1.Study on the differences of clinical application in pediatric patients for different brands infrared ear thermometers

Objective:To explore the differences in the clinical application of different brands of infrared ear thermometers in pediatric patients,and to guide the rational use of infrared ear thermometers.Methods:A total of 164 inpatients and outpatients who received treatment in the Children's Hospital of Zhejiang University School of Medicine from May 2023 to July 2023 were randomly selected,and three different brands of ear thermometers were used to measure the ear temperature of the children in the same measurement method.The differences in temperature and time measured by different brands of ear thermometers in children of different ages were compared.Results:The temperature and time measurements of the three ear thermometers were compared,the difference was statistically significant(F=5.007,264.988,P<0.05).The temperature of children aged≤3 months was measured by the three ear thermometers,and the difference was statistically significant(F=3.868,P<0.05).there was no difference in children aged over 3 months.Conclusion:For children of different ages,medical institutions can make choices based on the comprehensive consideration of the use cost,the speed of measurement time and other factors.For children aged over 3 months,choose product B with lower cost and faster measurement time,for children aged 0-3 months,it is recommended to choose product A with more accurate temperature measurement values.
2.Construction and application of competency-based portfolios for medical students

Objective:To retrospectively construct competency-based portfolios for medical students to analyze the factors influencing competency self-assessment, and to explore an approach to efficient portfolio construction.Methods:The participants were randomly selected among medical graduates of 2015 in Shantou University Medical College. Through records collection and an online questionnaire survey, portfolios were built for the participants based on their development data during eight years from college entrance, college education, to post-graduation medical education. The correlation between variables was determined using Kendall's tau-b correlation coefficients. The inter-group differences were analyzed using the Mann-Whitney U test. Results:A total of 126 usable questionnaires for effective developmental portfolios were collected. There were a total of 208 indicators collected, including 79 questionnaire indicators (including 36 for competency self-assessment). The reliability coefficient (Cronbach's α) of the questionnaire was 0.984. The factors related to competency self-assessment were identified. Conclusion:This study provides a basis for the implementation of portfolio assessment, which can promote students' self-assessment and competency development. Optimizing the indicator system, building an online platform, increasing participants' participation motivation, and emphasizing self-reflection and feedback will help improve the efficiency of developmental portfolio construction and its performance.
3.Supraglottic laryngeal carcinoma resection by radiofrequency coblation under a multifunctional opener in four cases and literature analysis

Objective:To investigate the efficacy and feasibility of radiofrequency resection of supraglottic laryngeal carcinoma under a multifunctional opener.Methods:The clinical data of four cases of supraglottic laryngeal carcinoma (type T1N1M0 in two cases, T2N1M0 in one case, and T1N0M0 in one case) who received treatment in the Affiliated Hospital of Jining Medical University during January-June 2019 were retrospectively analyzed. Radiofrequency resection of supraglottic laryngeal carcinoma under a multi-functional opener combined with bilateral neck II-IV region lymph node dissection was performed. Swallowing, breathing and phonation were observed and analyzed based on references.Results:Among the four cases, two cases had a normal diet at 3 days after surgery, one case had a normal diet at 7 days after surgery, and one case had a normal diet at 16 days after surgery. Tracheotomy was not performed in any case. After surgery, breathing and speech communication were not affected.Conclusion:Radiofrequency surgery under a multifunctional opener can be used for treatment of early supraglottic laryngeal cancer. It is an effective treatment with minimal trauma, mild postoperative pain and promotes the early recovery of normal swallowing function.
4.Clinical study of continuous intracranial pressure monitoring after decompressive craniectomy in severe traumatic brain injury patient

Objective To clarify the relationship between intracranial pressure monitoring and prognosis of patients with traumatic brain injury after decompressive craniectomy.Methods From December 2015 to December 2017,48 head-injured patients in Affiliated Suzhou Hospital of Nanjing Medical University were enrolled,who were underwent decompressive craniectomy in this retrospective study.The patients were subdivided into 2 groups based on whether postoperative was monitored (n =19) or not (n =29).The prognosis was evaluated by Glasgow Outcome Scale score,with 1 point of prognosis death,2 to 3 points of poor prognosis,and 4 to 5 points of good prognosis.Count data were expressed as a percentage (%).Count data were expressed as percentage (%).The chi-square test was used to compare the difference in the rate of good prognosis and mortality between the two groups.Results The mortality of monitoring group (10.5%) was significantly lower than that of control group (37.9%) (x2 =4.365 5,P =0.036 7) during hospitalization,The rate of good prognosis in the monitoring group (68.4%) and the control group (44.8%) was not statistically significant (x2 =2.573 8,P =0.108 6).Condusion The study showed that continuous monitoring in patients with severe craniocerebral injury could reduce the mortality of patients during hospitalization,but had no significant effect on the improvement of prognosis.
5.The value of contrast-enhanced CT and MR DWI in differentiating between autoimmune pancreatitis and pancreatic carcinoma

Objective To investigate the diagnostic value of contrast-enhanced CT and MR diffusion-weighted imaging (DWI) in differentiating autoimmune pancreatitis (AIP) from pancreatic carcinoma (PC).Methods Twelve patients with AIP (AIP groups) and 15 patients with PC (PC groups) were included in this study.Contrast-enhanced CT and DWI were performed in all patients.The manifestations of the pancreatic lesions,pancreatic duct and bile duct were observed.The CT attenuation values and the apparent diffusion coefficient (ADC) values of the lesions were calculated.Frequencies of the CT manifestations,and the mean CT attenuation values,and the ADC values were compared between the two groups.Results Most of the AIP showed diffuse enlargement,sausage shape,peripancreatic capsule,penetrating sign of pancreatic duct and thickness of the bile duct.The frequencies of the findings mentioned above were significantly different between AIP and PC (P<0.05).The CT attenuation values of the lesions,and the rate of lesions' CT attenuation values to livers' in 3 phases of contrast enhanced CT were significantly different between AIP and PC (P<0.05).The ADC values of AIP were significantly lower than that of PC (P<0.05).Conclusion The changes of morphology of pancreas,pancreatic and bile duct may facilitate in differentiating AIP from PC.Quantitative analysis of CT attenuation values and ADC values measured on MR images may be helpful in differentiating AIP from PC.
7.The research on quality control parameters detection of neonatal jaundice treatment cases

Objective: Through researching the quality control parameters detection of neonatal jaundice treatment cases, we can control the quality of treatment cases and preventive maintenance to make sure the therapy to newborn is effective and safe. Methods:Using INCU portable analyzer and BiliBlangket Light MeterII blu-ray irradiation apparatus to respectively test the temperature control parameters and the intensity of blue light phototherapy of the jaundice treatment cases, a lot of research data can be acquired. Results:On the base of researching the quality control parameters detection of neonatal jaundice treatment cases (30 sets in total), we can control the quality of treatment cases smoothly. Through these tests, we can fix the medical equipments which cannot pass the detections. Conclusion:The research on the quality control parameters detection of neonatal jaundice treatment cases makes the preventive maintenance and quality control quick and effective to ensure the treatment effect and safety for the neonatal.
8.The Resolution of Wet Wrap Bag after Disinfected in the Pulsation Sterilizer

Our hospital has instaled a couple of high temperature and high pressure pulsation sterilizers. However, after the process of disinfection and sterilization, we found that many non-woven bags in which wrapped the contaminated medical devices were wet. This abnormal phenomenon continuously has happened a couple of times. Having excessive condensed water inside the wrap bag wil severely affect the sterilization effect, so we did a scrupulous survey of the phenomenon and the structure of the machine and the steam boiler. At last, we came up with a perfect solution, that is, to make a redesign of the steam boiler and the steam conveying pipeline network.
9.Synthesis and antitumor activity of S-hexyl(heptyl) substituted ethanethioate derivatives.

To simplify the macrocyclic fragment and to modify the zinc binding group of the natural product apicidin, two series of S-hexyl (heptyl) ethanethioate derivatives were designed and synthesized. Twenty-six compounds were synthesized and confirmed with 1H NMR, IR, MS and HR-MS spectrum, which were not reported. Take vorinostat as control, their antiporliferative activities against cancer cell lines, MCF-7 and HL-60, were tested with MTT assay or trypan blue staining method. Generally in both series it was found that, the chiral carbon atom at 7 position is not necessary, compounds II-1, II-3, II-6 and II-13 showed good activity on HL-60 cells in vitro, with the IC50 values less than 10 micromol x L(-1). II-7 and II-8 showed stronger activity against MCF-7 than Vorinostat, with the IC50 of 3.19 and 6.29 micromol x L(-1), respectively.
10.Effect of evidence-based nursing on ventilator-associated pneumonia in patients with mechanical ventilation

Objective To explore the clinical effect of evidence-based nursing in ventilator-associated pneumonia (VAP).Methods One hundred and sixty patients with mechanical ventilation in our ICU from January 2011 to June 2013 were randomly divided into the observation group and the control group.The control group received the traditional nursing,and the observation group received evidence-based nursing on the basis of the routine nursing.The time of invasive ventilation,the duration of ventilation,the off-line time,the occurrence time of VAP,and the incidence and mortality rates of VAP were compared between two groups.Results The time of invasive ventilation,the duration of ventilation,the off-line time,the occurrence time of VAP in the observation group were respectively (3.3 ± 0.6),(10.3 ± 3.8),(2.6 ± 4.3),(9.2 ± 2.8) d,and were significantly lower or later than (5.6 ± 3.1),(13.1 ± 3.4),(6.5 ± 4.8),(4.8 ± 2.7) d in the control group,and the differences were statistically significant (t =2.58,2.63,2.86,2.79,respectively; P <0.05).The incidence and mortality rates of VAP in the observation group were respectively 16.9% and 10.4%,and were significantly lower than 48.1% and 34.9% in the control group,and the differences were statistically significant (x2 =5.36,4.93,respectively; P < 0.01).Conclusions Evidence-based nursing can significantly shorten the time of invasive ventilation,the duration of ventilation and the off-line time; and can also effectively delay and reduce the occurrence of VAP.
