1.Study on the mechanism of Yigong Powder in the treatment of anemia of chronic disease by means of formula components analysis based on activating spleen method

Objective To observe the role of dried tangerine peel in Yigong Powder improves iron metabolism and promotes red blood cell generation in anemia of chronic disease (ACD).Methods With a two-by-two factorial design,the Yigong Powder was divided into dried tangerine peel and Chenpi absent Decoction. According to the random number table method,32 zymosan-induced generalized inflammation (ZIGI) mice were randomly divided into the model group,the dried tangerine peel group,the Chenpi absent Decoction group,and the Yigong Powder group. The dried tangerine peel group,Chenpi absent Decoction group and the Yigong Powder group were given dried tangerine peel(3.083 g/kg),Chenpi absent Decoction(12.33g/kg),and Yigong Powder(15.413g/kg)by gavage to the corresponding group of mice. The model group was given an equal amount of physiological saline by gavage,and treated continuously for 7 days. After the completion of administration,the body weight of each group of mice was recorded. The hemoglobin content of each group of mice was detected using a fully automatic cell counter,the serum iron content was detected using colorimetry,the serum ferritin content was detected using enzyme-linked immunosorbent assay (ELISA),and the spleen index was calculated. The liver tissue inflammatory factors interleukin-1β (IL-1β),interleukin-6 (IL-6),tumor necrosis factor-α (TNF-α),interferon-γ (IFN-γ),interleukin-4 (IL-4),and interleukin-10 (IL-10) levels were detected using Luminex method. The mRNA expressions of liver tissue hepcidin gene (HAMP) and membrane iron transporter ( Fpn) were detected using real-time fluorescence PCR method. Results Dried tangerine peel and Chenpi absent Decoction both showed interactive effects in regulating hemoglobin,serum iron,serum ferritin content,improving spleen index,and regulating the mRNA expressions of HAMP,Fpn,as well as IL-1β and IFN-γ (P<0.05). Compared with the model group,dried tangerine peel significantly increased hemoglobin,serum iron content,and Fpn mRNA expression in ZIGI model mice,while decreasing ferritin content,spleen index,HAMP mRNA expression,and the levels of IL-1β,IL-6,TNF-α,and IFN-γ (P<0.05). Chenpi absent Decoction significantly increased serum iron content and Fpn mRNA expression in ZIGI model mice,while reducing spleen index,ferritin content,HAMP mRNA expression,and the levels of IL-1β and IFN-γ、IL-4 (P<0.05). Conclusion The effects of dried tangerine peel on inflammatory factors (IL-6 and TNF-α) and Fpn may play a key role in the improvement effects of Yigong Powder on ACD and iron metabolism.
2.Hand-brain perception and movement training based on mirror neuron theory promote the recovery of upper limb function after a stroke

Objective:To explore the effect of combining hand-brain perception training with hand-brain motor training based on mirror neuron theory on the recovery of upper limb function after a stroke.Methods:A group of 105 stroke survivors with upper limb dysfunction were randomly divided into a hand-brain perception (HP) group, a hand-brain motor (HM) group, and a combination (C) group, each of 35. In addition to conventional rehabilitation treatment (including exercise therapy, occupational therapy and physical factor therapy), the HP and HM groups were given hand-brain perception training and hand-brain motor training respectively, while group C was provided with both. Before the intervention and after 4 weeks, the upper limb motor functioning of all of the participants was assessed using the simplified version of the Fugl-Meyer upper limb motor function scale (FMA-UE). Sensory functioning was quantified using the tactile Semmes Weinstein monofilament examination (SWME), and the modified Barthel index (MBI) was used to quantify the participants′ ability in the activities of daily living.Results:After the intervention the average FMA-UE, MBI and SWME scores of all three groups had improved significantly, with group C′s average FMA-UE and MBI scores significantly better than the other two groups′ averages. The average SWME score of group C was then significantly better than that of group HM.Conclusions:Hand-brain perception combined with hand-brain motor training based on mirror neuron theory can further promote the recovery of upper limb sensory and motor functioning of stroke survivors., Such therapy is worthy of clinical promotion and application.
3.Strategy for frozen-thawed cycle blastocyst transfer after hysteroscopic adhesiolysis

Objective To explore the assisted reproductive strategy and influencing factors for patients undergoing frozen-thawed cycle blastocyst transfer after hysteroscopic adhesiolysis for intrauterine adhesions.Methods A total of 275 patients who underwent frozen-thawed cycle blastocyst transfer after hysteroscopic adhesiolysis for infertility reasons at the Reproductive Genetics Center of the First Affiliated Hospital of University of Science and Technology of China from January 2018 to December 2022 were included in the study.They were divided into a single blastocyst transfer group(n=182)and a double blastocyst transfer group(n=93).The clinical outcomes were analyzed and compared retrospectively between the group.Results The two groups showed no statis-tically significant differences in terms of age,day of endometrial thickness conversion,endometrial preparation method,clinical pregnancy rate,miscarriage rate,preterm birth rate,gestational week at delivery,and mode of delivery(P>0.05).The single blastocyst transfer group had significantly lower infertility duration(years)(2.43±1.64 vs.3.03±2.13,P<0.05),significantly lower AFS prognosis score(5.13±2.25 vs.5.72±2.19,P<0.05),and significantly lower multiple pregnancy rate(1.33%vs.28.57%,P<0.05),but significantly higher quality embryo rate(90.66%vs.46.24%,P<0.05),implantation rate(50.00%vs.34.41%,P<0.05),and live infant mass(g)(3236.84±565.35 vs.2976.44±692.79,P<0.05)compared to the double blastocyst transfer group.Binary logistic regression analysis showed that the number of high-quality embryos transferred and AFS score were independent influencing factors for clinical pregnancy(P<0.05).Conclusions The number of high-quality embryos transferred and the AFS score are independent influencing factors for clinical pregnancy in patients undergoing frozen-thawed cycle blastocyst transfer after hysteroscopic adhesiolysis for intrauterine adhesions.Single high-quality blastocyst transfer is a preferred treatment for patients after hysteroscopic adhesiolysis,and double blastocyst transfer is favor-able for patients with a poor prognosis to achieve better pregnancy outcomes.
4.Analysis of the development trajectory and influencing factors of endocrine therapy associated arthralgia in breast cancer patients

Objective To analyze the development trajectory and predictors of endocrine therapy associated arthralgia in breast cancer patients.Methods From January 2022 to July 2022,breast cancer patients in the breast medicine department or outpatients of a tertiary cancer hospital in Hunan Province were selected as respondents using a convenience sampling method.A baseline survey was conducted using the General Information Questionnaire,the Symptom Assessment Scale for Patients Treated with Endocrine Therapy for Breast Cancer,and the Self-Rating Anxiety Scale within 1 week prior to patient treatment.The Symptom Assessment Scale for Patients Treated with Endocrine Therapy for Breast Cancer was used to assess patients'arthralgia levels at 3,6,and 9 months after treatment,and data were analyzed using growth mixed model,univariate analysis of variance,and multiple logistic regression.Results A total of 418 breast cancer patients completed the follow-up,with 235 cases(56.22%)developing arthralgia.3 potential categories of arthralgia develop-ment trajectories were identified:high level-slowly increasing group(11.48%),low level-slowly increasing group(44.74%),and asymptomatic group(43.78%).The results of multifactorial analysis showed that anxiety,history of bone and joint disease,sleep duration,place of residence,monthly household income,and frequency of exercise were predictors of potential categories for the development of trajectory of arthralgia levels associated with endocrine therapy in breast cancer patients(P<0.05).Conclusion Arthralgia levels associated with endocrine therapy in breast cancer patients exhibit different trajec-tories,and clinical staff should emphasize the assessment and intervention of pain levels in patients with the anxiety,a history of bone and joint disease,poor sleep,poor finances,living in urban areas,and low frequency of exercise.
5.Mining of gene clusters for biosynthesis of secondary metabolites and analysis of genes encoding antibiotic resistance and virulence in 4 644 representative human gut strains.

Genome sequences of 4 644 representative strains from human gut microbiota were analyzed to mine gene clusters for biosynthesis of novel secondary metabolites, as well as genes encoding antibiotic resistance and virulence factors. AntiSMASH analysis showed that more than 60% of the representative strains encoded at least one secondary metabolite gene cluster, and 8 potential novel secondary metabolite gene clusters were identified from 8 unculturable bacteria. The secondary metabolite gene clusters in human intestine are mainly composed of nonribosomal peptide synthetase (NRPS), bacteriocin, arylpolyene, terpene, betalactone and NRPS like gene clusters distributed in Clostridia, Bacilli, Gammaproteobacteria, Bacteroidia, Actinobacteria and Negativicutes. PathoFact analysis showed that genes encoding antibiotic resistance and virulence factors are widely distributed in representative strains, but the frequency encoded by potential pathogens is significantly higher than that of non-potential pathogens. The frequency of genes encoding secretory toxins such as outer membrane protein, PapC N-terminal domain, PapC C-terminal domain, peptidase M16 inactive domain, and non-secretory toxins such as nitroreductase family, AcrB/AcrD/AcrF family, PLD-like domain, Cupin domain, putative hemolysin, S24-like peptidase, phosphotransferase enzyme family, endonuclease/ exonuclease/ phosphatase family, glyoxalase/ bleomycin resistance was high in potential pathogens. This study may facilitate mining new microbial natural products from the intestinal microbiome, understanding the colonization and infection mechanism of intestinal microorganisms, and providing targeted prevention and treatment of intestinal microbial related diseases.

6.Status and influencing factors of activities of daily living among the elderly in Mianyang community

Objective:To investigate the current situation of activity of daily living (ADL) among the community elders in Mianyang City in Sichuan Province and analyze its influencing factors, so as to provide reference for improving the ADL of the community elders.Methods:Using the convenient sampling method, from April 2020 to December 2021, a total of 17 259 community elders (≥65 years old) in 3 counties/districts in Mianyang City in Sichuan Province were selected as the research objects, and The Elderly Ability Assessment Standard Table was used to investigate them. A total of 17 259 questionnaires were sent out in this study and 16 482 were effectively received, with the effective recovery of 95.50%.Results:Among the 16 482 community elderly, a total of 20.59% (3 394/16 482) had ADL impairment, including 2 557 (75.34%) with mild ADL impairment, 517 (15.23%) with moderate ADL impairment and 320 (9.43%) with severe ADL impairment. The results of binomial Logistic regression analysis showed that age, educational level, marital status, children's status, mental state and social participation ability, perception and communication ability were the influencing factors of ADL among the elderly in community ( P<0.05) . Conclusions:The ADL impairment rate of the elderly in the community in Mianyang City is relatively high. Community medical staff should focus on the elderly. For those with low educational level, no spouse, children, impaired mental state and social participation ability and impaired perception and communication ability, the community medical staff give targeted interventions as soon as possible to improve the abilities of daily living of the elderly in the community.
7.The effects of observing good swallowing on the swallowing ability of stroke survivors

Objective:To observe the effect of observing good swallowing on the swallowing action of stroke survivors with dysphagia.Methods:Eighteen stroke survivors with dysphagia were randomly divided into a treatment group ( n=9) and a control group ( n=9). In addition to routine swallowing rehabilitation therapy, the treatment group was asked to simulate swallowing after watching a video of normal people′s swallowing action. They did so 5 times a week for 10 minutes, while the control group just watched landscape videos at the same time. The treatment lasted 8 weeks. Before and after the treatment, both groups were assessed using the eating assessment tool (EAT-10), the functional oral intake scale (FOIS) and the penetration and aspiration scale (PAS). Functional magnetic resonance imaging (fMRI) was also used to observe their swallowing action. Results:There was no significant difference between the two groups in any of the measurements before the treatment. After the 8 weeks of treatment the average EAT-10, FOIS and PAS scores of the treatment group were all significantly better than before the treatment and better than the control group′s averages at the time. fMRI showed significantly more areas activated in the precuneus, parietal lobe, posterior central gyrus, BA7, BA5, frontal lobe and paracentral lobule in the treatment group compared with before the intervention and also more than in the control group.Conclusions:Observing proper swallowing action can improve dysphagia and activation of the swallowing-related brain areas of stroke survivors.
8.Collateral circulation compensation mode in patients with severe bilateral internal carotid artery stenosis/occlusion

Objective:To investigate the correlation between collateral flow compensation mode and interventional treatment decision in patients with severe bilateral internal carotid artery stenosis/occlusion.Methods:According to the location of internal carotid artery lesions, patients with severe stenosis/occlusion of bilateral internal carotid artery were selected at the Second Affiliated Hospital, Qiqihar Medical University and the Sixth Medical Center of PLA General Hospital from May 2017 to June 2020.Results:A total of 42 patients were finally enrolled and divided into 4 types, including 34 males and 8 females with median age 61±8(48-82)years. The collateral circulation pathways manifested as following modes: anterior communicating artery collateral, posterior communicating artery collateral, ophthalmic artery collateral, posterior cerebral middle cerebral artery pial anastomosis collateral, posterior choroidal artery anterior choroidal artery collateral, external carotid internal carotid artery C4 segment collateral, pericallosal artery anastomosis collateral, dural and pial collateral and neovascularization. Type Ⅰ severe stenosis/occlusion of C1 segment was found in 20 cases (47.6%). There were 5 cases (11.9%) of type Ⅱ severe stenosis/occlusion from C2 to C6 prior to ophthalmic artery branch. Type Ⅲ severe stenosis/occlusion occurred in 2 cases (4.8%) after the split of ophthalmic artery. Type Ⅳ was mixed type in 15 cases (35.7%).Conclusions:The compensatory pathway of collateral circulation is closely related to the lesion location. To explore the compensatory pattern of collateral circulation is helpful for clinicians to accurately analyze the lesion characteristics and guide individualized interventional therapy.
9.The effects of transcranial magnetic stimulation on post-stroke dysphagia

Objective:To observe the effect of applying repetitive transcranial magnetic stimulation (rTMS) on swallowing function and auditory evoked potentials in the brain-stems of stroke survivors with dysphagia.Methods:Sixty stroke survivors with dysphagia were randomly assigned to an observation group and a control group, each of 30. Both groups were given 30 minutes of routine swallowing training 6 times a week for 5 weeks, while the observation group was additionally provided with two-second bursts of rTMS on the left and right brain hemispheres alternately for 20 minutes daily. The frequency was 3.0Hz and the strength was 80% of the resting motor threshold (RMT), with an interval of 10 seconds between bursts. Before and after the treatment, both groups′ swallowing was assessed using the functional communication measure (FCM) and the modified Mann swallowing capacity evaluation scale (MMASA). Brainstem auditory evoked potentials (BAEP) were recorded to detect any changes in the peak latency (PL) and inter-peak latency (IPL). The clinical efficacy was also observed.Results:After the treatment, the average FCM results of both groups had improved significantly, but the observation group had improved significantly more. The average MMASA total scores and the average scores on all the sub-items had improved significantly more in the observation group except for the expressive speech disorder and anarthria scores. After the treatment, improvement was observed in respiration, anarthria, range of motion of the tongue muscles, strength of the tongue muscles, gag reflex and soft palate movement compared with before the intervention. Peak and inter-peak latencies had also improved significantly more in the observation than in the control group, on average.Conclusions:rTMS combined with conventional swallowing training can significantly improve the swallowing of dysphagic stroke survivors, and shorten the PL of the BAEP so as to better regulate deglutition.
10.The Effect of Temporal Information in Cochlear Implant Analog Sounds on the Tone Recognition of Normal Hearing Young People

Objective By comparing the tone recognition rates for different cochlear implant (CI) analog sounds ,the effects of the temporal coding strategy on tone recognition were investigated .Methods The professional announcer read 6 vowel (/a/,/o/,/e/,/i/,/u/,/ü/) of 4 different tones at a normal speed .After adjusting the am-plitude envelope and increasing the fine structure ,the audios with different sampling accuracy below 500 Hz(125 ,250? ?1500 pps) were obtained ,including 288 audio amplitude envelope adjusted and 288 not adjusted .Thirty young participants of normal hearing had tone recognition tests .The results and the tone recognition rates under two dif-ferent temporal properties were compared .Results When the temporal fine structures were combined in considera-tion ,the amplitude envelope as adjusted to match the fundamental frequency (F0) had a significant effect on the tone recognition .The tone recognition rate after amplitude envelope adjusted (80 .22% ± 16 .32% ) was higher than before (74 .83% ± 20 .24% ) [F(1 ,9)=16 .91 ,P=0 .002] .When the amplitude envelopes were combined in consider-ation ,changing the fine structure of the frequency below 500 Hz had a significant effect on the tone recognition [F (11 ,99)=38 .86 ,P<0 .001] .When the sampling precision was <375 pps ,the tone recognition had improved re-markably with the improvement of sampling precision (P<0 .004) .When the sampling precision was ≥375 pps , the effect on the tone recognition was not significant (P>0 .004) .The interaction between changing amplitude en-velopes and increasing the fine structure had a significant effect on the tone recognition [ F(11 ,99 )= 3 .78 , P<0 .001] .When the fine structure ≤375 pps ,adjustment on the amplitude envelope to increase the information of F0 had a significant impact on the tone recognition (P<0 .05) .When the fine structure was >375 pps ,the difference of the tone recognition before and after the adjustment tended to 0 (P>0 .05) .Conclusion In the CI coding strate-gy ,adjusting the amplitude envelope to get more F0 information improves tone recognition .To a certain degree ,the tone recognition improves when the sampling precision of frequency below 500 Hz is improved .Adjusting the ampli-tude envelope to F0 and increasing the temporal fine structure below 500 Hz can be combined in the same speech coding strategy to optimize the tone recognition .
