1.Neuroprotective Mechanism of Yifei Xuanfei Jiangzhuo Prescription on VaD Rats Based on NF-κB/NLRP3 Signaling Pathway

ObjectiveTo investigate the molecular mechanism by which Yifei Xuanfei Jiangzhuo prescription regulates the nuclear factor-κB (NF-κB)/NOD-like receptor protein 3 (NLRP3) signaling pathway to improve neuronal function in vascular dementia (VaD) rats. MethodsA VaD model was established by intermittently clamping the bilateral common carotid arteries (CCA) combined with bilateral vascular occlusion (2-VO). Eighty-four SD rats were randomly divided into a blank group, sham group, model group, piracetam group (0.2 g·kg-1), and low-, medium-, and high-dose Yifei Xuanfei Jiangzhuo prescription groups (6.09, 12.18, and 24.36 g·kg-1). Drug administration began on day 7 after surgery, once daily for 28 consecutive days. Behavioral experiments were used to evaluate learning and spatial memory. Hematoxylin-eosin (HE) staining was applied to observe pathological morphological changes in the CA1 region of the hippocampus. Transmission electron microscopy was used to examine the ultrastructure of hippocampal neurons. Terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) was used to detect neuronal apoptosis in the CA1 region. Immunohistochemistry was performed to determine the positive expression rate of neuronal nuclear antigen (NeuN). Immunofluorescence single staining was used to assess nuclear expression of NF-κB p65 in brain tissue. Western blot was used to detect the protein expression levels of inhibitor of κB kinase (IKK), NF-κB p65, NLRP3, Caspase-1, apoptosis-associated speck-like protein (ASC), and interleukin-1β (IL-1β). ResultsCompared with the blank group, the model group showed a significant reduction in platform-crossing frequency (P0.01), aggravated hippocampal injury, a significant increase in neuronal apoptosis (P0.05), decreased NeuN positivity in the CA1 region (P0.05), increased nuclear expression of NF-κB p65 (P0.05), and significantly elevated expression of p-IKK, p-NF-κB p65, NLRP3, cleaved Caspase-1, ASC, and cleaved IL-1β (P0.05). Compared with the model group, all drug-treated groups improved learning and spatial memory in VaD rats, alleviated hippocampal pathological injury and neuronal apoptosis, and protected neuronal ultrastructure. Yifei Xuanfei Jiangzhuo prescription at doses of 12.18 and 24.36 g·kg-1 reduced hippocampal expression levels of p-IKK, p-NF-κB p65, NLRP3, Caspase-1, ASC, and cleaved IL-1β in VaD rats (P0.05), showing dose-dependent inhibition of the NF-κB/NLRP3 signaling pathway. ConclusionYifei Xuanfei Jiangzhuo prescription may exert neuroprotective effects by regulating the NF-κB/NLRP3 signaling pathway, thereby reducing neuroinflammation and inhibiting hippocampal neuronal apoptosis.

3.Effects of Wenpi Tongluo Kaiqiao Prescription combined with scalp acupuncture on autophagy markers Beclin1, P62 and MAP1-LC3 in hippocampus of AD rats

Objective:To investigate the effects of Wenpi Tongluo Kaiqiao Prescription combined with scalp acupuncture on the expression levels of hippocampal autophagy markers Beclin1, P62 and MAP1-LC3 in Alzheimer's disease (AD) model rats.Methods:The AD rat model was established by stereotactic drug injection, and randomly divided into model group, acupuncture-medicine group, and Western medicine group. Sham-operation group and control group were set, with 10 rats in each group. The acupuncture-medicine group was treated with scalp acupuncture plus Wenpi Tongluo Kaiqiao Prescription (16.8 g/kg); the Western medicine group was treated with donepezil hydrochloride tablets (0.08 mg/kg in week 1, 0.16 mg/kg in week 2, 0.24 mg/kg in week 3, 0.32 mg/kg in week 4) for gavage. The sham-operation group, the control group and the model group were treated with equal volume of normal saline for gavage. The drug was administered 2 weeks after modeling. Morris water maze was used to test the learning and memory abilities of rats in each group after continuous administration for 4 weeks, and the hippocampus of the animal was taken after Morris water maze. HE staining and TUNEL staining were used to observe the pathological changes of the hippocampal CA1 region in each group. Western blot was used to detect the expression levels of Beclin1, P62 and MAP1-LC3 protein genes.Results:Compared with the model group, the escape latency of rats in the acupuncture-medicine group and Western medicine groups was significantly shortened ( P<0.05), and the number of times they crossed the platform increased ( P<0.05). The HE and TUNEL results indicated that the model group rats had abnormal tissue structure and cell morphology in the CA1 area of the hippocampus, with significant cell degeneration. There were more autophagic vesicles in the cytoplasm of hippocampal neurons, and the structure of autophagic lysosomes was clear and complete; the tissue structure, cell morphology, and cellular degeneration of the CA1 area in the hippocampus of rats treated with acupuncture and medicine were significantly improved. The percentage of cell apoptosis in the acupuncture-medicine group and Western medicine group decreased ( P<0.05), the expression of Beclin1 protein in hippocampal tissue increased, and the expressions of MAP1-LC3 and P62 proteins decreased ( P<0.05). Conclusion:Wenpi Tongluo Kaiqiao Prescription combined with scalp acupuncture can suppress the apoptosis of hippocampal CA1 neurons, promote cognitive and memory recovery in AD rats by promoting the expression of Beclin1 in the hippocampus, inhibiting the expression of P62 and LC3, enhancing autophagy levels, and increasing autophagic activity.
4.Intraductal carcinomas of the salivary glands: a clinicopathological and molecular genetic analysis of twenty-seven cases

Objective:To investigate the clinicopathological features, molecular genetic features, and differential diagnosis of intraductal carcinomas (IDC) of the salivary glands.Methods:Twenty-five cases of salivary gland IDC diagnosed at the Department of Oral Pathology, Shanghai Ninth People′s Hospital and two cases from Department of Pathology, Henan Provincial People′s Hospital, Zhengzhou, China from January 2008 to July 2023 were collected. Their clinical and pathological features were analyzed retrospectively. Fluorescence in situ hybridization and Sanger sequencing were performed. The patients were followed up and related literatures were reviewed.Results:There were 27 patients with IDC, including 15 males and 12 females, ranging in age from 20.0 to 80.0 years (mean 55.9 years). Clinically, the tumor often presented as a painless mass with a tumor diameter of 1.0-3.0 cm (mean 2.0 cm). All patients received surgical treatment. Twenty patients were followed up. One of them (1/20) died of lung cancer, while the rest survived without tumor recurrence. Histologically, IDC were classified as: intercalated (63.0%, 17/27), apocrine (25.9%, 7/27), oncocytic (7.4%, 2/27) and mixed (3.7%, 1/27) types. Intercalated tumors showed positive S-100 and negative androgen receptor (AR) immunoreactivity. Ki-67 proliferation index was low (about 1%-5%). Nine cases had the RET gene disruption, and 2 cases showed the BRAF V600E mutation. Apocrine tumors showed strong AR immunoreactivity but no S-100 immunoreactivity. Ki-67 proliferation index was high (about 10%-60%), and the RET gene rupture was detected in 1 case. Oncocytic tumors were similar to that of intercalated type in 2 cases, and RET gene disruption was detected in the both cases. Mixed tumors showed histologic features of oncocytic and apocrine patterns and harbored the RET gene disruption.Conclusions:IDC is a rare low-grade malignant tumor of the salivary gland and easily confused with other salivary gland tumors with similar morphology. Molecular testing is helpful for its differential diagnosis.
5.Based on Network Pharmacology and Experimental Verification the Mechanism of Lung Xuan Lung Turbidity Reduction Formula in Thein the Treatment of VaD was Studied

Objective The molecular mechanism of Yi Lung Xuan Lung Subduing Turbidity Formula in treating rats with vascular dementia(VaD)model was investigated by network pharmacology and animal experiments.Methods The TCMSP database platform was used to screen the active ingredients and related targets of Yi Lung,Xuan Lung and Turbidity Subduing Formula.Since Maitong could not be found in the TCMSP database,the components of Maitong were retrieved by reviewing the literature and using the BATMAN-TCM Bioinformatics Analysis Tool.The GeneCards database was used to obtain VaD-related targets and predict the potential targets of Yi Lung Xuan Lung Turbidity Reducing Formula for the treatment of VaD.The network structure of"active ingredient-target intersection"of the formula was mapped by STRING and Cytoscape 3.7.2 software,and the PPI network model was established to find the key targets.GO enrichment and KEGG pathway enrichment analyses were performed on the targets of VaD treated with Yi Lung Xuan Lung Turbidity Reducing Formula.29 rats were randomly divided into the sham-operation group,the model group,the traditional Chinese medicine group and the western medicine group.The 2-VO method was used for modelling,and the water maze was used to test the memory behaviours of the rats;HE staining was used to observe the pathological changes in the hippocampal CA1 region of the animals in each group,fluorescence detection of VEGF content in the brain of the rats in each group,and ELISA to detect the content of IL-6 and TNF-α in the brain tissues of the rats in each group;Western blot was used to detect the levels of Nrf2,HO-1,P Nrf2,HO-1,P-Akt/Akt and NF-κB.Results A total of 380 targets were obtained from Yi Lung Xuan Lung Turbidity Reducing Formula,and 183 targets were intersected with VaD;The PPI network of intersected core target genes was constructed with 92 nodes and 2610 edges.The GO results suggested that the biological processes related to VaD treatment included responses to lipopolysaccharide,oxidative stress,cell migration,etc.KEGG enrichment analysis suggested that the key pathways included NF-κB,Akt,VEGF,NOD-like receptor signalling.Compared with the model group,the number of crossing platforms in the traditional Chinese medicine group was significantly increased,the histological structure and cellular morphology of hippocampal CA1 area were intact and there were few cellular degeneration,the expression levels of Nrf2,Akt,and HO-1 were significantly increased,and the expression level of NF-κB was significantly decreased(P<0.05),and inflammatory indexes,IL-6 and TNF-α,were significantly down-regulated(P<0.01),and the immunological monoclonal fluorescence showed that the vascular endothelial cell in the traditional Chinese medicine group was significantly increased,while the activity of antioxidant factor SOD increased and the content of oxidative damage factor MDA decreased(P<0.05).Conclusion The network pharmacological analysis suggested that Yi Lung Xuan Lung and Turbidity Reducing Formula could treat VaD by regulating multiple signalling pathways and biological processes.Yi Lung Xuan Lung and Turbidity Reducing Formula increased the protein expression levels of Nrf2,P-Akt/Akt and HO-1,VEGF,and improved the degeneration of neuronal cells of hippocampal CA1 area of rats with VaD,which may be through the activation of AKT/Nrf2/HO-1,and improved the lesions of hippocampal area of VaD,inhibited oxidative damage,and decreased the content of MDA,a factor that can damage oxidative processes.lesions,inhibiting oxidative damage and down-regulating the NF-κB pathway,reducing neuroinflammation,and thus improving cognitive function in VaD model rats.
6.The influence of three level quality control nursing management model on the quality of nursing service in the department of general surgery

Objective To analyze effect of three level nursing quality control management model on the quality of the nursing service in the department of general surgery.Methods Analysis in the implementation of nurs-ing quality control before and after nursing quality and the improvement of the examination results of nurses situation in general surgery department of our hospital were compared between the 2012 -2013 without the implementation of three level quality control mode and 2013 -2014 of establishment of the three level nursing quality control manage-ment mode,which was set the nursing quality monitoring nurses,quality monitoring group leader and quality monito-ring nurses in three level nursing quality control system.Results After the implementation of quality control,the quality of basic nursing score[(94.02 ±2.01)points],ward management quality score[(94.13 ±3.04)points],first aid management quality score[(96.22 ±4.06)points]and disinfection and isolation,and the quality score[(95.13 ± 3.04)points]and quality control of basic nursing quality score[(8.121 ±3.31 )points],ward management quality score[(81.35 ±2.34)points],emergency management quality score [(91.42 ±4.36 )points]and disinfection [(81.74 ±3.44)points]degrees and isolation quality scores were significantly higher than those of before three level quality control nursing management model(t =33.07,33.32,8.06,29.17,all P <0.05).Nursing quality control of nursing personnel the technical operation of the average score [(96.35 ±1.03)points],theory test average score [(95.73 ±0.98)points],average and quality control of nursing personnel and technical operation [(89.42 ± 2.49)points],theory test average score[(90.36 ±1.79)points]were obviously higher.The results had statistical significance (t =25.72,26.32,all P <0.05).Conclusion Three level nursing quality control management model established in the general surgery department had remarkable effect on the quality of the nursing service,which is safe and reliable,and improving the quality of nursing.
7.Effect of continuous quality improvement nursing on grasping disease knowledge and satisfaction of patients with thyroid tumors

Objective To investigate the effect of continuous quality improvement nursing on grasping disease knowledge and satisfaction of patients with thyroid tumors.Methods 86 cases of patients with thyroid cancer were enrolled.They were devided into the observation group(42 cases)and the control group(46 cases)according to the date of admission.Patients in control group were treated with routine care,while patients in the observation group were treated with continuous quality improvement nursing on the basis of the control group.The effects of nursing in the two groups were compared and analysized.Results After intervention,the scores of cognitive thyroid tumors,daily preventive health care,thyroid cancer surgery and surgical considerations in the observation group were higher than those in the control group,theses differences were statistically significant between the two groups (t =9.806,10.271, 9.924,12.053,all P <0.05).The rate of satisfaction in the observation group was significantly higther than that in the control group(92.86%/75.00%),theses difference was statistically significant between the two groups (χ2 =9.025,P <0.05).Conclusion Continuous quality improvement nursing can effectively improve the diseases -related knowledge in patients with thyroid benign tumor,which contributes to the smooth operation and improves surgical results.
8.Clinical investigation of entecavir combined with adefovir dipivoxil in the treatment of lamivudine -resistant hepatitis B cirrhosis

Objective To investigate the clinical effect of entecavir combined with adefovir dipivoxil in the treatment of lamivudine -resistant hepatitis B cirrhosis,to provide a reference for clinical treatment.Methods 117 cases of liver cirrhosis with lamivudine resistance were selected,they were divided into the control group and the observation group according to treatment.60 cases in the observation group used entecavir and adefovir combination therapy,57 cases in the control group was given lamivudine combined with adefovir dipivoxil.The HBeAg conversion rate,HBV -DNA negative rate,liver function,liver function Child -pugh score were compared between two groups.Results After treatment for 24 weeks,48 weeks,the HBV -DNA negative conversion rates in the observation group were 75.00%, 95.00%,which were higher than those in the control group,the differences were statistically significant (χ2 =4.251, P =0.024;χ2 =4.535,P =0.018).In the observation group,ALB,ALT,TBiL,PT improved better than the control group,the differences were statistically significant(t =4.229,P =0.025;t =6.214,P =0.008;t =5.514,P =0.014;t =5.233,P =0.017).After treatment,CTP of the observation group was (7.15 ±1.05)points,which was significantly lower than the control group (8.86 ± 1.47)points,the difference was statistically significant (t =5.874,P =0.010).The incidence rate of adverse reactions between the two groups showed no statistically significant difference (P >0.05).Conclusion Entecavir combined with adefovir dipivoxil therapy has good effect for lamivudine -resistant liver cirrhosis,which will help to improve liver function,inhibit HBV replication,it is worthy of clinical application.
9.Prevalence rate of ovine hepatic cystic echinococcosis in Quaker Wusu area of Bayinbuluke of Xinjiang, 2014

Objective To investigate the prevalence rate of ovine hepatic cystic echinococcosis (HCE) in sheep in Quaker Wusu area of Bayinbuluke of Xinjiang by ultrasonography and provide evidence for the prevention and control of HCE in sheep.Methods The prevalence screening of HCE in sheep was conducted based on ultrasound images in this area in July 2014.The sheep were divided into different groups by dental age to calculate the age specific prevalence rate of HCE and analyzed the correlation between the dental age and the prevalence rate.Results The total prevalence rate of HCE in sheep in this area was 36.9%.The prevalence rates of none-calcified HCE and calcified HCE were 7.3% and 29.6%,respectively.The prevalence rates of none-calcified HCE in different age groups were 1.2% (1-2 years old),1.4% (2-3 years old),14.0% (3-4 years old),10.0% (4-5 years old),15.6%(5-6 years old) and 4.2%(>6 years old) respectively.The prevalence rate of calcified HCE in different age groups were 9.9%(1-2 years old),16.2% (2-3 years old),31.6%(3-4 years old),47.8%(4-5 years old),42.2%(5-6 years old) and 41.7%(>6 years old) respectively.The prevalence rate of HCE in 1-2 years old group was lower than those in other groups,the prevalence rate of HCE in age groups >3 years increased significantly.There was positive correlation between the prevalence rate of HCE and dental age (r=0.372,R2=0.107,F=44.176,P=0.000).Conclusion HCE is highly endemic in Quaker Wusu area.The prevalence rate of HCE is low in sheep with young age and high in sheep aged 3-4 years.It is necessary to conduct early prevention of HCE in sheep in this area.
10.Relationship between coping style and social relational quality in patients with thyroid carcinoma

Objective To investigate the coping style and social relational quality of patients with thyroid carcinoma, and analyze the relationship between them,so as to provide a new perspective to improve the coping style of patients with thyroid carcinoma.Methods 311 patients with thyroid carcinoma were surveyed by the Social Relational Quality Scale and the Medical Coping Style Questionnaire ( Chinese version) .The level of coping style was compared among the patients with different clinical features, and the pearson correlation analysis and multiple linear regression analysis were used to explore the relationship between coping style and social relational quality of patients with thyroid carcinoma.Results The scores of facing, yield and avoidance in the coping style were (18.96 ±2.54), (13.15 ±1.48) and (17.76 ±2.23), respectively.The scores of social relationship of family intimacy, family responsibility and friendship were (20.47 ±2.61),(15.22 ± 2.22) and (14.75 ±2.03), respectively.The total score of social relational quality was (50.44 ±5.51).The score of facing was positively correlated with the total score and the each dimension score of social relational quality (P<0.05).The scores of yield and avoidance were negatively correlated with the total score and the each dimension score of social relational quality ( P <0.05 ) .The differences of coping style in the patients with different family income, model of payment, course of disease and lymph node metastasis were significant ( P<0.05).Multiple linear regression analysis showed that the family income, payment method, lymph node metastasis and social relational quality were the predictors of coping style of patients with thyroid carcinoma(P<0.05). Conclusions The coping style was significantly correlated with social relational quality in patients with thyroid carcinoma.We should improve the coping style by improving the social relational quality.
