1. Comparison of epidemic characteristics between SARS in2003 and COVID-19 in 2020 in Guangzhou

Objective By analyzing the epidemic characteristics and related indicators of SARS (2003) and COVID-19(2020), to explore the reasons for the similarities and differences of the two epidemics, so as to provide reference for epidemic prevention and control. Methods The general situation, clinical classification, activity history, contact history, family members’ contact and incidence of the two infectious diseases in Guangzhou were collected and used to analyze the time characteristics, occupational characteristics, age characteristics and other key indicators of the two diseases, including the number of cases, composition ratio (%), mean, median, crude mortality, etc. Results A total of 1 072 cases of SARS (2003) were included in the study. 353 of which were severe cases with the incidence of 30.13%. 43 cases of death were reported with a mortality rate of 4.01%. The average age was 46 years old, and 26.31% of the cases were medical staff. The interval time between first report to continuous zero reports was 129 days. As to COVID-19 (2020), a total of 346 cases were included. 58 of which were severe cases with the incidence of 16.67%. One case of death was reported with a mortality rate of 0.29%. The average age was 38 years old, and no hospital infection among medical staff was reported. The interval time between first report to continuous zero reports was 35 days. Conclusions The prevention and control strategies for COVID-19 (2010) are more effective compared to that of SARS (2003), and the emergency response procedures are worth to be evaluated and summarized.

The incidence ratio of male to female was 1∶1.26. 43 cases of death were reported with a mortality rate of 4.01%. The median age was 36 years old. The proportion of medical staff in the early stage of the epidemic was 29.04% (88 cases). As to COVID-19 in Wuhan, a total of 49 978 cases were included, The incidence ratio of male to female was 1.04∶1. The 2 423 cases of death were reported with a mortality rate of 4.85%. The median age was 56 years old. The proportion of medical staff in the early stage of the epidemic was 30.43% (42 cases).Conclusion:The COVID-19 in Wuhan has the characteristics of high incidence and wide population. However, the epidemic situation is falling rapidly, and the prevention and control strategy needs to be adjusted timely. The prevention and control of nosocomial infection should be addressed in future.
3.A qualitative study on differences of nurses' cognition on workplace violence under different perspectives

Objective:To understand the cognition of experiencers and witnesses on workplace violence (WPV) in nurses and to analyze the differences among them so as to provide a theoretical basis for formulating intervention.Methods:From February to May 2018, totals of 26 nurses, experiencers and witnesses of WPV, at a hospital in Suzhou were selected as subjects by purposive sampling to carry out semi-structured interview with the method of descriptive qualitative study. Content analysis was used to the data.Results:Under different perspectives, the differences of nurses' cognition on WPV included three aspects involving subject responsibility attribution, pre-recognition in advance and organizational support of WPV. Experiencers thought that patients had great responsibilities on violence, WPV was difficult to identify, lack of organizational support. Witnesses showed that nurses and patients matched each other in responsibilities, WPV could be identified in advance, the ways of organizational support needed diversification.Conclusions:Under different perspectives, there are differences in nurses' cognition on WPV. Hospital managers should strengthen the building of security, and improve nurses' violence identification and coping capacity to reduce the incidence of hospital violent incidents.

5.Efficacy and Safety of Hydrotalcite Combined with Omeprazole versus Omeprazole for Gastric Ulcer :A Meta-analysis

OBJECTIVE: To systematically evaluate the efficacy and safety of hydrotalcite combined with omeprazole for gastric ulcer, and to provide evidence-based reference for clinical treatment. METHODS: Retrieved from PubMed, Embase, Medline, the Cochrane library, CNKI, VIP and Wanfang database, randomized controlled trials (RCTs) about hydrotalcite combined with omeprazole (trial group) versus omeprazole alone (control group) for gastric ulcer during the database establishment to Aug. 2018. After data extraction of included literatures met inclusion criteria, and quality evaluation with Cochrane evaluator manual 5.0.1, Meta-analysis was performed for response rate, the incidence of ADR, recurrence rate of gastric ulcer bleeding, needed time of clinical symptom improvement and hospitalization stays by using Rev Man 5.3 statistical software. RESULTS: A total of 16 RCTs, involving 1 802 patients were included. The results of Meta-analysis showed that response rate [RR=1.24, 95%CI(1.19,1.29), P<0.001] of trial group was significantly higher than that of control group; recurrence rate of gastric ulcer [RR=0.27,95%CI(0.17,0.45),P<0.001], clinical symptom improvement time [MD=-2.04,95%CI(-2.25, -1.83),P<0.001] and hospitalization time [MD=-4.25,95%CI(-4.55,-3.95),P<0.001] of trial group were significantly lower or shorter than those of control group, with statistical significance. There was no statistical significance in the incidence of ADR [RR=0.68,95%CI(0.46,1.02),P=0.06] between 2 groups. CONCLUSIONS: Compared with omeprazole alone, hydrotalcite combined with omeprazole for gastric ulcer can obviously increase the clinical response rate, decrease the recurrence rate of gastric ulcer and shorten the needed time of clinical symptom improvement and hospitalization time, but do not increase the incidence of ADR.
6.Meta-analysis of Therapeutic Efficacy and Safety of Mirtazapine Combined with Selective Calcium Ch annel Blocker in the Treatment of Irritable Bowel Syndrome

OBJECTIVE: To systematically review therapeutic efficacy and safety of mirtazapine combined with selective calcium channel blocker (SCCB) in the treatment of irritable bowel syndrome (IBS), and provide evidence-based reference for clinical medication. METHODS: Retrieved from the Cochrane Library, PubMed, Embase, Medline, CNKI, VIP and Wanfang database, randomized controlled trials (RCTs) about mirtazapine combined with SCCB (trial group) versus SCCB (control group) for IBS were collected. After literature screening and data extraction, quality evaluation was performed by using Cochrane system evaluator manual 5.1.0 recommend bias risk evaluation tool. Meta-analysis was performed by using Stata 14.0 software. RESULTS: A total of 14 RCTs involving 1 005 patients were included. The results of Meta-analysis showed that the total response rate [RR=1.34,95%CI(1.25,1.44),P<0.001],neuropeptide-Y level after treatment [SMD=0.77,95%CI(0.49,1.05),P<0.001], response rate of abdominal pain therapy [RR=1.32,95%CI(1.06,1.66),P=0.014] and response rate of treatment for abnormal stool characteristics [RR=1.75,95%CI(1.36,2.27), P<0.001] were significantly higher than control group; the scores of depression scale after treatment [SMD=-1.87, 95%CI (-2.35, -1.39), P<0.001], anxiety scale after treatment [SMD=-2.25, 95%CI (-3.35, -1.15), P<0.001], abdominal pain symptom score after treatment [SMD=-7.41, 95%CI (-8.30,-6.51), P<0.001], diarrhea symptom score after treatment [SMD=-6.39, 95%CI (-7.96,-4.81), P<0.001] were significantly lower than those of the control group. There were no statistical significance in response rate of abdominal distension therapy [RR=1.07,95%CI(0.90,1.28),P=0.421] and response rate of abnormal defecation therapy [RR=1.05,95%CI(0.88,1.26),P=0.588], the incidence of abdominal pain [RR=0.45,95%CI(0.11,1.97), P=0.291] and exhaustion [RR=5.00,95%CI(0.60,41.79),P=0.137] between 2 groups. CONCLUSIONS: Mirtazapine combined with SCCB can significantly improve therapeutic efficacy of IBS patients, promote clinical symptoms, but do not increase the occurrence of ADR as abdominal pain and exhaustion.
7.Prevalence and influential factors of workplace violence against nurses in general hospitals of Suzhou

Objective To investigate the prevalence and influential factors of workplace violence (WPV) against nurses in general hospitals of Suzhou. Methods A total of 2110 staff nurses from 13 general hospitals in Suzhou were investigated with the Modified Version of Hospital Workplace Violence Questionnaire. Results During the past one year,68.9% (1454/2110) nurses investigated suffered from WPV.Working departments,marital status, strong support,etc,were the leading impact factors of WPV. Conclusions Nurses′experience of WPV is a prevalent phenomenon in general hospitals of Suzhou. It is urgent for hospital administration to raise attention to WPV and take effective measures,to reduce the occurrence of WPV.
8.Bacteriological analysis and treatment strategy in patients with biliary sepsis

Objectives To access the bacteriology in patients with sepsis due to biliary tract infection to provide a basis for empirical selection of proper antibiotic treatment.Methods This is a single-center retrospective study on 214 patients with biliary tract infection admitted from August 2014 to July 2016 to the surgical intensive care units (ICU) of The First Affiliated Hospital of Sun Yat-sen University.To study the demographic information,sequential organ failure assessment (SOFA),usage of antibiotics before ICU and duration of ICU were analyzed.Bile,peritoneal drainage and blood samples were collected.Results 47 septic shock patients and 25 septic patients due to biliary tract infection were enrolled in the trial.The two groups (the shock group vs.the sepsis group) had a significant difference in the duration of ICU stay [(6.4 ± 4.6) d vs.(2.3 ± 1.8) d,P < 0.05].48 strains of pathogens were isolated from the bile samples.The major pathogens were Escherichia coli (E.coli) (n =23,47.9%),Enterococcus faecalis (n =8,16.7%) and Enterococcus faecium (n =2,4.2%).80 strains of pathogens were isolated from the peritoneal drainage culture samples.E.coli,pseudomonas aeruginosa,and Klebsiella pneumoniae ranked the top 3 species,accounting for 26.3%,11.3% and 7.5%,respectively.The sensitivity of E.coli isolated from bile to amikacin,imipenem and panipenem were all over 90.0%.Conclusions E.coli was the principal gram-negative bacterium in biliary infection induced sepsis.Early administration of carbapenemes may reduce the occurrence of septic shock in these patients.
9.Arachidonic acid metabolic pathway-related genes interactions increase the incidences of stroke and vulnerable plaques

Objective To investigate whether metabolic pathway-related gene polymorphisms are associated with arterial plaque stability and their gene-gene interactions increase the risk of cerebral infarctions.Methods Totally 294 patients with atherothrombosis stroke admitted to the Department of Neurology, the Third Affiliated Hospital of Wenzhou Medical University from September 2010 to December 2012 were divided into a carotid vulnerable plaque group ( n=69 ) and a stable plaque group ( n=225 ) according to the results of carotid B-mode ultrasonography.A total of 282 healthy volunteers excluded carotid plaque and stroke were enrolled as well.Genetic polymorphisms of ALOX5AP and CYP3A5, CYP2C9*2, CYP2C9*3 and EPHX2 were genotyped using polymerase chain reaction and mass spectrometry analysis.The SPSS16.0 software was used to compare genotype frequencies and the generalized multifactor dimensionality reduction ( GMDR ) method was applied for gene-gene interaction analyses.Results The results showed that EPHX2 GG genotype might protect against stroke ( OR =0.520, 95% CI 0.288 -0.940, P=0.030).The distribution of CYP3A5 genotypes showed statistically significant differences (χ2 =7.284, P=0.026) between the vulnerable plaque ( AA: 5 cases, AG: 36 cases, GG: 28 cases) and stable plaque ( AA: 26 cases, AG: 77 cases, GG: 122 cases ) groups.Multivariate Logistic regression analysis showed that the GG genotype of CYP3A5 was protective factor for unstable plaques ( OR=0.405, 95%CI 0.178 -0.920, P =0.031 ).Differences in other SNPs did not reach statistical significance between the two groups.The GMDR analysis showed a significant gene-gene interaction between SG13S114 and A6986G, with scores of 10 for cross-validation consistency and 9 for the sign test (P=0.011).The best model for ischemic stroke was found to be SG13S114 AA and A6986G AA.Adjusting for age, hypertension and diabetes, the certain gene-gene interaction predicted a significantly higher risk of cerebral infarction (OR=1.804, 95%CI 1.180-2.759, P=0.006).Conclusions The EPHX2 G860A gene might be linked with the incidence of cerebral infarctions.Only a CYP3A5 gene polymorphism might be associated with carotid plaque instability in patients with stroke.The gene-gene interaction predicts a significantly higher risk of cerebral infarction.There is a 1.804-fold increased risk for ischemic stroke in individuals with these combined genetic factors.
10.The clinical application of prenatal serum blood group IgG antibody titer determination

Objective To assess the relationship between the serum IgG antibody titer of pregnant women and the hemolytic dis‐ease of newborn(HDN) .Methods Using microcolumn gel coombs card assay method to determine titer of 255 cases of couples an‐tenatal serum IgG antibody and ABO blood group .Results with 195 cases of ABO‐incompatible couples ,the positive rate of abnor‐mal serum IgG antibody(≥64)was 93 .8% .The titer of anti‐A/B IgG :in 12(6 .2% ) cases was <64;in 12(6 .2% )cases was 64;in 60(30 .8% )cases was 128 ;in 39(20% )cases was 256 ;in 45(23 .0% )cases was 512 ;in 27(13 .8% ) cases was 1 024 .There were no statistical differences between IgG anti‐A(B) titers distribution between A/O blood group matching and B/O blood group matching (χ2 =4 .361 ,P=0 .499) ,IgG anti‐A( B) titers was higher in AB/O blood group matching .Conclusion we can take early and effec‐tive prevention ,treatment ,reducing the incidence of HDN by the determination of prenatal serum IgG antibody titers in ABO‐in‐compatible couples ,which is important for the population of eugenics .
