Liver being substantial Yin and functional Yang maintain normal function of Qi, blood and meridians. In clinical practice, it is often found that pan-vascular lesions with atherosclerosis as the predominant pathological change often co-occur with metabolic dysfunction-associated fatty liver disease(MAFLD). MAFLD leads to increased risk and worse prognosis for many pan-vascular diseases, including cardiovascular disease. Dysregulation of energy homeostasis disrupts the hepatic homeostasis of body use, and representative drugs to improve metabolism, such as metformin, sodium-glucose co-transporter 2 inhibitors, and glucagon-like peptide-1 agonists, not only have a clear cardiovascular benefit, potential improvement of MAFLD has also been demonstrated. The liver stores blood and the heart pumps blood, and liver diseases affect the heart, that's why the unsmoothness of vessels appears. So the treatment should from the standpoint of liver, restoring liver function, soothing the liver and nourishing heart, activating blood and dredging meridian. It is of great significance to explore in depth the pathogenesis and treatment of pan-vascular lesions caused by MAFLD, and to restore the energy homeostasis by adjusting the balance of liver Yin and Yang.

3.Impact of the CACNA1G rs757415 polymorphism on grey matter volume in patients with bipolar disorder

Background Bipolar disorder is a severe mental disorder characterized by cycling between mania/hypomania and depression,yet its underlying pathophysiological mechanism remains unclear.Several prior studies have suggested a potential role for voltage-gated calcium channel subunit genes in the etiology of bipolar disorder,particularly in their influence on brain structure.Objective To investigate the differences in grey matter volume(GMV)for individuals with bipolar disorder compared to healthy controls,and to explore the potential influence of calcium channel voltage-dependent T-type α1 G subunit(CACNA1G)rs757415 polymorphism on GMV in bipolar disorder and clarify the specific brain regions associated with this genetic variation,thus offering a new opportunity to gain insight into the pathophysiological mechanism of bipolar disorder.Methods A cohort of 289 patients who met the Diagnostic and Statistical Manual of Mental Disorders,fourth edition(DSM-IV)criteria for bipolar disorder were selected for participation.These patients were either admitted to hospital or examined in outpatient clinic for bipolar disorder at the Mental Health Center of West China Hospital,Sichuan University between September 2013 and December 2022.Another 322 healthy individuals were concurrently recruited as a control group from Sichuan University and surrounding communities.All participants underwent brain imaging using a 3.0 T magnetic resonance scanner to acquire data on GMV.Additionally,the presence of the CACNA1G rs757415 polymorphism was validated using the imLDRTM technique.Spearman correlation analysis was utilized to investigate potential relationship between abnormal brain regions identified through GMV data and clinical characteristics of the patients.Then the genotype-by-diagnosis interaction effect for CACNA1G rs757415 on GMV was observed using the full factor method.Results The study successfully enrolled 173 patients with bipolar disorder and 207 healthy controls who completed all the necessary procedures.Analyses revealed decreased GMV for patients with bipolar disorder compared to healthy controls in the left cerebellar declive extending to cerebellar anterior/posterior lobe,fusiform gyrus,parahippocampal gyrus and inferior occipital gyrus(t=5.664,P<0.05);in the right cerebellar anterior/posterior lobe,fusiform gyrus,parahippocampal gyrus extending to lingual gyrus(t=4.583,P<0.05);in the bilateral anterior cingulate/paracingulate gyri,superior frontal gyrus and precuneus(t=7.543,P<0.05);in the left lingual gyrus and superior temporal gyrus(t=6.593,P<0.05);and in the right insula entending to central operculum(t=7.153,P<0.05).Correlation analysis indicated that the duration of bipolar disorder was positively correlated with cerebrospinal fluid volume(r=0.258,P=0.003),whereas negatively correlated with the GMV in the left cerebellar declive extending to cerebellar anterior/posterior lobe,inferior occipital gyrus and parahippocampal gyrus(r=-0.204,P=0.019),in the right cerebellar anterior lobe extending to right cerebellar posterior lobe,fusiform gyrus,parahippocampal gyrus and lingual gyrus(r=-0.238,P=0.006),in the bilateral superior frontal gyrus extending to anterior cingulate/paracingulate gyri and precuneus(r=-0.219,P=0.012),in the left lingual gyrus extending to superior temporal gyrus(r=-0.296,P=0.001),and in the right insula extending to central operculum(r=-0.257,P=0.003).A significant genotype-by-diagnosis interaction effect for CACNA1G rs757415 on GMV was observed in the right parahippocampal gyrus-fusiform gyrus-cerebellum 4-5(F=19.967,P<0.05).In the control group,individuals carrying the non-risk allele showed increased GMV in the right parahippocampal gyrus-fusiform gyrus-cerebellum 4-5 compared to those carrying the risk allele.In contrast,within the patient group,risk allele carriers exhibited increased GMV in the same brain regions when compared to non-risk allele carriers.Moreover,the GMV in the right parahippocampal gyrus-fusiform gyrus-cerebellum 4-5 of patients with bipolar disorder carrying risk alleles was increased compared to healthy controls.Conclusion CACNA1G rs757415 polymorphism may affect the GMV in the right parahippocampal gyrus,fusiform gyrus and cerebellum 4/5 of patients with bipolar disorder.
4.Relationship Between Gut Microbiota Dysbiosis and Cardiometabolic Diseases Based on "Heart-spleen-intestine" Axis

Cardiometabolic disease is a clinical syndrome with a causal relationship between metabolic abnormalities and cardiovascular damage. With its global incidence and related mortality rates continually rising, it has become a major health concern worldwide. The role of the gut microbiome and its metabolic products in cardiovascular metabolic health has received widespread attention, with gut microbiota dysbiosis considered a key factor in promoting the development of cardiometabolic disease. Dysbiosis disrupts the balance of the "heart-spleen-intestine" axis, leading to dysfunction of the spleen and intestines, which triggers metabolic disorders and accelerates the progression of cardiometabolic disease. Cardiac dysfunction can also negatively affect spleen and intestinal function, leading to imbalances in the heart and spleen, disharmony in Qi and blood, and exacerbating metabolic anomalies and further dysbiosis, thus forming a vicious cycle. From a modern biological perspective, the gut microbiome and its metabolic products can influence disease progression by modulating inflammatory responses and immune imbalances, leading to endothelial dysfunction and metabolic disorders, thereby increasing the risk of cardiometabolic disease. Additionally, the article proposes strategies for managing cardiometabolic disease by regulating the gut microbiome through a combination of Chinese and western medicine approaches. Traditional Chinese medicine(TCM) treatment starts from the gut microbiome, using the "heart-spleen-intestine" axis as a mediator to regulate cardiovascular metabolic health, highlighting the unique advantages of TCM in targeting the gut microbiome to treat cardiometabolic disease. This article takes the TCM theory of the "heart-spleen-intestine" axis as a starting point, discusses the pivotal role played by this axis in the connection between gut microbiome dysbiosis and the development of cardiometabolic disease, aiming to provide a new perspective for the integrated traditional Chinese and western medical research on cardiometabolic disease, offering scientific evidence and practical guidance to improve the prognosis and quality of life for patients.
5.Analysis of the impact of bone quality on the accuracy of computer-aided design of implant guide plate

Objective To explore the influence of different bone quality around the implant on the implantation accuracy,the com-puter-aided design of implant guide plate was used for patients with poor bone quality to evaluate its accuracy.Methods Retrospective analysis of relevant clinical data from 29 single-tooth implant patients with relatively poor bone quality meeting inclusion criteria was conducted.Using 3Shape Implant Studio software,implant plans were designed and fully guided digital templates were fabricated before surgery.Implantation was guided by the templates throughout,with torque and ISQ values recorded to evaluate the initial stability of the implant.Postoperative implant positions were reconstructed using CBCT images,and the three-dimensional deviation of implant place-ment was evaluated in conjunction with the preoperative design.Bone quality around the implants was also recorded and analyzed before and after surgery.Results In this study,digital guide plates were used in patients with poor bone quality,and the initial stability of implants was≥20 N·cm except for three cases.In cases of poor bone quality,the accuracy of the fully guided digital templates was as follows:cervix deviation(0.94±0.59)mm,apex deviation(1.40±0.81)mm and angle deviation 4.10°±2.99°.Bone quality had a greater impact on angle deviation(P<0.05).The higher the proportion of D3 bone was around the implant body 1/3,the smaller the deviation of neck,apex and angle was,while the higher the proportion of D5 bone,the greater the deviation of implantation.Conclu-sion For patients with poor bone quality,under the premise of restoration-oriented,the proportion of D3 bone around the implant can be increased by computer-aided design,and the initial stability of the implant can be improved by guiding the implantation with digital guide plate.Attention should be paid to the influence of bone on the angle deviation of implant during the application of guide plate.
6.The international cases of vaccinology education and its enlightenment to the discipline development in China

However, China currently confronts significant challenges, including a dearth of skilled professionals in the field of vaccination and disparities in the capacity for immunization services. This review introduced the experiences of four prime international vaccinology education models, including London School of Hygiene & Tropical Medicine, Johns Hopkins University, Leading International Vaccinology Education, and the Bill & Melinda Gates Foundation, in the aspect of personnel development, academic research and communication platforms establishment. It is supposed to give some insights and feasible suggestions on the establishment and advancement of vaccinology as a sub-discipline within high-level public health school in China, with the aim of development of a robust vaccinology education framework in China, which is essential for nurturing the next generation of public health leaders and practitioners for our country.
7.Influence of midwife led accompany on the psychological status and delivery outcome of gestational diabetes patients

Objective:To analyze the influence of midwifeled accompanyon the psychological status and delivery outcomes of gestational diabetes patients, and to provide reference for the implementation of clinical intervention programs for gestational diabetes patients.Methods:A semi-randomized controlled trial was adopted.Convenience sampling method was used to select 120 gestational diabetes patients admitted to Langfang People′s Hospital from March 2021 to September 2023, and they were divided into control group and experimental group according to the admission order. Both groups were given intraspinal delivery analgesia, while the control group was combined with routine midwifery management, and the experimental group was combined with a midwife led accompany. Intervention lasted until 2 h postpartum in both groups, and follow-up for 1 month. The delivery indicators, psychological status, delivery outcome and satisfaction of the two groups were compared.Results:Finally, 120 gestational diabetes patients were included, 60 patients in the control group, aged (28.73 ± 2.79) years, and 60 patients in the experimental group, aged (27.98 ± 1.09) years. The first and second stages of labor in the experimental group were (416.35 ± 29.87) and (60.95 ± 7.44) min, respectively, which were shorter than (501.33 ± 37.59) and (80.28 ± 8.95) min in the control group, and the differences were statistically significant ( t = 13.71, 12.87, both P<0.05). The blood glucose during delivery and 2 h postpartum hemorrhage was (6.81 ± 0.10) mmol/L and (236.18 ± 37.58) ml, respectively, lower than (7.48 ± 0.45) mmol/L and (325.70 ± 59.88) ml in the control group, the differences were statistically significant ( t = 11.26, 9.81, both P<0.05). After 1 month of follow-up, the scores of Self-rating Anxiety Scale and Self-rating Depression Scale of the experimental group were (30.86 ± 2.55) and (21.45 ± 2.67) points, which were lower than (42.19 ± 3.86) and (42.97 ± 3.15) points in the control group, and the scores of Psychological Resilience Scale were (71.22 ± 6.54) points, which was higher than that of the control group (55.38 ± 6.06) points, the differences were statistically significant ( t = 18.97, 40.37, 13.76, all P<0.05). After intervention, the incidence rates of lateral resection and postpartum hemorrhage in the experimental group were 28.3%(17/60) and 5.0%(3/60), respectively, lower than 48.3%(29/60) and 18.3%(11/60) in the control group, and the total satisfaction rate of the experimental group was 93.3%(56/60), higher than 80.0% (48/60) of the control group. The differences were statistically significant ( χ2 = 5.08, 5.18, 4.62, all P<0.05). Conclusions:The midwife led accompany could shorten the labor process of gestational diabetes patients, reduce their blood glucose during delivery and the amount of blood 2 h postpartum, and improve their psychological status and delivery outcomes, which was conducive to improving patient satisfaction.

9.Detection and clinical significance of serum ALDH2 and CAT activities in type 2 diabetic patients with retinopathy

Objective To analyze the activity of serum aldehyde dehydrogenase 2(ALDH2)and catalase(CAT)in patients with type 2 diabetes mellitus(T2DM)complicated with diabetic retinopathy(DR),and to explore its clinical diagnostic value.Methods A total of 206 T2DM patients in this hospital from April 2020 to November 2021 were selected and divided into T2DM group(T2DM group,n=103)and T2DM combined with DR group(T2DM+DR group,n=103)according to the presence or absence of retinopathy.Another 103 physical examination healthy people during the same period in the hospital were selected as the control group.Serum ALDH2 and CAT activities were measured.Pearson correlation analysis was used to analyze the rela-tionship between serum ALDH2,CAT and glycosylated hemoglobin,triglyceride,fasting blood glucose(FBG),1 h postprandial blood glucose(1 hPG)and insulin resistance index(HOMA-IR)in T2DM patients with DR.Multivariate Logistic regression was used to analyze the influencing factors of T2DM with DR.Re-ceiver operating characteristic(ROC)curve was used to analyze the clinical diagnostic value of serum ALDH2 and CAT for T2DM with DR.Results The serum ALDH2 and CAT activities were gradually decreased in the control group,T2DM group and T2DM+DR Group(P<0.05).Multivariate Logistic regression analysis showed that glycosylated hemoglobin,1 hPG,HOMA-IR,duration of diabetes,ALDH2 and CAT activity were the influencing factors of T2DM patients with DR(P<0.05).Pearson correlation analysis showed that the levels of ALDH2 and CAT were negatively correlated with glycosylated hemoglobin,triglyceride,FBG,1 hPG and HOMA-IR in T2DM patients with DR(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of ALDH2 combined with CAT in the diagnosis of T2DM patients with DR was significantly higher than AUC of ALDH2 alone(Z=2.563,P=0.010)and CAT alone(Z=1.984,P=0.047).Conclusion The serum ALDH2 and CAT activities are decreased in T2DM patients with DR.The combined detection of AL-DH2 and CAT has a high diagnostic value for T2DM patients with DR.
10.The application value of SWE in early hepatic fibrosis and renal fibrosis in MAFLD

Objective To explore the value of shear wave elastography(SWE)in the early assessment of hepatic and renal fibrosis in patients with metabolic associated fatty liver disease(MAFLD).Methods A total of 52 MAFLD patients admitted to the Second Affiliated Hospital of Wenzhou Medical University from July 2023 to May 2024 were selected as MAFLD group,and 40 non-MAFLD patients treated during the same period were served as control group.General information,laboratory data and ultrasound data were collected and compared between two groups.The differences as well as the correlation of the indicators between two groups were compared.The value of SWE in hepatic fibrosis and renal fibrosis in MAFLD patients was assessed.Results Liver function indicators,uric acid and aspartate aminotransferase-to-platelet ratio index(APRI)in MAFLD group were higher than those in control group(P<0.01);There were no statistically significant differences between two groups in fibrosis 4 index,estimated glomerular filtration rate,serum creatinine and blood urea nitrogen(P>0.05).The brightness of the liver,liver-kidney ratio,and liver elasticity value were higher in MAFLD group than those in control group(P<0.05);There was no significant difference in the elasticity value of the right renal cortex between two groups(P>0.05).Liver elasticity values was positively correlated with alanine aminotransferase(ALT);The liver-kidney ratio was positively correlated with body mass index(BMI),ALT,aspartate aminotransferase,alkaline phosphatase,y-glutamyl transferase and APRI.No significant correlation was found between the right renal cortex elasticity and BMI,estimated glomerular filtration rate,serum creatinine,blood urea nitrogen,or serum uric acid.Conclusion SWE helps in early identification of liver hardness in the MAFLD patients.But the application of SWE in early renal fibrosis in the MAFLD patients needs further evaluation.
