Objective To determine the prognostic biomarkers and new therapeutic targets of the lung adenocarcinoma (LUAD), based on which to establish a prediction model for the survival of LUAD patients. Methods An integrative analysis was conducted on gene expression and clinicopathologic data of LUAD, which were obtained from the UCSC database. Subsequently, various methods, including screening of differentially expressed genes (DEGs), Gene Ontology (GO), Kyoto Encyclopedia of Genes and Genomes (KEGG) analysis and Gene Set Enrichment Analysis (GSEA), were employed to analyze the data. Cox regression and least absolute shrinkage and selection operator (LASSO) regression were used to establish an assessment model. Based on this model, we constructed a nomogram to predict the probable survival of LUAD patients at different time points (1-year, 2-year, 3-year, 5-year, and 10-year). Finally, we evaluated the predictive ability of our model using Kaplan-Meier survival curves, receiver operating characteristic (ROC) curves, and time-dependent ROC curves. The validation group further verified the prognostic value of the model. Results The different-grade pathological subtypes' DEGs were mainly enriched in biological processes such as metabolism of xenobiotics by cytochrome P450, natural killer cell-mediated cytotoxicity, antigen processing and presentation, and regulation of enzyme activity, which were closely related to tumor development. Through Cox regression and LASSO regression, we constructed a reliable prediction model consisting of a five-gene panel (MELTF, MAGEA1, FGF19, DKK4, C14ORF105). The model demonstrated excellent specificity and sensitivity in ROC curves, with an area under the curve (AUC) of 0.675. The time-dependent ROC analysis revealed AUC values of 0.893, 0.713, and 0.632 for 1-year, 3-year, and 5-year survival, respectively. The advantage of the model was also verified in the validation group. Additionally, we developed a nomogram that accurately predicted survival, as demonstrated by calibration curves and C-index. Conclusion We have developed a prognostic prediction model for LUAD consisting of five genes. This novel approach offers clinical practitioners a personalized tool for making informed decisions regarding the prognosis of their patients.

BackgroundThe existing tools in China for assessing psychiatric nurses' knowledge, attitude and practice towards the use of protective restraint do not sufficiently consider the specialty in psychiatric practice, and the scale items are somewhat cumbersome to use, which together restrict their further promotion and application. Accordingly, there is an urgent need for developing a more scientific assessment tool. ObjectiveTo construct a scale for assessing psychiatric nurses' knowledge, attitude and practice towards the use of protective restraint and to verify its reliability and validity, so as to provide a generic tool for the assessment. MethodsBased on the theoretical framework of the knowledge-attitude-practice model, the preliminary scale was formed through systematic literature review, qualitative interview and Delphi method. From July to December 2021, a stratified sampling was utilized to select 729 psychiatric nurses from tertiary, secondary and primary (including unclassified medical institutions and grassroots hospitals) psychiatric hospitals in Guangdong Province. The formal scale was developed through item analysis, exploratory factor analysis and confirmatory factor analysis. The validity of the scale was evaluated by content validity and structure validity, and the reliability was verified by procedures including Cronbach's α coefficient, test-retest reliability, and split-half reliability. ResultsA total of 12 items of three dimensions (knowledge, attitude, practice) were included in the scale for assessing psychiatric nurses' knowledge, attitude and practice towards the use of protective restraint. The scale-level content validity index (S-CVI) was 0.941, and the item-level content validity index (I-CVI) ranged from 0.812 to 1.000. Exploratory factor analysis extracted three common factors, and the cumulative variance contribution rate was 62.948%. The confirmatory factor yielded adequate fit. Cronbach's α coefficient was found to be 0.887 for the scale, 0.819 for knowledge dimension, 0.842 for attitude dimension, and 0.831 for practice dimension. The split-half reliability and test-retest reliability for the scale were 0.712 and 0.922, respectively. ConclusionThe scale shows satisfactory reliability and validity, which can be used to assess psychiatric nurses' knowledge, attitude and practice towards the use of protective restraint. The expression level of NOD-like receptors(NLRP3)inflammasome is significantly elevated in breast cancer tissues, and a high level of NLRP3 inflammasomes is closely associated with breast cancer metastasis.Activation of NLRP3 inflammasome can induce the release of inflammatory factors into the tumor microenvironment(TME).These inflammatory factors,by signaling to cancer cells, reshape the TME to promote tumor growth and invasion, ultimately facilitating the process of epithelial-mesenchymal transition. This equips cancer cells with the ability to establish distant metastases and increase the formation of metastatic lesions. This review addresses the current research status and prospects of NLRP3 inflammasomes in the breast cancer TME and their role in breast cancer metastasis. The goal is to provide new insights for the study of breast cancer metastasis mechanisms and treatment strategies.
7.The Role and Mechanism of Bone Muscle Crosstalk in The Prevention and Treatment of Osteoscarcopenia

Progress in Biochemistry and Biophysics 2024;51(11):2936-2946
Osteoscarcopenia (OS) is a common degenerative syndrome in the elderly, which is caused by a decrease in both bone and muscle mass during the aging process, leading to osteoporosis and sarcopenia, a decrease in body balance, and a risk of falls and fractures, posing a serious threat to the quality of life and lifespan of the elderly. Osteoskeletal dystrophy increases with age, and its occurrence is higher in females than that in males. At present, there is no unified diagnostic standard, making it impossible to achieve early detection and intervention. The commonly used diagnostic methods include quantitative computed tomography (CT), magnetic resonance imaging (MRI), dual energy X-ray absorptiometry (DXA), muscle mass bioelectrical impedance analysis (BIA), as well as daily gait speed (UGS), short physical performance battery (SPPB), timed start test (TUG), and biochemical evaluation indicators to improve early diagnosis and screening. Due to the fact that both bones and muscles belong to the motor system, osteoporosis and sarcopenia share common pathogenic factors in genetics, endocrine, paracrine, and fat infiltration, which interact and regulate each other, inducing the occurrence of osteoscarcopenia. Osteoporosis and sarcopenia, two age-related diseases, share the same pathogenesis and regulatory pathways, as well as common drug targets. For example: somatostatin α‑actin-3, peroxisome proliferator activated receptor γ coactivation factor-1α (PGC-1α), myocyte enhancer factor-2 (MEF2C), sterol regulatory element binding transcription factor 1 (SREBF1), protoadhesion 7 (PCDH7) and methyltransferase like 21C (METTL21C), osteocalcin and bone derived bone factor gap junction connexin 43 (Cx43), growth hormone (GH), sex hormones, and diseases (such as tumors, diabetes, polycystic ovary syndrome, cardiovascular disease, anemia, disability, inflammatory disease), aging, nutrition, and poor living habits are closely related to osteosarcopenia. Osteoporosis is characterized by low bone mass and microstructural degeneration of bone tissue, while sarcopenia is characterized by loss of muscle mass, strength, and function, both of which often coexist in the elderly population. Exercise regulates muscle and skeletal cytokines such as myostatin (MSTN) and irisinβ‑aminoisobutyric acid (BAIBA), brain derived neutrophil factor (BDNF), interleukin, prostaglandin E2, Wnt, osteocalcin (OCN), and transforming growth factor‑β (TGF‑β) and receptor activator of NF-κB ligand (RANKL) interfere with each other to prevent and treat osteoscarcopenia. Wnt/β‑catenin signaling pathway can simultaneously regulate the growth and metabolism of bones and muscles, and promote osteoblast proliferation, maturation, and mineralization by increasing OPG/RANKL, which is beneficial for bone mass increase and induces proliferation of muscle satellite cells, stimulating and promoting increased muscle synthesis. NF‑κB pathway is the main regulatory factor for inflammation mediated muscle atrophy. Meanwhile, NF‑κB DNA can participate in RANKL inducing osteoclast differentiation in bone tissue, thereby reducing bone mass. Although exercise and nutrition can improve the symptoms of osteoporosis, they cannot be completely cured, and there are no specific drugs in clinical practice that can cure sarcopenia. Because osteoscarcopenia has a common crosstalk mechanism in the aging process, it is of great significance to prevent osteoscarcopenia by improving bone mass and muscle content through exercise, nutrition, and medication.

Objective To investigate the clinical features and prognosis of childhood acute lymphoblastic leukemia (ALL) with CREBBP gene mutation. Methods A retrospective analysis was performed for the clinical data of 14 ALL children with CREBBP gene mutation who were admitted to Children's Hospital of the First Affiliated Hospital of Zhengzhou University from January 2016 to December 2023. Results The ALL patients with CREBBP gene mutation accounted for 1.5% (14/963) among all children diagnosed with ALL during the same period of time, among whom there were 4 boys (29%) and 10 girls (71%),with a median age of 4 years and 3.5 months. All children had an immunological type of B-cell ALL and concurrent mutations in other genes including NRAS,KRAS,ETV6,FLT3,PAX5,SH2B3,CDKN2A,and CDKN2B,and 4 children had karyotype abnormality. All 14 children received induction therapy with the VDLP regimen, with a complete remission (CR) rate of 79% (11/14) after the first course of treatment. Three children experienced bone marrow recurrence alone, with a recurrence rate of 21% (3/14),among whom 1 child achieved CR after blinatumomab therapy and 2 received bridging hematopoietic stem cell transplantation after chemotherapy for recurrence. Among the 14 children,1 died due to treatment discontinuation and 13 achieved disease-free survival. The 5-year overall survival rate was 92%±7%,and the event-free survival rate was 73%±13%. Conclusions ALL with CREBBP gene mutation is more common in girls and has a low induction remission rate and a high recurrence rate, and it is often accompanied by other types of gene mutations and abnormal karyotypes. Most children with recurrence can achieve long-term survival after immunotherapy or hematopoietic stem cell transplantation.
