1.The Value of MSCT Based Radiomics in Differential Diagnosis of Borrmann Ⅳ Gastric Cancer and Primary Gastric Lymphoma

ObjectiveTo explore the predictive value of CT based radiomics model in differentiating Borrmann type Ⅳ gastric cancer (GC) from primary gastric lymphoma (PGL). MethodsA total of 186 cases (Borrmann type Ⅳ GC: 132; PGL: 86) pathologically diagnosed by surgical resection and/or endoscopic biopsy were enrolled from June 2008 to April 2018 retrospectively. Radiomics features were extracted from CT arterial phase and venous phase images by computed algorithm, and selected by least absolute shrinkage and selection operator (Lasso) logistic regression, and then the CT-based radiomics models were established. CT subjective signs were reviewed to build CT subjective signs model, while CT subjective signs and radiomics signature were assembled to build combined model. The receiver operating characteristic (ROC) curve was used to evaluate the performance of CT subjective sign model, radiomics model and the combined model. ResultsTwo signs(the bright line sign of serosa and the irregular nodular protrusion on the serosa side)were selected into the CT subjective sign model. Among the radiomics features, 9 venous phase features, 8 arterial phase features and 14 arteriovenous combination features related to tumor classification were selected, and the corresponding radiomics models were constructed respectively. When the cut-off value of CT subjective sign model was 0.188, the area under curve (AUC) was 0.846, the sensitivity was 61.9%, the specificity was 81.7%, and the accuracy was 76.5%. The cut-off values of arterial phase, venous phase and arteriovenous phase radiomics model were -0.315, -0.669 and -0.858, respectively, and the AUCs were 0.864, 0.955 and 0.890, the sensitivity were 71.4%, 95.2% and 81.0%, the specificity were 85.0%, 88.3% and 80.0%, the accuracy were 81.5%, 90.1% and 80.3%, respectively. The cut-off values of arterial phase, venous phase and arteriovenous phase in the combined model were 0.257, 0.556 and 0.497, respectively, and the AUCs were 0.883, 0.956 and 0.918, the sensitivity was 71.4%, 90.5% and 71.4%, the specificity was 85.0%, 93.3% and 90.0% and the accuracy were 81.5%, 92.6% and 85.2%, respectively. The diagnostic performance of the models from high to low were the combined model, radiomics model and CT subjective finding model ( P< 0.001), and CT venous phase images were more effective in the differential diagnosis of the two tumors. ConclusionsThe radiomics model based on the arterial and venous phases CT images could differentiate Borrmann type Ⅳ gastric carcinoma from primary gastric lymphoma effectively.
2.Electroacupuncture Delays Cartilage Degeneration by Modulating Nuclear Factor-κB Signaling Pathway.

OBJECTIVE:
To illustrate the molecular mechanisms underlying the therapeutic effects of electroacupuncture (EA) on knee osteoarthritis (OA).
METHODS:
Twenty-seven six-month-old New Zealand white rabbits were allocated into three groups in accordance with a random number table: normal group (no surgery-induced OA; without treatment), model group (surgery-induced OA; without treatment) and EA group [surgery-induced OA; received treatment with EA at acupoints Dubi (ST 35) and Neixiyan (EX-LE 5), 30 min twice a day]. After eight consecutive weeks of treatment, the histopathological alterations in cartilage were observed using optical microscopy and transmission electron microscopy, cartilage degeneration was evaluated by modified Mankin's score principles, the synovial fluid concentration of interleukin-1β (IL-1β), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and matrix metalloproteinase-3 (MMP-3) were evaluated by enzyme-linked immunosorbent assay, and the protein expression levels of IL-1β, IL-6, TNF-α, MMP-3, IκB kinase-β (IKK-β), nuclear factor of α light polypeptide gene enhancer in B-cells inhibitor α (IκB-α) and nuclear factor-κB (NF-κB) p65 were quantified by Western blot analysis.
RESULTS:
EA treatment significantly improved cartilage structure arrangement and reduced cellular degeneration. The IL-1β, IL-6, TNF-α and MMP-3 of synovial fluid in the EA-treated group were significantly decreased compared with the model group (all P<0.01). Compared with the model group, the IL-1β, IL-6, TNF-α, MMP-3, IKK-β and NF-κB p65 protein expressions in cartilage of EA-treated group were significantly decreased (all P<0.01), whereas IκB-α expression was significantly up-regulated (P<0.01).
CONCLUSION
EA treatment may delay cartilage degeneration by down-regulating inflammatory factors through NF-κB signaling pathway, which may, in part, explain its clinical efficacy in the treatment of knee OA.
3. Expression of aquaporin 4 in 1,2-dichloroethane-induced toxic brain edema in rats

OBJECTIVE: To explore the effects of aquaporin 4( APQ4) in rat toxic brain edema induced by subacute 1,2-dichloroethane( 1,2-DCE) exposure. METHODS: Thirty-two specific pathogen free healthy adult female SD rats were randomly divided into control( 8 rats),low-dose( 12 rats) and high-dose( 12 rats) groups. The treatment groups were exposed to 1,2-DCE( low-dose: 600 mg / m3; high-dose: 1 800 mg/m3,nose-only) and the control group was exposed to fresh air by dynamic inhalation for 8 hours per day for consecutive 7 days. After exposure,histopathologic changes were examined in the cerebral cortex. Real-time polymerase chain reaction was used to detect the mRNA relative expression of matrix metalloproteinase 2( MMP2),Na-K-Cl cotransporter-1( NKCC1) and AQP4. The Western blotting was used to detect the expression of AQP4 protein in the cerebral cortex. RESULTS: The pathological results showed that the cerebral cortex tissues were loose around the peripheral vessels and the vessels tissue space appeared widen in low-dose exposure group. The pathological change was more serious in high-dose group than low-dose group,with obvious loosen vessels and vacuole. Compared with those of the control group and the low-dose group,the relative expression level of MMP2 mRNA in the high-dose group increased significantly[( 1. 07 ± 0. 41) vs( 1. 56 ± 0. 55),( 1. 21 ± 0. 59) vs( 1. 56 ± 0. 55),P <0. 05],while the the relative expression level of AQP4 mRNA in the high-dose group significantly decreased [( 1. 03 ±0. 25) vs( 0. 81 ± 0. 12),( 1. 00 ± 0. 20) vs( 0. 81 ± 0. 12),P < 0. 05]. The relative expression levels of NKCC1 mRNA in all groups showed no statistical difference [( 1. 03 ± 0. 31) vs( 1. 14 ± 0. 43) vs( 1. 36 ± 0. 50),P > 0. 05]. The relative expression level of AQP4 protein in the high-dose group was lower than that of the control group [( 0. 80 ± 0. 25) vs( 1. 19 ± 0. 42),P < 0. 05]. CONCLUSION: The brain edema induced by subacute inhalation of 1,2-DCE is of mixed types with vasogenic edema as its main symptom. Its pathogenesis is related to the changes of AQP4 expression.
4.Expression of Na(v)1.6 in rats after cerebral ischemic injury

Objective To observe the changes of Na(v)1.6 expression in rats after acute cerebral ischemia and the effect of Riluzole (the sodium channel blocker) on these changes, and discuss the relationship between level of Na(v)l.6 and cerebral ischemia. Methods One hundred and five healthy SD rats were randomly divided into sham-operated group (n=15), ischemia control group (IC, n=45) and Riluzole therapy group (RT, n=45). Rat models of focal acute cerebral ischemia in the later 2 groups were established by permanent occlusion of right middle cerebral artery. Riluzole at a dosage of 8 mg/kg was given once daily to the rats of the RT group 30 min after ischemia. Tissues from the striatum were collected at different time points (6 h, and 1, 2, 3 and 7 d after ischemia); the expressions of Na(v)1.6 in the striatum were detected by immunofluorescence staining and real-time quantitative PCR at each time point; and the infarct volume was observed by triphenyltetrazolium chloride staining at each time point.Results The rats in the IC group and RT group showed neurologic impairment, especially 2 d after ischemia; rats of the IC group presented significantly higher scores of neurological function scale than those of the RT group at the same time point (P<0.05). Immunofluorescence staining showed that the expression of Na (v)1.6 was up-regulated, and reached its peak level 1 d after ischemia but then, was down-regulated both in the IC group and RT group. Real-time quantitative PCR showed that the expression of Na(v)1.6 in the IC group was up-regulated 1 d after ischemia, and then down-regulated 2, 3 and 7 d after ischemia, however, that in the RT group was down-regulated 6 h after ischemia; the mRNA expression of Na (v)1.6 in the RT group was obviously down-regulated as compared with that in the IC group at the same time point (P<0.05). The infarction volume became the largest 3 daRer ischemia both in the IC group and RT group; the infarction volume in RT group was smaller than that in IC group at the same time point (P<0.05). Conclusion The expression of Na(v)1.6 is down-regulated after cerebral ischemic injury to mitigate acute cerebral ischemic injury, indicating that Na (v)1.6 might involve in the development of cerebral ischemic injury.

6.Mechanisms and effects of biosynthesis and apoptosis in repair of full-thickness skin defect with collagen-chitosan dermal stent.

OBJECTIVETo investigate biosynthetic and apoptotic mechanisms in repair of full thickness skin defect with collagen-chitosan porous scaffold transplantation, and to determinate differences between wound repair with the scaffold transplantation and scar healing without the scaffold transplantation.
METHODSThe full thickness skin defects were made on 10 Bama miniature pigs and the bilayer dermal equivalent (BDE) composed of collagen-chitosan porous scaffold and silicone membrane was transplanted on wounds. Surfaces of wounds were observed at 1, 2, and 3 weeks after the BDE transplantation, and so were done the wound repairs after epidermis had been grafted for 2 weeks on surface of the scaffold which had been transplanted on skin defect wounds for 2 weeks. At the same time, TGF-beta1 expressions, apoptosis and self collagen replacement of scaffolds in wounds were detected in situ by immunohistochemical staining, terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate-biotin nick end labeling (TUNEL) and picrosirius red polarized light. Wounds without scaffold transplantation were studied as control.
RESULTS1) Wounds with the scaffold transplantation were different from granulation tissue. 2) The peak of TGF-beta1 expression in the scaffold wounds was from 1 to 2 weeks after BDE transplantation, and TGF-beta1 expressions decreased continuously from 3 to 4 weeks. TGF-beta1 expressions increased continuously in the control wounds from 1 to 3 weeks and decreased on 4 weeks. TGF-beta1 expressions in the scaffold wounds on 1st and 2nd week were significantly higher than those in the corresponding control wounds, whereas, TGF-beta1 expressions in the scaffold wounds on 3rd and 4th week were significantly lower than those in the corresponding control wounds. 3) Apoptosis increased continuously in the scaffold wounds from 2 to 4 weeks after BDE transplantation, and so did in the control wounds from 3 to 4 weeks. However, apoptosis signals in the scaffold wounds on 2nd, 3rd, and 4th week after BDE transplantation were significantly more than those in the corresponding control wounds, and there was no difference between apoptosis signals in the scaffold wounds on 1st week after BDE transplantation and those in the corresponding control wounds. 4) Observation by picrosirius red polarized light method: self collagen began to synthesize in the scaffold wounds on 1st week after BDE transplantation, and scaffolds had been replaced by self collagen from 2 to 3 weeks after BDE transplantation.
CONCLUSIONSCollagen-chitosan porous scaffold plays a very important role in wound healing of full thickness skin defect. The mechanisms of wound repair by dermal scaffold are different from those by granulation and scar healing. It has a good future in repairing skin defect.

7.Study on repair of full-thickness skin defect with collagen-chitosan dermal stent in pigs.

OBJECTIVETo investigate angiogenesis of collagen-chitosan porous scaffold, and to study survive of skin grafts on the scaffold after bilayer dermal equivalent (BDE) was transplanted on wounds with full thickness skin defects.
METHODSThe full thickness skin defects were made on 10 Bama miniature pigs and the BDE composed of collagen-chitosan porous scaffold and silicone membrane was transplanted on wound. Angiogenesis in dermal equivalent, wound healing, and healing and survive of skin grafts on dermal equivalent were observed in 1, 2, and 3 weeks after the BDE transplantation. At the same time, CD34 positive signals (neo-forming micro-vessels) were detected by immunohistochemical staining.
RESULTSInflammatory cells and fibroblasts infiltrated into dermal equivalent and a few new micro-vessels had been formed in 1 week after the BDE transplantation; neo-forming micro-vessels perpendicular to wound bed had increased significantly in 2 weeks after the BDE transplantation; neo-forming micro-vessels could be observed in almost all dermal equivalents in 3 weeks after the BDE transplantation. CD34 positive signals (neo-forming micro-vessels) in 3 weeks after the BDE transplantation was much more than those in 2 weeks after the BDE transplantation, and CD34 positive signals in 2 weeks after the BDE transplantation was much more than those in 1 week after the BDE transplantation. Survival rate of intermediate split thickness skin graft were 10%, 70% and 100% respectively after the skin grafts had been grafted for 2 weeks on surface of the scaffold which had been transplanted for 1, 2 and 3 weeks. Epidermis which had been grafted on surface of the scaffold for 1 or 2 weeks could perfectly survive after BDE had been transplanted for 1 or 2 weeks.
CONCLUSIONSCollagen-chitosan porous scaffold plays a very important role in wound healing of full thickness skin defect and can induce fibroblast infiltration and new micro-vessel formation. Epidermis grafted on surface of collagen-chitosan porous scaffold can perfectly repair wounds, and it has brilliant applied prospects in repairing skin defect.

8.Edge detection and modeling in frontal facial contour image for plastic surgery.

OBJECTIVETo find a new method for edge detection and modeling in frontal facial contour image.
METHODSSearching circle-based edge detection algorithm was developed on the basis of Sobel edge detector. Apriori knowledge of the facial contour and searching limitations as the minimum curvature radius, concave-convex property, and maximum edge disconnected distance were used to detect the edge of frontal facial contour. The frontal facial contour model was established with least squares curve fitting methods, and the relationship between the model rank and model precision was analyzed.
RESULTSThe edge detected by the new method was consistent with the actual edge of the facial contour and the irrelevant edge was well eliminated. Variation of the 2,4,10 time models from the actual image were compared, which identified the 10 time model as the best one.
CONCLUSIONThe effectiveness and practicability of this new method for edge detection and modeling has been tested, which provides a theoretic basis for designing facial contouring image system.
