1.Evaluation of ZOPOMED-60 mycobacterium tuberculosis microscanner for detecting acid-fast bacilli in sputum smears

Objective To evaluate the effect of ZOPOMED-60 mycobacterium tuberculosis microscanner on detecting acid-fast bacilli in sputum smears.Methods A total of 203 qualified sputum specimens from patients treated in Chest Hospital of Guangxi Zhuang Autonomous Region in May 2023 were collected.The samples were stained by using Ziehl-Neelsen staining and fluorecent staining,respectively,and the acid-fast bacillus was detected by the manual microscopy and ZOPOMED-60 mycobacterium tuberculosis microscanner.The sensitivity,specificity,positive predictive value and negative predictive value of acid-fast bacillus were calculated using different methods based on liquid culture of mycobacterium.Results The positive rates of manual reading and instrument intelligent reading of fluorecent staining were 12.3%and 18.7%,respectively,and the positive rates of manual reading and instrument intelligent reading of Ziehl-Neelsen staining were 13.3%and 17.7%,respectively.There was no significant difference between the four reading results(P>0.05).Based on the results of liquid culture of mycobacterium,sensitivity of manual reading and instrument intelligent reading of fluorecent staining and manual reading and instrument intelligent reading of Ziehl-Neelsen staining were 40.0%,45.0%,45.0%and 53.3%,specificity were 99.3%,92.3%,100.0%and 97.2%,positive predictive values were 96.0%,71.0%,100.0%and 88.9%,and negative predictive values were 79.8%,80.0%,81.2%and 83.2%,respectively.The consistency was good.Conclusion The performance of ZOPOMED-60 mycobacterium tuberculosis microscanner in the detection of acid-fast bacilli by two staining methods is good,and it is worth popularizing.

The peripherally inserted central catheter (PICC) in neonates has solved the long-term venous access needs of hospitalized infants in the Neonatal Intensive Care Unit (NICU) . The application of technology is accompanied by the risk of complications. Providing safe and high-quality vascular access for neonates through standardized operations is of great significance. The National Association of Neonatal Nurses (NANN) has formulated the Practice Guideline for PICC in Neonates and revised it into the the Practice Guideline for Peripherally Inserted Central Catheter in Neonates (Third Edition) in 2015, aiming to support the bedside nursing practice of neonatal PICC, reduce the occurrence of complications, and promote neonatal safety. This article interprets the recommended points for early identification, management, and prevention of complications after neonatal PICC catheterization in the guidelines.
3.Analysis of the mediating effects of parental perceptions of child vulnerability and child anxiety between parental social support and quality of life in children with epilepsy

Objective:To explore the mediating effects of parental perceptions of child vulnerability (PPCV) and child anxiety in the relationship between parental social support and the quality of life in children with epilepsy.Methods:This was a cross-sectional study. Totally 315 children with epilepsy and their 315 parents who visited the neurologic outpatient clinic for video EEG monitoring from September 2021 to April 2022 were selected by convenience sampling and investigated with General Information Questionnaire, Child Vulnerability Scale (CVS), Social Support Rating Scale (SSRS), PROMIS Parents Report Anxiety Short Form, and the Quality of Life in Children with Epilepsy-16 (QOLCE-16). Pearson correlation was used to analyze the relationships between quality of life in children with epilepsy and variables such as parental social support, PPCV, and child anxiety. AMOS 22.0 was utilized to establish a structural equation model to examine the relationships among the variables. A total of 315 questionnaires were distributed, with 295 valid questionnaires retrieved, yielding a response rate of 93.7%.Results:The scores for the 295 parents on the SSRS, CVS, PROMIS Parents Report Anxiety Short Form, and QOLCE-16 were (40.75±10.24), (8.08±4.84), (16.40±5.98), and (72.01±15.43), respectively. Parental social support was negatively correlated with PPCV and child anxiety ( P<0.01), and both PPCV and child anxiety were negatively correlated with the child's quality of life ( P<0.01), while parental social support had a positive correlation with the child's quality of life ( P<0.01). PPCV and child anxiety mediated the relationship between parental social support and the child's quality of life ( P<0.01), with the direct and indirect effects being 0.301 and 0.205 respectively. Conclusions:Parental social support can directly affect the quality of life of children with epilepsy, and also indirectly influence it through PPCV and child anxiety. Clinical nursing staff should provide more care and support for children with epilepsy and their parents, helping to deepen their understanding of the disease, reduce levels of PPCV and child anxiety, thereby enhancing the quality of life for children with epilepsy.
4.Current status and influencing factors of health-related quality of life in children with epilepsy

Objective:To investigate the current status of health-related quality of life (HRQoL) in children with epilepsy using the Quality of Life in Childhood Epilepsy Questionnaire (QOLCE-16) and identify its influencing factors.Methods:Totally 289 epileptic children and their parents who visited the epilepsy center of Shanghai Children's Hospital from May 2021 to April 2022 were selected for the study by convenience sampling. Data were collected using general information surveys, epilepsy knowledge questionnaires, Social Support Rating Scale (SSRS) , and the QOLCE-16. A stepwise linear regression was performed to analyze the factors influencing the HRQoL of children with epilepsy. Out of 289 questionnaires distributed, 261 valid responses were received, yielding a valid response rate of 90.31% (261/289) .Results:The QOLCE-16 score for the 261 children with epilepsy was (71.05±14.04) . Stepwise linear regression revealed that single-parent family, presence of comorbidities, utilization of social support, and the accuracy rate of the epilepsy knowledge questionnaire were influencing factors for HRQoL in children with epilepsy.Conclusions:The HRQoL in children with epilepsy requires further enhancement. Clinical nursing processes should pay attention to the child's family situation, alleviate negative emotions, encourage active treatment cooperation, assist in social interaction, and provide family-centered health education to improve the quality of life for children with epilepsy.
5.Prevalence of osteoporosis and its influence factors in monitored population in Chongqing

Objective To understand the prevalence of osteoporosis and its influence factors in residents of Chongqing, and to provide the reference for osteoporosis prevention and treatment. Methods The data were from “An epidemiological survey of osteoporosis in Chongqing in 2018”, and the multi-stage stratified cluster random sampling method was used to select 1885 subjects (≥20 years old) from 32 villages/neighborhood committee in 4 areas (countries) of Chongqing. The investigation was performed by questionnaire (sociodemographic factors, lifestyle factors, history of orthopedics) and the bone mineral density was measured by QDR 4 500A dual energy X-ray absorptiometry. The chi-square test and multi factor multi-variant logistic regression analysis were utilized to analyze the data, the used software was SPSS 25.0. Results The morbidities of osteoporosis and osteopenia in residents (>20 years old) of Chongqing in China were 12.99% and 48.70%, respectively. The osteoporosis morbidity (19.08%) in females was significantly higher than that (4.42%) in males, and increased with age. The multi-variant logistic regression indicated that women (OR=6.10, 95% CI:4.08-9.14), the past medical history of fracture (OR=1.79, 95%CI:1.18-2.73) and age (OR=16.80, 95% CI:9.19-30.77) were risk factors for osteoporosis; milk intake (OR=0.65, 95%CI:0.47-0.89), overweight (OR=0.51, 95%CI:0.36-0.93)and obesity (OR=0.30, 95%CI:0.19-0.47), as compared with ≤primary school group, junior high school group (OR=0.57, 95%CI:0.39-0.82);≥high school group (0.40, 95%CI:0.26-0.61) were considered to be protective factors for osteoporosis. Conclusion The prevalence of osteoporosis among residents with age above 50 years old of Chongqing was very high. Health education and behavior intervention should be focused on the elderly or female residents to promote the formulation of healthy lifestyle.
6.The influence of scene dialogue on the experience of blood donors during blood donation

【Objective】 To explore the effect of repeated use of situational dialogues in the process of blood donation of voluntary blood donors. 【Methods】 From September to November 2019, blood donors in the center were investigated in the form of questionnaire, and the influencing factors of blood donation experience were analyzed by regression analysis.The direction of blood donation service should be improved according to the influencing factors.Compared with the survey data before and after the improvement, the effectiveness of the new blood donation service measures, centering on the situation dialogue, on improving blood donation experience was evaluated. 【Results】 A total of 304 and 187 valid questionnaires were returned from the two surveys. Regression analysis of the data from the first study revealed that the main factors affecting the blood donation experience were emotion, value, body and service. The regression coefficient for emotional experience was 2.587 (OR=13.292). Based on this evidence, improvements were implemented to the blood donation service, and the core strategy was to increase emotional communication with blood donors through multiple situational dialogues to achieve psychological support for blood donors. The results of the comparison of the data before and after the measure were as follows: emotional experience 3.490±0.879 vs. 3.754±0.771, value experience 3.461±0.957 vs. 3.722±0.854, and service experience 3.355±0.908 vs. 3.663±0.909; all were improved (F=9.365~23.025, P<0.01 for all). The control analysis illustrated that the improved orientation of the blood donation service was effective in improving the donor experience (χ2=4.776, P<0.05). 【Conclusion】 Increasing verbal situational dialogues with blood donors is an effective way for blood centers to improve donors′ donation experience, which helps donors to develop a positive evaluation of blood donation and leads to an increased intention to return.
7.Sinicization of the Quality of Life in Children with Epilepsy and its reliability and validity

Objective:To translate the 16-item version of Quality of Life in Children with Epilepsy (QOLCE-16) into Chinese, and conduct reliability and validity analysis in Chinese children with epilepsy.Methods:The International Functional Assessment of Chronic Illness Therapy (FACIT) translation method was adopted. Three bilingual translators pre-translated the source scale, and another three bilingual translators performed back translation. The QOLCE-16 was adjusted through the cognitive interview and expert consultation. This study was a cross-sectional study. Convenience sampling was used to select 256 children with epilepsy who were admitted to the Department of Neurology of Children's Hospital of Shanghai from June to December 2020. All questionnaires were filled out by long-term caregivers, and 256 copies were finally recovered effectively to analyze the reliability and validity of the QOLCE-16 after cultural adjustment in the evaluation of the health-related quality of life of children with epilepsy. A total of 40 subjects were selected from the sample, and the Chinese version of QOLCE-16 was filled out again at an interval of 2 to 3 weeks. Finally, 32 samples were effectively recovered to evaluate the test-retest reliability of the scale.Results:The Chinese version of QOLCE-16 had 16 items in total, including 4 dimensions of cognition, emotion, social interaction, and physical function. Each item had a good degree of discrimination, and there was a statistical correlation with the total score of the scale ( P<0.05) . The scale-level content validity index ( S- CVI) was 1.00, and the item-level content validity index ( I- CVI) was from 0.89 to 1.00. The KMO value of exploratory factor analysis was 0.909, the eigenvalues of each dimension were 3.482, 3.223, 3.136, 2.955 respectively, and the cumulative variance contribution rate was 79.976%. The Cronbach's α coefficient of the scale was 0.938, and the Cronbach's α coefficient of each dimension was from 0.866 to 0.939. The split-half reliability was 0.814, and the split-half reliability of each dimension ranged from 0.840 to 0.942. The test-retest reliability of the scale was 0.724, and the test-retest reliability of each dimension was from 0.615 to 0.716. Conclusions:The Chinese version of QOLCE-16 has good reliability and validity, and can be used as a simple tool for evaluating the health-related quality of life of children with epilepsy between 4 and 18 years of age.
8.Association between self-reported snoring and hypertension among Chinese Han population aged 30-79 in Chongqing, China.

BACKGROUND:
We aim to explore the association between self-reported snoring and hypertension among adults aged 30-79 in Chongqing, China.
METHODS:
A total of 23,342 individuals aged 30-79 were included at baseline from August 2018 to January 2019, and the final sample size for the analysis was 22,423. Face-to-face interviews and physical examinations were conducted by trained investigators. Logistic regression was performed to study age-specific and gender-specific associations between snoring and hypertension.
RESULTS:
Frequent snoring was associated with the risk of hypertension for each age and gender group, and the frequency of snoring was positively correlated with the risk for hypertension. For the three age groups (< 45, 45-59, ≥ 60), compared with the non-snoring group, those who snore often had a 64.5%, 53.3%, and 24.5% increased risk of hypertension (< 45: OR = 1.65, 95%CI 1.34-2.02; 45-59: OR = 1.53, 95%CI 1.37-1.72; ≥ 60: OR = 1.25, 95%CI 1.09-1.42), respectively. For men and women, those who snore often had a 46.8% and 97.2% increased risk of hypertension, respectively, than the non-snoring group (men: OR = 1.47, 95%CI 1.33-1.63; women: OR = 1.97, 95%CI 1.75-2.23).
CONCLUSIONS
People who snore frequently should pay close attention to their blood pressure levels in order to achieve early prevention of hypertension, particularly for snorers who are female and aged under 45; importance should be attached to their blood pressure control.

9.Preliminary study of risk management mode for investigator-initiated clinical trials

Objective To explore the risk management model of investigator-initiated clinical trials (IITs) from the prospective of clinical research management personnel,to provide further reference for its construction and implementation in China.Methods The risks in IITs and its current situation of risk management were analyzed.Risk management standards including ISO 31000 and related guidelines were adopted to explore the risk management mode for China-based IIT.Results This article proposed the new risk management mode for IITs and also introduced the specified frame and procedures of related risk management.Conclusions The risk management model proposed in this article provided reference for clinical research management.
10.Problem exploration and measures on the investigator initiated trial from the perspective of scientific research supervision

Objective By analyzing the problems existed in the Investigator Initiated Trial (IIT),this article put forward the corresponding countermeasures and therefore provides reference for the standardization of clinical research project management.Methods Four types of problems identified in the supervision of hospital IIT projects are analyzed according to literature review,data analysis of clinical research project,comparative study and summary.Identified problems are existed in the following aspects:scientific research supervision function,research method guidelines,technical specification of the diagnosis and treatment,scientific research design and project approval review,research funds,medical ethics committee,construction of Biobank and Regulation Conflicts.Results This article put forward 6 countermeasures for improvement:establishing and perfect IIT project scientific research supervision entity,bring in the IIT project steering group to strengthen the scientific review;Strengthen risk management to ensure medical safety,carry out IIT training,establish IIT management database information system,build a comprehensive integrated development multi-point application model of hospital BioBank.Conclusions The establishment of the hospital's IIT scientific research supervision system,management mode and technical standard system is of great importance to standardize clinical research,ensure research quality and guide the clinical research work of the hospital effectively.
