Purpose::Under-foot impact loadings can cause serious lower limb injuries in many activities, such as automobile collisions and underbody explosions to military vehicles. The present study aims to compare the biomechanical responses of the mainstream vehicle occupant dummies with the human body lower limb model and analyze their robustness and applicability for assessing lower limb injury risk in underfoot impact loading environments.Methods::The Hybrid III model, the test device for human occupant restraint (THOR) model, and a hybrid human body model with the human active lower limb model were adopted for under-foot impact analysis regarding different impact velocities and initial lower limb postures.Results::The results show that the 2 dummy models have larger peak tibial axial force and higher sensitivity to the impact velocities and initial postures than the human lower limb model. In particular, the Hybrid III dummy model presented extremely larger peak tibial axial forces than the human lower limb model. In the case of minimal difference in tibial axial force, Hybrid III's tibial axial force (7.5 KN) is still 312.5% that of human active lower limb's (2.4 KN). Even with closer peak tibial axial force values, the biomechanical response curve shapes of the THOR model show significant differences from the human lower limb model.Conclusion::Based on the present results, the Hybrid III dummy cannot be used to evaluate the lower limb injury risk in under-foot loading environments. In contrast, potential improvement in ankle biofidelity and related soft tissues of the THOR dummy can be implemented in the future for better applicability.

Herald of Medicine 2024;43(8):1259-1269
Objective To systematically evaluate the efficacy and safety of rituximab rapid desensitization therapy. Methods PubMed,Embase,Cochrane,Web of Science,Scopus,CNKI,Wanfang, and VIP databases were searched, and the search time frame was from the establishment of the database to February 2023.Two evaluators independently screened the literature, extracted data, and evaluated the quality of the included literature, and the resulting data were analyzed descriptively or statistically. The systematic review protocol was registered on PROSPERO(CRD:42022306557).Results Twenty-five studies(11 case reports and 14 case series)were included, with 138 patients desensitized,of whom 129 were successfully desensitized(i.e.,completed at least one target dose administration). The pretreatment regimen prior to desensitization was a combination of 2 or more of several classes of drugs, including H1 and H2 receptor antagonists, glucocorticoids, sedatives,NSAIDs,and acetaminophen, and 88%of the pretreatment regimens included H1 receptor antagonists or glucocorticoids.Sixteen studies(64.0%)reported on skin testing prior to desensitization, with positive rates of 12.50%for skin prick tests(n=16)and 49.15%for intradermal tests(n=59).Seventeen studies(68.0%)used the 3 bags-12 steps rapid desensitization protocol, the rest were for multiple desensitization methods such as 4 bags-16 steps or 2 bags-8 steps. The initial administration concentration of rituximab(i.e.,the concentration of the first desensitizing solution)was 1/10 X-1/10,000 X in most of the included studies, the first step infusion rate was 1.5-5 mL·min-1,and the total duration of single desensitization was approximately 4-10.5 h.Twenty-three studies(92%)reported the occurrence of anaphylactic reactions during patient desensitization,of which five studies had no anaphylactic reactions at the time of desensitization, and the majority of patients in the other 18 studies were able to continue to complete desensitization after being given symptomatic treatment or a modified desensitization regimen. Conclusions Current evidence suggests that rituximab rapid desensitization therapy has certain effectiveness and safety. However, there is still a lack of high-quality evidence or execution standards with strong operability and more large-sample,multicenter studies are needed to further explore the aspects of skin testing before desensitization, pretreatment, individualized desensitization steps, and management of allergic reactions during desensitization.

Objective This study aimed to investigate the potential relationship between urinary metals copper (Cu), arsenic (As), strontium (Sr), barium (Ba), iron (Fe), lead (Pb) and manganese (Mn) and grip strength. Methods We used linear regression models, quantile g-computation and Bayesian kernel machine regression (BKMR) to assess the relationship between metals and grip strength. Results In the multimetal linear regression, Cu (β=-2.119), As (β=-1.318), Sr (β=-2.480), Ba (β=0.781), Fe (β= 1.130) and Mn (β=-0.404) were significantly correlated with grip strength (P < 0.05). The results of the quantile g-computation showed that the risk of occurrence of grip strength reduction was -1.007 (95% confidence interval:-1.362, -0.652; P < 0.001) when each quartile of the mixture of the seven metals was increased. Bayesian kernel function regression model analysis showed that mixtures of the seven metals had a negative overall effect on grip strength, with Cu, As and Sr being negatively associated with grip strength levels. In the total population, potential interactions were observed between As and Mn and between Cu and Mn (Pinteractions of 0.003 and 0.018, respectively). Conclusion In summary, this study suggests that combined exposure to metal mixtures is negatively associated with grip strength. Cu, Sr and As were negatively correlated with grip strength levels, and there were potential interactions between As and Mn and between Cu and Mn.

OBJECTIVES:
To investigate the difference in intestinal microbiota between preterm infants with neurodevelopmental impairment (NDI) and those without NDI.
METHODS:
In this prospective cohort study, the preterm infants who were admitted to the neonatal intensive care unit of Maternal and Child Health Hospital of Guangxi Zhuang Autonomous Region from September 1, 2019 to September 30, 2021 were enrolled as subjects. According to the assessment results of Gesell Developmental Scale at the corrected gestational age of 1.5-2 years, they were divided into two groups: normal (n=115) and NDI (n=100). Fecal samples were collected one day before discharge, one day before introducing solid food, and at the corrected gestational age of 1 year. High-throughput sequencing was used to compare the composition of intestinal microbiota between groups.
RESULTS:
Compared with the normal group, the NDI group had a significantly higher Shannon diversity index at the corrected gestational age of 1 year (P<0.05). The principal coordinate analysis showed a significant difference in the composition of intestinal microbiota between the two groups one day before introducing solid food and at the corrected gestational age of 1 year (P<0.05). Compared with the normal group, the NDI group had a significantly higher abundance of Bifidobacterium in the intestine at all three time points, a significantly higher abundance of Enterococcus one day before introducing solid food and at the corrected gestational age of 1 year, and a significantly lower abundance of Akkermansia one day before introducing solid food (P<0.05).
CONCLUSIONS
There are significant differences in the composition of intestinal microbiota between preterm infants with NDI and those without NDI. This study enriches the data on the characteristics of intestinal microbiota in preterm infants with NDI and provides reference for the microbiota therapy and intervention for NDI in preterm infants.
