1.Transumbilical Single-port Laparoscopic Ovarian Cyst Excision With Protection of Ovarian Function

Objective To investigate the clinical application value of transumbilical single-port laparoscopic ovarian cyst excision with protection of ovarian function.Methods From July 2018 to December 2019,we performed 56 cases of single-port laparoscopic ovarian cyst debulking,in which the umbilicus was incised transversely for about 2 cm and a single-port trocar puncture was placed to form an artificial pneumoperitoneum.The ultrasonic knife was used to avoid the ovarian hilum and cut the ovarian cortex on the opposite side of the ovary to ensure a good blood supply.The ovary on the affected side was fixed to facilitate blunt separation of the cyst and preserve the normal tissues of the ovary as much as possible.The 2-0 absorbable suture was applied for hemostasis and ovarian reconstruction.Results The operations were successfully completed in all the 56 cases,including bilateral ovarian cyst excision in 3 cases,salpingectomy in 3 cases,tubal mesangial cyst excision in 1 case,hysteromyomectomy in 2 cases,appendectomy in 1 case(an additional puncture hole was added due to the need for abdominal drainage after the operation),hysteroscopic resection of endometrial polyps in 1 case,and pelvic adhesion release in 1 case.The operation time was(72.0±30.0)min,and the postoperative hospitalization time was(5.0±0.6)d.No complication occurred.The 56 patients were followed up for 2-3 years,with an average of 1.5 years,with no complications,poor wound healing,or recurrence of ovarian cysts.There was no significant difference in the number of ovarian antral follicles between 6 and 12 months after surgery and before surgery[7.02±1.57 vs.7.05±1.55,P = 1.000;6.93±1.46 vs.7.05±1.55,P =1.000].There was no significant difference in ovarian stromal blood flow(P>0.05),indicating no decrease in ovarian function.Conclusion The key to protecting ovarian function is the rational selection of surgical instruments and incisions,precise performance of resection of ovarian cysts,and protection of ovarian blood supply.
2.Impact of pembrolizumab combined with chemotherapy on angiogenesis and circulating endothelial cells in patients with advanced non-small cell lung cancer

Objective:To explore the impact of pembrolizumab combined with chemotherapy on angiogenesis and circulating endothelial cells in patients with advanced non-small cell lung cancer (NSCLC) .Methods:The retrospective analysis of clinical data from 121 patients diagnosed with advanced NSCLC who were admitted to the Second Affiliated Hospital of Xingtai Medical College from August 2021 to January 2023 was conducted. These patients were divided into a control group ( n=57) and an observation group ( n=64) based on the designated treatment protocol. Specifically, individuals in the control group received standard chemotherapy (cisplatin+paclitaxel), while those in the observation group underwent penpilimab therapy in conjunction with conventional chemotherapy. The comparative assessment encompassed short-term clinical efficacy, quality of life, immune function parameters, angiogenic factors [including endostatin, insulin-like growth factor 1 (IGF-1), and vascular endothelial growth factor (VEGF) ], circulating endothelial cells, and adverse reactions within the two groups. Results:After 6 courses of treatment, the objective response rate [67.19% (43/64) vs. 49.12% (28/57) ] and disease control rate [87.50% (56/64) vs. 70.18% (40/57) ] in observation group were higher than those in control group, with statistically significant differences ( χ2=4.06, P=0.044; χ2=5.52, P=0.019). The quality of life score of observation group [ (56.77±6.81) points] was significantly higher than that of control group [ (47.73±8.23) points], with a statistically significant difference ( t=6.61, P<0.001) ; The T cell subgroup CD3 + levels [ (63.59±9.00) % vs. (53.06±8.80%), t=6.49, P<0.001], CD4 + levels [ (46.54±8.20) % vs. (30.74±7.32) %, t=11.13, P<0.001] and CD4 +/CD8 + ratio (1.90±0.36 vs. 1.21±0.28, t=11.66, P<0.001) in observation group were significantly higher than those in control group, with statistically significant differences; Endostatin in observation group [ (48.99±3.43) μmol/L] was significantly higher than that in control group [ (31.35±3.87) μmol/L], with a statistically significant difference ( t=26.58, P<0.001), IGF-1 [ (102.31±20.35) μg/L vs. (134.98±19.02) μg/L] and VEGF [ (31.70±4.32) pg/ml vs. (58.71±5.99) pg/ml] were significantly lower in observation group than those in control group, with statistically significant differences ( t=18.73, P<0.001; t=28.14, P<0.001). The number of circulating endothelial cells in observation group [ (58.77±10.03) /ml] was significantly lower than that in control group [ (87.01±8.01) /ml], with a statistically significant difference ( t=17.20, P<0.001). During treatment, there were no statistically significant differences in the incidence of gastrointestinal reaction ( χ2=0.01, P=0.908), leukopenia ( χ2=0.64, P=0.424), thrombocytopenia ( χ2=0.28, P=0.597), anemia ( χ2=1.66, P=0.197), nephrotoxicity ( χ2=0.64, P=0.424), skin rash ( χ2=1.33, P=0.249) between the two groups. Conclusion:The combination therapy of pembrolizumab and chemotherapy for the treatment of advanced NSCLC has demonstrated noteworthy effectiveness. This regimen has the potential to enhance patients' immune functionality, ameliorate their overall quality of life, suppress angiogenesis, and exhibits a commendable profile of safety and reliability.

Traumatic supraorbital fissure syndrome (TSOFS) is a symptom complex caused by nerve entrapment in the supraorbital fissure after skull base trauma. If the compressed cranial nerve in the supraorbital fissure is not decompressed surgically, ptosis, diplopia and eye movement disorder may exist for a long time and seriously affect the patients′ quality of life. Since its overall incidence is not high, it is not familiarized with the majority of neurosurgeons and some TSOFS may be complicated with skull base vascular injury. If the supraorbital fissure surgery is performed without treatment of vascular injury, it may cause massive hemorrhage, and disability and even life-threatening in severe cases. At present, there is no consensus or guideline on the diagnosis and treatment of TSOFS that can be referred to both domestically and internationally. To improve the understanding of TSOFS among clinical physicians and establish standardized diagnosis and treatment plans, the Skull Base Trauma Group of the Neurorepair Professional Committee of the Chinese Medical Doctor Association, Neurotrauma Group of the Neurosurgery Branch of the Chinese Medical Association, Neurotrauma Group of the Traumatology Branch of the Chinese Medical Association, and Editorial Committee of Chinese Journal of Trauma organized relevant experts to formulate Chinese expert consensus on the diagnosis and treatment of traumatic supraorbital fissure syndrome ( version 2024) based on evidence of evidence-based medicine and clinical experience of diagnosis and treatment. This consensus puts forward 12 recommendations on the diagnosis, classification, treatment, efficacy evaluation and follow-up of TSOFS, aiming to provide references for neurosurgeons from hospitals of all levels to standardize the diagnosis and treatment of TSOFS.
4.Research on the clinical application effect of transcranial ultrasound patch probe

Objective:To evaluate the performance of the self-developed transcranial ultrasound patch probe as well as its stability and safety in clinical applications.Methods:This study was a cross-sectional study. Fifty healthy subjects in Tangdu Hospital from October to December 2023 were randomly and equally divided into two groups using a simple randomization method. The transcranial Doppler ultrasound (TCD) examinations were performed using the self-developed transcranial ultrasound patch probe and normal TCD probe alternatively in each group from squatting to sitting and then to standing posture. Two experienced sonographers (A, B) each completed one test for the same subject using the above two different probes. The differences of the ultrasound parameters of middle cerebral artery (MCA) blood flow measured by the self-developed transcranial ultrasound patch probe and the normal TCD probe in different postures (squatting, sitting, standing) were compared. At the same time, operator satisfaction with the use of the two probes was assessed in terms of time taken to obtain satisfactory images, operational comfort, and ease of handling. In addition, the subject's comfort during the examination was examined, and the signal acquisition stability of the two probes was comprehensively evaluated.Results:There was no significant difference in detection of various ultrasound parameters of MCA between using the transcranial patch probe and the normal TCD probe (all P>0.05). The overall satisfaction of transcranial ultrasound patch probe and normal TCD probe was quite similar. The transcranial ultrasound patch probe acquired favorable and stable signals. No subjects experienced adverse events/accidents during the examination of the subjects using the transcranial ultrasound patch probe as well as the normal TCD probe.Both probes had good and stable signal acquisition. Conclusions:TCD examinations could be well performed using the transcranial ultrasound patch probe, which might provide a potential new method for real-time monitoring and evaluating of cerebral blood flow of a moving subject.
5.Analysis of language and influencing factors of children with speech disorder in Beijing

Objective:To investigate the features and influencing factors of language in children with various types of speech disorders.Methods:A case-control study was carried out, 262 children with speech disorder had been diagnosed at the language-speech clinic of the Center of Children′s Healthcare, Children′s Hospital, Capital Institute of Pediatrics from January 2021 to November 2023, the children with speech sound disorder as the speech sound disorder group, the children with developmental stuttering as the stuttering group. There were 100 typically-developed children who underwent physical checkups at the Center of Healthcare during the same period as the healthy group. All children experienced a standardized evaluation of language with diagnostic receptive and expressive assessment of mandarin‐comprehensive(DREAM-C) and questionnaire, One-way ANOVA and LSD test were conducted to compare the differences in overall language, receptive language, expressive language, semantics, and syntax scores among 3 groups of children. According to the results of DREAM-C, the children with speech disorder were divided into language normal group and language delay group. Chi‐square test and multivariate Logistic regression were implemented to analyze the association between the linguistic development of children with speech disorder and potential influential factors.Results:There were 145 children in the speech sound disorder group, including 110 males and 35 females respectively, with an age of (5.9±1.0) years; 117 children in the stuttering group, including 91 males and 26 females, with an age of (5.8±1.0) years; 100 children in the healthy group, including 75 males and 25 females, with an age of (5.7±1.2) years. The variations in overall language, expressive language, and syntax scores among 3 groups of children were statistically significant (92±18 vs.96±11 vs. 98±11, 81±18 vs. 84±14 vs. 88±13, 87±16 vs. 89±11 vs. 91±10, F=5.46, 4.69, 3.68, all P<0.05). Pairwise comparison revealed that the speech sound disorder group had lower scores in overall language, expressive language, and syntactic compared to the healthy group, and the differences were statistically significant (all P<0.01) and the overall language score was lower than that of children with stuttering ( P<0.05). In terms of overall language and expressive language, there was a statistically significant difference in the incidence of language delay among the three groups of children (15.9% (23/145) vs. 20.5% (24/117) vs. 7.0% (7/100), 46.2% (67/145) vs. 39.3% (46/117) vs. 26.0% (26/100); χ2=7.93, 10.28; both P<0.05). In terms of overall language, the stuttering group took up the highest proportion. In terms of expressive language, the speech sound disorder group accounted for the highest amount. The incidence of language delay in children with speech disorder was 44.3% (116/262). Non-parent-child reading, daily screen time ≥1 hour and screen exposure before 1.5 years of age are risk factors for the development of language in children with speech disorder ( OR=1.87, 2.18, 2.01; 95% CI 1.07-3.27, 1.23-3.86, 1.17-3.45; all P<0.01). Negative family history are protective factors for the progress of language ability ( OR=0.37, 95% CI 0.17-0.81, P<0.05). Conclusions:Children with speech disorder tend to have easy access to language delay, especially in expressive language and syntax. The occurrence of language delay in children with speech disorder is tightly connected with factors such as the family medical history, parent-child reading, screen time, etc. Attention should be paid to the development of language in children who suffer from speech disorder.
6.Analysis of VSIG4 expression in clear cell renal cell carcinoma and its correlation with tumor infiltrating immune cells

Objective:To explore expression of VSIG4 in clear cell renal cell carcinoma(ccRCC)and its correlation with immune cells infiltration and prognosis.Methods:RNA-seq data of ccRCC and corresponding clinical data of patients were downloaded from The Cancer Genome Atlas(TCGA)database.Wilcoxon rank-sum test was used to analyze VSIG4 mRNA expression in ccRCC and normal renal tissues.Correlation between VSIG4 expression and clinicopathological parameters was analyzed by Wilcoxon rank-sum or Kruskal-Wallis rank-sum test.Kaplan-Meier method was used to analyze the correlation between VSIG4 expression and progno-sis of patients.Gene set enrichment analysis(GSEA)was used to explore the signaling pathway of VSIG4 in ccRCC.Correlation between VSIG4 level and tumor infiltrating immune cells was analyzed via CIBERSORT in R software.Western blotting was used to detect VSIG4 protein level in human renal epithelial cell line 293T,and human renal carcinoma cell line ACHN,A498,786-O and OS-RC-2.Results:Expression level of VSIG4 in ccRCC was significantly higher than that in normal tissues(P<0.05).VSIG4 expression was significantly correlated with the stages of distant metastasis and lymph node metastasis(P<0.05).Overall survival rate of patients with high VSIG4 expression was significantly lower than that of patients with low expression(P<0.05).GSEA enrichment analysis showed that VSIG4 was mainly enriched in apoptosis,chemokine signaling pathway,cell adhesion molecules and other signaling path-ways.VSIG4 expression was negatively correlated with M1 macrophages(r<0,P<0.05),while positively correlated with M2 macro-phages(r>0,P<0.05).Western blotting results showed that expression of VSIG4 in renal cell carcinoma cells was higher than that in normal renal cells.Conclusion:VSIG4 is highly expressed in ccRCC,and is negatively associated with prognosis,which may become a prognostic biomarker for ccRCC patients.
7.The predictive value of serum sorting protein combined with extracellular matrix metalloproteinase inducer for in stent restenosis after carotid stent placement surgery

Objective:To explore the predictive value of serum sorting protein (Sortilin) combined with extracellular matrix metalloproteinase inducible factor (EMMPRIN) for in stent restenosis (ISR) after carotid artery stent placement (CAS).Methods:A total of 197 patients with carotid artery stenosis who underwent CAS treatment at the Capital Medical University Daxing Teaching Hospital from January 2018 to October 2020 were selected as the study subjects. All patients were followed up for 24 months after surgery and were divided into an ISR group (28 cases) and a non ISR group (169 cases) based on the occurrence of ISR. Enzyme linked immunosorbent assay (ELISA) was used to detect and compare the preoperative serum Sortilin and EMMPRIN levels between the two groups. We collected clinical data from patients and analyzed the influencing factors of post CAS ISR using univariate and multivariate logistic regression models. The receiver operating characteristic (ROC) curve was used to analyze the predictive value of serum Sortilin and EMMPRIN for postoperative ISR in CAS.Results:The serum Sortilin, EMMPRIN, age, proportion of concomitant hypertension, degree of preoperative stenosis, neutrophil count (NEUT), platelet count (PLT), white blood cell count (WBC), high-sensitivity C-reactive protein (hs-CRP), triglycerides (TG), and total cholesterol (TC) in the ISR group were higher than those in the non ISR group (all P<0.05). The results of multivariate logistic regression analysis showed that PLT, WBC, and elevated serum Sortilin and EMMPRIN were independent risk factors for the occurrence of ISR after CAS surgery (all P<0.05). ROC curve analysis showed that the AUC under the ROC curve of serum Sortilin and EMMPRIN alone and in combination for predicting postoperative ISR in CAS were 0.735(95% CI: 0.546-0.918), 0.766(95% CI: 0.615-0.910), and 0.839(95% CI: 0.701-0.984), respectively. The predictive efficacy of the combined application was greater than that of the two indicators alone. Conclusions:Patients with postoperative ISR after CAS have abnormally elevated levels of serum Sortilin and EMMPRIN before surgery, which are independent risk factors for postoperative ISR. The combined detection of serum Sortilin and EMMPRIN has good predictive value for the risk of postoperative ISR in CAS.
8.Artery anastomosis only in reconstruction of digit-tip defects with fibular great toe flap: a report of 8 cases

Objective:To explore the clinical effect of free fibular great toe flap in reconstruction of digit-tip defect with only anastomosis of artery.Methods:From February 2022 to January 2023, a total of 8 patients with digit-tip defects received reconstruction surgery using free flap of fibular great toe with anastomosis of artery only in the Department of Hand Surgery, Shunde Heping Surgical Hospital. The patients were 5 males and 3 females, with an average age of 33 (14-55) years old. Two defected digit-tips were of thumbs, 3 of index fingers, 2 of middle fingers and 1 of little finger. The digit-tip defects were 1.5 cm×0.5 cm-2.0 cm×1.2 cm in size. During the surgery, a perforator flap of fibular great toe with fibular plantar digital artery and a branch of digital nerve was harvested. And the artery and nerve carried by the flap were anastomosed to the digital artery arch and nerve of the recipient site. No reflux vein was included in the free flap due to the small size of flap. The donor sites were directly closed. Postoperative anti-infection, anti-coagulation, and anti-vasospasm were offered. Scheduled postoperative follow-ups were conducted at outpatient clinics after surgery.Results:All flaps survived smoothly without any vascular compromise. All patients were included in the follow-up with an average of 7.1 (5-13) months. At the final follow-up, the appearances of the digit-tips were full and beautiful, with recovered finger print and good stability of flaps, as well as normal sweating. All flaps had good sensation recovery, with TPD at 6.0 mm to 7.5 mm and at 6.7 mm in average. There was no significant difference in skin temperature between the donor sites and the healthy sides. According to the Evaluation Trial Standards of Upper Limb Partial Functional of Hand Surgery of Chinese Medical Association, 7 patients were rated as excellent and 1 as good.Conclusion:The clinical effect is excellent for the free flap of fibular great toe with anastomosis of artery only in the reconstruction of digit-tip defect. It not only has satisfactory reconstruction but also restores the appearance and function of the flap donor site. Worth promoting and applying in clinical practice.
9.Supplementation of Clostridium butyricum Alleviates Vascular Inflammation in Diabetic Mice

Background:
Gut microbiota is closely related to the occurrence and development of diabetes and affects the prognosis of diabetic complications, and the underlying mechanisms are only partially understood. We aimed to explore the possible link between the gut microbiota and vascular inflammation of diabetic mice.
Methods:
The db/db diabetic and wild-type (WT) mice were used in this study. We profiled gut microbiota and examined the and vascular function in both db/db group and WT group. Gut microbiota was analyzed by 16s rRNA sequencing. Vascular function was examined by ultrasonographic hemodynamics and histological staining. Clostridium butyricum (CB) was orally administered to diabetic mice by intragastric gavage every 2 days for 2 consecutive months. Reactive oxygen species (ROS) and expression of nuclear factor erythroid-derived 2-related factor 2 (Nrf2) and heme oxygenase-1 (HO-1) were detected by fluorescence microscopy. The mRNA expression of inflammatory cytokines was tested by quantitative polymerase chain reaction.
Results:
Compared with WT mice, CB abundance was significantly decreased in the gut of db/db mice, together with compromised vascular function and activated inflammation in the arterial tissue. Meanwhile, ROS in the vascular tissue of db/db mice was also significantly increased. Oral administration of CB restored the protective microbiota, and protected the vascular function in the db/db mice via activating the Nrf2/HO-1 pathway.
Conclusion
This study identified the potential link between decreased CB abundance in gut microbiota and vascular inflammation in diabetes. Therapeutic delivery of CB by gut transplantation alleviates the vascular lesions of diabetes mellitus by activating the Nrf2/HO-1 pathway.

Objective To investigate the relationship between serum neutrophil elastase(NE),Sortilin and carotid atherosclerotic(CAS)plaque in patients with acute cerebral infarction(ACI)and its predictive value for poor prognosis.Methods A total of 155 patients with ACI admitted to Daxing Teaching Hospital of Cap-ital Medical University from December 2020 to November 2022 were selected as the study objects.According to carotid intima-media thickness(IMT),they were divided into normal IMT group,thickened group and plaque group,and 40 healthy subjects were selected as control group during the same period.The serum levels of NE and Sortilin in all groups were compared and their correlations with IMT were analyzed.ACI patients were followed up for 3 months and divided into good prognosis group and poor prognosis group.Serum NE and Sortilin levels were compared between the two groups.Multivariate Logistic regression was used to ana-lyze the risk factors of poor prognosis in ACI patients.Receiver operating characteristic(ROC)curve was used to predict prognosis.Results The levels of serum NE and Sortilin in control group,IMT normal group,thick-ened group and plaque group were increased successively(P<0.05).Pearson correlation analysis showed that serum NE and Sortilin levels were positively correlated with IMT(r=0.509,0.483,P<0.05).The incidence of poor prognosis in ACI patients was 39.35%.The proportion of diabetes mellitus,age,admission National Institutes of Health Stroke Scale(NIHSS)score,low density lipoprotein cholesterol,total cholesterol,fasting blood glucose,white blood cell count,serum creatinine,NE,uric acid(UA)and Sortilin levels in poor progno-sis group were higher than those in good prognosis group(P<0.05).Multivariate Logistic regression analysis showed that admission NIHSS score and UA,NE and Sortilin levels were independent risk factors for poor prognosis in ACI patients(P<0.05).ROC curve analysis showed that the area under the curve of serum NE,Sortilin and UA alone and combined to predict poor prognosis were 0.695,0.740,0.752 and 0.869,respective-ly.The combined prediction efficiency of serum NE,Sortilin and UA was higher than that of single detection of each indicator.Conclusion The increase of serum NE and Sortilin levels in ACI patients is positively corre-lated with CAS,and is an independent risk factor for poor prognosis in ACI patients.Detection of serum NE and Sortilin levels could help predict the short-term prognosis of ACI patients.
