Endo-periodontal lesions(EPLs)involve both the periodontium and pulp tissue and have complicated etiologies and pathogenic mechanisms,including unique anatomical and microbiological characteristics and multiple contributing factors.This etiological complexity leads to difficulties in determining patient prognosis,posing great challenges in clinical practice.Furthermore,EPL-affected teeth require multidisciplinary therapy,including periodontal therapy,endodontic therapy and others,but there is still much debate about the appropriate timing of periodontal therapy and root canal therapy.By compiling the most recent findings on the etiology,pathogenesis,clinical characteristics,diagnosis,therapy,and prognosis of EPL-affected teeth,this consensus sought to support clinicians in making the best possible treatment decisions based on both biological and clinical evidence.
4.Analysis of the positive rate of scanning laser ophthalmoscope in the retromode for different types of diabetic macular edema

Objective:To explore the positive rate of scanning laser ophthalmoscope in the retromode (RM-SLO) in different types of diabetic macular edema (DME), and to analyze its correlation with foveal thickness (CMT) and macular volume.Methods:From March to May 2021, 40 patients (65 eyes) were diagnosed as DME by fundus examination combined with optical coherence tomography (OCT) in Affiliated Eye Hospital of Wenzhou Medical University at Hangzhou were included in the study. All eyes underwent best corrected visual acuity (BCVA), OCT and RM-SLO fundus imaging examinations, 47 eyes underwent fluorescein fundus angiography (FFA) examination. RM-SLO fundus imaging examinations were performed with Mirante SLO, including retro mode illumination deviated right (RMDR) and retro mode illumination deviated left (RMDL). If one or more of the RMDR and RMDL of the examined patient can identify macular edema, RM-SLO was considered to be able to identify macular edema. The macular volume at CMT and 6 mm from the fovea was measured by OCT software. DME were divided into 3 types based on OCT images: diffuse retinal thinkening (DRT) type; cystoid macular edema(CME) type; serous retinal detachment (SRD) type, focal leakage type, diffuse leakage type and diffuse cystic leakage type. The consistency of RMDR and RMDL in the diagnosis of DME in RM-SLO fundus imaging was evaluated, as well as their positive rate in different classifications of DME. The correlation between the detection of macular edema by RM-SLO and the DME type, CMT and foveal volume, and the correlation between BCVA and edema type, CMT and macular volume were analyzed.Results:Among 65 eyes, the positive rates of RMDR and RMDL fundus imaging to detect DME were 46 (70.77%, 46/65) and 48 (73.85%, 48/65), respectively. There was good consistency in identifying DME (Kappa value=0.770; P<0.001). The positive rates of RMDR and RMDL fundus imaging DRT, CME and SRD type of DME were 42.11% (8/19), 57.89% (11/19), 77.78% (28/36), 77.78% (28/36), 100.00% (10/10), 90.00% (9/10), respectively. In the FFA classification of them, the positive rates of focal leakage, diffuse leakage and diffuse cystic leakage were 68.75% (11/16), 62.50% (10/16), 68.00% (17/25), 76.00% (19/25), 100.00% (6/6), 100.00% (6/6), respectively. The results of Spearman correlation analysis showed that whether RM-SLO could identify DME was associated with CMT and OCT classification ( r=0.310, 0.365; P=0.120, 0.003); there was no correlation between FFA classification and macular volume ( r=0.113, 0.117; P=0.449, 0.352). BCVA was correlated with CMT and macular volume ( r=0.307, 0.269; P=0.013, 0.030), however, there was no significant correlation with OCT type, angiographic type ( r=0.051, 0.175; P=0.684, 0.240). Conclusion:The diagnostic agreement of DME are good between RMDR, RMDL of RM-SLO image. DME of DRT type and patients with smaller CMT in OCT are difficult to identified by RM-SLO fundus imaging.
5.Analysis on 4 cases of polycythemia vera transforming to acute myeloid leukemia

Objective To explore the clinical characteristics,diagnosis and treatment of polycythemia vera(PV) transforming to acute myeloid leukemia(AML).Methods The clinical features and process of diagnosis and treatment in 4 cases of PV transforming to AML were analyzed.Results The case 1 had 10 years history of PV,after experiencing PV,had myelofibrosis and transformed to AML at the end stage of natural disease course;the case 2 had 7 years history of PV,orally took hydroxyurea(HU) treatment in recent 2 years and transformed to AML at present,his chromosome karyotype analysis showed 46,XY,del(7)(q31q36),del (18) (q22)[10],which was considered as treatment-related AML;the case 3 and 4 orally took H U for a long time after diagnosing HU,and respectively turned into AML during the pathologic polyemia stage after 6 and 7 years.Conclusion PV can be transformed to AML,the safety of HU in treatment should by paid attention to.
6.Effects of Zhige Oral Liquid on Lipid Metabolism in Rats with Alcoholic Liver Disease

Objective: To observe the effects of Zhige oral liquid on lipid metabolism in rats with alcoholic liver disease.Methods: Alcoholic liver disease rats were induced by alcoholic gavage plus normal diet. The intervention group was given different doses of Zhige oral liquid at the same time. The control group was given alcohol and oral administration of Jiejiuling oral liquid. After 12 weeks, the rats were sacrificed and serum ALT, AST, TC, TG and liver TC and TG levels were measured. Results: Compared with the normal group, the liver index, serum ALT, AST, TC, TG and liver TC and TG levels were significantly higher in the groups except the high dose group (P < 0.01 or 0.05), among which the model group and the low dose group had the most significant increase (P < 0.01) . Compared with the model group, the above indicators were decreased in the high, medium and low dose groups and the control group, and the decreasing trend was the same.That is, the high dose of the Zhige oral liquid was the most significant (P < 0.01), followed by the middle dose group and the control group (P < 0.05) . The decrease in the low dose group was the least significant. Compared with the control group, there were significant differences in the high-dose groups (P < 0.05) . Conclusion: A certain dose of Zhige oral liquid can inhibit or reduce the occurrence of alcoholic liver disease by improving lipid metabolism disorder.
7.Biofilm development and environmental determinants in Vibrio cholerae.

Biofilm associated Vibrio cholerae exhibits hypervirulence and supreme fitness against the harsh stresses during its infectious cycle. It is important to study the relationships between the regulation mechanism of V. cholerae biofilm development and its environmental adaption in host niche and aquatic habitat. Here, we summarize the recent advances in V. cholerae biofilm, including biofilm compositions, development and regulation. Particularly, we extensively discuss how V. cholerae fosters its biofilm architecture and assembly via sensing and responding various environmental determinants, such as bacterium self-produced molecules, natural environment components and host factors.
8.Effects of Risperidone on Brain Derived Neurotrophic Factor in First Episode Schizophrenic Patients

Objective To observe the effect of risperidone on serum brain derived neurotrophic factor ( BDNF) in first-episode schizophrenic patients. Methods In the treatment group, 90 first-episode schizophrenics were treated with risperidone for 16 weeks, and the dose of risperidone was (3.79±0.88) mg·d-1. Serum BDNF levels were measured before treatment, at 2, 8, and 16 weeks after treatment. The severity of schizophrenia symptoms was assessed by the positive and negative symptom scale ( PANSS) before and after sixteen weeks of treatment. Serum BDNF concentrations were measured in 90 healthy controls. Results BDNF in the control group was (22.867±6.051) ng·mL-1. The serum BDNF levels before treatment and at the end of week 2, 8, 16 after the treatment in the treatment group were (14.256±4.096), (13.078±3.462), (18.001±5.753), (21.089± 6.692) ng·mL-1 , and the serum BDNF level was significantly lower in the treatment group than that in the control group ( P<0.01) . After the treatment, the level of BDNF in the treatment group decreased at first and then increased, compared with that before treatment, the difference was significantly ( P<0.05 or P<0.01) . The level of BDNF in the treatment group at the end of week 16 was not significantly different from that of the control group ( P>0.05) . After treatment, the total score of PANSS scale and its subscales decreased, the difference was significantly (P<0.01). At the end of week 16, the PANSS subscale reduction rate was positively correlated to the serum BDNF concentration change (r=0.499, P=0.001). The change rate of serum BDNF concentration at the end of week 16 was not correlated with the dose of risperidone (r=0.103, P=0.335). Conclusion BDNF is abnormal in the first episode of schizophrenia, which can be improved by risperidone treatment.
9.Associations of left atrial enlargement and the severity and lesion patterns of cardioembolic stroke in patients with nonvalvular atrial fibrillation

Objective To investigate the associations of left atrial enlargement and the severity and lesion patterns of cardioembolic stroke in patients with nonvalvular atrial fibrillation (NVAF).Methods The patients with NVAF diagnosed as cardiogenic stroke within 48 h after onset were enrolled.The National Institutes of Health Stroke Scale (NIHSS) was used to evaluate the severity of stroke,and ≥ 10 was defined as moderate to severe stroke,and < 10 was defined as mild stroke.Transthoracic echocardiography was used to measure the left atrial diameter.The patients were divided into normal group,mild,moderate,and severe left atrial enlargement groups.According to the lesion patterns revealed by the diffusion weighted imaging,they were divided into either a multiple infarctions group or a single infarction group.Results A total of 137 patients were enrolled,including 86 patients with mild stroke (62.8%),51 with moderate to severe stroke (37.2%),69 with multiple infarctions,and 68 with single infarction (49.6%).Left atrial enlargement was found in 103 cases,in which 35 (25.5%) were mild enlargement,40 (29.2%) were moderate enlargement,and 28 (20.4%) were severe enlargement.Multivariate logistic regression analysis showed that the risk of severe stroke in patients with mild (odds ratio [OR] 15.662,95% confidence interval [CI] 1.821-134.677;P =0.012),moderate (OR 26.435,95% CI 3.201-218.319;P =0.002),and severe (OR 75.763,95% CI 8.536-672.441;P<0.001) left atrial enlargement were increased significantly,and there were no significant differences in the lesion patterns among the different severity of left atrial enlargement groups (x2 =7.050,P =0.07).Conclusions Left atrial enlargement may be independently associated with the severity of cardiogenic stroke in patients with NVAF,but it is not associated with the lesion patterns.
10.Root canal morphology and root variation of the mandibular first premolar: A study by cone-beam computed tomography

Objective Few studies are reported on the radicular groove and root canal variation of the mandibular first premolar.This study aimed to observe the root canal system and root variation of the mandibular first premolar by cone-beam computed tomography (CBCT).Methods We selected CBCT images of 643 mandibular first premolars of 336 patients and subjected the data obtained to statistical analysis.Results Of the 643 mandibular first premolars, 637 (99.1%) had a single root and the other 6 had double roots;502 (78.1%) had a single root canal, 133 (20.7%) had two, 5 (0.78%) had three, and 3 (0.47%) presented C-shaped root canal morphology.The average length of the 643 mandibular first premolars was (13.2±1.5) mm and the mean distance from the apical foramen to the anatomical apical end of the root was (0.93±0.35) mm.Totally, 123 (19.1%) of the mandibular first premolars had a radicular groove, of which 94 (76.4%) had double root canals and the root canal systems included typesⅠ in 24 (19.5%), Ⅱ in 2 (1.6%), Ⅲ in 19 (15.4%), Ⅳ in 1 (0.8%), Ⅴ in 69 (56.1%), and Ⅶ in 3 (2.4%).Conclusion The root canal system of the mandibular first premolar has a significant variation.CBCT can reveal the complex anatomical structure of the mandibular first premolar and provide reliable evidence for the root canal treatment.
