Objective:To understand the breastfeeding situation in the neonatal intensive care units (NICUs) in Qianxinan Prefecture, Guizhou Province, and to assess the knowledge, attitudes, and practices of doctors and nurses regarding breastfeeding, aiming to provide foundational data for improving breastfeeding quality. Methods:A questionnaire was developed to survey the knowledge, attitudes, and practices related to breastfeeding in NICUs. The questionnaire was divided into three dimensions: knowledge (seven items, total score of 7), attitudes (nine items, total score of 45), and practices (seven items, total score of 35). Lower scores indicated weaker recognition of breastfeeding. Additionally, five items were included to identify the most influential factors affecting breastfeeding. From November 25 to November 30, 2023, a survey was conducted among doctors and nurses with professional qualifications who had worked in the neonatal departments of nine hospitals in Qianxinan Prefecture for at least one year. Independent sample t-tests and Chi-square tests were used to compare the scores of doctors and nurses from different levels of hospitals and within the same level of hospitals across the three dimensions. Results:(1) Among the nine hospitals, three were tertiary grade A hospitals (referred to as "tertiary hospitals"), with 95.6% (43/45) of the doctors and 96.5% (110/114) of the nurses participating in the survey. Six were secondary grade A hospitals (referred to as "secondary hospitals"), with 95.0% (38/40) of the doctors and 97.6% (83/85) of the nurses participating. (2) All nine hospitals were baby-friendly hospitals and all had breastfeeding promotional materials. Six hospitals had NICUs that promoted breastfeeding, with an average NICU breastfeeding rate of 25.8% across the prefecture between year 2021 to 2023. (3) The proportion of doctors who had received breastfeeding training was higher than that of nurses within the same level of hospitals [tertiary hospitals: 69.8% (30/43) vs. 40.0% (44/110), χ 2=10.97, P=0.001; secondary hospitals: 47.4% (18/38) vs. 24.1% (20/83), χ 2=6.55, P=0.010], although the overall training rates were low. (4) In tertiary hospitals, doctors scored higher than nurses in the attitude dimension [(35.35±4.75) vs. (33.18±5.60) scores, t=-2.03, P=0.044] and also in the practice dimension [(26.98±3.00) vs. (25.60±3.75) scores, t=-2.17, P=0.032]. In secondary hospitals, the total knowledge dimension score of doctors was higher than that of nurses [(4.92±1.44) vs. (4.20±1.45) scores, t=-2.52, P=0.013]. In tertiary hospitals, the total scores for attitude and practice dimensions of doctors were higher than those of doctors in secondary hospitals, and the total scores for knowledge, attitude, and practice dimensions of nurses were higher than those of nurses in secondary hospitals (all P<0.05). (5) In the knowledge dimension, the lowest scoring item of doctors in the tertiary hospitals was "Breastfeeding is possible for maternal hepatitis B newborns after receiving vaccines and immunoglobulin"; the lowest scoring item of nurses in the tertiary hospital, and doctors and nurses in the secondary hospitals was "The duration of breastfeeding has a greater impact on neonatal outcomes". In the attitude dimension, the lowest scoring item for doctors and nurses in both tertiary and secondary hospitals was "You think the breastfeeding process is more troublesome than feeding preterm formula". In the practice dimension, the lowest scoring item of the doctors and nurses in the tertiary hospitals was "Your hospital had enough breastfeeding knowledge training", while for the doctors and nurses in the secondary hospitals were "You have more opportunities to participate in various breastfeeding-related training" and "Breast feeding should be started as soon as possible when the infant is stable after active treatment", respectively. (6) The most influential factors affecting breastfeeding were: lack of cooperation from parents (50.0%, 137/274), relative insufficient human resources for doctors and nurses (21.9%, 60/274), and the absence or poor implementation of breastfeeding management policies (18.3%, 50/274), etc. Conclusions:The breastfeeding rate in NICU of county-level hospitals is relatively low, and medical staff, especially nurses, have insufficient knowledge about breastfeeding. It is necessary to strengthen various breastfeeding training for medical staff to enhance their understanding of NICU breastfeeding.

Objective:To investigate the efficacy and safety of ticagrelor and clopidogrel in patients with coronary atherosclerotic heart disease (CHD) and hyperhomocysteinemia (Hhcy). Methods:A total of 101 patients with CHD complicated with HHcy who had successfully undergone percutaneous coronary intervention (PCI) were enrolled. They were divided into ticagrelor group ( n=49) and clopidogrel group ( n=52) according to the different antiplatelet drugs used. The clinical data of the two groups were collected, and the incidence of major cardiovascular and cerebrovascular adverse events (MACCE) and bleeding events in one year were compared between the two groups. Results:There was no statistical difference in baseline datas and PCI datas between the clopidogrel and ticagrelor groups ( P>0.05). Compared with clopidogrel group, ticagrelor can reduce the total MACCE (8.16% vs 32.69%, P<0.05) and the incidence of unstable angina pectoris events (0 vs 13.46%, P<0.05) in patients with hyperhomocysteinemia PCI 1 year after operation. The incidences of ischemic stroke, unstable angina pectoris, recurrent myocardial infarction, and cardiogenic death were compared separately between the two groups, and the difference was not statistically significant ( P>0.05). The incidence of major bleeding events, minor bleeding events, and minimal bleeding events were similar between the two groups, and the difference was not statistically significant (4.08% vs 0; 4.08% vs 1.92%; 20.41% vs 9.62%; P>0.05). Conclusions:In patients with CHD and Hhcy, ticagrelor can play a better antithrombotic effect, reduce the incidence of ischemic events, and has good security, which is more worthy of clinical recommendation.

Objective To explore the effect of a multimodal electroencephalogram ( EEG) data visualiza-tion system on the motor imagery ability of stroke survivors. Methods Twenty stroke patients were randomly di-vided into an experimental group and a control group, each of 10. Both groups were provided with brain-computer interface-based motor imagery ( MI) training. At the same time, the experimental group was monitored and guided using an online, multimodal EEG data visualization system developed in our department. The classification accuracy ( CA) and event-related desynchronization ( ERD) of the 2 groups′ motor imagery were compared before and after the treatment. Results Before the treatment, no significant differences in the average CA of MI were found be-tween the experiment group (50.92±2.08) and the control group (49.35±4.20)(P>0.05). After the treatment, however, the experimental group′s average CA had increased to (64.52±5.27), significantly higher than that of the control group (51.18±5.02). When the stroke patients imaged affected upper extremity movements, obvious ERD was observed in the α frequency around the bilateral central motor regions of both groups, especially in the experi-mental group, but without significant differences between the two groups. However, no significant changes were found in the ERD of theβwaves of the two groups( P>0.05) . Conclusion The proposed online multimodal elec-troencephalogram data visualization system can help stroke patients imagine movements actively. It is worth sprea-ding in clinical practice.

Journal of Central South University(Medical Sciences) 2016;41(4):380-387
OBJECTIVE:
To evaluate the diagnostic value of liver imaging reporting and classification criteria of data system (LI-RADS) MRI on primary hepatic carcinoma (HCC).
METHODS:
The MRI images of 100 patients (72 patients with HCC, 4 with regenerative nodules, 13 with hemangioma, 4 with liver cyst, 3 with Liver abscess, 2 with inflammatory pseudotumor, 1 with hepatic arteriovenous fistula, 1 with Limitations of steatosis) with liver disease were collected in Xiangya Hospital from January, 2014 to January, 2015. All patients were at the risk of HCC. The images were reviewed according to the LI-RADS categorization by American College of Radiology (ACR) under a condition of blindness to the pathological data. The sensitivity, specificity and the area under the ROC curve in the diagnosis of HCC with the LI-RADS categorization were calculated by three associate professors from the Department of Radiology, Xiangya Hospital.
RESULTS:
The area under the ROC curve was 0.925 in the 100 patients with HCC. Of the 100 pathologically confirmed patients, 20 cases were LR1 and LR2. However, the post-operation pathological examination showed benign tumor (negative predictive value was 100%). There were 25 HCC in 31 cases of LR3, 27 HCC in 29 cases of LR4 (positive predictive value was 93.1%). All of 20 cases of LR5 were HCC (positive predictive value was 100%). If LR1 and LR2 were considered negative, and LR3, 4, 5 were positive, then the sensitivity, the specificity and the area under the ROC curve was 100%, 71.4% and 0.925, respectively. If LR1, 2 and 3 were considered negative and LR4 and LR5 were positive, the sensitivity, the specificity and the area under the ROC curve was 100%, 90.9% and 0.974, respectively.
CONCLUSION
The MRI LI-RADS categorization is highly sensitive and specific in the diagnosis of HCC, which can improve the diagnostic accuracy.

10.Advances in soluble guanylate cyclase stimulator
Liqing HU ; Rui YI ; Qianbin LI
Journal of China Pharmaceutical University 2016;47(5):531-536
Soluble guanylate cyclase is a key signal transduction enzyme in the body, which can activate the NO-sGC-cGMP signaling pathway and inhibit the TGF-β signaling pathway. cGMP is one of the most important second messengers and also involved in a series of physiological or pathological responses including vasodilation, inhibition of platelet aggregation, inhibition of cell proliferation and so on. When TGF-β signaling pathway is inhibited, it can inhibit issue fibrosis and cell proliferation. Recent studies have shown that sGC can be activated directly to treat a variety of diseases. As a new class of potential drugs, sGC stimulators have shown many unique advantages. Herein, the mechanism and the latest research progress of sGC stimulators are reviewed, which could provide a useful information for further research of sGC stimulators.
