1.Synthesis and in vitro antiviral effects against hepatitis C virus of oleanolic acid and ursolic acid derivatives

Objective To design and synthesize derivatives of oleanolic acid and ursolic acid, and investigate their anti-hepatitis C virus (HCV) activity along with that of common triterpenoid acids. To explore the structure-activity relationship and provide a reference for the research of anti-HCV drugs derived from natural products through obtaining compounds with higher activity. Methods Oleanolic acid and ursolic acid were directly reacted with corresponding amines using PyBOP as a condensing agent in the presence of DIEA. Alternatively, the target compounds were prepared through PCC oxidation followed by the Baeyer-Villiger reaction catalyzed by m-CPBA. In vitro anti-HCV activity was tested using the HCVcc infection model. Molecular docking was performed by Autodock software to investigate the interaction between the active compounds and HCV NS5B. Results Oleanolic acid, glycyrrhetinic acid, ursolic acid, and asiatic acid all exhibited certain anti-HCV effects. Specifically, oleanolic acid derivatives OA2-OA4, OA6, and OA7, as well as ursolic acid derivatives UA1 and UA2, demonstrated superior anti-HCV activity compared to their parent compounds. Preliminary structure-activity relationship analysis revealed that introducing a bulky group to 28-COOH of oleanolic acid and ursolic acid enhanced their activity. Molecular docking results demonstrated that the active compounds could stably bind to HCV NS5B, thereby exhibiting antiviral activity. Conclusion Pentacyclic triterpenoids possessed anti-HCV effects, and their derivatives coud be synthesized to obtain more active compounds. The anti-HCV mechanism of these compounds may be associated with their inhibition of NS5B.
2.The chain mediating effect of perceived overqualification and job crafting on the relationship between platform leadership and career calling of clinical nurses

Objective:To explore the relationship between platform leadership, perceived overqualification, job crafting and career calling of clinical nurses, and provide references for formulating occupational calling strategies for clinical nurses.Methods:A total of 488 clinical nurses from Qingdao Hiser Hospital Affiliated of Qingdao University(Qingdao Traditional Chinese Medicine Hospital), Shandong University Qilu Hospital, and Qingdao Municipal Hospital were selected as the study subjects by the convenience sampling method from April to May 2023. A general data questionnaire, Platform Leadership Scale, Perceived Overqualification Scale, Job Crafting Scale, and Career Calling Scale were used to conduct a cross-sectional investigation on the research objects, and the relationship and mediating effect between platform leadership, perceived overqualification, job crafting, and career calling were analyzed.Results:Among the 488 clinical nurses, 79 were males and 409 were females, aged 23-51 (30.37 ± 6.40) years old. The scores of platform leadership, perceived overqualification, job crafting and career calling of clinical nurses were (86.12 ± 9.99), (21.79 ± 7.29), (70.61 ± 14.54) and (38.70 ± 7.54) points, respectively. The total effect of platform leadership on work engagement was 0.316. The mediating path of platform leadership to career calling included platform leadership→perceived overqualification→career calling; platform leadership→job crafting→career calling; platform leadership→perceived overqualification→job crafting→career calling. Three mediating effects accounted for 17.99%, 9.50% and 4.56% of the total effect, respectively.Conclusions:There is a chain-mediating effect of perceived overqualification and work remodeling on the relationship between platform leadership and the career calling of clinical nurses. Nursing managers should implement platform leadership to alleviate clinical nurses ′ perceived overqualification, improve job crafting, and then improve the level of clinical nurses ′ career calling.
3.Analysis of colorectal cancer screening results among residents in Baoshan District

Objective:
To analyze the results of colorectal cancer screening among residents in Baoshan District, Shanghai Municipality from 2013 to 2021, so as to provide the basis for promoting colorectal cancer screening and prevention.
Methods:
Permanent residents aged 50 to 74 years in Baoshan District from 2013 to 2021 were selected as the screening population. The initial screening was conducted using a risk assessment form and fecal occult blood test. Positive results on either the risk assessment form or fecal occult blood test were considered positive for the initial screening. Participants with positive initial screening results were invited to undergo colonoscopy. The positive rate of the initial screening, colonoscopy compliance rate, and colonoscopy results were analyzed.
Results:
A total of 264 907 individuals underwent the initial colorectal cancer screening in Baoshan District from 2013 to 2021, with 65 333 individuals (24.66%) testing positive. Among them, the positive rate of the risk assessment form was 12.16%, and the positive rate of fecal occult blood test was 14.64%. A total of 14 473 individuals completed colonoscopy, with a compliance rate of 22.15%. A total of 1 284 precancerous lesions were detected, with a detection rate of 8.87%, and 386 cases of colorectal cancer were identified, with a detection rate of 2.67%. The positive rate of the initial screening, colonoscopy compliance rate, precancerous lesion detection rate, and colorectal cancer detection rate were higher in males than in females (25.55% vs. 24.06%, 23.12% vs. 21.45%, 11.60% vs. 6.74%, 3.62% vs. 1.93%, all P<0.05). With increasing age, the positive rate of the initial screening increased, the colonoscopy compliance rate decreased, the precancerous lesion detection rate and colorectal cancer detection rate increased (all P<0.05). From 2013 to 2021, the positive rate of the initial screening among residents showed a downward trend, while the colonoscopy compliance rate showed an upward trend (both P<0.05).
Conclusions
The detection rate of precancerous lesions in colorectal cancer was 8.87%, and the detection rate of colorectal cancer was 2.67% in Baoshan District from 2013 to 2021. Male and older individuals were the key populations for screening, and the colonoscopy compliance among residents needs to be improved.
4.Research progress of supplementing qi and nourishing yin traditional Chinese medicine in treatment of palpitation

Arrhythmia is one of the most common diseases in clinical practice that can threaten the health and safety of patients and is referred to as "palpitations" in traditional Chinese medicine. Traditional Chinese medicine often uses the method of tonifying qi and nourishing yin to treat palpitations in clinical practice. However, there is currently a lack of relevant research on its treatment mechanism. From the perspective of pharmacology and through a literature search, this paper summarized the progress in the mechanism of traditional Chinese medicine for supplementing qi and nourishing yin commonly used in the treatment of palpitations, as well as traditional Chinese patent medicines and simple preparations for the treatment of arrhythmia, to explore their mechanism of action on palpitations. This paper also summarized that traditional Chinese medicine drugs for tonifying qi and nourishing yin mainly regulate various signal pathways and ion channels through multiple target genes, thereby regulating heart rhythm and improving clinical symptoms in patients with palpitations. The results can provide new ideas for colleagues on palpitations treatment.

7.Application research on construction of Pathogen Biology and Immunology memory clock-in mode based on wechat applet

Objective:To explore the application effect of memory clock-in mode to promote the quality of pre class memory in hybrid teaching.Methods:Total 778 nursing students were selected as research objects,and divided into control group and observation group.Control group memorized according to the learning requirements before the general class,observation group participated in memory punching on the basis of control group.The final examination scores,memory frequency and memory difficulty of the subjects were com-pared between the two groups,then investigate the satisfaction of memory clock.Results:Final examination score of observation group was higher than that of control group(P<0.05);the number of weekly memory in observation group was significantly higher than that in control group(P<0.05),while there was no significant difference in the length of each memory between observation group and control group(P>0.05);difficulty of subjective perception and memory in observation group was lower than that in control group(P<0.05);students who participated in the whole process of clock in memory were more satisfied with knowledge grid memory than those who did not participate in the whole process of clock in memory(P<0.05).Conclusion:The memory punch in mode improves the pre class memory quality of the hybrid teaching of Pathogenic Biology and Immunology.
8.Discussion on MA Shi's Critical Inheritance and Innovative Development of WANG Bing's Annotations in Suwen Zhuzheng Fa-wei

[Objective]To summarize the characteristics of MA Shi's refutations of WANG Bing's annotations in Suwen Zhuzheng Fawei(Commentaries and Discussions on Suwen of Yellow Emperor's Inner Canon),and analyze the relationship between MA's annotations and WANG's annotations.[Methods]After consulting the annotations of MA Shi refuting WANG Bing in Suwen Zhuzheng Fawei,this paper analyzes MA Shi's critical views on WANG's annotations,compares the different understandings of MA Shi and WANG Bing on scriptures,and summarizes the characteristics of MA's annotations from three aspects:expression of terms,method of interpreting scriptures and elucidation of medical theory.Finally it sums up the relationship between MA's annotations and WANG's annotations.[Results]When refuting WANG's annotations,MA Shi often used three types of words to express different degrees of refutation on WANG's annotations,which contained"opposed""superficial"and"lacking".The methods of interpreting scriptures can be summarized into five kinds of citation analysis and four levels of literary analysis,and the scriptures of Suwen and Lingshu were mostly cited as evidence.In the elucidation of medical theory,MA's annotations differed from WANG's in the understanding of theoretical scope,the analysis of theoretical connotation and the determination of acupoint position.[Conclusion]MA Shi used the more comprehensive interpretation methods,interpreted the scriptures from the broader theoretical level and refuted WANG's annotations,which not only formed a systematic way of interpreting the scriptures,but also inspired more possibilities of the meaning of the scriptures.MA's annotations had both critical inheritance and innovative development to WANG's annotations,which promoted the study of Suwen.
10.Clinical effects of free perforator propeller myocutaneous flap from buttock in repairing complex wounds in the buttock with deep dead cavity

Objective:To investigate the feasibility and clinical effects of using free perforator propeller myocutaneous flap from buttock in repairing complex wounds in the buttock with deep dead cavity.Methods:A retrospective observational study was conducted. From June 2020 to June 2022, 9 patients with complex wounds in the buttock with deep dead cavity who met the inclusion criteria were admitted to Lanzhou University Second Hospital, including 6 males and 3 females, aged 26 to 62 years, with original wound area ranging from 4.0 cm×3.0 cm to 8.0 cm×7.0 cm and dead cavity depth of 7 to 11 cm. All the wounds were repaired with free perforator propeller myocutaneous flap from buttock, with flap area of 6.0 cm×2.5 cm to 13.0 cm×7.0 cm and muscle flap length of 6 to 11 cm. All the wounds in the donor area were closed and sutured directly. Postoperative myocutaneous flap survival, complications, as well as donor and recipient wound healing were observed, and the shape of donor and recipient areas were followed up.Results:Congestion occurred under the myocutaneous flap of one patient due to poor drainage on post surgery day 2, which was healed after 15 days of drainage and dressing change. The myocutaneous flaps of other patients survived successfully after surgery. The wounds in the donor and recipient areas were all well healed. During the follow-up of 3 to 10 months, the donor and recipient areas were full in shape, with little difference from the healthy side, and were able to bear pressure.Conclusions:The free perforator propeller myocutaneous flap from buttock can repair the deep dead cavity and surface wounds at the same time. The use of this myocutaneous flap in repairing complex wounds in the buttock with deep dead cavity results in minimal damage to the donor area, allows pressure-bearing of the donor and recipient areas after surgery, and ensures a full buttock shape.
