1.Classification and influencing factors of family resilience and post-traumatic growth in spinal tumor patients based on potential profile analysis

Objective:To explore the classification and influencing factors of family resilience and post-traumatic growth in patients with spinal tumor.Methods:A cross-sectional investigation was conducted among 219 inpatients with spinal tumor admitted from July 2021 to July 2022. The General Demographic Information questionnaire, Chinese-Family Resilience Assessment Scale, Posttraumatic Growth Inventory, Family Crisis-Oriented Personal Evaluation Scales (F-COPES), and Social Support Rating Scale (SSRS) were used in the study. The ordinal and multivariate logistic regression analyses was applied to identify the factors associated with the classification of family resilience and post-traumatic growth.Results:Of the 219 patients, there were 62 cases of primary spinal tumors (28.3%). According to the results of latent profile analysis, the respondents were classified into three categories by family resilience and post-traumatic growth, namely family difficulty-resistant type ( n=38, 17.4%), general resilience-struggle type ( n=99, 45.2%) and family adaptation-growth type ( n=82, 37.4%). There were significant differences in occupational status, commitment to housework, family atmosphere( χ2=10.75, P=0.025; χ2=6.95, P=0.031; χ2=11.37, P=0.017), and total score of F-COPES and SSRS ( F=25.95, P<0.001; F=19.06, P<0.001)among three groups. Ordinal and multivariate logisitc regression analyses showed that retirement ( OR=2.928, 95% CI:1.098-7.808, P<0.05), family coping ( OR=1.113, 95% CI:1.063-1.165, P<0.05), and social support ( OR=1.226, 95% CI:1.103-1.362, P<0.05) were independently associated with family resilience and post-traumatic growth in patients with spinal tumor. Conclusion:Patients with spinal tumor have significant differences in characteristics by family resilience and post-traumatic growth. As a result, more targeted interventions should be provided for different categories of spinal tumor patients in the future.
2.Construction and application of a spinal cord injury rehabilitation care platform based on patient portrait

Objective To construct a rehabilitation care platform for spinal cord injury patients,based on patient portrait technology and to validate its application effect.Methods Artificial intelligence technology was used to realize the fusion of data.We constructed a patient portrait rehabilitation nursing system that can provide systematic and personalized information support.By the convenience sampling method,53 perioperative patients with spinal cord injury from July 2023 to December 2023 were selected as the test group to implement the intelligent intervention based on patient portrait technology;53 perioperative patients with spinal cord injury who were hospitalized from January 2023 to June 2023 were selected as the control group to implement the conventional information support intervention.The self-efficacy,self-care ability,and quality of life were compared before the intervention,and 2 weeks,4 weeks,12 weeks after the intervention between the 2 groups.Results 45 cases in the experimental group and 43 cases in the control group reached the endpoint of the study after 12 weeks of intervention.The ANOVA of the 2 groups showed the test group having a greater advantage over the control group,with statistically significant differences in self-efficacy,ability to perform daily living tasks,and quality of life between groups,intergroup effect,and interaction effect(P<0.05).Conclusion The rehabilitation care platform based on patient portraits has a positive effect on improving self-efficacy,self-care ability and quality of life for spinal cord injury patients.
3.Clinicopathological characteristics of diffuse midline gliomas with H3K27-altered:an analysis of 102 children cases

Purpose To investigate the clinicopathological characteristics of pediatric diffuse midline gliomas(DMG)with H3K27-altered.Methods Clinical data of 102 patients with diffuse midline glioma with H3K27-altered were collected,HE and immunohistochemistry EnVision two-step staining was used,and Sanger sequencing for molecular detection was used to ana-lyze their clinical and pathological characteristics and reviewed relevant literatures.Results The age of patients ranged from 1 to 14 years(median age,7 years).Brainstem was the predilec-tion site(81.4%)while other sites included basal ganglia,pin-eal,etc(18.6%).The main clinical manifestations were dizzi-ness,headache,gait instability,etc.The lesions mainly showed hypointensity or isointensity on T1 and hyperintensity on T2 in MRI.Histology showed high-grade gliomas in 64 cases(62.7%)and low-grade gliomas in 38 cases(37.3%).Immunohisto-chemistry showed downexpression or loss of H3K27me3 in 81 ca-ses(100%),H3K27M expression in 100 cases(98%)cases,and EZHIP expression in 2 cases(2%).BRAF V600E expres-sion was detected in only 2.2%(2/89)while p53 mutation in 53.6%(52/97).Additionally,loss of ATRX expression was de-tected in 19.1%(18/94)while the expression of IDH1 was neg-ative in all cases(89/89).Molecular tests showed that of BRAF mutation was 1.7%(1/59)and EGFR mutation was 9.1%(1/11).Conclusion Children DMG with H3K27-altered tend to occur in the brainstem.The histological grade is mostly high.Immunohistochemistry staining of H3K27me3 show varying de-grees of deficiency.Molecular variants include H3K27M muta-tion or EZHIP overexpression and EGFR mutation.
4.Retrospection and reflection on standardized residency training system from the perspective of medical education history

Based on the national policies, regulations and documents on residency training, this paper sorts out the historical evolution of the standardized residency training system in China, and divides its development into four stages: preliminary exploration, local pilot, national trials, and implementation. It also puts forward some practical thoughts on its development law and future trend, such aspects as that the system reform follows the top-down administrative order and indicative plan, the system pays attention to the gradual implementation on the basis of summing up practical experience, and the system needs continuous implementation and improvement from the overall perspective.
5.Research progress on the application of infra-acetabular channel screws in acetabular fractures

The infra-acetabular channel is a channel parallel to the quadrilateral body of the pelvis that passes through the anterior and posterior columns at the medial side of the hip joint. With the widespread use of infra-acetabular channel screws, the acetabular frame screw fixation technique has been proposed to provide stronger fixation for acetabular fractures. There are gender differences in the anatomical parameters of the infra-acetabular channel screws (the length and diameter of the infra-acetabular channel in males are larger than those in females, and the angle between the long axis of the channel and the sagittal plane is smaller). Due to the narrow infra-acetabular channel, it may increase the risks of screw insertion into the hip joint and iatrogenic injury. 3D navigation system-guided placement of infra-acetabular channel screws improves the precision of nail placement. The application of infra-acetabular screws in the fixation of anterior and posterior column fractures has been proved by biomechanical experiments, however, in bicolumnar fractures and T-shaped fractures is still controversial. In order to standardize the placement of infra-acetabular channel screws and to clarify their indications, many studies have been conducted to observe the anatomical parameters of the infra-acetabular channel screws, the surgical technique of placement, biomechanics, and clinical efficacy. This article reviews these studies in order to provide theoretical basis and guidance for the application of infra-acetabular channel screws in acetabular fractures.
6.Visualization analysis of hotspots and trends in healthcare transition for chronic disease adolescents based on Web of Science

Objective:To explore the hotspots, trends, and frontiers of international research on the healthcare transition of adolescent patients with chronic diseases in the past 20 years, so as to provide reference for research in China.Methods:We retrieved articles on the healthcare transition of adolescents with chronic diseases published in the core collection of the Web of Science from April 1, 2002 to April 30, 2022. CiteSpace software was used for visual analysis of article keywords, research institutions, and journal co citations.Results:A total of 865 articles were included, and the overall number of publications showed a wave of growth. The country with the highest volume of publications was the United States, and the institution with the highest volume was the University of Toronto. The UK emphasized cooperation between institutions and formed a close network of institutional cooperation. The most cited magazine was Pediatrics. Keyword analysis showed that the research hotspots of healthcare transition in adolescents with chronic diseases mainly focused on qualitative research on the experiences of different members during the transition period, interventions during the transition period, and mental health during the transition period. Conclusions:The research on the healthcare transition of chronic disease adolescents abroad has a certain depth and breadth. Future researchers should pay attention to the research results of authoritative institutions and journals, and construct personalized transitional intervention plans for adolescents that are suitable for China's medical background.
7.Qualitative studies on the real experience of living related organ transplantation donors from relatives: a Meta-synthesis

Objective:To systematically evaluate the real experience of living related organ transplantation donors from relatives and provide evidence-based basis for medical and nursing staff to develop personalized intervention measures.Methods:Qualitative study on the real experience of living related organ transplantation donors from relatives was searched on PubMed, Web of Science, Cochrane Library, Embase, China National Knowledge Infrastructure, WanFang Data, VIP, and Biology Medicine disc. The search period was from database establishment to August 31, 2022. The article was evaluated using the quality evaluation criteria for qualitative study of the Australian Joanna Briggs Institute (JBI) Evidence-Based Healthcare Center. We converged the results and performed Meta-synthesis.Results:A total of 15 articles were included, 32 research results were extracted, and summarized and integrated into 13 new categories. A total of 5 integration results were synthesized, including the motivation to donate, the pressure and challenges faced before transplantation, the physical and mental discomfort caused by donation, the positive impact of donation, and the desire for support and needs from various aspects.Conclusions:Donors of living related organ transplantation have multiple psychological experiences during the process of organ donation. Nurses should dynamically assess the pressure and challenges faced by donors, provide diverse support to meet their needs, help donors actively respond, and improve their quality of life.

9.Medication Analysis and Care for Acute Renal Failure with Epileptic Seizure Caused by Excretion Delay of HD- MTX by Clinical Pharmacists

OBJECTIVE:To investi gate the role of clinical pharmacists in the treatment of delayed excretion of acute renal failure (ARF) with epileptic seizure caused by HD-MTX in a patient ,and to provide reference for rational drug use and pharmaceutical care in such type of patients. METHODS :A patient with diffuse large B-cell lymphoma was given HD-MTX for chemotherapy,and ARF caused by delayed methotrexate excretion occurred on the second day after methotrexate administration. Clinical physicians adjusted the rescue dose and frequency of calcium folinate but the effect was poor. Clinical pharmacists analyzed the causes of delayed methotrexate excretion by reviewing literature and combining with the patient ’s condition. It was suggested to monitor the blood concentration of methotrexate ,strengthen alkalization and hydration ,increase the volume of intravenous sodium bicarbonate from 125 mL to 250 mL,take Sodium bicarbonate tablets orally ,and monitor the pH value of urine (pH value of urine maintained above 7). In addition ,the pharmacist told the patient to drink water as much as possible to ensure the daily urine output reached 3 000 to 4 000 mL. The blood concentration of methotrexate was 16.14 μmol/L 44 h after administration ,which proved to be excretion delay. The patient had epileptic seizure on the 13th day after methotrexate medication. The physician gave sodium valproate 0.8 g intravenously to control epilepsy. The clinical pharmacist conducted pharmaceutical care for the patient ,and found that the compliance of the patient taking Sodium bicarbonate tablets and Sodium valproate tablets orally was not good ,so medication education and pharmaceutical care were conducted ,then the patient accepted and took the drugs on time. RESULTS : The physician adopted the suggestions of the pharmacist to monitor the blood concentration of methotrexate and performed symptomatic treatment. The urine volume of the patient increased ,the edema was reduced ,serum creatinine gradually returned to normal,and renal function recovered gradually ;the symptoms of epilepsy was controlled. CONCLUSIONS :In the treatment process of ARF complicated with epileptic seizure caused by excretion delay of HD-MTX ,the clinical pharmacist assisted physician to improve the treatment plan and conducted pharmaceutical care and medication education for the patient ,therefore ensure the safe and rational use of drugs .
10.A systematic review of pseudoaneurysm after intertrochanteric fracture

Objective:To summarize the age, gender difference, cause, location, time, treatment and prognosis of femoral intertrochanteric fracture complicated with pseudoaneurysm, and analyze the influence of different treatment methods on the occurrence of femoral intertrochanteric fracture complicated with pseudoaneurysm.Methods:Using the method of systematic literature review, a total of 76 literatures were retrieved from multiple databases at home and abroad, and 45 literatures were screened out through the second screening of title, abstract and full text. The patients with intertrochanteric fracture complicated with pseudoaneurysm were selected as the research objects, and the duplicate cases were deleted; the age, gender, onset age, etiology, location, treatment and prognosis of the disease were retrospectively analyzed.Results:A total of 54 cases of femoral intertrochanteric fracture related pseudoaneurysms were selected from 45 literatures, including 25 males and 27 females, and 2 cases had no gender; aged from 43 to 94 years old, and 4 cases were younger than 60 years old, 6 between 61 and 70, 17 between 71 and 80, and 25 between 81 and 90. Forty-five cases occurred in deep femoral artery, 3 cases in superficial femoral artery, 1 case in superior gluteal artery, 1 case of medial circumflex artery, 1 case of lateral circumflex femoral artery, 1 case of femoral artery, and 2 cases of unknown location. The occurrence time of pseudoaneurysm: 17 cases within 1 week after fracture, 8 cases from 1 week to 3 weeks, 19 cases from 3 weeks to 3 months, 2 cases more than 3 months, and 8 cases unknown. The causes of pseudoaneurysm: among the 54 patients, 41 cases had definite causes, including 21 cases of vascular injury caused by fracture block; 20 cases of iatrogenic injury, of which the incidence of direct injury of blood vessels by screw or drill was the highest, accounting for 55% (11/20). Twenty-two cases were treated with vascular embolization, 10 cases with surgical sutures, 8 cases with surgical ligation, 4 cases with resection of pseudoaneurysm, 5 cases with covered stent, and 4 cases with thrombin injection. There were 42 cases who had follow-up data, and no recurrence and other complications were found during the follow-up period.Conclusion:The peak age of femoral intertrochanteric fracture complicated with pseudoaneurysm was 71-90 years old. The incidence of femoral intertrochanteric fracture complicated with pseudoaneurysm was more often within 1 week after fracture and 3 weeks to 3 months after fracture, due to fracture fragments and iatrogenic vascular injury caused by the location of the profundus femoris artery. Treatment options include vascular embolization, surgical ligation, suture or resection of pseudoaneurysm, covered stent and thrombin injection, and the prognosis is generally good.
