1.Expert consensus on standardized clinical applications of minimally invasive tooth extraction techniques

Tooth extraction is a common and widely employed therapeutic procedure in oral and maxillofacial surgery.Minimally invasive tooth extraction can reduce both physical and psychological trauma to the patients,and is widely recommended as a first-line clinical treatment.But currently no guidelines or consensus has been available to provide a systematic introduction of minimally invasive tooth extraction to guide the clinical practices.To address this issue,this consensus,based on a comprehensive literature review and clinical experiences of experts,systematically summarizes the indications,target patients,and contraindications of minimally invasive tooth extraction,the overall workflow of this procedure(preoperative preparation,surgical steps,postoperative management,postoperative instructions,medications,and follow-up),and its common postoperative complications to provide a comprehensive guidance for clinical application of this technique.
2.Effect of internal fixation with mini plate and cannulated screw on postoperative recovery of joint function in patients with large fracture of posterior malleolus

Objective To analyze the impact of mini plate and cannulated screw internal fixation on joint function recovery in patients with posterior malleolus fracture.Methods A total of 150 patients with posterior malleolus fractures,treated at our hospital from March 2021 to June 2023,were included in this study.They were divided into two groups using the odd-even number method.The control group consisted of 75 patients who underwent cannulated screw internal fixation,while the study group comprised 75 patients who received mini plate internal fixation.Clinical indicators,ankle range of motion,ankle function,and health status were compared and analyzed between the two groups.Additionally,levels of inflammatory factors,postoperative complications,and clinical efficacy were assessed.Results In terms of operation time and intraoperative blood loss,there was no statistically significant difference between the two groups(P>0.05).However,the study group exhibited shorter ambulation days,fracture healing time,and hospitalization days compared to the control group(P<0.05).Moreover,the study group demonstrated significantly improved ankle dorsiflexion,ankle plantar flexion,foot varus and foot valgus range of motion compared to the control group(P<0.05).Additionally,higher AOFAS score and KPS score were observed in the study group as compared to the control group(P<0.05).Furthermore,levels of IL-6,IL-8 and CRP were lower in the study group than in the control group(P<0.05).The incidence of postoperative complica-tions was also lower in the study group than in the control group(P<0.05).Conclusion Mini plate internal fixa-tion for posterior malleolus fracture yields ideal outcomes by promoting improvement in clinical indicators and ankle range of motion while effectively enhancing ankle function,reducing inflammatory reaction as well as minimizing postoperative complications.

Coronary heart disease (CHD) and stroke are the most well-known cardiovascular diseases, which share many common pathological basis. Yindan Xinnaotong soft capsule (YDXNT) is a commonly used Chinese patent medicine in the treatment of stroke and CHD. However, its action of mechanism of co-treatment for stroke and CHD is still unclear. The aim of this study was to explore the common mechanism of YDXNT in co-treatment of CHD and stroke using network pharmacology, experimental verification and molecular docking. An integrated literature mining and databases of IPA, ETCM, HERB, Swiss Target Prediction, OMIM and GeneCards were used to screen and predict active ingredients and potential targets of YDXNT in co-treatment of CHD and stroke. The protein-protein interaction network, GO analysis and pathway analysis were analyzed by IPA software. The effect of YDXNT on core targets was verified by immunofluorescence. UPLC-QTOF/MS and molecular docking were used to screen and predict the main active constituents of YDXNT and their interactions with core targets. In the past 37 years, human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) has undergone various major transmission routes in China, with the world most complex co-circulating HIV-1 subtypes, even the prevalence is still low. In response to the first epidemic outbreak of HIV in injecting drug users and the second one by illegal commercial blood collection, China issued the Anti-Drug Law and launched the Blood Donation Act and nationwide nucleic acid testing, which has avoided 98,232 to 211,200 estimated infections and almost ended the blood product-related infection. China has been providing free antiretroviral therapy (ART) since 2003, which covered >80% of the identified patients and achieved a viral suppression rate of 91%. To bend the curve of increasing the disease burden of HIV and finally end the epidemic, China should consider constraining HIV spread through sexual transmission, narrowing the gaps in identifying HIV cases, and the long-term effectiveness and safety of ART in the future.

8.The spectrum of chronic diseases and influential factors of immigrants moving water source area of South-to-North Water Diversion Project

Objective To understand the prevalence and disease distribution of migrants within the water source area of the Middle Route Project of South-to-North Water Diversion Project and analyze the influencing factors. Methods By multi-stage stratified cluster sampling method, 307 immigrants and 676 immigrants were selected from Danjiangkou reservoir area, Shiyan city, Hubei Province. Results The prevalence of chronic diseases among in-migration was 51.8%. The prevalence rates of the top five chronic diseases were waist and leg pain, hypertension, gastritis, heart disease and diabetes. The prevalence of chronic diseases among out-migration was 65.1%. The prevalence rates of the top five chronic diseases were waist and leg pain, hypertension, gastritis, heart disease and bronchitis. The rate of chronic disease, the rate of two weeks and the rate of two weeks' visit were higher than that of emigration(all P<0.05). The results of multivariate analysis showed that the factors influencing the chronic diseases of immigrants were 60 years old (OR=59.928,P<0.001), unsatisfied family income (OR=0.377, P=0.002) and poor economic status (OR=0.517, P=0.028). The factors influencing the chronic diseases of immigrants were 60 years old (OR=26.190, P<0.001), smoking (OR=1.856,P=0.004), unsatisfactory family income (OR=0.598,P=0.029) and poor economic status (OR=0.460,P<0.001). Conclusions The prevalence of migrants within a high level is at a high level. Actually raising the income of immigrants is conducive to improving the health condition of immigrants.
9.Experience and reference to China's trans-provincial healthcare from European cross-border healthcare management

With the development of integration process, how to manage citizens' cross-border healthcare be-tween member countries in an organized way has become a major agenda for European Union (EU). Through great efforts over years,a well-functioning mechanism has been established by EU's social health insurance and has been of global interest. It's believed that China's trans-provincial healthcare management can learn a lot from this mechanism. This study aims to provide experiences and reference to China's trans-provincial healthcare management by overview EU's mechanism for cross-border healthcare and then analyzing the differences between China and European Union in terms of object of management,authorization,service institutions,types of healthcare services,and reimbursement of medical insurance in different places. This study suggests that it's necessary for each province to officially introduce relevant policies in order to speed up and facilitate the establishment of trans-provincial healthcare management mech-anism. Moreover,to achieve orderly seeking medical treatment, health insurance agencies should be endowed with approval power to authorize patient trans-provincial application. Meanwhile, the network of health institutions which are entitled to provide services to trans-provincial patients should be expanded. Along with such expansion,and given the lack of understanding on behaviors of health institutions in different provinces for a specific local health insurance agency,it's required that national trans-provincial healthcare management system should take an active role in putting such health institutions under supervision.
10.Verifying performance characteristics of the SLCO1B1 & APOE gene polymorphism detection kit

Objective To verify the performance characteristics of the human SLCO1B1 & APOE gene detection kit (PCR-fluorescent probe technique).Methods The verification protocol for the human SLCO1B1 & APOE gene detection kit manufactured by Wuhan YZY Medical Science and Technology Co.,Ltd.was designed by referring to the requirements of professional standard and pertinent literatures,and its accuracy,specificity,lowest detection limit and anti-interference ability were compared with those of sequencing technique (gold standard).Results The results coincidence of 20 clinical samples from fluorescence PCR and sequencing technique was 100%,which met the requirement of accuracy.Forty clinical samples with wild type loci determined by fluorescence PCR were verified by sequencing and no any mutation was detected,which met the requirement of specificity.The lowest detection limit of the kit was 10 ng/μL.Three interfering substances,including hemoglobin,bilirubin and triglyceride,were individually added into the blood samples with known genotype,and the results determined by the kit were coincident with that without interfering substances,which met the requirement of anti-interference ability.Conclusion The verification parameters of the human SLCO1B1 & APOE gene detection kit are basically consistent with the manufacturer's statement,indicating that the kit meets the requirements of the relevant quality management and may be applied to the detection of clinical samples.
