1.Analysis of diabetes mortality characteristics among the residents in Fengxian District, Shanghai, 2012‒2021

ObjectiveTo investigate the trends in diabetes mortality rate and the characteristics of decreased population in Fengxian District, Shanghai from 2012 to 2021. MethodsData from the death registration records of the residents in Fengxian District between 2012 and 2021, sourced from the Shanghai Death Surveillance System, were analyzed. Indicators such as the crude mortality rate due to diabetes, the standardized mortality rate, years of life lost (YLL), and the probability of premature death were estimated. Annual percentage change (APC) was used to analyze the temporal trends of mortality and the probability of premature death due to diabetes. Rate decomposition analysis was used to assess the contributions of demographic and non-demographic factors to diabetes mortality. ResultsFrom 2012 to 2021, there were 1 471 deaths due to diabetes in Fengxian District, with a crude mortality rate of 27.51/100 000 and a standardized mortality rate of 17.58/100 000. The crude mortality rate showed an overall increasing trend (APC=4.58%, Z=3.49, P<0.05). The potential years of life lost (PYLL) due to diabetes over this period amounted to 9 715 person-years, with a PYLL rate of 1.82 ‰, and the average years of life lost (AYLL) was 11.94 years. The probability of premature death was 0.41% (APC=3.36%, t=2.33, P<0.05). Both population aging and non-aging factors contributed to the increase in diabetes mortality, with overall contribution rates of 67.99% and 32.01%, respectively. Among men, the contribution rates were 60.57% and 39.43%, while among women, they were 79.43% and 20.57%, respectively. ConclusionFrom 2012 to 2021, both the crude mortality rate and the probability of premature death due to diabetes showed an upward trend among the residents in Fengxian District, with a higher YLL. Population aging was the main factor causing the increase in mortality rate, while non-demographic factors had a greater impact on the rise in diabetes mortality among men than that in women. Therefore, the management on male diabetes patients should be strengthened.
2.Relationship between Phenotypic Changes of Dendritic Cell Subsets and the Onset of Plateau Phase during Intermittent Interferon Therapy in Patients with CHB

Objective This study aimed to evaluate whether the onset of the plateau phase of slow hepatitis B surface antigen decline in patients with chronic hepatitis B treated with intermittent interferon therapy is related to the frequency of dendritic cell subsets and expression of the costimulatory molecules CD40,CD80,CD83,and CD86. Method This was a cross-sectional study in which patients were divided into a natural history group(namely NH group),a long-term oral nucleoside analogs treatment group(namely NA group),and a plateau-arriving group(namely P group).The percentage of plasmacytoid dendritic cell and myeloid dendritic cell subsets in peripheral blood lymphocytes and monocytes and the mean fluorescence intensity of their surface costimulatory molecules were detected using a flow cytometer. Results In total,143 patients were enrolled(NH group,n = 49;NA group,n = 47;P group,n = 47).The results demonstrated that CD141/CD1c double negative myeloid dendritic cell(DNmDC)/lymphocytes and monocytes(%)in P group(0.041[0.024,0.069])was significantly lower than that in NH group(0.270[0.135,0.407])and NA group(0.273[0.150,0.443]),and CD86 mean fluorescence intensity of DNmDCs in P group(1832.0[1484.0,2793.0])was significantly lower than that in NH group(4316.0[2958.0,5169.0])and NA group(3299.0[2534.0,4371.0]),Adjusted P all<0.001. Conclusion Reduced DNmDCs and impaired maturation may be associated with the onset of the plateau phase during intermittent interferon therapy in patients with chronic hepatitis B.
3.Association of Cytokines with Clinical Indicators in Patients with Drug-Induced Liver Injury

Objective To explore characteristics of clinical parameters and cytokines in patients with drug-induced liver injury(DILI)caused by different drugs and their correlation with clinical indicators. Method The study was conducted on patients who were up to Review of Uncertainties in Confidence Assessment for Medical Tests(RUCAM)scoring criteria and clinically diagnosed with DILI.Based on Chinese herbal medicine,cardiovascular drugs,non-steroidal anti-inflammatory drugs(NSAIDs),anti-infective drugs,and other drugs,patients were divided into five groups.Cytokines were measured by Luminex technology.Baseline characteristics of clinical biochemical indicators and cytokines in DILI patients and their correlation were analyzed. Results 73 patients were enrolled.Age among five groups was statistically different(P=0.032).Alanine aminotransferase(ALT)(P=0.033)and aspartate aminotransferase(AST)(P=0.007)in NSAIDs group were higher than those in chinese herbal medicine group.Interleukin-6(IL-6)and tumor necrosis factor alpha(TNF-α)in patients with Chinese herbal medicine(IL-6:P<0.001;TNF-α:P<0.001)and cardiovascular medicine(IL-6:P=0.020;TNF-α:P=0.001)were lower than those in NSAIDs group.There was a positive correlation between ALT(r=0.697,P=0.025),AST(r=0.721,P=0.019),and IL-6 in NSAIDs group. Conclusion Older age may be more prone to DILI.Patients with NSAIDs have more severe liver damage in early stages of DILI,TNF-α and IL-6 may partake the inflammatory process of DILI.
4.The Protection Mechanism of Huaiqihuang Granules from Diabetic Kidney Disease in Rats and for Renal Cells in High-Glucose Environment

Objective To observe the protection mechanism of Huaiqihuang Granules from diabetic kidneydisease(DKD)in rats and for renal cells in high-glucose environment.Methods(1)In vivo experiment:the rat model was constructed by high-fat diet feeding combined with STZ intraperitoneal injection.The experiment was divided into normal group,model group,Huaiqihuang Granules low-,medium-and high-dose groups and Losartan group,with 10 rats in each group.After corresponding treatment,the main biochemical indexes such as urinary microalbumin,fasting blood glucose,serum creatinine,total cholesterol and low density lipoprotein cholesterol were measured,and the pathological changes of renal tissue were observed by hematoxylin-eosin staining,periodic acid Schiff's staining and Masson staining,respectively.(2)In vitro experiments:a high glucose-induced renal tubular epithelial cell HK2 model was constructed.The experiment was divided into normal glucose group,high-glucose group,Huaiqihuang Granules group and mannitol group.After corresponding treatment,the cell proliferation was detected by MTT assay,and the generation of reactive oxygen species(ROS)in cells and mitochondria was detected by 2',7'-Dihydrodichlorofluorescein diacetate(DCFH-DA)method and MitoSOX/Hoechst 33342 staining method,respectively.Western Blot was used to detect the expressions of inflammasome-related proteins NOD-like receptor protein 3(NLRP3),cleaved-caspase-3 and interleukin-1β(IL-1β)and renal fibrosis-related proteins fibronectin(FN)and type Ⅳ collagen(Col Ⅳ).Results(1)In vivo experiments,Huaiqihuang Granules significantly improved the urinary microalbumin excretion and serum creatinine,hyperglycemia and lipid metabolism disorders in DKD rats,and reduced the pathological damage of renal tissue.(2)In vitro studies,Huaiqihuang Granules significantly inhibited proliferation and ROS overproduction of high-glucose-induced HK2 cells,reduced the activation of NLRP3 inflammasome and the expressions of renal fibrosis-related proteins FN and Col Ⅳ in high-glucose-induced HK2 cells.Conclusion Huaiqihuang Granules has an impovement effect on DKD in rats,and its protection mechanism for renal cells in high-glucose environment is relavent to inhibiting ROS generation,activation of NLRP3 inflammasome and expression of fibrosis-related proteins.
5.Analysis of the short-term efficacy and safety of percutaneous liver puncture in the local treatment of portal vein thrombosis

Objective To evaluate the short-term efficacy and safety of percutaneous liver puncture for local management of portal vein thrombosis(PVT).Methods Variations in thrombus,blood flow,and laboratory examination results were observed before and after percutaneous liver puncture in 197 patients with PVT,and the occurrence of comorbidities was recorded and followed up for one year after treatment.Results After treatment,the thrombus in the main portal vein vessels almostly disappeared in 119 patients(60.41%)with PVT,the thrombus had a significant reduction in 57 patients(28.93%),and the thrombus had a smaller change or an increase in 21 patients(10.66%);146 patients(74.11%)had smooth blood flow in the main portal vein vessels,29 patients(14.72%)showed significant improvement in blood flow,and 22 patients(11.17%)showed no significant improvement or worsening of blockage.The mean portal venous pressure was significantly lower than that before treatment(P<0.001);thrombin time,activated partial thromboplastin time,and prothrombin time were prolonged compared to those before thrombolysis(P<0.001),and fibrinogen were reduced compared to those before thrombolysis(P<0.001).A total of 35 patients(17.77%)occured comorbidities during treatment.One year after treatment,196 patients(99.49%)with PVT survived,of which thrombus essentially disappeared in 141(71.94%),thrombus stabilized(or decreased)in 42(21.43%),and thrombus increased in 13(6.63%).Conclusion percutaneous liver puncture for local management of PVT is effective and reliable in the short-term and requires standardized management of the entire process.
6.A-485 alleviates tubular lipid accumulation by inhibiting H3K18ac/H3K27ac induced by P300/CBP in diabetic mice

Purpose To investigate the protective effect and mechanism of A-485 on renal tubular injury in diabetic mice.Methods Eighteen male C57BL/6J mice were randomly divided into three groups:Control group,diabetic kidney dis-ease(DKD)group and A-485 treatment group.The DKD mice model was established by feeding high-fat diet for 8 weeks and intraperitoneal injection of streptozotocin for 5 days.Subsequent-ly,the A-485 treatment group was given A-485(10 mg/kg/day)by intraperitoneal injection every other day for 4 weeks.After treatment,the renal function,P300 enzyme activity and lipid deposition in renal tissue were measured.Western blot a-nalysis was performed to detect SREBP-1,FASN,ACC,ChREBP,P300,CBP,H3K18ac and H3K27ac protein levels.Results Compared with control mice,the levels of FBG,BUN,Scr and UAE were significantly increased in diabetic mice(FBG:2.52 times,BUN:2.89 times,Scr:2.13 times,UAE:4.21 times),while diabetic mice treatment with A-485 exhibi-ted a remarkable decrease on BUN,Scr and UAE(BUN:0.511 times,Scr:0.636 times,UAE:0.574 times,P<0.01).The results of the transmission electron microscopy and oil red O stai-ning showed that A-485 treatment prevents lipid droplets forma-tion and up-regulation of SREBP-1,FASN,ACC and ChREBP in renal tubular cells of diabetic mice(SREBP-1:0.544 times,FASN:0.449 times,ACC:0.306 times,ChREBP:0.317 times,P<0.01).Furthermore,A-485 intervention downregu-lated the enzyme activity of P300(0.546 times)and suppressed the expression of H3K18ac(0.337 times)and H3K27ac(0.308 times,P<0.01).Conclusion A-485 can significant-ly improve renal lipid metabolic disorder in diabetic mice,which may be achieved by inhibiting p300-induced H3K18ac and H3K27ac.

Epigenomic imbalance drives abnormal transcriptional processes,promoting the onset and progression of cancer.Although defective gene regulation generally affects carcinogenesis and tumor suppression networks,tumor immunogenicity and immune cells involved in antitumor responses may also be affected by epigenomic changes,which may have significant implications for the development and application of epigenetic therapy,cancer immunotherapy,and their combinations.Herein,we focus on the impact of epigenetic regulation on tumor immune cell function and the role of key abnormal epigenetic processes,DNA methylation,histone post-translational modification,and chromatin structure in tumor immunogenicity,and introduce these epigenetic research methods.We emphasize the value of small-molecule inhibitors of epigenetic modulators in enhancing antitumor immune responses and discuss the challenges of developing treatment plans that combine epigenetic therapy and immuno-therapy through the complex interaction between cancer epigenetics and cancer immunology.
8.Impact of different angles of pulmonary surfactant administration on bronchopulmonaryplasia and intracranial hemorrhage in preterm infants:a prospective randomized controlled study

Objective To investigate the effects of different angles of pulmonary surfactant(PS)administration on the incidence of bronchopulmonary dysplasia and intracranial hemorrhage in preterm infants.Methods A prospective study was conducted on 146 preterm infants(gestational age<32 weeks)admitted to the Department of Neonatology,Provincial Hospital Affiliated to Anhui Medical University from January 2019 to May 2023.The infants were randomly assigned to different angles for injection of pulmonary surfactant groups:0° group(34 cases),30° group(36 cases),45° group(38 cases),and 60° group(38 cases).Clinical indicators and outcomes were compared among the groups.Results The oxygenation index was lower in the 60° group compared with the other three groups,with shorter invasive ventilation time and oxygen use time,and a lower incidence of bronchopulmonary dysplasia than the other three groups(P<0.05).The incidence of intracranial hemorrhage was lower in the 60° group compared to the 0° group(P<0.05).The cure rate in the 60° group was higher than that in the 0° group and the 30° group(P<0.05).Conclusions The clinical efficacy of injection of pulmonary surfactant at a 60° angle is higher than other angles,reducing the incidence of intracranial hemorrhage and bronchopulmonary dysplasia in preterm infants.[Chinese Journal of Contemporary Pediatrics,2024,26(4):337-342]
9.Correlation Analysis of the Number of Hemophagocytes and Peripheral Blood Cells in Bone Marrow

Objective:To study the correlation between the number of hemophagocytes and peripheral blood cells in bone marrow of patients with fever of unknown origin.Methods:A total of 465 patients with fever of unknown origin in our hospital from January 2019 to December 2021 were selected as the research objects,which was to reviewed retrospectively the correlation between the number of hemophagocytes and peripheral blood cells in bone marrow.Results:The positive rates of hemophagocytes detected in the three lines decreased group,the two lines decreased group,the one line decreased group,normal group of the three lines and at least one of the three lines increased group were 86.4%,62.1%,38.3%,34.6%and 33.3%,respectively.The number of hemophagocytes per unit area in the three lines decreased group was significantly higher than that in the other four groups(P<0.001).The number of hemophagocytes per unit area in the two lines decreased group was higher than that in the one line decreased group,normal group of three lines and at least one of the three lines increased group(P<0.01).There was no significant difference in the number of hemophagocytes per unit area between the group with a decreased number of one line and the other two groups with a normal number of three lines and the group with at least one increased number of three lines(P>0.05).The missed rates of hemophagocytes in the five groups were 15.78%,22.03%,62.22%,77.78%and 53.84%,respectively.Conclusion:For patients with fever of unknown origin,especially those with obvious decrease in the number of three lines and two lines in peripheral blood cells,which should pay attention to the detection of hemophagocytes in bone marrow.Meanwhile,if the number of three lines was normal even at least one of the three lines increased,the presence of hemophagocytes in the bone marrow slice should be also carefully observed.

Ulcerative colitis (UC) is a chronic inflammation of the gastrointestinal tract and is characterized by alternating periods of inflammation and remission. Although UC incidence is lower in Taiwan than in Western countries, its impact remains considerable, demanding updated guidelines for addressing local healthcare challenges and patient needs. The revised guidelines employ international standards and recent research, emphasizing practical implementation within the Taiwanese healthcare system. Since the inception of the guidelines in 2017, the Taiwan Society of Inflammatory Bowel Disease has acknowledged the need for ongoing revisions to incorporate emerging therapeutic options and evolving disease management practices. This updated guideline aims to align UC management with local contexts, ensuring comprehensive and context-specific recommendations, thereby raising the standard of care for UC patients in Taiwan. By adapting and optimizing international protocols for local relevance, these efforts seek to enhance health outcomes for patients with UC.
