1. Influence of Ineffective Swallows on Esophageal Motility and Gastroesophageal Reflux

Background : Studies showed that esophageal body dysmotility is associated with gastroesophageal reflux disease (GERD), however, their interactions are still unclear. Aims: To explore the influence of proportion of ineffective swallows on esophageal motility and gastroesophageal reflux in esophageal high-resolution manometry (HRM). Methods: Patients who completed esophageal HRM and 24 h esophageal impedanee-pH monitoring and were identified as normal esophageal motility or mild dysmotility from March 2018 to December 2019 at the First Affiliated Hospital of Nanjing Medical University were recruited retrospectively. According to the times of ineffective swallows in 10 warm water swallows in HRM, these patients were allocated into four groups; Group A (0 times), Group B (1-4 times), Group C (5-7 times), Group D (8-10 times). The parameters of esophageal HRM and 24 h esophageal impedance-pH monitoring were analyzed, and the value of ineffective swallows for assistant diagnosis of pathological acid reflux was assessed. Results: A total of 142 patients were included. There were no significant differences in abnormal manometric parameters between the four groups (all P > 0. 05). In Group D, the number of weak and non-peristalsis were increased, while the mean and maximum value of distal contractile integral (DCI) were decreased as compared with those in Group A and Group B (all P < 0. 05). Between Group C and Group D, the differences in mean and maximum DCI value were significant (all P < 0. 05). As the times of ineffective swallows increased, the acid exposure time (AET) and DeMeester score gradually increased from Group A to Group D (all P < 0. 05). Times of ineffective swallows and weak peristalsis were moderately and positively correlated with AET, DeMeester score, and the total number of acid exposure (all P < 0. 05). ROC curve analysis revealed that the area under the curve (AUC) of ineffective swallows for assistant diagnosis of pathological acid reflux was 0. 625 (95 % CI; 0.523-0. 727, P = 0. 027); the optimal cut-off value was 4. 5, and the sensitivity and specificity were 62. 9% and 61. 7 %, respectively. The diagnostic performance of weak peristalsis was superior to that of non-peristalsis (AUC: 0. 625 vs. 0. 590). Conclusions: Different proportions of ineffective swallows suggest different clinical significance. The clinical relevance of ineffective swallows > 70 % might be most significant, and to a certain extent, can predicts pathological acid reflux.
2.Effects of Mangiferin on Glucose and Lipid Metabolism of Insulin-resistant HepG2 Cells

OBJECTIVE:To analyze the effects of mangiferin (MGF)on glucose and lipid metabolism in insulin resistance (IR)HepG2 cells,and to explore the potential mechanism. METHODS :Using human hepatoma HepG 2 cells as research objects , 1 mmol/L palmitic acid and 2 mmol/L oleic acid were used to establish the IR-HepG 2 cell model. Using metformin hydrochloride as positive control ,the effects of low-concentration ,medium-concentration and high-concentration MGF (125,250,500 μmol/L)on the corrected glucose consumption ,the contents of triglyceride (TG)and total cholesterol (TC)in IR-HepG 2 cells were detected. The mRNA expression of APN ,AdipoR2,APPL1,AMPK in the upstream of AMPK signaling pathway and IRS- 1,Akt and GLUT4 in the downstream insulin signaling pathway were detected by RT-PCR. The phosphorylation level of AMPK protein was detected by Western blot assay. RESULTS :Compared with control group ,corrected glucose consumption ,mRNA expression of APN,AdipoR2,APPL1,AMPK,IRS-1 and GLUT 4,as well as the phosphorylation level of AMPK protein were decreased significantly in model group ,while the contents of TG and TC were increased significantly (P<0.05 or P<0.01). Compared with model group , corrected glucose consumption , mRNA expression of APN (except for MGF medium-concentration and high-concentration groups ),AdipoR2,APPL1,AMPK(except for MGF medium-concentration and high-concentration groups ), IRS-1(except for MGF medium-concentration and high-concentration groups ),Akt(except for positive control group ),GLUT4 (except for MGF high-concentration group )were increased significantly in administration groups ,while the contents of TG and TC were decreased significantly (P<0.05 or P<0.01). CONCLUSIONS :Mangiferin may activate APN ,which is the upstream target of pathway ,and then regulate AMPK signaling pathway ,so as to promote glucose uptake of IR-HepG 2 cells,reduce TG and TC contents,and improve IR and abnormal glucose and lipid metabolism.
3.Establishment of Modified Aqueous Two-phase Extraction System of Polyphenols from Archidendron clypearia

OBJECTIVE: To e stablish modified aqueous two-phase extraction (ATPE) system of polyphenols from Archidendron clypearia. METHODS :Taking the polyphenol content ,extraction efficiency and partition coefficient of A. clypearia as indexes ,the solid-liquid ratio ,ethanol mass fraction and ultrasonic time of ATPE system were selected by single factor tests . The aqueous two-phase extracts from different polar parts of A. clypearia was prepared. The modifier was selected by taking polyphenol content and antioxidant activity [IC 50 of 1,1-diphenyl-2-trinitrophenylhydrazine (DPPH), 2,2′-hydrazine bis (3-ethylbenzothiazole-6-sulfonic acid ) diammonium salt (ABTS)] as indicators ,combined with the grey correlation analysis between polyphenols and antioxidant activity. The effects of modified ATPE system with different mass fraction (0-90%)of modifier on the extraction of polyphenols from A. clypearia were investigated and compared with traditional extraction technology (ultrasonic extraction and heating reflux extraction ). RESULTS :The optimal ATPE system included solid-liquid ratio of 1 ∶ 45, ethanol mass fraction of 40%,(NH4)2SO4 mass fraction of 11%,ultrasonic extraction time of 20 min,at room temperature. In 3 validation tests ,polyphenol content ,extraction efficiency and partition coefficient were (28.35±1.01)%(RSD=3.56%,n=3), (98.87±0.19)%(RSD=0.19%,n=3)and 13.25±0.71(RSD=5.39%,n=3),respectively. The modifier was ethyl acetate. When the mass fraction of ethyl acetate in the ethyl acetate-ethanol mixed solvent was 70%,there was no significant difference in the content and anti-oxidant activity of polyphenols from A. clypearia of modified ATPE (P>0.05). The yield and transfer rate of it were si gnificantly higher than those of ultrasonic extraction and heating reflux extraction (P<0.05). CONCLUSIONS :The modified ATPE system with better extraction yield and transfer rate than the traditional extraction technology is successfully established,which can extract polyphenols from A. clypearia 1048214903@qq.com in one step.
4.Survey of neuromuscular monitoring and assessment of postoperative residual neuromuscular block in a postoperative anaesthetic care unit.

INTRODUCTION:
The use of neuromuscular blocking agents (NMBAs) is common during general anaesthesia. Neuromuscular monitoring with a peripheral nerve stimulator (PNS) is essential to prevent postoperative residual neuromuscular block (PRNB), defined as a train-of-four (TOF) ratio < 0.9. PRNB remains a common complication and may contribute to morbidity in the postoperative anaesthetic care unit (PACU).
METHODS:
An online survey was sent to anaesthesiologists in our department to assess their knowledge and clinical practices related to neuromuscular blockade. Next, a study was conducted on adult patients scheduled for elective surgery under general anaesthesia requiring NMBAs. Upon admission to the PACU, TOF monitoring was performed.
RESULTS:
A large proportion of anaesthesiologists showed a lack of knowledge of neuromuscular blockade or non-adherence to the best clinical practices associated with it. The majority (98.7%) stated that they did not routinely use PNS monitoring. In the clinical study, TOF monitoring was only used in 17.9% of the 335 patients who were assessed. The prevalence of PRNB was 33.4% and was associated with the elderly (age ≥ 65 years), a higher dose of NMBA used, a shorter duration of surgery, and a shorter duration between the last dose of NMBA and measurement of PRNB in the PACU. The incidence of adverse symptoms in the PACU was observed to be higher in patients with PRNB.
CONCLUSION
PRNB remains a clinically significant problem, but routine PNS monitoring is rare in our institution. This is compounded by inadequate knowledge and poor adherence to best clinical guidelines related to neuromuscular blockade.
5.How do Singapore patients view post-anaesthesia adverse outcomes? A single-centre willingness-to-pay study.

INTRODUCTIONKnowing how patients value the quality of anaesthesia helps anaesthesiologists to customise their service. However, generalising findings from Western population-based willingness-to-pay studies across different cultures and societies might result in the oversight of some contextualised perspectives of the anaesthesia experience. This study aimed to capture the Singapore perspective of undesired post-anaesthesia outcomes.
METHODS132 patients recruited in a pre-anaesthetic evaluation clinic were given questionnaires describing ten possible post-anaesthetic outcomes. Outcomes were ranked for undesirability and assigned relative value through the hypothetical proportioning of SGD 100 to avoid their occurrence. Data was analysed with reference to patients' background and anaesthetic history.
RESULTSA response rate of 69.1% (n = 132/191) was achieved. Outcomes from the most to least undesirable were pain; vomiting; nausea; shivering; orodental trauma; sore throat; abrasions; somnolence; and thirst. Relative values allocated, in descending order, were pain; vomiting; nausea; orodental trauma; abrasions; sore throat; shivering; somnolence; and thirst.
CONCLUSIONSimilar to previous studies in Western populations, pain, vomiting and nausea were the top three adverse outcomes that Singapore patients wished to avoid. However, discrepancies with Western patients were seen in spending attitudes, possibly accounted for by differences in healthcare socioeconomics. This study provided a better understanding of Singapore patients' perspectives on post-anaesthesia adverse outcomes and could help to improve treatment strategy and resource management.

Break of a stent delivery catheter and subsequent stent loss (SL) has been a rare event in the drug-eluting stent (DES) era. We here report a case of successful retrieval of a stent after a break if the delivery catheter and SL from a balloon catheter at a culprit lesion. We finally resolved this situation using a simple balloon technique for both the broken stent catheter inside of the guide catheter and the unexpanded stent in the culprit lesion. Thus balloons are an important weapon in our armamentarium in the cardiac catheterization laboratory for urgent retrieval of a lost stent. Their apt use definitely allowed our patient to avoid undergoing emergency cardiovascular thoracic surgery.

7.A Case of ABO*Ael02/O04 Genotype with Typical Phenotype O.

Ael is a rare blood type which has the least amount of A antigen among A subgroups. It can be detected by special tests performed to resolve the discrepancy between red cell and serum typing in routine serological typing. The presence of A antigen on Ael red cell is demonstrable only by adsorption and elution tests. An Ael individual does not secret A substance in the saliva and may have anti-A antibody in the serum which is usually less reactive with the reagent red cells than anti-B antibody. In Korea, Ael02 has been reported more frequently than other Ael alleles. We report a case of Ael02/O04 who presented as typical phenotype O with strong anti-A and anti-B antibodies and no A antigen detected even by adsorption and elution tests. The case has been proved to be Ael02/O04 by direct sequencing analysis. In individuals with history of discrepancies in the results of ABO phenotyping, ABO genotyping is needed for an accurate evaluation of their blood type.

8.Detection of Mutations in Chronic Hepatitis B Virus under Lamivudine Treatment: Direct Sequencing and CLIP Sequencing.

BACKGROUND: Hepatitis B Virus (HBV) is a major risk factor for hepatocellular carcinoma, and about five to six percents of people are infected with HBV in Korea. Lamivudine is a first-line drug having good control against HBV replication, but long-term treatment by lamivudine induces drug resistance. We analyzed the rate of HBV resistance mutation for lamivudine by direct sequencing and CLIP sequencing. METHODS: HBV DNA was isolated from 371 patients who were in treatment, or were planning to be treated with lamivudine. The direct sequencing for lamivudine resistance mutation was performed in 371 patients and CLIP sequencing in 138 patients. We analyzed the mutation rate and the type of mutations for lamivudine resistance. RESULTS: The mutation was detected in 203 patients (54.7%) and (CTG) L180M (ATG) was most common (36.1%) followed by (ATG) M204I (ATT) (29.9%) and (ATG) M204V (GTG) (18.6%). According to the duration of treatment, mutation rates were as follows: 45.3% for less than one year, 71.7% for one to two years, 66.7% for two to three years, and 87.9% for more than three years. The results of the direct sequencing and CLIP sequencing agreed in 134 out of 138 patients, in whom both tests were performed. CONCLUSIONS: We confirmed that HBV mutation rates for lamivudine resistance increased as the lamivudine treatment period increased. The lamivudine resistance mutations detected were similar to the previous studies. CLIP sequencing showed good correlation with the direct sequencing and gave additional mutation information. CLIP sequencing is a promising tool for the detection of lamivudine resistance mutation in HBV that can assist treatment plans.

10.Neuroprotective Therapy in Parkinson's Disease: Current Status and New Directions from Experimental and Genetic Clues.

Despite successful treatment of Parkinson's disease (PD) with a wide variety of symptomatic therapy, the disease continues to progress and drug-resistance symptoms become the predominant factors producing the disability of PD patients. Neuroprotective therapies have been tested, but clinically effective drugs have not been found yet. New insights gained from studies of genetic forms of PD point to the common pathogenic mechanisms that have been suspected in sporadic forms of the disease and may provide new approaches for the future neuroprotective therapies.
