1.Translational Research of Electromagnetic Fields on Diseases Related With Bone Remodeling: Review and Prospects

Electromagnetic fields can regulate the fundamental biological processes involved in bone remodeling. As a non-invasive physical therapy, electromagnetic fields with specific parameters have demonstrated therapeutic effects on bone remodeling diseases, such as fractures and osteoporosis. Electromagnetic fields can be generated by the movement of charged particles or induced by varying currents. Based on whether the strength and direction of the electric field change over time, electromagnetic fields can be classified into static and time-varying fields. The treatment of bone remodeling diseases with static magnetic fields primarily focuses on fractures, often using magnetic splints to immobilize the fracture site while studying the effects of static magnetic fields on bone healing. However, there has been relatively little research on the prevention and treatment of osteoporosis using static magnetic fields. Pulsed electromagnetic fields, a type of time-varying field, have been widely used in clinical studies for treating fractures, osteoporosis, and non-union. However, current clinical applications are limited to low-frequency, and research on the relationship between frequency and biological effects remains insufficient. We believe that different types of electromagnetic fields acting on bone can induce various “secondary physical quantities”, such as magnetism, force, electricity, acoustics, and thermal energy, which can stimulate bone cells either individually or simultaneously. Bone cells possess specific electromagnetic properties, and in a static magnetic field, the presence of a magnetic field gradient can exert a certain magnetism on the bone tissue, leading to observable effects. In a time-varying magnetic field, the charged particles within the bone experience varying Lorentz forces, causing vibrations and generating acoustic effects. Additionally, as the frequency of the time-varying field increases, induced currents or potentials can be generated within the bone, leading to electrical effects. When the frequency and power exceed a certain threshold, electromagnetic energy can be converted into thermal energy, producing thermal effects. In summary, external electromagnetic fields with different characteristics can generate multiple physical quantities within biological tissues, such as magnetic, electric, mechanical, acoustic, and thermal effects. These physical quantities may also interact and couple with each other, stimulating the biological tissues in a combined or composite manner, thereby producing biological effects. This understanding is key to elucidating the electromagnetic mechanisms of how electromagnetic fields influence biological tissues. In the study of electromagnetic fields for bone remodeling diseases, attention should be paid to the biological effects of bone remodeling under different electromagnetic wave characteristics. This includes exploring innovative electromagnetic source technologies applicable to bone remodeling, identifying safe and effective electromagnetic field parameters, and combining basic research with technological invention to develop scientifically grounded, advanced key technologies for innovative electromagnetic treatment devices targeting bone remodeling diseases. In conclusion, electromagnetic fields and multiple physical factors have the potential to prevent and treat bone remodeling diseases, and have significant application prospects.
2.Treatment of Chronic Urticaria with Traditional Chinese Medicine by Regulating PI3K/Akt Molecular Pathway: A Review

Chronic urticaria (CU) is a common skin disease worldwide, and its incidence is increasing year by year in various regions. Clinical manifestations such as severe itching can affect normal work, sleep, and daily life and increase the negative psychological burden caused by stress, anxiety, and depression. Mast cell activation and degranulation induced by immunoglobulin(Ig)E hypersensitivity is one of the core pathogenic mechanisms of CU, and there is no cure. Antihistamines such as cetirizine and loratadine are preferred for the clinical treatment of CU. Although they can effectively improve clinical manifestations such as itchiness, long-term application can increase the risk of adverse reactions and drug resistance. The phosphatidylinositol kinase/serine-threonine protein kinase B(PI3K/Akt) signaling pathway, as a classical signaling pathway regulated by phosphatidylinositol and tyrosine kinase receptor (RTK), is a key target regulating the production and release of cytokines in macrophages and affecting the migration of leukocytes and the activation of mast cells and inflammation, and it can be involved in a variety of metabolic processes, such as mast cell activation and degranulation induced by IgE hypersensitivity and abnormal activation of the complement system so that the PI3K/Akt molecular pathway could be an important target for the future eradication of CU. However, the mechanism and potential role of the PI3K/Akt signaling pathway in the treatment of CU are less reported in China. Now, this paper reviewed the molecular mechanism of PI3K/Akt signaling pathway regulation in the treatment of CU and provided corroborative evidence and therapeutic strategy choices for the treatment of CU with traditional Chinese medicine (TCM) from the perspectives of molecular regulation and network pharmacology analysis.
3.Mingshi Formula (明视方) for Low Myopia in Children with Heart Yang Insufficiency Syndrome: A Multicentre, Double-Blind, Randomised Placebo-Controlled Study

ObjectiveTo observe the effectiveness and safety of the Chinese herbal medicine Mingshi Granules (明视方颗粒) for low myopia in children with heart yang insufficiency. MethodsA multicentre, prospective, double-blind randomised controlled study was conducted, in which 290 children with low myopia from 8 centres were randomly divided into 145 cases in the treatment group and 145 cases in the control group, and the treatment group was given education, dispensing glasses, and Chinese herbal medicine Mingshi Granules, while the control group was given education, dispensing glasses, and granules placebo. Both Mingshi Granules and placebo granules were taken orally, 1 bag each time, twice daily, 4 weeks of oral intake and 2 weeks of rest as 1 course of treatment, a total of 4 courses of treatment (24 weeks). Equivalent spherical lenses, best naked-eye distance visual acuity, ocular axis, corneal curvature K1, adjustment amplitude, traditional Chinese medicine (TCM) symptom scores, calculate the amount of progression of equivalent spherical lenses, were observed at the 12th and the 24th week of treatment, at the 36th week and 48th week of follow-up, resectively, the control rate of myopia progression was evaluated at the 24th week, and safety indexes were observed before treatment. ResultsThe amount of progression of equivalent spherical lenses was lower in the treatment group than in the control group at the 48-week follow-up (P<0.05). The control rate of myopia progression at 24 weeks after treatment in the treatment group was higher (57.60%, 72/125) than that in the control group (44.63%, 54/121) (P<0.05). The best naked-eye distance visual acuity at 36-week follow-up in the treatment group was higher than that in the control group (P<0.05). Equivalent spherical lenses were significantly lower in both groups at all observation time points compared with pre-treatment (P<0.05), and were higher in the treatment group than in the control group at the 48-week follow-up (P<0.05). The ocular axes of both groups were significantly higher at each observation time point after treatment and at follow-up compared with before treatment (P<0.05). The amount of eye axis growth in the treatment group was lower than that in the control group at 24 weeks after treatment and at the 48-week follow-up (P<0.05). Corneal curvature K1 was significantly lower in the treatment group at the 24th week of treatment compared to pre-treatment (P<0.05). The magnitude of adjustment in the treatment group was significantly higher at the 36-week follow-up and at the 48-week follow-up than before treatment (P<0.05). The scores of white/dark complexion, white coating thin pulse, fatigue and total TCM symptom scores of children in both groups at the 12th, 24th, 36th and 48th weeks of follow-up were significantly lower than those before treatment (P<0.05); the scores of blurred vision at the 24th and 36th weeks of follow-up were significantly lower than those before treatment (P<0.05); and the scores of blurred vision in the treatment group at the 48th week of follow-up were signi-ficantly lower than those before treatment (P<0.05). In the treatment group, the score of fatigue was higher than that of the control group at the 36-week follow-up, and the score of blurred vision was lower than that of the control group at the 48-week follow-up (P<0.05). No adverse reactions or obvious abnormalities of the safety indexes were observed of the two groups during the treatment. ConclusionChinese herbal medicine Mingshi Granules showed the effect of controlling the progression of low myopia, improving the best naked eye distance visual acuity, slowing down the growth of the eye axis, improving some of the TCM symptoms, with good safety.
4.Improvement effects of poria acid on insulin resistance in rats with polycystic ovary syndrome and its mechanism

OBJECTIVE To study the improvement effects of poria acid on insulin resistance in rats with polycystic ovary syndrome (PCOS) and its mechanism. METHODS One hundred and twenty-six female rats were randomly separated into blank group, PCOS group, poria acid low-dose group (8.33 mg/kg), pachymic acid high-dose group (33.32 mg/kg), ethinylestradiol cyproterone group (positive control group, 0.34 mg/kg), recombinant rat high mobility group protein B1 protein (rHMGB1) group (8 μg/kg), and poria acid high dose+rHMGB1 group (33.32 mg/kg poria acid+8 μg/kg rHMGB1), with 18 rats in each group. Except for the blank group, the rats in all other groups were given Letrozole suspension intragastrically to construct the PCOS model. After successful modeling, administration was performed once a day for 4 weeks. After medication, the fasting blood glucose and fasting insulin levels, and insulin resistance index (HOMA-IR) were measured in rats; the levels of follicle-stimulating hormone (FSH), luteinizing hormone (LH) and testosterone (T) in rat serum, and the levels of interleukin-1β (IL-1β) and tumor necrosis factor- α (TNF- α) in ovarian tissue were detected; ovarian coefficients of rats were calculated; the pathological changes of ovarian tissue were observed; the expressions of HMGB1, receptor for advanced glycosylation elaine_ tanghong@sina.com end product (RAGE) and phosphorylated nuclear factor κB p65 (p-NF-κB p65) proteins were determined in ovarian tissue of rats. RESULTS Compared with the blank group, the pathological injury of ovarian tissue of rats in the PCOS group was serious, the levels of fasting blood glucose and fasting insulin, HOMA-IR and ovarian coefficient were increased, the levels of serum LH and T were increased, while the levels of FSH were decreased; the levels of IL-1β and TNF-α, the expressions of HMGB1, RAGE and p-NF-κB p65 protein in ovarian tissue were increased, with statistical significance (P<0.05). Compared with the PCOS group, pathological damage of ovarian tissue was reduced in poria acid low-dose and high-dose groups and ethinylestradiol cyproterone group, and fasting blood glucose, fasting insulin levels, HOMA-IR and ovarian coefficient were decreased; serum LH and T levels were decreased, while FSH levels were increased; the levels of IL-1β and TNF-α and the expressions of HMGB1, RAGE and p-NF-κB p65 protein in ovarian tissue were decreased, with statistical significance (P<0.05). The trend of corresponding indexes in rHMGB1 group was opposite to the above (P<0.05). Compared with poria acid high-dose group, the changes of the above indexes were reversed significantly in poria acid high-dose+rHMGB1 group (P<0.05). CONCLUSIONS Poria acid may improve insulin resistance and inhibit inflammatory reaction in PCOS rats by inhibiting HMGB1/ RAGE pathway.
5.Mechanism of Qizhenziyin mixture in the treatment of hypogonadism based on network pharmacology analysis and molecular docking

Objective To investigate the mechanism of Qizhenziyin mixture in the treatment of hypogonadism by using the network pharmacology approach. Methods The active components of Qizhenziyin mixture were obtained by searching TCMSP ,TCMID and HIT databases.The related targets of candidate compounds were obtained by searching STITCH databases. The potential targets of Qizhenziyin mixture in the treatment of hypogonadism were obtained by mapping the disease genes of hypogonadism with Genecards and DisGeNet databases. The protein interaction platform database (STRING) was used to construct the interaction relationship between action targets. The target protein interaction (PPI) network was constructed by introducing Cytoscape software. The mechanism of Qizhenziyin mixture in the treatment of hypogonadism was explained through the enrichment analysis of GO, KEGG and molecular docking technology. Results A total of 148 drug-disease chemical compounds, 96 drug-disease intersection targets, 1085 disease targets were obtained;the components for treating diseases are: quercetin,kaempferol, luteolin, etc; enrichment analysis of GO revealed 1792 biological processes (BP), 31 cellular components (CC) and 79 molecular functions (MF);the results of KEGG pathway enrichment analysis indicated such as FOXO signaling pathway, prostate cancer, AGE-RAGE signaling pathway in diabetic complications, HIF-1 signaling pathway, etc.The results of molecular docking showed that kaempferol and LEP had the best and stable binding energy. Conclusion The active components of Qizhenziyin mixture may play a role of the treatment of hypogonadism by improving insulin resistance and the expression of testosterone synthetase of Leydig cells.
6.Clinical observation of Aiyou balloon combined with modified B-Lynch suture in the treatment of intractable postpartum hemorrhage

Objective:To study the effect of Aiyou balloon combined with modified B-Lynch suture in the treatment of intractable postpartum hemorrhage (IPH) .Methods:Sixty-two patients with IPH in Wuxi maternal and Child Health Hospital from Feb. 2017 to Feb. 2022 were randomly divided into control group and observation group, with 31 cases in each group. The control group was treated with modified B-Lynch suture, and the observation group was treated with Aiyou balloon combined with modified B-Lynch suture. The operation conditions (operation time, intraoperative bleeding, 24 h postoperative bleeding and blood transfusion) of the two groups were compared, the hemostatic effect and coagulation function [prothrombin time (PT) , activated partial thromboplastin time (APTT) , thrombin time (TT) and fibrinogen (FIB) ] of the two groups were compared, and the complications one month after operation were counted.Results:There was no significant difference in operation time between the two groups ( P > 0.05) . The amount of intraoperative bleeding (851.26±81.25) mL, 24 h postoperative bleeding (142.37±24.89) mL and blood transfusion (1.57±0.39) U in the observation group were lower than those in the control group (1089.47±100.39) mL, (312.58±36.25) mL, (2.48±0.43) U ( P < 0.05) . The grade distribution of hemostatic effect in the observation group was better than that in the control group ( P < 0.05) . At 24 hours postpartum, Pt, APTT and TT were prolonged and FIB decreased in both groups ( P < 0.05) . Pt (13.55±1.42) s, APTT (37.58±4.18) s and TT (16.89±1.57) s in the observation group were shorter than those in the control group (14.78±1.63) s, (40.74±4.32) s, (18.95±1.81) s at 24 hours postpartum, and FIB (1.89±0.32) g/L was greater than those in the control group (1.71±0.28) g/L ( P < 0.05) . The incidence of complications in the observation group was 3.23% lower than 22.58% in the control group ( P < 0.05) . Conclusion:Aiyou balloon combined with modified B-Lynch suture in the treatment of IPH can reduce the amount of bleeding and blood transfusion, improve the hemostatic effect, improve the coagulation function and reduce the incidence of complications.
7.Characteristic of immune function decline in the process of “Declined of Vital Qi” caused by aging in mice

[摘 要] 目的:基于小鼠渐进衰老模型探讨衰老所致“正虚”的免疫功能衰退表征的特点。方法:使用不同月龄(2、6、15月龄)C57BL/6小鼠,通过流式细胞术检测并比较小鼠外周血和脾组织中T细胞、髓源性抑制细胞(MDSC)及其亚群的丰度变化。结果:外周血中T细胞亚群表型为CD3+CD4+CD44-CD62L+的幼稚CD4+ T细胞(2 vs 6月龄,P=0.137;2 vs 15月龄,P=0.004;6 vs 15月龄,P=0.105)和表型为CD3+CD8+CD44-CD62L+的幼稚CD8+ T细胞(2 vs 6月龄,P=0.179;2 vs 15月龄,P=0.001;6 vs 15月龄,P=0.015)出现与衰老有关的细胞比例降低,差异具有统计学意义。表型为CD3+CD4+CD44+CD62L+的中央记忆CD8+ T细胞出现与衰老有关的比例升高,差异具有统计学意义(2 vs 6月龄,P=0.01;2 vs 15月龄,P=0.007;6 vs 15月龄,P=0.164)。对脾组织的检测结果具有与外周血相同特点。同时,CD8+ T细胞比例随衰老逐渐升高(2 vs 6月龄,P=0.027;2 vs 15月龄,P<0.001;6 vs 15月龄,P<0.001);表型为CD8+CD28+的活化CD8+ T细胞亚群比例也出现随月龄增长的上升(2 vs 6月龄,P=0.863; 2 vs 15月龄,P=0.016;6 vs 15月龄,P=0.024),差异均具有统计学意义。结论:衰老所致“正虚”过程中,不同免疫细胞亚群变化并不都反映免疫抑制特点,虽然总体免疫功能下降,但单一表型难以反应整体免疫功能变化。

9.Intervention of Lung Cancer by Chinese Medicine and Active Components via Affecting Gut Microbiota:A Review

As a malignancy with the highest morbidity and mortality in the world, lung cancer poses a huge threat to the health and life safety of all human beings. Most lung cancer patients are already in the advanced stage when they are diagnosed, and the treatment of advanced lung cancer often brings heavy mental pressure and economic burden to patients but has little effect. Therefore, the early diagnosis and treatment of lung cancer has become a major problem for medical researchers. At present, chemotherapy, immunotherapy, targeted therapy, and other treatment methods still have problems such as intolerance of patients and drug resistance, so there is an urgent need for human beings to seek new methods to treat lung cancer. Currently, the relationship between gut microbiota and disease occurrence, development, and prognosis, and the treatment of diseases by regulating gut microbiota have become a hot field of medical research. There are significant differences in gut microbiota between lung cancer patients and healthy people. Intestinal microorganisms can act on the respiratory system through the gut-lung axis, thereby affecting the occurrence, development, and prognosis of lung cancer and other respiratory diseases. As a peculiar means of treatment in China, Chinese medicine can effectively delay tumor progression, prevent postoperative recurrence and metastasis, reduce complications in the course of treatment, improve the quality of life, and prolong the survival of patients. Therefore, Chinese medicine is widely involved in the treatment of malignancies. Some Chinese medicine monomers, compounds, and active components have been found to regulate the gut microbiota. They can regulate the metabolism of the body, control the inflammatory response, build an immune barrier, or play a synergistic effect with various lung cancer treatments by affecting gut microbiota, so as to achieve the anti-tumor purpose. This article systematically reviewed the research on Chinese medicine and effective components in regulating gut microbiota, creating tumor-suppressing microenvironment, and intervening in the treatment of lung cancer, in order to provide new research ideas for the treatment of lung cancer.
10.Establishment of contralateral arteriovenous fistula by using the waste vein on the side of central venous lesion: a case report.

Central venous lesion is a difficult problem in the vascular access complications of hemodialysis, which can cause serious clinical symptoms and affect the quality of hemodialysis and life of patients. We established arteriovenous fistula of the contralateral graft blood vessel with the used vein on the diseased side of the central vein of the patient. The arteriovenous fistula of the graft blood vessel was successfully punctured and hemodialysis was performed 2 weeks later. In this way, we not only solved the problem of venous hypertension and subsequent vascular access in the patient, but also reserved more vascular resources.
