1.Assessment of respiratory protection competency of staff in healthcare facilities

Objective To understand the respiratory protection competency of staff in hospitals.Methods Staff from six hospitals of different levels and characteristics in Beijing were selected,including doctors,nurses,medical technicians,and servicers,to conduct knowledge assessment on respiratory protection competency.According to exposure risks of respiratory infectious diseases,based on actual cases and daily work scenarios,content of respira-tory protection competency assessment was designed from three aspects:identification of respiratory infectious di-seases,transmission routes and corresponding protection requirements,as well as correct selection and use of masks.The assessment included 6,6,and 8 knowledge points respectively,with 20 knowledge points in total,all of which were choice questions.For multiple-choice questions,full marks,partial marks,and no mark were given respective-ly if all options were correct,partial options were correct and without incorrect options,and partial options were correct but with incorrect options.Difficulty and discrimination analyses on question of each knowledge point was conducted based on classical test theory.Results The respiratory protection competency knowledge assessment for 326 staff members at different risk levels in 6 hospitals showed that concerning the 20 knowledge points,more than 60%participants got full marks for 6 points,while the proportion of full marks for other questions was relatively low.Less than 10%participants got full marks for the following 5 knowledge points:types of airborne diseases,types of droplet-borne diseases,conventional measures for the prevention and control of healthcare-associated infec-tion with respiratory infectious diseases,indications for wearing respirators,and indications for wearing medical protective masks.Among the 20 knowledge questions,5,1,and 14 questions were relatively easy,medium,and difficult,respectively;6,1,4,and 9 questions were with discrimination levels of ≥0.4,0.30-0.39,0.20-0.29,and ≤0.19,respectively.Conclusion There is still much room for hospital staff to improve their respiratory protection competency,especially in the recognition of diseases with different transmission routes and the indications for wearing different types of masks.
2.Evaluation and Analysis of Health Informatization Construction Policies Based on PMC Index Model

Purpose/Significance To quantitatively analyze and evaluate 26 health informatization construction policies issued at the national level,so as to provide basis for improving related policies.Method/Process Based on the policy modeling consistency(PMC)index model,a health informatization construction policy evaluation index system is constructed,which includes 9 first-level indicators and 36 second-level indicators.Appling it to the evaluation of various policy texts,and the health informatization construction policies are analyzed in a holistic and concrete way.Result/Conclusion The overall score of the health informatization construction policies is ex-cellent.2 policies are at the perfect level,80.6%of policies are at the excellent level,and there are no bad level policy texts.However,the driving force of the policy and the balance of policy recipients need to be further improved.
3.Safety and the Short-Term Efficacy of Venetoclax Combined with Azacitidine Followed by Cladribine in Children with Refractory/Relapsed Acute Myeloid Leukemia.

OBJECTIVE:
To investigate the safety and the short-term efficacy of venetoclax combined with azacitidine followed by cladribine (VAC regimen) in children with refractory/ relapsed acute myeloid leukemia (AML).
METHODS:
The clinical data, treatment outcomes, complications, and blood product consumption of 6 children with refractory/relapsed AML treated with VAC regimen in the Children's Hospital of Soochow University from August 2021 to December 2021 were retrospectively analyzed.
RESULTS:
Among the 6 children, there were 1 male and 5 females. 5 cases were refractory AML, and 1 case was relapsed AML, which recurred again 16 months after allogeneic hematopoietic stem cell transplantation. 4 children were accompanied by chromosomes or genes that predicted poor prognosis, such as RUNX1, FLT3-ITD, KMT2A exon 2-exon 8 dup, MLL-AF6, 7q-, KMT2A exon 2-exon 10 dup, etc. After received VAC regimen, 4 cases achieved CR+CRi, 1 case achieved PR (only MRD did not relieve, MRD was 0.59%), and 1 case was NR (but the proportion of bone marrow blasts decreased). All 6 patients had grade Ⅳ neutropenia, and 4 patients had grade Ⅳ thrombocytopenia. During the period of neutropenia, none of the 6 children developed symptoms of infection such as fever, cough, and diarrhea. No treatment-related death occurred.
CONCLUSION
Venetoclax combined with azacitidine followed by cladribine provides a new treatment option for patients with relapsed/refractory AML who have poor efficacy in early induction remission theragy, showing good efficacy and safety.

4.Molecular biological identification of ticks and tick-borne pathogens in Yadong,Tibet

This study was aimed at identifying species of ticks and investigating infections with tick-borne pathogens a-mong people in Yadong,Tibet.The total of 23 ticks(Ixodes and Haemaphysalis)collected from livestock in Yadong,Tibet in July 2021,the gene sequences of cytochrome oxidase subunitⅠ(COX Ⅰ),mitochondrial 16S ribosomal DNA(16S rDNA)and mitochondrial 12S ribosomal DNA(12S rDNA)were identi-fied on the basis of molecular biology classification.The gene sequences of spotted fever group rickettsia(SFGR),Borrelia burgdorferi,Francisella tularensis and Coxiella burnetii in ticks were amplified by PCR and sequenced,and phylogenetic ana-lyses were performed.In addition,184 serum samples collected from Yadong in 2021 were screened for antibodies to Lyme dis-ease,Q fever,spotted fever,and tularemia by ELISA,and infections with tick-borne pathogens in Yadong were comprehen-sively analyzed.The ticks(Ixodes)collected in Yadong,Tibet,were grouped into two categories.The COX Ⅰ,16S rDNA,and 12S rDNA gene sequences of the ticks had 99.5%,97.57%,and 99.12% similarity,respectively,to those of Ixodes nuttallia-nus in GenBank.The similarities between the COX Ⅰ and 16S rDNA gene sequences of another Ixodes tick species and the COXⅠ and 16S rDNA gene sequences of Ixodes ovatus in GenBank were 88.29% and 95.75%,respectively.The similarity between the COX Ⅰ,16S rDNA,and 12S rDNA gene sequences of Haemaphysalis ticks and those of Haemaphysalis Tibet in GenBank were 96.04%,96.17%,and 97.47%,respectively.The sequence amplification results for Borrelia burgdorferi,Francisella tularensis,and Coxsiella burnetii in 23 ticks were negative,whereas gltA gene amplification of spotted fever group Rickettsia in 23 ticks was positive.The similarity of gltA gene sequences with respect to those of Candidatus Rickettsia tarasevichiae,Rickettsia raoultii,Rickettsia canadensis,and a Rickettsia endosymbiont of Eucalyptus brunneri in GenBank was 96.77%,96.76%,95.35%,and 95.35%,respectively.Among 184 serum samples from Yadong,the positivity rates of Lyme disease,Q fever,spotted fever,and tularemia were 6.52%,12.50%,8.70%,and 10.87%,respectively.These results indicated that the 23 parasitic ticks collected in Yadong,Tibet,in July 2021 were Ixodes nuttallianus,Ixodes ovatus and Haemaphysalis ti-betensis,and 23 ticks were infected with SFGR.Lyme disease,Q fever,spotted fever,and tularemia infection were prevalent in the population of Yadong.Therefore,efforts to control tick-borne diseases must be strengthened.

6.Wutou Chishizhi Wan Regulates Autophagy and PI3K/Akt/GSK-3β Signaling Pathway in Rats with Myocardial Ischemia-reperfusion Injury

ObjectiveTo explore the mechanism of Wutou Chishizhi Wan in regulating autophagy and phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt)/glycogen synthase kinase-3β (GSK-3β) signaling pathway in rats with myocardial ischemia-reperfusion injury (MIRI). MethodSixty male SD rats were randomly assigned into the normal group (normal saline), model group (normal saline), positive control (trimetazidine, 5.4 mg·kg-1) group, and low-, medium-, and high-dose (1.63, 4.9, 14.7 g·kg-1, respectively) Wutou Chishizhi Wan groups, with 10 rats in each group. The rats in other groups except the normal group underwent left anterior descending coronary artery ligation for modeling. Electrocardiogram was employed to detect the ST-segment elevation to evaluate the modeling. Hematoxylin-eosin (HE) staining was performed to reveal the damage of myocardial tissue. The levels of aspartate aminotransferase (AST) and creatine kinase (CK) were determined by colorimetry, and those of cardiac troponin T (cTnT) and myoglobin (MYO) by enzyme-linked immunosorbent assay (ELISA). Western blot was carried out to determine the protein levels of microtubule-associated proteins 1 light chain 3 (LC3), autophagy-related gene Beclin-1, PI3K, Akt, GSK-3β, p-GSK-3β, and p-Akt. ResultCompared with the normal group, the modeling elevated the serum levels of AST, CK, cTnT, and MYO (P<0.01), destroyed the arrangement of myocardial cells abd nucle, twisted and broken myocardial fibers, up-regulated the protein levels of LC3Ⅱ/Ⅰ and Beclin-1 (P<0.01), and down-regulated the protein levels of PI3K, p-Akt, and p-GSK-3β (P<0.01). Compared with the model group, trimetazidine and Wutou Chishizhi Wan (all the doses) lowered the levels of AST, CK, cTnT, and MYO in serum (P<0.01), restored the arrangement of myocardial cells and muscle fibers, reduced necrosis, down-regulated the protein level of Beclin-1 (P<0.01), and up-regulated the protein levels of PI3K, p-Akt, and p-GSK-3β (P<0.01). Additionally, Wutou Chishizhi Wan (all the doses) down-regulated the protein level of LC3Ⅱ/Ⅰ (P<0.05, P<0.01). Compared with those in the trimetazidine group, the serum AST level rose in the low-dose Wutou Chishizhi Wan group (P<0.05) and declined in the high-dose group (P<0.01), and the protein level of Beclin-1 was down-regulated in the medium-dose group (P<0.01). Additionally, the trimetazidine group had higher protein level of LC3Ⅱ/Ⅰ than medium- and high-dose Wutou Chishizhi Wan groups (P<0.05, P<0.01), higher protein level of PI3K than low-, medium-, and high-dose groups (P<0.01), lower protein level of p-Akt than low- and medium-dose groups (P<0.01), and higher p-GSK-3β protein level than the medium-dose group (P<0.01). ConclusionDifferent doses of Wutou Chishizhi Wan can ameliorate MIRI, and the high dose has the best effect. Wutou Chishizhi Wan can reduce the activity of myocardial injury markers AST, CK, cTnT, and MYO, and alleviate the pathological damage of myocardial tissue. It can down-regulate the protein levels of beclin-1, LC3Ⅱ/Ⅰ, and up-regulate those of PI3K, p-Akt, and p-GSK-3β. In summary, Wutou Chishizhi Wan may inhibit excessive autophagy and regulate the PI3K/Akt/GSK-3β signaling pathway to exert protective effect on MIRI rats.
7.Processing theory of "leading vinegar-processing Chinese medicine into liver".

The processing of Chinese medicine is a unique and dialectical treatment of traditional Chinese medicine in clinic.The processing theory of "leading vinegar-processing Chinese medicine into liver" is one of the traditional processing theories of Chinese medicine.The vinegar-processing Chinese medicine under the guidance of the processing theory typically reflects the characteristics of "reducing toxicity and enhancing efficacy" of the processing of Chinese medicine.This paper traced the origin and discussed the connotation of the traditional theory of "leading vinegar-processing Chinese medicine into liver".Combined with the research status of "lea-ding vinegar-processing Chinese medicine into liver", this paper explored the mechanism of "leading vinegar-processing Chinese medicine into liver" from the aspects of material basis, medicine effect, and traditional Chinese medicine(TCM) meridian, and analyzed the existing problems in the current research.This paper reviewed the modern study on reducing toxicity and enhancing efficacy of vinegar-processing Chinese medicine, and deeply explored the scientific connotation of the traditional processing theory of "leading vinegar-processing Chinese medicine into liver".At the same time, the research trend and idea of the effect mechanism of "leading vinegar-processing Chinese medicine into liver" based on the Quality markers(Q-Marker) of TCM, biological targets, and clinical prescriptions were put forward, providing references for the further study on "leading vinegar-processing Chinese medicine into liver".This paper also provided a scientific basis for the rational selection of processed products in TCM clinical practice.

8.Identification of chemical components based on UPLC-Q-TOF-MS/MS and network pharmacology of Zhuru Decoction.

This study analyzed the main chemical components of Zhuru Decoction via ultra-performance liquid chromatography coupled with quadrupole time-of-flight tandem mass spectrometry(UPLC-Q-TOF-MS/MS), and then predicted the mechanism of Zhuru Decoction in clearing heat, resolving phlegm, detoxifying, and treating vomiting and alcohol-related vomiting caused by heat in stomach based on network pharmacology. The gradient elution was conducted in Agilent ZORBAX extend-C_(18) column(2.1 mm×100 mm, 1.8 μm) with the mobile phase of 0.1% formic acid aqueous solution(A)-acetonitrile(B) at a flow rate of 0.3 mL·min~(-1) and the column temperature of 35 ℃. The MS adopted the positive and negative ion mode of electrospray ionization(ESI), and the data were collected in the scanning range of m/z 100-1 500. A total of 98 compounds in Zhuru Decoction were identified via BATMAN, SYMMAP, TCMSP, and relevant literature, including 36 flavonoids, 7 triterpenoids, 8 gingerols, 20 organic acids, 5 amino acids, and 22 other compounds. On the basis of the available studies, 9 components were selected as index components, and the protein-protein interaction(PPI) network of the common targets was established with STRING 11.0. Finally, 10 core targets associated with the pharmacodynamic effect were screened out. This study established the UPLC-Q-TOF-MS/MS method for identifying the chemical components in the classic prescription Zhuru Decoction, and employed network pharmacology to explore the core targets of its efficacy, which provided a refe-rence for the quality control and the research of the pharmacodynamic substances of Zhuru Decoction.

Objective: To analyze the clinical epidemiological characteristics including composition of pathogens , clinical characteristics, and disease prognosis acute bacterial meningitis (ABM) in Chinese children. Methods: A retrospective analysis was performed on the clinical and laboratory data of 1 610 children <15 years of age with ABM in 33 tertiary hospitals in China from January 2019 to December 2020. Patients were divided into different groups according to age,<28 days group, 28 days to <3 months group, 3 months to <1 year group, 1-<5 years of age group, 5-<15 years of age group; etiology confirmed group and clinically diagnosed group according to etiology diagnosis. Non-numeric variables were analyzed with the Chi-square test or Fisher's exact test, while non-normal distrituction numeric variables were compared with nonparametric test. Results: Among 1 610 children with ABM, 955 were male and 650 were female (5 cases were not provided with gender information), and the age of onset was 1.5 (0.5, 5.5) months. There were 588 cases age from <28 days, 462 cases age from 28 days to <3 months, 302 cases age from 3 months to <1 year of age group, 156 cases in the 1-<5 years of age and 101 cases in the 5-<15 years of age. The detection rates were 38.8% (95/245) and 31.5% (70/222) of Escherichia coli and 27.8% (68/245) and 35.1% (78/222) of Streptococcus agalactiae in infants younger than 28 days of age and 28 days to 3 months of age; the detection rates of Streptococcus pneumonia, Escherichia coli, and Streptococcus agalactiae were 34.3% (61/178), 14.0% (25/178) and 13.5% (24/178) in the 3 months of age to <1 year of age group; the dominant pathogens were Streptococcus pneumoniae and the detection rate were 67.9% (74/109) and 44.4% (16/36) in the 1-<5 years of age and 5-<15 years of age . There were 9.7% (19/195) strains of Escherichia coli producing ultra-broad-spectrum β-lactamases. The positive rates of cerebrospinal fluid (CSF) culture and blood culture were 32.2% (515/1 598) and 25.0% (400/1 598), while 38.2% (126/330)and 25.3% (21/83) in CSF metagenomics next generation sequencing and Streptococcus pneumoniae antigen detection. There were 4.3% (32/790) cases of which CSF white blood cell counts were normal in etiology confirmed group. Among 1 610 children with ABM, main intracranial imaging complications were subdural effusion and (or) empyema in 349 cases (21.7%), hydrocephalus in 233 cases (14.5%), brain abscess in 178 cases (11.1%), and other cerebrovascular diseases, including encephalomalacia, cerebral infarction, and encephalatrophy, in 174 cases (10.8%). Among the 166 cases (10.3%) with unfavorable outcome, 32 cases (2.0%) died among whom 24 cases died before 1 year of age, and 37 cases (2.3%) had recurrence among whom 25 cases had recurrence within 3 weeks. The incidences of subdural effusion and (or) empyema, brain abscess and ependymitis in the etiology confirmed group were significantly higher than those in the clinically diagnosed group (26.2% (207/790) vs. 17.3% (142/820), 13.0% (103/790) vs. 9.1% (75/820), 4.6% (36/790) vs. 2.7% (22/820), χ2=18.71, 6.20, 4.07, all P<0.05), but there was no significant difference in the unfavorable outcomes, mortility, and recurrence between these 2 groups (all P>0.05). Conclusions: The onset age of ABM in children is usually within 1 year of age, especially <3 months. The common pathogens in infants <3 months of age are Escherichia coli and Streptococcus agalactiae, and the dominant pathogen in infant ≥3 months is Streptococcus pneumoniae. Subdural effusion and (or) empyema and hydrocephalus are common complications. ABM should not be excluded even if CSF white blood cell counts is within normal range. Standardized bacteriological examination should be paid more attention to increase the pathogenic detection rate. Non-culture CSF detection methods may facilitate the pathogenic diagnosis.

Molecular pharmacognosy is a science of classification and identification, cultivation and protection, and production of active ingredients of graduated drugs at the molecular level. The proposal of molecular pharmacognosy allows the research of crude drugs to advance from the microscopic level to the genetic level. Pueraria lobata root, as a medicinal and edible plant, has high application value and economic value. There are many varieties that are easy to cause confusion, and it is not easy to distinguish and identify according to traditional identification methods. Moreover, the research of P. lobate root at the genetic level is still relatively shallow. the study received extensive attention of scholars. This article reviews recent research on molecular identification of P. lobate, transcriptome sequencing, cloning and synthesis of functional genes of P. lobate root in recent years in order to provide references for further promoting the development and utilization of P. lobate root and its active ingredients.
