1.Clinical and imaging analyses of primary mediastinal yolk sac tumor

		                        			Objective·To summarize the clinical features,imaging features,and diagnosis and treatment experience of primary mediastinal yolk sac tumor(YST).Methods·Data of 29 patients with primary mediastinal YST,who attended Shanghai Chest Hospital,Shanghai Jiao Tong University School of Medicine from September 2016 to May 2023,were collected and comprehensively analyzed,including imaging examination results,serum indicators,pathology reports and treatment methods.Results·There were 22 cases of pure YST and 7 cases of mixed YST comprising 28 males and 1 female.The mean age of onset was(24.5±5.9)years.The initial symptoms were chest tightness(34.5%),chest pain(27.8%),cough(34.5%),expectoration(34.5%)and no specific symptoms(24.1%).Chest computerized tomography(CT)enhancement showed that all the 29 lesions were located in the anterior mediastinum.The maximum diameter of the lesions ranged from 5.6 cm to 18.2 cm.The lesions were irregular in shape,uneven in density,partially cystic and solid in density.The enhancement scan showed the solid part was slightly and moderately enhanced,and the low-density area was not enhanced.Tumor boundary was not clear because tumors often compressed and invaded surrounding tissues.Among the 29 newly diagnosed patients,serum alpha-fetoprotein(AFP)was significantly increased in 28 cases(1 case was not tested).Patients received multidisciplinary comprehensive treatment,including chemotherapy(25/29),surgery(26/29),and radiotherapy(8/29).Seven patients directly received surgery after diagnosis.Nineteen patients received chemotherapy first and then surgery;16(84.2%)cases were evaluated as lesion shrinkage after chemotherapy.After surgery,73.1%(19/26)patients had a significant decrease in serum AFP.After chemotherapy,56.0%(14/25)patients had decreased serum AFP.Conclusion·Primary mediastinal YST usually occurs in middle-aged and young men,with certain clinical and radiographic features and elevated serum AFP,which requires multidisciplinary comprehensive treatment.
		                        		
		                        		
		                        		
		                        	
2.The Effects of Different Delivery Methods on Levator Ani Muscle of Primipara were Evaluated by Three-dimensional Perineal Ultrasonogra-phy Combined with Shear-wave Elastography

		                        			Objective To explore the application value of transperineal three-dimensional ultrasound combined with SWE in evalu-ating the effect of delivery mode on the morphology and function of levator ANI muscle in primipara,and to provide a new method for early diagnosis of pelvic floor dysfunction(PFD).Methods A total of 92 primiparas who were re-examined 6-8 weeks after delivery in our hospital were selected as postpartum group,and divided into vaginal delivery group(n=45)and cesarean section group(n=47)accord-ing to their delivery mode,and healthy women without childbirth at the same period were selected as control group(n=43).Transperineal three-dimensional ultrasound combined with SWE was used to examine the differences in the morphological parameters and elastic param-eters of levator anal hiatus in different states among all groups.Results The hiatus area of levator anal muscle(LHA)at rest,maximum anal retraction and maximum Valsalva in the vaginal delivery group was greater than that in the cesarean section group and control group,and the cesarean section group was greater than the control group,the differences were statistically significant(P<0.01).Compared with control group and cesarean section group,the elastic parameters of vaginal delivery group at rest and maximum anal retraction were de-creased,the difference was statistically significant(P<0.01).There was no significant difference in elastic parameters between the cesar-ean section group and the control group(P>0.05).The difference between maximum Valsalva state and resting state LHA(Δ A')was negatively correlated with the difference before and after contraction of bilateral PR Young's modulus(ΔE)(left r=-0.444,P=0.008;Right r=0.488,P=0.002).Conclusion Three-dimensional perineal ultrasonography combined with SWE can quantitatively evaluate the morphology and function of levator ANI muscle,and provide evidence for the early diagnosis of PFD.

4.Bibliometric analysis of trauma informed care based on Web of Science

		                        			Objective:To explore the research hotspots and trends of trauma informed care based on the Web of Science Core Collection, so as to provide reference for future research in China.Methods:Articles related to trauma informed care was searched on the Web of Science Core Collection from February 1, 2002 to February 1, 2022. Article duplication, screening, and data extraction were performed in EndNote X9. CiteSpace 5.0 R1 software was used for visual analysis.Results:A total of 510 articles were included. Since 2021, there had been a sharp increase in the number of articles, with the largest number of published articles in the United States. The overall collaboration between authors was not close, with 10 of the most productive authors, and a core author group was not formed. The top 10 research institutions were affiliated with universities. Published journals, disciplines, and keywords focused on the psychology, children, adolescents, psychiatry, women, violence, and public health services, and were developing towards a wide range of population.Conclusions:Research on trauma informed care currently focuses on the application of service objects in mental and psychological service environments, as well as the personnel training and development of measurement tools. 		                        			On January 22, 2020, China National Center for Bioinformation (CNCB) released the 2019 Novel Coronavirus Resource (2019nCoVR), an open-access information resource for the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). 2019nCoVR features a comprehensive integra-tion of sequence and clinical information for all publicly available SARS-CoV-2 isolates, which are manually curated with value-added annotations and quality evaluated by an automated in-house pipeline. Of particular note, 2019nCoVR offers systematic analyses to generate a dynamic landscape of SARS-CoV-2 genomic variations at a global scale. It provides all identified variants and their detailed statistics for each virus isolate, and congregates the quality score, functional annotation,and population frequency for each variant. Spatiotemporal change for each variant can be visualized and historical viral haplotype network maps for the course of the outbreak are also generated based on all complete and high-quality genomes available. Moreover, 2019nCoVR provides a full collection of SARS-CoV-2 relevant literature on the coronavirus disease 2019 (COVID-19), including published papers from PubMed as well as preprints from services such as bioRxiv and medRxiv through Europe PMC. Furthermore, by linking with relevant databases in CNCB, 2019nCoVR offers data submission services for raw sequence reads and assembled genomes, and data sharing with NCBI. 		                        			To analyze clonal evolution and clinical significance of trisomy 8 in patients with acquired bone marrow failure.

		                        			The clinical data of 63 patients with acquired bone marrow failure accompanied with isolated trisomy 8 (+8) from June 2011 to September 2018 were analyzed retrospectively, the clonal evolution patterns and relationship with immmunosuppressive therapy were summarized.

		                        			Totally 24 male and 39 female patients were enrolled, including 39 patients with aplastic anemia (AA) and 24 patients with relatively low-risk myelodysplastic syndrome (MDS) . 7.Effect of ERH gene on migration and invasion of human bladder cancer cells T24 and 5637

		                        			Objective To study the effect of enhancer of rudimentary homolog (ERH) gene on migration and invasion in human bladder cancer T24 and 5637 cells.Methods After knocking out the ERH gene of human bladder cancer T24 and 5637 cells,Wound healing assay,Transwell cell migration assay and Transwell cell invasion assay were used to verify the migration and invasion function.Cell migration related protein was detected by Western blot.Nude mouse tail vein transfer assay was used to study the metastasis ability of bladder cancer cells in vivo.Results (1) The Wound healing assay showed that there were significant differences in the migration cell counts of human bladder cancer 5637 and T24 (P < 0.05).(2) There were significant differences in migration and invasion cell counts of Transwell assay (P <0.05).(3) Western blot showed that the expression of E-Cadherin in human bladder cancer 5637 cells and T24 cells was significantly increased (P < 0.05) after knocking out ERH gene,while the expression of Fibronectin,Twist,Vimentin and Snail2 protein were significantly decreased (P < 0.05).(4) Nude mouse tail vein transfer assay showed that lung metastases were significantly reduced in the ERH knockout group compared with the normal ERH group.Conclusions Both in vitro and in vivo experiments suggest that ERH knockout affects the migration and invasion of human bladder cancer T24 and 5637 cells.

9.Clinical Observation of Pregabalin Combined with Gabapentin in the Treatment of Central Pain after Cerebral Infarction

		                        			OBJECTVE:To investigate the clinical effect and safety of pregabalin combined with gabapentin in the treatment of central pain after cerebral infarction.METHODS:One hundred and fifty patients with central pain after cerebral infarction in our hospital from Jan.2010 to Dec.2015 in our department were randomly divided into group A,B,C,with 50 cases in each group.Group A was given Pregabalin capsule 75 mg,bid combined with Gabapentin capsule 0.1 g,tid;group B was given Pregabalin capsule 75 mg,bid;group C was given Gabapentin capsule 0.1 g,tid;3 groups were treated for 4 weeks.VAS score,NRS score,PSQI and SF-36 score were observed among 3 groups before and after treatment to evaluate clinical efficacies of 3 groups;the occurrence of ADR were recorded in 3 groups.RESULTS:The clinical total response rate of group A,B,C were separately 94.00%,74.00%,70.00%.The clinical total response rate of group A was significantly better than that of group B and C,with statistical significance (P<0.05).After treatment,VAS score of group A,B,C were separately(3.87 ± 0.74),(5.10 ± 1.26),(5.03 ± 1.23);NRS score were separately (3.91 ± 0.88),(5.29 ± 1.25),(5.37 ± 1.30);VAS score and NRS score of group A were signifi cantly lower than group B,C and before treatment,with statistical significance (P<0.05);PSQI score of group A,B,C were separately(4.03 ± 0.85),(5.92 ± 1.16),(5.83 ± 1.11);SF-36 score were separately (372.84 ± 73.25),(348.07 ± 60.54),(345.67 ± 59.72);PSQI score and SF-36 score of group A were significantly better than group B,C and before treatment,with statistical sig nificance (P<0.05).There was no statistical significance in the incidence of ADR among 3 groups (P>0.05).CONCLUSIONS:Compared with pregabalin and gabapentin alone,pregabalin combined with gabapentin in the treatment of central pain after cerebral infarction can efficiently relieve the perceived pain,improve sleep quality and daily life quality and not increase the risk of ADR;therefore,drug combination plan is recommended for patient with central pain after cerebral infarction,especially with poor effect of two single drug.
