1.Effect observation of Kanglaite injection assisted with camrelizumab combined with chemotherapy in the treatment of advanced NSCLC

OBJECTIVE To observe the clinical efficacy of Kanglaite injection assisted with camrelizumab combined with chemotherapy in the treatment of advanced non-small cell lung cancer (NSCLC). METHODS A total of 192 patients with advanced NSCLC and hospitalized in the TCM oncology department of our hospital from January 1st, 2018 to December 1st, 2022 were retrospectively selected as the study objects, and were divided into observation group (additional use, n=104) and control group (without additional use, n=88) according to whether the patients additionally received Kanglaite injection based on camrelizumab combined with chemotherapy (carboplatin+pemetrexed). The short-term therapeutic effects of 2,4 and 6 cycles were compared between the two groups. The levels of peripheral blood immune function indexes and serum tumor markers were compared before treatment, after 3 cycles of treatment and after treatment. The long-term therapeutic effects as well as the occurrence of adverse drug reaction(ADR) during hospitalization were compared between the two groups. RESULTS After 3 treatment cycles and at the end of treatment, the CD4+ T lymphocyte ratio and CD4+/CD8+ in the observation group were notably greater than the control group (P<0.05); the levels of serum carcinoembryonic antigen and cytokeratin 19 fragment antigen 21-1 were significantly lower than those in the control group (P<0.05). The overall survival of the observation group was significantly longer than that of the control group (P<0.05), and the median overall survival was (185.27±38.21) d and (132.11±34.23) d, respectively. There were no significant differences in the whole ADR and grade ≥3 ADR between the two groups during hospitalization(P>0.05). CONCLUSIONS Based on camrelizumab combined with chemotherapy, the addition of Kanglaite injection can enhance immunological response and prolong overall survival in advanced NSCLC patients.
2.Effects of Changweiqing (肠胃清) on Transplanted Tumor Growth of Colorectal Cancer Cells and Expression of STAT3 and Bcl-2 Gene Splicing Isoforms

ObjectiveTo explore the possible mechanism of Changweiqing (肠胃清) in the treatment of colorectal cancer. MethodsHCT 116 cancer cells were used to prepare intestinal cancer cells with silenced polypyrimidine region binding protein 3 (PTBP3) gene and stably transfected cells with overexpressed PTBP3 gene. Stably transfected cells with silenced PTBP3, stably transfected cells with overexpressed PTBP3 and untransfected cancer cells were injected into the armpit of 72 nude mice to construct three different subcutaneous transplanted tumor models of colorectal cancer cells, including the silenced model, the overexpressed model and the control model, with 24 mice per model. Mice of each transplanted tumor modelwere randomly divided into Changweiqing (CWQ) group, oxaliplatin (OXA) group and normal saline (NS) group, with 8 mice in each group. The CWQ groups were given intragastric administration of 35.9625 g/kg of Changweiqing oral liquid and were intraperitoneally injected with 0.2ml of normal saline; the NS groups were given 0.5ml of normal saline by gavage, and intraperitoneal injection of 0.2ml of normal saline; the OXA groups were intraperitoneally injected with 5 mg/kg (0.2 ml) of oxaliplatin and given 0.5ml of normal saline by gavage. Each group was given intragastric administration once a day and intraperitoneal injection three times a week. After 31 days, the weight of subcutaneous tumors in each group was measured, and the tumor inhibition rate of the groups in each model were measured. Immunohistochemistry and other methods were used to detect the expression level of cell proliferation cell nuclear antigen Ki67 and apoptosis index. Real-time PCR and Western Blot were used to detect mRNA and protein expressions of PTBP3, signal transducer and activator of transcription 3 (STAT3) splicing isoform α (STAT3α), STAT3 splicing isoform β (STAT3β), B-cell lymphoma/leukemia-2 (Bcl-2) splicing isoform α (Bcl-2α), and Bcl-2 splicing isoform β (Bcl-2β) in subcutaneous tumor cells in each group. ResultsFor all three transplanted tumor models, the weight of the subcutaneous tumors and Ki67 expression level of subcutaneous tumor tissue in all CWQ groups and OXA groups were lower than those of the corresponding NS groups, while the apoptosis level were higher (P<0.05 or P<0.01). The mRNA and protein expressions of PTBP3, STAT3α, and Bcl-2α in the subcutaneous tumor tissues of the silenced model CWQ group and the overexpressed model CWQ group were lower than those of the corresponding NS groups, while the mRNA and protein expression levels of STAT3β and Bcl-2β were higher (P<0.05 or P<0.01). All there groups of silenced model had lower subcutaneous tumor weight, Ki67 expression level, and mRNA and protein expression levels of PTBP3, STAT3α, and Bcl-2α in subcutaneous tumor tissue, as well as higher apoptosis level and mRNA and protein expression levels of STAT3β and Bcl-2β than those in all groups of control model; all groups of overexpressed model had higher subcutaneous tumor weight, Ki67 expression level, and mRNA and protein expression levels of PTBP3, STAT3α, and Bcl-2α , while lower apoptosis level and mRNA and protein expression levels of STAT3β and Bcl-2β than those in all control model groups (P<0.05 or P<0.01). In the control model, compared with the NS group, The tumor inhibition rate of all OXA groups was higher than that of corresponding CWQ groups, respectively. Compared to that of each control model group, the tumor inhibition rate was positive value of each silenced model group, and negative value of each overexpressed model group. ConclusionPTBP3 can promote the proliferation and inhibit apoptosis of intestinal cancer cells, upregulate the expression of STAT3α and Bcl-2α, and downregulate the expression of STAT3β and Bcl-2β in intestinal cancer cells. The meachnism of action of Changweiqing in the treatment of colorectal cancer maybe related to the inhibition of PTBP3, and regulation of the expression of STAT3α, STAT3β, Bcl-2α, and Bcl-2β.
3.Folic acid treatment regulates C2C12 myoblast diferentiation via JNK/p38 MAPK signaling pathway

AIM:To observe the effect of folic acid(FA)on C2C12 myoblast proliferation and differentia-tion,and to explore its mechanism.METHODS:During the proliferation stage,C2C12 myoblasts were treated with vari-ous concentrations of FA(0,2.5,5,10 and 20 μmol/L).The cell status was observed under a microscope,cell viability was detected using the MTT method,and cell proliferation was assessed using the EdU method.In the differentiation stage,C2C12 cells were divided into control(Ctrl)group(0 μmol/L FA)and FA group(10 μmol/L FA).On day 2 or 4 of differentiation,immunofluorescence staining and Western blot were employed to detect the expression of myoblast differen-tiation-related proteins,myoblast determination protein 1(MyoD),myogenin(MyoG)and myosin heavy chain(MyHC).The myotubule formation in each group was analyzed.On day 4 of differentiation,C2C12 cells were treated with FA for 0,1,3 and 6 h,and the protein levels of p-JNK,JNK,p-p38 MAPK and p38 MAPK at each time point were detected by Western blot.Additionally,C2C12 cells after 4-day differentiation were divided into Ctrl group,FA group,FA+ SP600125(specific inhibitor of JNK)group,and FA+SB203580(specific inhibitor of p38)group.The cells in FA+ SP600125 and FA+SB203580 groups were treated with 10 μmol/L SP600125 or SB203580 for 1 h,followed by treatment with 10 μmol/L FA for 24 h.The cells in FA group were treated with 10 μmol/L FA for 24 h,while the cells in Ctrl group were left untreated.The protein levels of p-JNK,JNK,p-p38 MAPK,p38 MAPK and MyHC were detected by Western blot.RESULTS:(1)Compared with 0 μmol/L FA group,the number of the cells in other concentration groups in-creased,cell viability was raised(P<0.05 or P<0.01),and the rate of EdU positive cells increased(P<0.05).(2)Com-pared with Ctrl group,the expression levels of MyoD,MyoG and MyHC in FA group were increased(P<0.05),and the myotube fusion index was raised(P<0.05 or P<0.01).(3)Compared with 0 h group,the ratios of p-JNK/JNK and p-p38 MAPK/p38 MAPK were elevated after FA treatment for 1,3 and 6 h(P<0.05 or P<0.01),and showed a trend of gradual increase with the extension of treatment time.(4)After FA treatment,the ratios of p-JNK/JNK and p-p38 MAPK/p38 MAPK,and the expression of MyHC were elevated(P<0.01).Treatment with SP600125 decreased the ratio of p-JNK/JNK and the expression of MyHC(P<0.05),while SB203580 intervention cut down the ratio of p-p38 MAPK/p38 MAPK and the expression of MyHC(P<0.05 or P<0.01).CONCLUSION:Folic acid can promote the differentiation of C2C12 myoblasts by activating the JNK/p38 MAPK signaling pathway.
4.Incidence of acute kidney injury following cardiac surgery and related risk factors in 4 878 patients

Objective:To investigate the incidence of acute kidney injury (AKI) following cardiac surgery and related risk factors in 4 878 patients.Methods:The information from patients who underwent cardiac surgery through March 2015 to October 2015 was collected retrospectively from the electronic database of Beijing Anzhen Hospital. A total of 4 878 patients were divided into AKI group and non-AKI group according to whether AKI occurred within 7 days after cardiac surgery. The incidence of AKI was calculated, and the AKI incidence in different types of cardiac surgeries were compared. Clinical data such as baseline clinical information, operation information, comorbidity, hospital stay time, life ability score in discharge from the hospital, and so on, were compared between AKI group and the non-AKI group using univariate analysis. Risk factors for AKI following cardiac surgery were analyzed using the binary multivariate logistic regression.Results:A total of 933 patients suffered from AKI (19.1%) following cardiac surgery. The time of stay in the hospital was longer in AKI group than that in the non-AKI group [(14.4±8.9) vs (13.7±7.7) d, P<0.05)]. The incidence of AKI in different types of cardiac surgeries varied significantly ( P<0.001). The logistic regression analysis showed that male, diabetes, hypertension, the elevated basic serum creatinine, cardiac dysfunction (NYHA grade≥Ⅲ), cardiopulmonary bypass, a combination of operations≥3, the rethoracotomy exploration and hemostasia, and using an invasive ventilator for over 96 hours were the independent risk factors for the AKI following cardiac surgery (all P<0.05), and the odds ratio (95% confidence interval) were 1.81(1.46-2.24), 1.29(1.03-1.62), 5.85(4.73-7.22), 1.81(1.36-2.40), 4.49(3.60-5.60), 1.84(1.49-2.27), 23.24(18.25-29.59), 2.34(1.45-3.77) and 1.94(1.09-3.43) respectively. Conclusions:The incidence of AKI after cardiac surgery in Beijing Anzhen Hospital is 19.1%. AKI following cardiac surgery prolongs the time of stay in the hospital. Independent risk factors for AKI following cardiac surgery are multiple, and one of the most critical factors is a combination of operations≥3.

6.Effect of comfort-care theory on improving quality nursing in post-anesthesia care unit

Objective To explore methods and effects of implementing comfort nursing care in post-anesthesia care unit by using relative theories of comfort nursing.Methods In anesthesia recovery period, a series of comfort-care measures were conducted to the patients, including nursing care with improved position, relieving postoperative dry mouth by spraying lemonade, protecting body temperature and health education on early post-operation pain, and many others.Results Satisfaction of patients, anesthesiologists and surgeons were significantly improved after developing comfort nursing care in post-anesthesia care unit.Conclusions Development of comfort nursing care in post-anesthesia care unit can improve comfort degree of patients in anesthesia recovery period, reduce their psychological issues like fear and anxiety, improve their satisfaction and promote postoperative recovery.
7.Effects of minocycline on the cognition and expression of BDNF, Bcl-2 and Bax in hippocampus of Alzheimer’s disease model rats

Objective To investigate the effect of minocycline on the cognition and expressions of brain-derived neurotrophic factor (BDNF), apoptosis related factor Bcl-2 and Bax in hippocampus of rats with Alzheimer’s disease (AD).
Methods The rat model was established by microinjection of Aβ25-35 into lateral ventricle. Thirty healthy male SD rats were randomly divided into three groups:control group, model group and minocycline treatment group. Normal saline 1 mL/(kg·d) was intraperitoneally injected in control group and model group. The minocycline treatment group was intraperitoneally injected with minocycline 50 mg/(kg · d) for 14 days. Morris water maze was used to detect the behaviors of animals. The expressions of BDNF, Bcl-2 and Bax in hippocampus were measured by Western blotting and enzyme linked immunosorbent assay (ELISA). The apoptosis of neurons was detected by TdT-mediated dUTP nick-end labeling (TUNEL). Results Minocycline greatly improved the behaviors of AD rats, up-regulated the expressions of BDNF and Bcl-2, and down-regulated the expression of Bax in hippocampus, and reduced cell apoptosis. Conclusion Minocycline plays a protective role in neural function by promoting the growth of neurons and inhibiting the neuronal apoptosis.
8.A qualitative research on work experience of nurses in post anaesthesia care unit

Objective To explore nurses′real work experience of post anaesthesia care unit (PACU) in a level three class A hospital so as to provide reference for nursing managers to meet nurses′demands. Methods The phenomenology method of qualitative research was used to in-depth interview 6 nurses of PACU in a level three class A hospital of Beijing.The data was analyzed and concluded by Colaizzi phenomenology method and Nvivo 1 0.0 encoding software.Results Inner experience of nurses in PACU during working could be divided into 4 themes including occupational values,multiple roles,facing pressure currently,occupational planning and demands by analysing and concluding 6 interview data.Conclusions The special role of nurses in PACU enhanced their occupational identity and the sense of mission.Managers and leadership should pay attention to nurses′pressure and demands in this kind of work and should go into overdrive in support and guarantee for nurses so as to further improve the quality of care in PACU.
9.Clinical research and application of self-made pressurized oxygen bag

Objective To explore the clinical application of self-made pressurized oxygen bag . Methods A total of 200 patients in anesthesia recovery room were randomly divided into experimental group and control group , 100 cases respectively .Patients were escorted from recovery room to the ward .On the way patients in control group used the traditional oxygen bag while patients in experimental group used self -made pressurized oxygen bag .Vital signs, comfort and nurses′satisfaction in transit were compared between two groups.Results After leaving PACU with oxygen saturation for 10 minutes, blood oxygen saturation spo (SpO2) of patients was (98.73 ±1.52) %in experimental group and (94.59 ±2.53)%in control group, the difference between two groups was statistically significant (P<0.05).There was no significant difference in heart rate (HR), blood pressure (BP) and respiratory rate (RR) at different time points after leaving PACU between two groups (P>0.05).The differences in the use of convenience , the fixed reliability and oxygen flow stability between two groups were statistically significant (P<0.05).Conclusions The self-made pressurized oxygen bag has the advantages in the use of convenience , credible fixation and stable oxygen flow , and could effectively maintain the SpO 2 so as to meet the requirements for oxygen inhalation in clinical transport .
10.Comparison of the effects of different intervention methods on alleviating xerostomia in patients during anesthesia recovery period

Objective To compare the effects of different intervention methods on relieving the xerostomia and to explore the best method to relieve xerostomia in patients during anesthesia recovery period.Methods A total of 108 patients in post anesthesia care unit after general anesthesia were recruited from March 2015 to June 2016 in Chinese PLA General Hospital.They were randomly divided into the experimental group 1 (27 cases),experimental group 2 (26 cases),condition conditional group (28 cases),blank control group (27 cases) with different methods of moistening oral cavity after extubation with 2 ml at a time for every 10 minutes including lemon water sprayer,vitamin C sprayer,warm water sprayer and non-intervention.And then,the visual analogue scale (VAS) for the intensity of xerostomia and the prevalence of adverse reactions including nausea,vomiting,abdominal pain,abdominal distension and so on were compared before intervention,20 minutes and 50 minutes after intervention.Results The results showed that the scores of VAS in the experimental group 1 were lower than those in the experimental group 2,the conditional control group and the blank control group 20 minutes and 50 minutes after intervention with significant differences [(Dunnett-t =-1.200,-1.733,-1.733;P< 0.01;20 minutes after intervention),(Dunnett-t =-2.150,-2.500,-5.567;P<0.01;50minutes after intervention)].Conclusions The application of the lemon water sprayer for moistening oral cavity can effectively relieve the xerostomia in patients during anesthesia recovery period.Besides,the effects of the lemon water sprayer are better than those of vitamin C sprayer and warm water sprayer.
