Objective:To investigate the effect of nano silver dressing as adjuvant therapy on wound healing, vascular endothelial growth factor (VEGF), and epidermal growth factor (EGF) levels in patients with deep and extensive burns. Methods:A total of 100 patients with deep and extensive burns admitted to the Zhangjiakou First Hospital from May 2022 to May 2023 were retrospectively selected and divided into a control group and an observation group according to different treatment methods, with 50 patients in each group. The control group was treated with recombinant human epidermal growth factor, while the observation group was treated with nano silver dressing in addition to the control group. A comparative analysis was conducted on the levels of serum EGF, VEGF, tumor necrosis factor alpha (TNF-α), interleukin-10 (IL-10), and intercellular adhesion molecule-1 (ICAM-1) between two groups, as well as their recovery status, pain rating scale (NRS) score, wound healing rate, scar color, thickness, vascular distribution, and softness score. Results:Before treatment, there was no statistically significant difference in the levels of VEGF, EGF, TNF-α, IL-10, and ICAM-1 between the two groups (all P>0.05); After treatment, the levels of VEGF, EGF, IL-10, and ICAM-1 increased in both groups, while the level of TNF-α decreased. The observation group had higher levels of VEGF, EGF, IL-10, and ICAM-1 than the control group, and lower levels of TNF-α than the control group, with statistically significant differences (all P<0.001). Compared with the control group, the observation group had a shorter disappearance time of burn secretions, wound healing time, and inflammation symptoms (all P<0.001). After dressing change, the NRS score level of the observation group was lower than that of the control group ( P<0.001). The wound healing rate in the observation group was higher than that in the control group at 1, 3, and 6 months (all P<0.001). After 6 months of treatment, the color, thickness, vascular distribution, and softness scores of both groups were lower than before treatment, and the observation group was lower than the control group (all P<0.001). Conclusions:The use of nano silver dressings as adjuvant therapy for patients with deep and extensive burns can shorten wound healing time, improve EGF, VEGF, and ICAM-1 levels, alleviate inflammatory reactions, and improve the condition.

To achieve continuously physiological monitoring on hospital inpatients, a ubiquitous and wearable physiological monitoring system SensEcho was developed. The whole system consists of three parts: a wearable physiological monitoring unit, a wireless network and communication unit and a central monitoring system. The wearable physiological monitoring unit is an elastic shirt with respiratory inductive plethysmography sensor and textile electrocardiogram (ECG) electrodes embedded in, to collect physiological signals of ECG, respiration and posture/activity continuously and ubiquitously. The wireless network and communication unit is based on WiFi networking technology to transmit data from each physiological monitoring unit to the central monitoring system. A protocol of multiple data re-transmission and data integrity verification was implemented to reduce packet dropouts during the wireless communication. The central monitoring system displays data collected by the wearable system from each inpatient and monitors the status of each patient. An architecture of data server and algorithm server was established, supporting further data mining and analysis for big medical data. The performance of the whole system was validated. Three kinds of tests were conducted: validation of physiological monitoring algorithms, reliability of the monitoring system on volunteers, and reliability of data transmission. The results show that the whole system can achieve good performance in both physiological monitoring and wireless data transmission. The application of this system in clinical settings has the potential to establish a new model for individualized hospital inpatients monitoring, and provide more precision medicine to the patients with information derived from the continuously collected physiological parameters.

Chinese Journal of Preventive Medicine 2018;52(2):195-200
The human gastro-intestinal tract is not only the habitat of gut microbiota, but also the main place that the body gets available nutrients. Therefore, the gut microbiota of human can be inseparable associated with the human nutrition. The common technologies used among gut microbiota research included metageonomic, metatranscriptomics, metaproteomics, and metabolomics. The research of gut microbiota based on above omics methods confirmed that diets were the main factors influencing the composition and expression of gut microbiota. The proportion, quantity, stable state, and metabolic changes of gut microbiota were closely related to obesity, diabetes mellitus, cardiovascular disease, and other nutritional-related diseases. Reasonable dietary intervention can adjust the disorders of gut microbiota, which can achieve prevention and treatment of obesity, diabetes, cardiovascular diseases, and other nutritional-related diseases. Although the single omics methods can be used to study the problems of some aspects of gut microbiota, the combination of multi-omics is needed to achieve the above objectives.

Objective To observe the effect of spleen-warming and kidney-tonifying therapy on Toll-like receptor 4 (TLR4) signaling pathway of cerebral infarction sequelae patients. Methods A total of 60 cerebral infarction sequelae patients with spleen and kidney(yang) deficiency were divided into control group and treatment group, 30 cases in each group. The two groups were given function rehabilitation training following by the theory of modern rehabilitation medicine, and additionally the treatment group was treated with spleen-warming and kidney-tonifying herbs orally. The treatment period for the two groups averaged 3 months. Before and after treatment, the scores of traditional Chinese medicine (TCM) symptoms and signs, peripheral blood TLR4 mRNA expression level, and the inflammation-related factors in the two groups were observed, and the observation parameters were compared with 30 healthy volunteers. Meanwhile, the changes of modern rehabilitation evaluation indexes were also evaluated before and after treatment. Results (1) After treatment, the Fugl-Meyer scores for motor function and Modified Barthel Index(MBI) scores for activities of daily living in the treatment group were obviously increased(P < 0.05 or P < 0.01 compared with those before treatment); the MBI scores in the control group was much increased(P < 0.05 compared with those before treatment), but the Fugl -Meyer scores only showed an increasing trend(P > 0.05). (2) After treatment, the predominant symptoms and signs of hemiplegia, facial distortion and dysphasia in the two groups were much improved(P < 0.01 compared with those before treatment), and the improvement in the treatment group was superior to that in the control group(P < 0.05). The control group had no significant effect on relieving the symptoms and signs of dizziness and blurred vision, numbness of limbs, spontaneous sweating, shortness of breath and weakness, and darkish tongue(P > 0.05 compared with those before treatment); the treatment group had obvious effect on the above symptoms and signs, and the effect was superior to that in the control group(P < 0.05 or P < 0.01).(3) For the inflammation-related factors, tumor necrosis factor alpha(TNF-α) level in the treatment group was much decreased and even arrived to the normal level(P > 0.05), and TNF-α level in the control group was decreased but did not arrive to the normal level(P < 0.05); levels of interleukin(IL)-1β, IL-6 and TLR4 mRNA in the two groups were decreased but did not arrive to the normal(P < 0.01), and the decrease in the treatment group was superior to that in the control group(P < 0.05 or P < 0.01). (4) During the treatment period, no obvious adverse effect was found in the two groups. Conclusion Oral use of spleen -warming and kidney –tonifying Chinese medicine combined with modern rehabilitation therapy is effective for the treatment of the sequelae of cerebral infarction patients by relieving the clinical TCM symptoms, and one of the therapeutic mechanisms is probably related with the TLR4 signaling pathway through regulating the expression of inflammatory factors.
