2.Longitudinal Changes in Liver Aminotransferases Predict Metabolic Syndrome in Chinese Patients with Nonviral Hepatitis.

OBJECTIVEThis study explored the correlation of longitudinal changes in serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels with the incidence of metabolic syndrome (Mets) based on a dynamic health examination cohort.
METHODSA Mets-free dynamic cohort involving 4541 participants who underwent at least three health examinations from 2006 to 2011 was included in the study. Mets was defined according to the Chinese Medical Association Diabetes Branch definition that included hypertension, obesity, hyperlipidemia, and hyperglycemia. Generalized estimating equation (GEE) model was used to analyze multivariate relative risk (RR) of repeated observations of ALT and AST in quartiles for Mets or its components according to gender.
RESULTSIn all, 826 Mets cases were reported. Adjustment of relevant parameters indicated that time-varying changes in ALT and AST levels were positively associated with the incidence of Mets in a dose-response manner. Positive association between high ALT levels and fatty liver was much stronger than that between high AST levels and fatty liver, particularly in male participants. These associations were consistently observed in the following subgroups: participants with ALT and AST levels of <40 U/L, participants with of <25 kg/m2, and participants with non-fatty liver. Furthermore, participants with 2 Mets components at baseline showed lower multivariate adjusted RRs of ALT and AST for Mets than participants with 0-1 Mets component.
CONCLUSIONThese results suggested that elevated serum ALT and AST levels were early biomarkers of Mets or its components.

3.Assessing Adverse Effects of Aroclor 1254 on Perinatally Exposed Rat Offspring.

To assess the neurotoxic effects and redox responses of Aroclor 1254 (A1254) on perinatally exposed rat offspring, A1254 was administered by gavage from gestational day (GD) 6 to postnatal day (PND) 21. Neurobehavioral development, antioxidant enzyme activities, lipid peroxidation (LPO), nitric oxide (NO), and NO synthase (NOS) levels were analyzed in the offspring. Neurobehavioral development analysis revealed delayed appearance of the righting reflex, negative geotaxis, and cliff drop test responses in A1254 exposed group. Developmental A1254 exposure also caused oxidative stress in the brain of PND 22 offspring via reductions in the activity of SOD and GSH-Px, and by promoting a rise in the levels of NO and NOS.

4.Propensity score analysis of liver and kidney function changes when shenmai is parenterally administered over extended period.

Since parenterally administered Shenmai directly enters the bloodstream, avoiding gastrointestinal absorption, there are concerns as to its toxicity on the liver and kidneys. Previously, the risk of toxicity to the liver and kidneys through overdose has been discussed. This paper investigates changes in liver and kidney functions as indicated by ALT, AST, Cr and Bun, obtained from hospital information system (HIS) data, when it is administered over a longer than the recommended period of 14 days. Methods applied include: column joint analysis, logistic regression, propensity score and propensity score weighting. Results indicate that a longer course of administration does not have a significant effect on the function of liver and kidneys. However, continued monitoring of the toxicity to the liver and kidneys is a long-term and important issue; further exploration through toxicology experiments and prospective clinical research is required.

OBJECTIVETo study the clinical features of children with 3β-hydroxy-Δ(5)-C27-steroid dehydrogenase deficiency and review the literature.
METHODClinical features and treatment of one Chinese infant with 3β-hydroxy-Δ(5)-C27-steroid dehydrogenase deficiency confirmed by HSD3B7 gene mutation analysis were retrospectively reviewed, and 51 cases of 3β-hydroxy-Δ(5)-C27-steroid dehydrogenase deficiency who were internationally reported since 2000 were also reviewed in this paper.
RESULT(1) A 3-month-old infant with neonatal cholestasis was admitted to our hospital because of hyperbilirubinemia and abnormal liver dysfunction (total bilirubin 110.7 µmol/L, direct bilirubin 74.5 µmol/L, γ-glutamyltransferase 24.4 IU/L, total bile acid 0.1 µmol/L).His jaundice disappeared within a few weeks, serum liver biochemistries improved and his growth in weight and height was excellent after oral cholic acid therapy.HSD3B7 gene analysis using peripheral lymphocyte genomic DNA from the patient identified compound heterozygous mutations. This child was confirmed as the most common inborn error of bile acid metabolism-3β-hydroxy-Δ(5)-C27-steroid dehydrogenase deficiency by molecular analysis.(2) Retrospective review of the literature showed that the clinical features of 3β-hydroxy-Δ(5)-C27-steroid dehydrogenase deficiency included neonatal cholestasis, some patients progressed to severe liver disease and needed liver transplantation without effective therapy; however, serum biochemical characteristics of normal γ-glutamyltransferase activity, normal or low total bile acid concentrations were not consistent with cholestasis, the replacement treatment with cholic acid produced a dramatic improvements in symptoms, biochemical markers of liver injury; 31 cases were diagnosed by HSD3B7 gene mutation analysis.
CONCLUSIONThe clinical characteristics of 3β-hydroxy-Δ(5)-C27-steroid dehydrogenase deficiency include neonatal cholestasis, normal serum γ-glutamyltransferase activity, and normal or low serum total bile acid concentration.Oral cholic acid replacement is an effective therapy; definitive diagnosis of 3β-hydroxy-Δ(5)-C27-steroid dehydrogenase deficiency can be identified by molecular genetic testing technology.

6.Real world study of affect on liver function of overdose of salvianolate extract injection.

OBJECTIVETo understand the affect on liver function of using salvianolate injection above the recommended dose.
METHODNationwide, research of hospital information system (HIS) data from 18 general hospitals comprising 14 191 patients was carried out. Two groups of patients were studied and their blood tests monitored before and after injections. One group received the recommended dose of 200 mg, the other exceeded it. To take into account group differences the propensity score method and logstic regression was used alongside liver function.
RESULTWhen salvianolate injection was administered above the recommended dose patients were more likely to have abnormal AST and ALT levels. However, the difference between the groups was not statistically significant.
CONCLUSIONAccording to our data the affect of exceeding the recommended dose of salvianolate injection on liver function is not significant. However, monitoring of liver function should be carried out in clinic.

7.Data analysis of real world clinical changes in indexes of liver and kidney function to use of parenterally administered kudiezi at different doses and time periods.

This is a retrospective study based on a 18 hospital information system data warehouse. Records of 1 982 patients who used Kudiezi intravenous infusion (KDZ) were extracted from the data warehouse. All the patients were divided into two groups, one group of 1 707 patients used KDZ, < or = 14 days under the instruction, the other group of 275 patients used KDZ, > 14 days, off label use. Generalized boosted models (GBM) with propensity score were applied to compare the two groups on four indexes of liver and kidney functions, such as alanine aminotransferase (ALT), aspartate aminotransferase (AST), creatinine (Cr) and blood urea nitrogen (BUN). Seventy one confounders were identified and balanced by GBM. And another two logistic analysis methods were used to confirm the results from GBM. The results indicated that there were no significant difference on the four indexes except the AST (P < 0. 05) by all the three analyzing models. It is hard to conclude that ultra long, off label using Kudiezi intravenous infusion could influence the four indexed of liver and kidney from this data analysis. More conclusive evidence should be collected by further prospective study.

8.Outcome research of shuxuening injection different dosage impact on patient's liver function.

The safety of parenterally administered Chinese medicine has caught the attention of clinicians, patients and drug administration departments. However, there are few studies that have assessed liver function damage from these medications. This study retrospectively analyzed hospital information system (HIS) data, from 20 nationwide general hospitals, of patients who were treated with parenterally administered Shuxuening. Patients that were given doses of 21-50 mL, which is above the recommended dose, acted as the observation group. Those receiving the recommended dose of < 20 mL were the control group. Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were measured before and after parenteral administration of Shuxuening, changes after treatment were outcomes. Also recorded were: age, costs, length of hospitalization and the patients' condition on admission. Propensity score method was used to balance 71 confounding variables such as gender, age, mortality, and costs. The influence of dosage of ALT and AST can then be known. Analysis of the impact of parenterally administered Shuxuening on ALT, before logistic regression based on adjusted propensity score, then the P value is 0.007. However, after logistic regression based on adjusted propensity score method the P value became 0. 254. Hence, no differences were found between the two groups; using parenterally administered Shuxuening at the higher dose of 21 to 50 mL had no significant effects on ALT compared with the normal dose. Similarly analysis of AST changes before and after logistic regression based on adjusted propensity score method, P value changes from 0.192 to 0.568. There were no differences between the two groups using parenterally administered Shuxuening; a dosage of 21 to 50 mL each time had no significant effects on AST. Nevertheless, this study is a retrospective analysis, whilst known confounding factors have been taken into account there may be unknown ones to include inanalysis. Logistic regression based on adjusted propensity score method can control known confounding factors very well. This research serves as an aid to the safe clinical use of parenterally administered Shuxuening.

OBJECTIVETo analyze the effect on liver function of parenterally administered Shuxuetong Injection with different doses.
METHODThe study is based on hospital information system (HIS) data. We group the cases by dose: The observation group receive a dose > 6 mL (recommended daily dose), the matched group receive the recommended dose. Chi-square test, logistic regression, logistic regression with propensity score, logistic regression with GBM propensity score were used to find the between group differences in ALT and AST after using parentyerally administered Shuxuetong injection.
RESULTAll tests show that there were no significant differences between groups in AST or ALT after we control for confounding factors.
CONCLUSIONBased on HIS data, there was no evidence to associate the use of Shuxuetong over the recommended dose and effects on liver function.

10.Study on intervention effect of Danggui Shaoyao San on rats with cirrhotic ascites.

OBJECTIVETo investigate the intervention effect of Danggui Shaoyao San on rats with cirrhotic ascites, and discuss the effect of arginine vasopressin (AVP) on cirrhotic ascites.
METHODMale SD rats were randomly divided into the control group, the model group, Danggui Shaoyao San low, middle and high dose groups. The cirrhotic ascites rat model was established by CCl4 combined with phenobarbital. Their urines were collected at 24 h to observe urine excretion of each group. Filter papers were used to determine the amount of ascites. The levels of serum alanine aminotransferasa (ALT) , aspartate aminotransferase (AST) were detected by the automatic biochemistry analyzer. Plasma prothrombin time (PT) was evaluated by the blood coagulation analyzer. The concentration of AVP in plasma was detected by enzyme-linked immunosorbent assay (ELISA). Pathological changes in livers were observed by HE staining.
RESULTCompared with the model group, the Danggui Shaoyao San group showed significant improvement in live indexes, with notable decrease in serum ALT and AST and the time of PT, improvement in liver pathological changes. Simultaneously, the amount of ascites decreased to varying degrees, with notable increase in urine in 24 h and decrease in AVP concentration in plasma.
CONCLUSIONDanggui Shaoyao San can notably improve liver functions of rats with cirrhotic ascites, reduce the generation of ascites and delay the progress of liver pathological changes. Its mechanism may be related to AVP.
