher than 75.0% in the control group; the difference between the two groups was statistically significant (P<0.05). Compared with those before treatment, the scores of physical signs and each low back pain scale, and the levels of serum pain factors in the two groups were decreased (P<0.05), and those in the observation group were lower than those in the control group (P<0.05). Conclusion: Acupuncture at Yuan-Primordial and Luo-Connecting points by host-guest combination plus Tuina is effective in the treatment of the third lumbar transverse process syndrome; it can improve the patient's physical signs, relieve low back pain, and reduce the levels of serum pain factors.
9.Summary of the best evidence for prevention and management of falls in the frail elderly

Objective:To search, evaluate and summarize the best evidence on falls prevention and management in the frail elderly to provide ideas and references for clinical practice.Methods:Evidences related to falls prevention and management in the frail elderly were systematically searched on BMJ Best Practice, UpToDate, Registered Nurses' Association of Ontario, National Guideline Clearinghouse, National Institute for Health and Clinical Excellence, Medlive, Joanna Briggs Institute (JBI) Evidence Based Health Care Center Database in Australia, Scottish Intercollegiate Guideline Network, Cochrane Library, CINAHL, PubMed, Web of Science, Embase, CNKI, Wanfang Database, VIP and China Biology Medicine disc. The search period was from the establishment of the database to March 1, 2023. Three researchers independently evaluated quality of the included literature, and extracted, integrated and graded the evidence.Results:A total of 24 literatures were included, including 13 systematic reviews, 4 expert consensus, 6 guidelines and 1 evidence summary. A total of 30 evidences were summarized from 5 aspects of frailty screening, assessment, disease and medication management, exercise management and nutrition management.Conclusions:This study summarizes the best evidence for fall prevention and management in the frail elderly. It is suggested that practitioners should formulate a management plan in accordance with the actual situation in combination with the application scenario and the situation of the elderly, so as to improve the intervention level and reduce the incidence of falls in the frail elderly.
10.Meta-synthesis of qualitative studies on recurrence risk perception in stroke patients

Objective:To systematically integrate qualitative study on stroke patients' perception of recurrence risk, so as to providing evidence for optimizing stroke risk communication education and secondary prevention strategies.Methods:Qualitative studies related to the perception of recurrence risk in stroke patients were searched through computers on PubMed, Web of Science, Embase, CINAHL, Cochrane Library, Joanna Briggs Institute (JBI) Evidence-Based Health Care Centers Database, China National Knowledge Infrastructure, WanFang Data, VIP, and China Biomedical Medicine Database. The search period was from the establishment of the database to September 2022. The articles quality were evaluated using the quality evaluation criteria for qualitative research of the Australian JBI Evidence-Based Health Care Center (2020) . The results were integrated by Meta-synthesis.Results:A total of 7 articles were included, 37 research results were extracted, 9 new categories were summarized, and 3 integrated results were obtained, which were respectively the perceived characteristics of recurrence risk in stroke patients, the barriers to accurate perception of recurrence risk, and the ways to actively or negatively address recurrence risk.Conclusions:In clinical practice, it is necessary to carry out early dynamic assessment of recurrence risk perception among stroke patients, attach importance to and optimize communication and education on recurrence risk, improve comprehensive support mechanisms, so as to promote stroke patients to participate in and adhere to active risk management.
