1.Clinical Observation on Treatment of Hip Joint Pain with Mailuoning Compound Solution via Nerve Blocks around Hip Joint

		                        			ObjectiveTo observe the clinical efficacy and safety of Mailuoning compound solution in the treatment of hip joint pain via nerve blocks around the hip joint. MethodsFrom March 2015 to March 2019,a total of 136 patients with hip joint pain who met the inclusion criteria were admitted and divided into an observation group and a control group according to the random number table method. Among them,six cases fell off due to failure to complete five treatments,and finally, 130 patients entered clinical observation,with 65 cases in each group. The observation group used Mailuoning compound solution for nerve blocks around the hip joint(including obturator nerve,femoral nerve branch,superior gluteal nerve, and hip fascia). The control used Mailuoning compound solution for a simple obturator nerve block. The differences in the visual analogue scale (VAS) and Harris score of hip joint of the two groups before and after treatment were observed. Any adverse drug reactions and adverse events during the treatment of the patients were recorded. ResultsThe VAS score of the two groups was significantly decreased after treatment (P<0.01). The observation group had a more significant decrease compared to the control group(P<0.01). The total Harris score of hip joint, pain degree,function score, and motion of joint of the two groups were significantly improved after treatment (P<0.01). Compared with the control group,the improvement in the total Harris score of hip joint, pain degree,and function score was more significant in the observation group (P<0.01). The clinical efficacy based on the Harris score of hip joint of the two groups was compared. The excellent and good rate of the observation group was 84.62% (55/65), which was significantly better than that of the control group [56.92% (37/65)] (χ2=12.05,P<0.01). The follow-up results showed that the patients who achieved excellent and good results had stable curative effects and low recurrence rates,and there was no significant difference in recurrence rate between the two groups. Case analysis showed that after treatment of femoral head necrosis,the saccular transparent shadow of the femoral head was significantly reduced,and the number of bone trabeculae increased. The low-density shadow decreased as can be seen on hip X-rays. In patients with hip osteoporosis after treatment,the number of bone trabeculae increased, and the low density shadow reduced. ConclusionThe use of Mailuoning compound solution for nerve blocks around the hip joint gives full play to the synergistic effect of Mailuoning compound solution and nerve block. It can effectively relieve hip joint pain,promote the recovery of hip joint function,reduce the disability rate,and improve the quality of life of patients. Early intervention is an important link in the treatment of hip joint pain diseases,which can effectively control the development of the patient's disease. Mailuoning compound solution is a new idea and method to treat hip joint pain through neuroregulation,which is easy to operate,with high safety and good therapeutic effect. In future studies,a larger sample size is needed,and more in-depth research should be conducted on the imaging changes and mechanisms of action for various hip joint pain diseases. 
		                        		
		                        		
		                        		
		                        	
2.Pregnancy management in patients with chronic kidney disease

		                        			As the incidence of chronic kidney disease (CKD) increases year by year, the number of patients with CKD is increasing year by year before or during pregnancy. Pregnancy management is increasingly important for patients with kidney disease, and women with CKD face a number of challenges during pregnancy, such as the risk of kidney disease progression, fetal growth restriction, the potential teratogenicity of medications, and an increased risk of complications such as preeclampsia and preterm birth. In order to ensure a good outcome for pregnant women and the fetus, it is necessary for multidisciplinary physicians in the department of nephrology, obstetrics, reproductive health and other departments to assess the risk of pregnancy in patients with CKD in a standardized manner, adjust medication in a timely manner, and closely monitor to detect maternal and fetal complications early, stabilize the patient′s condition, and determine the safest time for pregnancy and delivery.
		                        		
		                        		
		                        		
		                        	
3.Regulatory molecules related with microglia-neuron two-way interaction

		                        			As resident immune cells of central nervous system(CNS),microglia play a key role in immune surveillance and tissue repair.They can be activated by various types of pathological factors to form M1 or M2 phenotype,which mediates neuroinflam-mation or neuroprotective effects.At the same time,when the homeostasis of the microenvironment in which the neuron is located is broken,"signal"molecules are also released to induce microglial activation through ligand-receptor binding.Therefore,the regulation of microglia-neuron connection is of great significance for treatment of nervous system diseases.This article reviews the signaling mole-cules involved in the two-way interaction between microglial cells and neurons,and elucidates the immune mechanisms of microglia-neuron,thus providing a basis for targeted treatment of CNS diseases related to microglia activation.
		                        		
		                        		
		                        		
		                        	
4.Outcome of COVID-19 Infection in Patients With Multiple Sclerosis Who Received Disease-Modifying Therapies: A Systematic Review and Meta-Analysis

		                        			 Background:
		                        			and Purpose A systematic review and meta-analysis was performed of the outcome of Coronavirus disease 2019 (COVID-19) infection in patients with multiple sclerosis (MS) who received disease-modifying therapies (DMTs). 
		                        		
		                        			Methods:
		                        			Relevant studies published before November 2022 in the PubMed, Cochrane Library, Chinese National Knowledge Infrastructure, and Web of Science databases were retrieved using the following search expression: (“multiple sclerosis” OR “MS”) AND (“DMT” OR “disease modifying therapies”) AND (“COVID-19”). Two authors independently screened the articles and extracted the data. Qualitative analyses and a meta-analysis constituted 22 of the 794 retrieved articles. Differences in the hospitalization and mortality rates were used as the main measures of efficacy, and the meta-analysis was performed using RevMan software. 
		                        		
		                        			Results:
		                        			22 clinical trials were selected. The hospitalization rate was lower in the 3,216 patients who received DMTs than in the 774 patients who did not receive any treatment, with a moderate effect size of 0.43 (p<0.00001). The mortality rate was also lower among patients with MS treated using DMTs than in controls (odds ratio [OR]=0.19, 95% confidence interval [CI]=0.13–0.27, p<0.00001). The hospitalization rates for COVID-19 infection in patients with MS treated with anti-CD20 therapy also increased markedly (OR=3.32, 95% CI=2.63–4.20, p<0.00001). However, there was no significant difference between patients with MS who did and did not receive DMTs. 
		                        		
		                        			Conclusions
		                        			In summary, the application of DMTs was found to be valuable for patients with MS infected with COVID-19. However, more clinical studies are needed to determine the use of anti-CD20 drugs in patients with MS during the COVID-19 pandemic. 
		                        		
		                        		
		                        		
		                        	
5.Pan-immune-inflammation value predicts in-hospital mortality in patients with acute ischemic stroke admitted to the intensive care units

		                        			Objective:To investigate the predictive value of pan-immune-inflammation value (PIV) for in-hospital mortality in patients with acute ischemic stroke (AIS) admitted to intensive care unit (ICU).Methods:The clinical data of the first-ever AIS patients admitted to the ICU in the Medical Information Mart for Intensive Care (MIMIC) -Ⅳ of the United States were retrospectively included and analyzed. According to whether the patients died in the hospital, they were divided into a survival group and a death group, and the differences in baseline data between the two groups were compared. Multivariate logistic regression model was used to analyze independent influencing factors of in-hospital mortality in patients. Receiver operating characteristic curve was used to evaluate the predictive value of PIV on in-hospital mortality. Results:A total of 1 068 first-ever AIS patients admitted to the ICU were included, with a median age of 69 years. There were 543 males (50.84%), and 182 (17.05%) experienced in-hospital mortality. Multivariate logistic regression analysis showed that after adjusting for potential confounding factors, a higher PIV (>1 555.71) was an independent risk factor for in-hospital mortality in patients (odds ratio 1.924, 95% confidence interval 1.093-3.387; P=0.023). The receiver operating characteristic curve analysis showed that the area under the curve for predicting in-hospital mortality by PIV was 0.605 (95% confidence interval 0.556-0.654), with an optimal cutoff value of 1 199.93. The sensitivity and specificity for predicting in-hospital mortality were 48.9% and 70.2%, respectively. Conclusions:A higher PIV is an independent risk factor for in-hospital mortality in AIS patients admitted to ICU, which may help identify AIS patients with a higher risk of in-hospital mortality in the ICU.
		                        		
		                        		
		                        		
		                        	
6.Quantitative evaluation for payment policy of diagnosis-related groups in China based on PMC index model

		                        			OBJECTIVE To quantitatively evaluate the existing payment policies of diagnosis-related groups (DRGs)in China , so as to provide reference for the formulation and improvement of policies. METHODS Totally 58 documents related to DRGs payment issued by the national and provincial medical security bureaus from 2017 to 2022 were processed by text mining method. PMC index evaluation model of DRGs payment policy was established. Nine typical DRGs payment policies were quantitatively evaluated and analyzed by 10 primary variables and 40 secondary variables. RESULTS Among the 9 policies,5 were excellent and 4 were acceptable. The average score of PMC index was 6.882. Generally ,there was still room for improvement because of the acceptable level. By comparing the two representative policies ,it was found that the main reasons was a lack of consideration in terms of supervision and management ,incentives and constraints when facing policy changes ,reason of the lower level of urban development. CONCLUSIONS Although DRGs payment policy in China is basically perfect ,there is still a lot of room for improvement in terms of extending the time limit of the policy ,summarizing and spreading the successful experience of pilot cities.
		                        		
		                        		
		                        		
		                        	
7.Application of clinical and ultrasound-based model in evaluating the severity of secondary hyperparathyroidism

		                        			Objective:To analyze the gray-scale ultrasound and contrast-enhanced ultrasound features in secondary hyperparathyroidism (SHPT) to construct a clinical and ultrasound-based model, and to investigate the relationship between this model and serum intact parathyroid hormone(iPTH) level in order to find proper indicators for evaluation of the severity of SHPT.Methods:From February 2016 to March 2021, a total of 59 SHPT patients with 181 parathyroid glands (PTGs) admitted to the First Affiliated Hospital of Sun Yat-Sen University were enrolled. Gray-scale ultrasound and contrast-enhanced ultrasound were performed in every participant. Patients were divided into low-iPTH group ( iPTH<800 ng/L) and high-iPTH group (iPTH≥800 ng/L) according to the serum iPTH level. The characteristics of gray-scale ultrasonic imaging and contrast-enhanced ultrasonic imaging were analyzed by 2 sonographers.Biochemical parameters were collected and combined with ultrasonic characteristics to construct the clinical and ultrasound-based model. The relationship between the model and serum iPTH level was analyzed by multivariate linear regression (stepwise). Independent influencing factors on serum iPTH level was investigated in SHPT patients without iPTH-reducing drugs using history.Results:There were 19 patients in low-iPTH group and 40 patients in high-iPTH group.Serum calcium, serum phosphorus, serum creatinine, PTG number, total PTG volume, blood scores, calcification and cysts scores, CEUS scores (washing-in phase and washing-out phase) were significantly different between two groups(all P<0.05). The multivariate linear regression (stepwise) showed that serum phosphorus, total PTG volume and blood scores were independently related with serum iPTH level (standardized β coefficient were 0.387, 0.254 and 0.242 respectively; all P<0.05). Conclusions:Serum phosphorus, total PTG volume and blood scores are independent influencing factors on serum iPTH level. 8.Elasticity characteristic analysis of boundaries and surrounding liver tissue of focal liver lesions and their influencing factors

		                        			Objective:To qualitatively analyze the elasticity characteristics of boundaries and surrounding liver tissue of focal liver lesions (FLL) by real-time shear wave elastography (SWE), and to analyze the relating influencing factors.Methods:One hundred and fifty-two patients with FLLs (152 lesions) from February 2012 to October 2013 in the First Affiliated Hospital of Sun Yat-Sen University were collected. SWE of FLLs and the surrounding liver tissue was performed and baseline clinical data were collected. The elastic boundary (clear or not clear) of FLLs and elasticity distributions of surrounding liver tissue and their influencing factors were analyzed.Results:A total of 26 benign lesions and 126 malignant lesions were included in this study. Twenty-five cases (96.2%) of benign lesions showed clear elastic boundaries and 1 case (3.8%) showed unclear elastic boundary while 82 cases (65.1%) of malignant lesions showed clear elastic boundaries and 44 cases (34.9%) showed unclear elastic boundaries. Multivariate analysis showed benign/malignant lesion was an independent influencing factor for elastic boundary ( P<0.05). Twenty cases (76.9%), 6 cases (23.1%) and 0 case of liver tissue around benign lesions showed type A, B and C elastic distribution respectively while 17 cases (13.5%), 60 cases (47.6%), and 49 cases (38.9%) of liver tissue around malignant lesions showed type A, B, and C elastic distribution. There were statistically significant differences of benign and malignant lesions, different viral hepatitis backgrounds, different gender, and different ALB, TBIL, ALT levels in elasticity distributions of surrounding liver tissue of FLLs among type A, B and C (all P<0.05) while there was no significant difference in different lesion size( P=0.036). Conclusions:The elasticity characteristics of boundaries and surrounding liver tissue of FLLs on SWE images can provide a reference for differential diagnosis of benign and malignant FLLs. The elastic characteristics of surrounding liver tissue of FLLs are influenced by the nature of lesions and the background of liver disease, but are not influenced by lesion size. Further quantitative studies of elasticity characteristics of boundaries and surrounding liver tissue are needed for the differential diagnosis of different types of FLLs.
		                        		
		                        		
		                        		
		                        	
9.A model based on shear wave elastography to predict post-hepatectomy liver failure in patients with hepatocellular carcinoma

		                        			Objective:To establish a model based on two-dimensional shear wave elastography (2D-SWE) for predicting post-hepatectomy liver failure (PHLF) among patients with hepatocellular carcinoma (HCC).Methods:One hundred and one consecutive patients with HCC undergoing hepatectomy from August 2018 to July 2019 were enrolled prospectively in the First Affiliated Hospital of Sun Yat-Sen University. Laboratory tests, shear wave elastography in liver parenchyma, and abdominal contrast-enhanced computed tomography (CT) or magnetic resonance imaging (MRI) were performed preoperatively. Liver functional reserve, liver stiffness (LS), and tumor-related imaging parameters were assessed. PHLF was defined according to the definition of International Study Group of Liver Surgery Recommendations (ISGLS). A predictive model was developed by logistic regression analysis and the performance thereof was evaluated by receiver operating characteristic (ROC) curve analysis and Hosmer-Lemeshow test.Results:PHLF occurred in 39 patients (38.9%). Logistic regression analysis identified that international normalized ratio ( OR=1.09, P=0.026), LS( OR=1.297, P=0.004) and the largest nodule diameter( OR=1.191, P=0.015) were independent risk factors of PHLF.The area under curve (AUC) of the model was 0.842(95% CI =0.763-0.921), which was significantly higher than those of ALBI score, MELD score and Child-Pugh score (AUC 0.626-0.688, P<0.05). The model also showed good calibration in Hosmer-Lemeshow test ( P=0.498). Conclusions:A model based on 2D-SWE provides good preoperative prediction of PHLF among patients with HCC, which might have the potential in better customizing treatment strategy in those patients.
		                        		
		                        		
		                        		
		                        	
10.Study on the relationships between the levels of trace elements and nutritional status of preschool children in Guangzhou

		                        			Objective   To investigate the levels of trace elements in preschool children in Guangzhou and their relationships with nutritional status, and to provide a scientific basis for guiding children's nutritional diet and timely and reasonable supplementation of trace elements. Methods   A cross-sectional study was used to perform physical examinations and collect blood samples from 5 002 children who were examined in the Department of Pediatric Health of a Maternal and Child Health Hospital in Guangzhou in 2018. The levels of trace elements including iron, zinc and copper were detected by atomic absorption spectrometer.   Results   The median (upper and lower quartile) levels of iron, zinc and copper for preschool children in a district of Guangzhou were 7.80 (7.39, 8.19) mmol / L, 69.0 (61.0, 76.5) μmol / L, 18.6 (15.9, 21.4) μmol / L. There was no significant difference in the levels of iron, zinc and copper between different genders (P>0.05). The levels of iron and zinc increased significantly with the age of children (P=0.000). There was no significant difference in the level of copper in different ages (P>0.05). The prevalence rates of iron, zinc and copper deficiency in preschool children were 14.93%, 21.93% and 0.24%, respectively, and there was no significant difference between different genders (P>0.05).There were no significant differences in the prevalence of iron and copper deficiency in different ages (P>0.05), while the zinc deficiency rate decreased with the age of children (P = 0.000).  (P<0.05). The levels of iron and zinc in overweight children, and those suffering from malnutrition,and obesity were lower than those in normal children. There was no statistically significant difference in the levels of copper in children with different nutritional status (P>0.05). The prevalence rates of iron and zinc deficiency in children with malnutrition, overweight and obesity were higher than those in normal children. There was no significant difference in the prevalence of copper deficiency in children with different nutritional status (P>0.05).   Conclusion   The prevalence of iron, zinc deficiency is high in preschool children in Guangzhou.
