1.Prognostic Factors of Liposarcoma in Head and Neck

Objective To explore the pathogenesis and prognostic factors of liposarcoma in the head and neck region, and simultaneously analyze the efficacy of different treatment regimens. Methods A retrospective analysis was performed on all patients with primary untreated head and neck liposarcoma who were diagnosed and underwent surgical treatment at our hospital from January 2008 to January 2024. All patients were monitored during follow-up, and their prognoses were analyzed using SPSS software. Results A total of 30 patients were included in the study. Liposarcoma accounted for up to 60% of the cases in the orbit, while the remaining liposarcomas were primarily located in various interspaces of the neck. Dedifferentiated liposarcoma was the most common type, comprising 33%, while myxoid pleomorphic liposarcoma was the rarest at 4%. The tumor pathological type (P<0.001) and Ki67 (P=0.014) significantly affected the tumor control rate. However, an analysis of disease-specific survival rates revealed no significant differences across various factors (all P>0.05). Conclusion The prognosis of head and neck liposarcoma is better compared to that of liposarcomas in other parts of the body. However, myxoid pleomorphic liposarcoma, pleomorphic fat sarcoma, and high Ki67 levels are indicators of poor prognosis. Additionally, postoperative adjuvant radiotherapy does not significantly enhance disease-specific survival rates.
2.Study on nephrotoxicity of the extracts from different parts of Miao medicine Wikstroemia indica

OBJECTIVE To study the nephrotoxicity of the extracts from different parts o f Miao medicine Wikstroemia indica in healthy rats ,and to provide reference for the study of its toxicity mechanism and clinical drug use. METHODS Using 70% ethanol as solvent ,total ethanol extract of W. indica was extracted with diacolation method. After dispersing the above extract with water,the fractions of corresponding fractions were obtained with petroleum ether ,ethyl acetate and n-butanol,and the rest was the extract of water fraction. SD rats were randomly divided into total ethanol extract group ,petroleum ether fraction group ,ethyl acetate fraction group ,n-butanol fraction group ,water fraction group and blank group ,with 12 rats in each group (half male and half female ). The rats in the administration groups were given the corresponding dose of drug solution intragastrically (total ethanol extract 317.520 mg/kg,petroleum ether fraction 7.875 mg/kg,ethyl acetate fraction 78.435 mg/kg,n-butanol fraction 53.865 mg/kg and water fraction 76.545 mg/kg),once a day ,for conse- cutive 2 weeks,and then stopped taking drug for 2 weeks; rats in the blank group were given equal volume of 1.0% . sodium carboxymethyl cellulose solution intragastrically. Duringthe experiment ,the general conditions of rats were observed. The samples of urine (on the 14th and 28th day ),serum and bilateral renal tissues (on the 15th and 29th day )were taken respectively,the renal index was calculated ,the levels of @qq.com renal function indexes in serum and urine were detected ,and the pathomorphological changes of renal tissues were observed. RESULTS During administration ,compared with blank group ,the rats in the total ethanol extract group and ethyl acetate fraction group showed poisoning behavior and activity characteristics such as mental depression ,decreased activity and diet ,thin stool and decreased body mass. The mental state of the rats in the petroleum ether fraction group ,n-butanol fraction group and water fraction group were slightly worse than that in blank group,and slightly decreased activity and diet as well as thin stool ,and slowly increased body mass were found ;however,there was no significant difference in anal temperature in each group. After 2 weeks of administration ,the renal index in total ethanol extract group ,the serum levels of N-acetylglucosaminidase(NAG),urea nitrogen (BUN)and creatinine (Cr)in total ethanol extract group and ethyl acetate fraction group ,serum level of NAG in n-butanol fraction group and serum level of Cr in water fraction group ,as while as NAG levels in urine of rats in total ethanol extract group and petroleum ether fraction group ,NAG and urinary protein levels in urine of rats in ethyl acetate fraction group were increased significantly (P<0.05 or P<0.01). In the pathomorphological observation ,renal tubules showed different degrees of unclear structure ,cell swelling and a few cell necrosis in the total ethanol extract group ,petroleum ether fraction group and ethyl acetate fraction group ,accompanying by glomerular pyknosis,renal tubular sclerosis and inflammatory cell infiltration ,compared with blank group. After drug withdrawal ,the mental state of rats in the administration groups were significantly improved ,the amount of activity and diet increased ,and the stool tended to be normal. Two weeks after drug withdrawal and recovery ,the levels of above indexes in serum and urine of rats in administration groups returned to be close to that in blank group (P>0.05);the glomerular structure of rats in each administration group gradually recovered clearly ,and cell swelling and inflammatory cell infiltration were rare in total ethanol extract group , petroleum ether fraction group and ethyl acetate fraction group. CONCLUSIONS The total ethanol extract ,petroleum ether fraction and ethyl acetate fraction of Miao medicine W. indica have certain nephrotoxicity and reversibility. The toxic component may
3.Analysis of IVD gene variants in four children with isovalerate acidemia.

OBJECTIVE:
To detect variants of IVD gene among 4 neonates with suspected isovalerate acidemia in order to provide a guidance for clinical treatment.
METHODS:
111 986 newborns and 7461 hospitalized children with suspected metabolic disorders were screened for acyl carnitine by tandem mass spectrometry. Those showing a significant increase in serum isovaleryl carnitine (C5) were analyzed for urinary organic acid and variants of the IVD gene.
RESULTS:
Four cases of isovalerate acidemia were detected, which included 2 asymptomatic newborns (0.018‰, 2/111 986) and 2 children suspected for metabolic genetic diseases (0.268‰, 2/7461). The formers had no obvious clinical symptoms. Analysis of acyl carnitine has suggested a significant increase in C5, and urinary organic acid analysis has shown an increase in isovaleryl glycine and 3-hydroxyisovalerate. Laboratory tests of the two hospitalized children revealed high blood ammonia, hyperglycemia, decreased red blood cells, white blood cells, platelets and metabolic acidosis. The main clinical manifestations have included sweaty foot-like odor, feeding difficulty, confusion, drowsiness, and coma. Eight variants (5 types) were detected, which included c.158G>A (p.Arg53His), c.214G>A (p.Asp72Asn), c.548C>T (p.Ala183Val), c.757A>G (p.Thr253Ala) and 1208A>G (p.Tyr403Cys). Among these, c.548C>T and c.757A>G were unreported previously. None of the variants was detected by next generation sequencing of 2095 healthy newborns, and all variants were predicted to be likely pathogenic based on the guidelines from the American College of Medical Genetics and Genomics.
CONCLUSION
The incidence of isovalerate acidemia in Liuzhou area is quite high. Screening of metabolic genetic diseases is therefore recommended for newborns with abnormal metabolism. The discovery of novel variants has enriched the mutational spectrum of the IVD gene.

4.Efficient expansion of rare human circulating hematopoietic stem/progenitor cells in steady-state blood using a polypeptide-forming 3D culture.

Although widely applied in treating hematopoietic malignancies, transplantation of hematopoietic stem/progenitor cells (HSPCs) is impeded by HSPC shortage. Whether circulating HSPCs (cHSPCs) in steady-state blood could be used as an alternative source remains largely elusive. Here we develop a three-dimensional culture system (3DCS) including arginine, glycine, aspartate, and a series of factors. Fourteen-day culture of peripheral blood mononuclear cells (PBMNCs) in 3DCS led to 125- and 70-fold increase of the frequency and number of CD34+ cells. Further, 3DCS-expanded cHSPCs exhibited the similar reconstitution rate compared to CD34+ HSPCs in bone marrow. Mechanistically, 3DCS fabricated an immunomodulatory niche, secreting cytokines as TNF to support cHSPC survival and proliferation. Finally, 3DCS could also promote the expansion of cHSPCs in patients who failed in HSPC mobilization. Our 3DCS successfully expands rare cHSPCs, providing an alternative source for the HSPC therapy, particularly for the patients/donors who have failed in HSPC mobilization.

Patients are prone to negative emotions that affect their physiological state during waiting time. 6. Clinical analysis of nine cases with critical corona virus disease 2019 in Hainan province

Objective To explore the clinical features of critical cases of coronavirus disease 2019 (COVID-19). Methods The clinical data of nine patients who were diagnosed with critical COVID-19 in Hainan General Hospital from January 21, 2020 to February 6, 2020 were retrospectively analyzed. RT-PCR testing for 2019 novel coronavirus (2019-nCoV) was performed with multi-sites synchronize specimens including pharyngeal swab, blood, excrement, and urine. The serum levels of leucocyte, C-reactive protein, procalcitonin and lactic acid between the improved group (five cases) and the deteriorated group (four cases) were compared. The t test was used for comparison of normally distributed continuous data between groups. Results There were eight males (88.9%) and 1 female enrolled. The patients aged 28-77 years old, with an age of (52.9±18.0) years. By March 4, 2020, all five cases in improved group were cured and discharged, three cases in deteriorated group died and 1case remained in critical condition. All multi-sites specimens of patients in improved group turned negative in 2-4 weeks of illness onset, while those of cases in deteriorated group showed sustained viral nucleic acid positive (up to 48th day of illness onset). The white blood cell counts ((13.52±8.24)×10 9 /L vs (10.49±4.46) ×10 9 /L), C-reactive protein ((139.71±87.46) mg/L vs (78.60±55.40) mg/L) and procalcitonin ((2.32±4.03) ng/mL vs (0.28±0.58) ng/mL) , lactic acid ((3.70±4.14) mmol/L vs (2.33±0.53) mmol/L) in deteriorated group were all significantly higher than those in improved group ( t =2.908, 5.009, 4.391 and 2.942, respectively, all P <0.01). A rapid rise of serum IL-6 level up to 8 500 pg/mL was observed in one patient three days prior to death. Conclusion Among the patients with critical COVID-19, serum levels of inflammatory cytokines of the death cases are higher than those of improved and discharged cases.
7.Effects of “Sweat Soaking Method ”on the Content of Genkwanin in Wikstroemia indica and Its Anti-oxidation Ability

OBJECTIVE:To compare the content changes of active/toxic ingredient genkwanin in ethanol extract from Wikstroemia indica before and after processing with “sweat soaking method ”and the effects of processing method on its anti-oxidation ability. METHODS :HPLC method was adopted to determine the content of genkwanin in W. indica before and after processing with “sweat soaking method ”. The separation was performed on Diamonsil C 18 column with 0.2% phosphoric acid solution-methanol as mobile phase (gradient elution )at the flow rate of 1 mL/min. The column temperature was 30 ℃ and detection wavelength was set at 346 nm. The sample size was 20 µL. SD rats were randomly divided into blank group ,W. indica raw product ethanol extract group (317.52 mg/kg,called“raw-product group ”as short )and W. indica processed product ethanol extract group (317.52 mg/kg,called“processed-product group ”as short ),with 6 rats in each group. Blank group was given constant volume of 1.0%CMC-Na solution intragastrically ,and administration groups were given relevant medicine suspension intragastrically;all of them were given 20 mL/kg,once a day ,for consecutive 14 days. The contents of serum oxidant stress indexes(MDA,CAT,SOD)in rats were determined by ELISA. RESULTS :The linear range of genkwanin were 0.147-27.360 μg (r=0.999 9);RSDs of precision ,reproducibility and stability tests were all lower than 3% ;average recoveries were 98.64%-98.92%(RSD<1%,n=3). Before and after processing with “sweat soaking method ”,average contents of genkwanin in W. indica were 0.377 6 and 0.234 0 mg/g. Compared with blank group ,the serum content of SOD in raw-product group was increased significantly ,while CAT content was decreased significantly (P<0.05 or P<0.01);the serum content of MDA was decreased significantly in processed-product group ,while SOD content was increased significantly (P<0.05 or P<0.01). MDA content of processed-product group was significantly lower than that of raw-product group ,while SOD content was significantly higher than raw-product group (P<0.05). CONCLUSIONS :After proce ssing with “sweat soaking method ”,the content of genkwanin in W. indica is decreased ,and antioxidant activity is increased .“Sweat soaking method ”processes certain function of “reducing toxicity and increasing efficiency ”.
8.Th1/Th2 cytokines and chemokine CXCL13 levels in peripheral blood and cerebrospinal fluid of patients with neurosyphilis and their clinical significance

Objective To investigate the Th1/Th2 cytokines and chemokine CXCL 13 levels in peripheral blood and cerebrospinal fluid(CSF)of patients with neurosyphilis and their clinical significance. Methods Forty seven HIV negative patients with neurosyphilis, 36 syphilis patients without neurological involvement(syphilis group)and 23 patients without infectious intracranial diseases(control group) admitted in Hangzhou Third Hospital during July 2011 to December 2014 were enrolled in the study.CSF pressure, protein contents, white blood cell counts and IgG index were detected in patients with neurosyphilis.Enzyme-linked immunosorbent assay(ELISA)was used to detect the levels of Th1 cytokines (IL-2,IL-12 and IFN-γ),Th2 cytokines(IL-6,IL-10)and CXCL13 in serum and CSF in three groups.Th1/Th2 cytokines,CXCL13 levels and CSF routine were also examined in neurosyphilis patients after treatment for 3 and 12 months.ANOVA, Kruskal-Wallis test and Spearman correlation were used for data analysis.Results The levels of IL-6 and IL-10 in serum and CSF from neurosyphilis patients were higher than those in syphilis group and control group(χ2IL-6=15.43, 15.39 and 14.44, 20.01,χ2 IL-10 =16.46, 23.86 and 15.11,24.44;P<0.05 or <0.01), while the levels of IL-2, IL-12 and IFN-γin the serum and CSF were lower than those in syphilis group and control group(χ2IL-2=14.55,17.14 and 16.14,17.97;χ2IL-12=13.65,20.50 and 18.48,21.04;χ2IFN-γ=16.95,17.53 and 16.00,15.21;P<0.05 or <0.01). CXCL13 contents in CSF of neurosyphilis patients were significantly higher than those in other two groups (χ2=52.51 and 53.76, P <0.01), and were positively correlated with leucocyte counts, protein concentrations,IgG index,IL-6 and IL-10(r=0.325,0.544,0.750,0.333 and 0.382,P<0.05 or <0.01),but were negatively correlated with IL-12 levels in neurosyphilis patients(r=-0.303,P<0.05). In neurosyphilis patients,CXCL13 and IL-6 levels were reduced after treatment for 12 months(χ2=102.00 and 22.17,P<0.05 and <0.01), while the levels of IL-2 and IL-12 were increased(χ2=18.28 and 24.10,P<0.05 and <0.01).Conclusion Chemokine CXCL13 and Th1/Th2 cytokines are involved in the immune response in neurosyphilis patients, which may be used for the diagnosis and evaluation of therapeutic efficacy in patients with neurosyphilis.
9.Distribution of algesia sensitized acupoints in the patients of intestinal cancer.

OBJECTIVETo judge whether algesia sensitization of some acupoints is existed and whether the acupoint algesia sensitization area is expanded in the patients of intestinal cancer.
METHODSTotally, 30 patients of intestinal cancer and 30 healthy subjects were included. The electronic Von Fray was used to determine the pressure-pain thresholds at 13 acupoints relevant with gastrointestinal disorders and the reference points at the sites 1and 2lateral to those points as well as the sites at the corresponding nerve segments. Compared with the pressure-pain thresholds at the reference points of the different segments, the relative value was calculated. The changes were analyzed in the pressure-pain thresholds at the relevant acupoints on the body surface in the patients of intestinal cancer as compared with the relative pressure-pain thresholds in the healthy volunteers.
RESULTSThe pressure-pain thresholds at Zusanli (ST 36), Shangjuxu (ST 37), Xiajuxu (ST 39), Quchi (LI 11) and Dachangshu (BL 25) in the patients of intestinal cancer were all significantly reduced as compared with those of the healthy subjects (<0.05,<0.01,<0.001). At the non-acupoint sites 1and 2lateral to those acupoints as well as at the sites of the same segments, the pressure-pain thresholds were reduced significantly as compared with the control group (<0.05,<0.01,<0.001). Particularly, the sensitization zone of Yinlingquan (SP 9) focused on the acupoint, the site 1lateral to it as well as the non-acupoint sites of the same segments (<0.01,<0.001).
CONCLUSIONThe acupoint sensitization is displayed at Zusanli (ST 36), Shangjuxu (ST 37), Xiajuxu (ST 39), Quchi (LI 11), Dachangshu (BL 25) and Yinlingquan (SP 9) and the sensitization area is expended in the patients of intestinal cancer.
10.Clinical analysis of 2 261 cases of esophageal cancer

Objective To analyze the clinical features and onset factors of esophageal cancer.Methods The SPSS 13.0 software was adopted to establish the computer records management system of esophageal cancer.The clinical data in 2 261 clinically cured discharged cases of esophageal cancer from January 2011 to December 2013 were statistically analyzed.Results Forty-nine cases (2.17%) were in the 30-<40 years old group,324 cases (14.33%) were in the 40-<50 years old group,963 cases (42.59%) were in the 50-<60 age group,790 cases (34.94%) were in the 60-<70 age group and 135 cases (5.97%) were in the 70 years old or more group.Among 2 240 cases,2 031 cases (90.36 %) showed the different differentiation degrees of squamous epithelial carcinoma by histological analysis,the invaded range displayed fibrous membrane(T3) in 759 cases(33.57%) and peripheral tissue(T4) in 682 cases(30.16%);having smoking and drinking history was in 1 281 cases(56.67%) and 1 025 cases (45.33 %) respectively;596 cases (26.36 %) had genetic family history,75.08 % was father positive or mother positive.Conclusion Esophageal cancer is mainly squamous cell carcinoma,the onset age is mainly concentrated at the age 50-<70 years old,which is correlated with smoking and drinking years,moreover has obvious genetic susceptibility.
