1.The influencing factors of adverse pregnancy outcomes in patients with preeclampsia and the predictive value of serum trace elements in the second trimester

Objective To investigate the influencing factors of adverse pregnancy outcomes in patients with preeclampsia and the predictive value of serum trace elements in the second trimester.Methods A total of 98 patients with preeclampsia admitted to Qujing First People's Hospital from January 2019 to June 2022 were enrolled in the study.Patients were divided into poor outcome group and good outcome group according to whether they had adverse pregnancy outcomes.The clinical data of all patients enrolled in the study were col-lected and the serum levels of trace elements calcium,copper,zinc and iron were detected in the second trimes-ter.Univariate analysis and multivariate Logistic regression were used to analyze the influencing factors of ad-verse pregnancy outcomes in patients with preeclampsia.The levels of serum trace elements in the second tri-mester of pregnancy were compared between the poor outcome group and the good outcome group.The re-ceiver operating characteristic(ROC)curve was used to evaluate the predictive value of serum trace elements calcium,copper,zinc and iron for adverse pregnancy outcomes in patients with preeclampsia.Results Univari-ate analysis showed that compared with the good outcome group,the poor outcome group had significantly higher systolic blood pressure,24 h urinary protein quantitation,and D-dimer level(P<0.05)and significantly less gestational age and platelet count at admission(P<0.05).Multivariate Logistic regression analysis showed that 24 h urinary protein quantification,D-dimer and platelet count were the influencing factors of ad-verse pregnancy outcomes in patients with preeclampsia(P<0.05).The levels of serum trace elements calci-um,copper,and zinc in the poor outcome group were significantly lower than those in the good outcome group(P<0.05),and the level of iron was significantly higher than that in the good outcome group(P<0.05).ROC curve analysis showed that the areas under the curves(AUCs)of serum calcium,copper,zinc,and iron in the second trimester of pregnancy for predicting adverse pregnancy outcomes in preeclampsia patients were 0.830(95%CI:0.780-0.880),0.855(95%CI:0.805-0.905),0.847(0.797-0.897)and 0.861(95%CI:0.811-0.911),respectively.Conclusion Adverse pregnancy outcomes in patients with preeclampsia are re-lated to 24 h urine protein,D-dimer and platelet count.The levels of serum trace elements calcium,copper,zinc and iron in the second trimester of pregnancy change significantly in patients with adverse pregnancy out-comes,which may become predictive markers of adverse pregnancy outcomes.

Background::Atopic dermatitis (AD) affects approximately 10% of adults worldwide. CM310 is a humanized monoclonal antibody targeting interleukin-4 receptor alpha that blocks interleukin-4 and interleukin-13 signaling. This trial aimed to evaluate the efficacy and safety of CM310 in Chinese adults with moderate-to-severe AD.Methods::This multicenter, randomized, double-blind, placebo-controlled, phase 2b trial was conducted in 21 medical institutions in China from February to November 2021. Totally 120 eligible patients were enrolled and randomized (1:1:1) to receive subcutaneous injections of 300 mg CM310, 150 mg CM310, or placebo every 2 weeks for 16 weeks, followed by an 8-week follow-up period. The primary endpoint was the proportion of patients achieving ≥75% improvement in the Eczema Area and Severity Index (EASI-75) score from baseline at week 16. Safety and pharmacodynamics were also studied.Results::At week 16, the proportion of EASI-75 responders from baseline was significantly higher in the CM310 groups (70% [28/40] for high-dose and 65% [26/40] for low-dose) than that in the placebo group (20%[8/40]). The differences in EASI-75 response rate were 50% (high vs. placebo, 95% CI 31%–69%) and 45% (low vs. placebo, 95% CI 26%–64%), with both P values <0.0001. CM310 at both doses also significantly improved the EASI score, Investigator’s Global Assessment score, daily peak pruritus Numerical Rating Scale, AD-affected body surface area, and Dermatology Life Quality Index compared with placebo. CM310 treatment reduced levels of thymus and activation-regulated chemokine, total immunoglobulin E, lactate dehydrogenase, and blood eosinophils. The incidence of treatment-emergent adverse events (TEAEs) was similar among all three groups, with the most common TEAEs reported being upper respiratory tract infection, atopic dermatitis, hyperlipidemia, and hyperuricemia. No severe adverse events were deemed to be attributed to CM310. Conclusion::CM310 at 150 mg and 300 mg every 2 weeks demonstrated significant efficacy and was well-tolerated in adults with moderate-to-severe AD.Trial Registration::ClinicalTrials.gov, NCT04805411.
3.Genetic Polymorphism of 42 Autosomal STR Loci from Chinese Han Population in Shenzhen

[Objective]To investigate the genetic polymorphism of 42 short tandem repeats(STRs),including 41 non-CODIS loci from the Shenzhen Han population and evaluate their potential values in forensic application.[Methods]In our research,the AGCU 21+1 STR kit and Microreader? 23sp Direct ID System were applied to analyze the polymorphism of STR loci from 435 unrelated individuals of Shenzhen Han population.Modified-Powerstates and Arlequin v3.5 software were used to analyze the allele frequencies and forensic parameters,and perform the Hardy-Weinberg equilibrium test.[Results]A total of 418 alleles were detected from 435 unrelated individuals in Shenzhen,all consistent with Hardy-Weinberg equilibrium(P>0.05/42),with the allele frequency ranging from 0.001 1 to 0.552 9.Besides,the discrimination power(DP)ranged from 0.798 8(D1S1627)to 0.968 6(D7S3048),the polymorphic information content(PIC)ranged from 0.568 0(D1S1627)to 0.859 8(D7S3048),and the heterozygosity(H)ranged from 0.627 6(D1S1627)to 0.878 2(D20S470).Among all the STRs tested in the study,both D1S1656 and D21S1270 have 16 alleles and show the highest polymorphism.In comparison,only five alleles were observed in the D4S2408 locus,which displays the least polymorphism.[Conclusions]The 42 autosomal STR loci with high genetic polymorphism in Shenzhen Han population showed potential as an effective means for individual identification and paternity testing,especially in the cases with single parent or mutation detected.The obtained information can provide basic data for STR population genetics.
4.Relationship of creatinine/cystatin C ratio and monocyte/HDL-C ratio with hypertension complicated with coronary heart disease

Objective To analyze the correlation of serum creatinine(Cr)/cystatin C(Cys C)ratio and monocyte/high-density lipoprotein-cholesterol(HDL-C)ratio with hypertension complicated with coronary heart disease(CHD).Methods A total of 200 hypertension patients with concomi-tant CHD admitted in our hospital from August 2021 to August 2023 were recruited and served as the observation group,and another 100 patients with hypertension during the same period served as the control group.Based on the number of vascular lesions,the patients in the observation group were divided into single-,double-and multi-vessel disease subgroups(104,75 and 21 cases).According to their Gensini score(<30,30-90,>90),they were also assigned into mild,moderate and severe stenosis subgroup(98,78 and 24 cases).After the fasting venous blood sam-ples were harvested from all subjects,the serum levels of Cys C,Cr and HDL-C were measured,and monocyte count was calculated,and Spearman correlation analysis was used to determine the correlation of serum Cr/Cys C ratio and monocyte/HDL-C ratio with number and severity of cor-onary artery lesions.Results The observation group had significantly higher Cr and Cys C levels,monocyte count and monocyte/HDL-C ratio,while lower Cr/Cys C ratio and HDL-C level than the control group(P<0.01).The conditions of above indicators were more severe in the multi-vessel lesion subgroup than the single-and double-vessel lesion subgroups(P<0.05).Similar re-sults were observed in the severe stenosis subgroup when compared with the mild and moderate stenosis subgroups(P<0.05).Spearman correlation analysis showed that the Cr/Cys C ratio was negatively correlated with the number of coronary artery lesions and the severity of coronary ar-tery stenosis(r=-0.743,P<0.05;r=-0.673,P<0.05),and the monocyte/HDL-C ratio was positively correlated with the number of coronary artery lesions and the degree of coronary artery stenosis(r=0.692,P<0.05;r=0.611,P<0.05).Conclusion The serum Cr/Cys C ratio and monocyte/HDL-C ratio are closely associated with hypertension combined with concomitant CHD.And the Cr/Cys C ratio is negatively correlated with the severity of the disease,and the monocyte/HDL-C ratio is positively correlated with the severity of the disease in the patients.
5.The status and its influencing factors of tissue silence of nurses in 3 Grade A general hospitals

Objective To investigate the status quo and its influencing factors of nurses'organizational silence in 3 Grade A general hospitals.Methods Convenient sampling method was used to investigate clinical nurses in 3 Grade A general hospitals in Xi'an from April to August 2023 by general data questionnaire,nurses'organizational silence questionnaire and hospital magnetic factor scale.Multiple linear regression was used to analyze the influencing factors of organizational silence.Results A total of 855 nurses completed the study.The total silence score of nurses was(56.33±8.55);The total score of hospital magnetic level was(107.63±12.85).There was a negative correlation between nurse tissue silence and hospital magnetic level(r=-0.318,P<0.01).Hospital magnetic level,age,job title and working time were the influential factors of nurses'organizational silence(all P<0.001),which together explained 62.60%of the variation.Conclusions The silence of nurses'tissue and the level of hospital magnetism are in the low-medium level.Nurses are younger in age,lower in professional title,shorter in nursing age and lower in hospital magnetism level,the higher the tissue age level is,the nursing managers can reduce the tissue silence of nurses by improving the hospital magnetism level.
6.Experiences in use of kissing pancreatojejunostomy in 267 cases of pancreatoduodenectomy

Objective:To investigate feasibility, efficiency and safety of kissing pancreatojejunostomy after pancreatoduodenectomy.Methods:From Jan 2006 to Sep 2020, the clinical data of 267 patients undergoing pancreatoduodenectomy and kissing pancreatojejunostomy were retrospectively analyzed.Results:Grade B postoperative pancreatic fistula (POPF) occurred in 6.37%, Grade C POPF in 2.25% of patients. There was no mortality within 30 days postoperatively as to pancreatic fistula, by cut-off of pancreatic duct diameter at 3mm, there was no significant difference between two subgroups (15/140 vs. 8/127, P=0.20). also, when grouped by texture of the pancreas, no there was significant difference (20/194 vs. 3/73, P=0.11). Conclusions:Kissing pancreatojejunostomy is feasible and easy to perform. It also does not increase the POPF rate when applied to the pancreatojejunostomy with thin pancreatic duct and soft texture.
7.Ovarian cancer cells promote glycolysis metabolism of peripheral CD4 + T cells

Objective:
To investigate the effects of ovarian cancer cells on the glycolysis metabolism and functions of peripheral CD4 + T cells.
Methods:
We established a coculture system of human CD4 + T cells with ovarian cancer cell SKOV3, and the mRNA expression levels of 8 glycolysis-related genes (mTORC1, HIF1α, GLUT1, GPI, ENO1, PKM2, LDHα and PDK1) in cocultured CD4 + T cells were determined by real-time fluorescence quantitative PCR. The levels of glucose uptake and glycolysis were detected by colorimetry. The proportion of CD4 + CD25 + Foxp3 + T cells was tested by flow cytometry. The proliferation levels of CD4 + T cells were measured by tritium incorporation method. The expression levels of 8 glycolysis-related genes in peripheral CD4 + T cells were determined by real-time fluorescence quantitative PCR in 10 ovarian cancer (OC) patients, 10 benign ovarian tumor (BOT) patients and 10 healthy control (HC).
Results:
Results: Compared with the CD4 + T cells in single culture without SKOV3, the expression levels of 8 glycolysis-related genes in CD4 + T cells cocultured with SKOV3 were elevated (all P<0.01). The levels of glucose uptake and glycolysis were increased (P<0.05). The proportion of CD4 + CD25 + Foxp3 + T cells was up-regulated (P<0.05) and the proliferation levels of CD4 + T cells were decreased (P<0.05). Moreover, the mRNA expression levels of all the glycolysis-related genes were increased (all P<0.05) in the peripheral CD4 + T cells of OC patients compared with BOT patients and HC.
Conclusion
Ovarian cancer cells could promote the glycolysis metabolism of CD4 + T cells, which may be relevant to its differentiation and functions.
8.Prenatal diagnosis of a fetus affected with Finnish type congenital nephrotic syndrome.

OBJECTIVE:
To explore the genetic basis for a fetus suspected for congenital nephrotic syndrome of Finland (CNF).
METHODS:
Genomic DNA was extracted from peripheral and umbilical cord blood samples derived from both parents and the fetus. Potential variants were detected by using next-generation sequencing. Suspected variants were confirmed by Sanger sequencing.
RESULTS:
The fetus was found to carry compound heterozygous variants c.1440+1G>A and c.925G>T of the NPHS1 gene, which were respectively inherited from its mother and father.
CONCLUSION
Identification of the compound heterozygous NPHS1 variants has enabled diagnosis of CNF in the fetus and genetic counseling for the affected family.

10.Predictive value of HIT-antibodies detection for new thrombosis in heparin-induced thrombocytopenia

Objective To investigate the predictive value of HIT-antibodies(HIT-Ab) detection for new thrombus in suspected Heparin-Induced thrombocytopenia (HIT). Methods Retrospective cohort study. 472 suspected HIT patients were collected from July 2016 to November 2018, and all subjects under-went a 4Ts score and were sent for HIT-Ab tests. According to the results of HIT-Ab, there were four groups:412 cases of negative HIT-Ab (0-0.9 U/ml), 45 cases of weak-positive HIT-Ab (1.0-4.9 U/ml), 12 cases of moderate-positive HIT-Ab (5.0-15.9 U/ml), and 3 cases of strong-positive HIT-Ab (≥16.0 U/ml) respective-ly. Ultrasound or CT examination was used to confirm new thrombosis as a standard to evaluate the value of HIT-Ab for predicting new thrombus. The diagnostic efficacy of HIT-Ab for HIT was evaluated in clinically confirmed HIT. Results The incidence rates of new thrombus in each group were: 15.8% in Negative HIT-Ab group (62/412), 48.9%in Weak-positiveHIT-Ab group (22/45), 75.0%in Moderate-positive HIT-Ab group (9/12), and100%in Strong-positive HIT-Ab group (3/3)(P<0.00). When HIT-Ab≥1.0 U/ml, the speci-ficity for diagnosing new thrombus was 93.0%, the sensitivity was 34.2%, the negative predictive value (NPV) was 84.2%, and the positive predictive value (PPV) was 56.5%. The diagnostic rates of HIT in each group were:negative 0%(0/412), weak-positive 62.2%(28/45), moderate-positive (12/12) and strong-positive (3/3) were 100%. When HIT-Ab≥ 1.0 U/ml, the specificity for HIT diagnosis was 96.0%, the sensitivity was 100%, NPV was 100%, and PPV was 71.5%. Conclusions In suspected HIT patients, the incidence of new thrombosis increases with the elevated HIT-Ab level. HIT-Ab detection can be used as a crucial tool for new thrombosis prediction and HIT diagnosis in suspected HIT patients. Clinicians can develop treatment strategies based on HIT-Ab levels.
