1.Panax notoginseng saponins regulate differential miRNA expression in osteoclast exosomes and inhibit ferroptosis in osteoblasts

		                        			BACKGROUND:Steroid-induced femoral head necrosis is mostly caused by long-term and extensive use of hormones,but its specific pathogenesis is not yet clear and needs further study. OBJECTIVE:To screen out the differential miRNAs in osteoclast exosomes after the intervention of Panax notoginseng saponins,and on this basis,to further construct an osteogenic-related ferroptosis regulatory network to explore the potential mechanism and research direction of steroid-induced osteonecrosis of the femoral head. METHODS:MTT assay was used to detect the toxic effects of different concentrations of dexamethasone and different mass concentrations of Panax notoginseng saponins on Raw264.7 cell line.Tartrate resistant acid phosphatase staining and TUNEL assay were used to detect the effects of Panax notoginseng saponins on osteoclast inhibition and apoptosis.Exosomes were extracted from cultured osteoclasts with Panax notoginseng saponins intervention.Exosomes from different groups were sequenced to identify differentially expressed miRNAs.CytoScape 3.9.1 was used to construct and visualize the regulatory network between differentially expressed miRNAs and mRNAs.Candidate mRNAs were screened by GO analysis and KEGG analysis.Finally,the differential genes related to ferroptosis were screened out,and the regulatory network of ferroptosis-related genes was constructed. RESULTS AND CONCLUSION:(1)The concentration of dexamethasone(0.1 μmol/L)and Panax notoginseng saponins(1 736.85 μg/mL)suitable for intervention of Raw264.7 cells was determined by MTT assay.(2)Panax notoginseng saponins had an inhibitory effect on osteoclasts and could promote their apoptosis.(3)Totally 20 differentially expressed miRNAs were identified from osteoclast-derived exosome samples,and 11 differentially expressed miRNAs related to osteogenesis were predicted by target mRNAs.The regulatory networks of 4 up-regulated differentially expressed miRNAs corresponding to 155 down-regulated candidate mRNAs and 7 down-regulated differentially expressed miRNAs corresponding to 238 up-regulated candidate mRNAs were constructed.(4)Twenty-four genes related to ferroptosis were screened out from the differential genes.Finally,12 networks were constructed(miR-98-5p/PTGS2,miR-23b-3p/PTGS2,miR-425-5p/TFRC,miR-133a-3p/TFRC,miR-185-5p/TFRC,miR-23b-3p/NFE2L2,miR-23b-3p/LAMP2,miR-98-5p/LAMP2,miR-182-5p/LAMP2,miR-182-5p/TLR4,miR-23b-3p/ZFP36,and miR-182-5p/ZFP36).These results indicate that Panax notoginseng saponins may regulate osteoblast ferroptosis by regulating the expression of miRNAs derived from osteoclast exosomes,thus providing a new idea for the study of the mechanism of steroid-induced femoral head necrosis.
		                        		
		                        		
		                        		
		                        	
2.Endothelial progenitor cell and mesenchymal stem cell therapy for vascular stent-associated diseases

		                        			BACKGROUND:With advancements in stem cell research,the therapeutic efficacy of adult stem cells such as endothelial progenitor cells and mesenchymal stem cells in atherosclerosis and complications arising from atherosclerosis and vascular stent implantation is gradually being recognized.Due to the limitations of intravenous infusion of adult stem cells,including poor targeting and low treatment efficiency,recent research has focused on surface modification of vascular stents to achieve localized aggregation and functional modulation of endothelial progenitor cells or mesenchymal stem cells. OBJECTIVE:To discuss the therapeutic progress of endothelial progenitor cells and mesenchymal stem cells in vascular stent-related diseases,summarize the research status of the design of vascular stents based on endothelial progenitor cells and mesenchymal stem cells. METHODS:Relevant literature was retrieved on CNKI,WanFang,PubMed,and Web of Science databases since their inception.The Chinese search terms were"endothelial injury,stenting,thrombosis,intimal hyperplasia,atherosclerosis,endothelial repair,endothelial progenitor cell,mesenchymal stem cell,vascular stent."English search terms were"endothelial injury,stenting,thrombosis,intimal hyperplasia,atherosclerosis,endothelial repair,endothelial regeneration,endothelial progenitor cell,mesenchymal stem cell,vascular stent,vascular scaffold."According to inclusion and exclusion criteria,127 articles were finally reviewed. RESULTS AND CONCLUSION:Endothelial progenitor cells and mesenchymal stem cells can treat atherosclerosis and complications of stent implantation through differentiation and paracrine effects,mainly by protecting endothelial cells,regulating the expression of inflammatory cells and cytokines,and modulating smooth muscle cell proliferation and phenotype.Mesenchymal stem cells may have adverse reactions such as thrombosis and vascular calcification in therapeutic applications,and using extracellular vesicles and co-administration with heparin for surface modification is a feasible solution.Currently,there is more research on stents based on endothelial progenitor cells,mainly focusing on recruitment,capture,proliferation,differentiation,and activity.Research on stents based on mesenchymal stem cell capture in the vascular field is relatively scarce,but exosome-loaded stents derived from mesenchymal stem cells have been found to have highly effective therapeutic efficacy.Additionally,some underlying diseases such as diabetes may affect the activity of adult stem cells,leading to the loss of effectiveness in stem cell-based stent designs.Therefore,in future stent designs,consideration should be given to the background diseases.
		                        		
		                        		
		                        		
		                        	
3.Effects of Different Modes in Hypoxic Training on Metabolic Improvements in Obese Individuals: a Systematic Review With Meta-analysis on Randomized Controlled Trail

		                        			This paper aimed to systematically evaluate the effects of hypoxic training at different fraction of inspired oxygen (FiO2) on body composition, glucose metabolism, and lipid metabolism in obese individuals, and to determine the optimal oxygen concentration range to provide scientific evidence for personalized and precise hypoxic exercise prescriptions. A systematic search was conducted in the Cochrane Library, PubMed, Web of Science, Embase, and CNKI databases for randomized controlled trials and pre-post intervention studies published up to March 31, 2025, involving hypoxic training interventions in obese populations. Meta-analysis was performed using RevMan 5.4 software to assess the effects of different fraction of inspired oxygen (FiO2≤14% vs. FiO2>14%) on BMI, body fat percentage, waist circumference, fasting blood glucose, insulin, HOMA-IR, triglycerides (TG), low-density lipoprotein cholesterol (LDL-C), and high-density lipoprotein cholesterol (HDL-C), with subgroup analyses based on oxygen concentration. A total of 22 studies involving 292 participants were included. Meta-analysis showed that hypoxic training significantly reduced BMI (mean difference (MD)=-2.29,95%CI: -3.42 to -1.17, P<0.000 1), body fat percentage (MD=-2.32, 95%CI: -3.16 to -1.47, P<0.001), waist circumference (MD=-3.79, 95%CI: -6.73 to -0.85, P=0.01), fasting blood glucose (MD=-3.58, 95%CI: -6.23 to -0.93, P=0.008), insulin (MD=-1.60, 95%CI: -2.98 to -0.22, P=0.02), TG (MD=-0.18, 95%CI: -0.25 to -0.12, P<0.001), and LDL-C (MD=-0.25, 95%CI: -0.39 to -0.11, P=0.000 3). Greater improvements were observed under moderate hypoxic conditions with FiO2>14%. Changes in HOMA-IR (MD=-0.74, 95%CI: -1.52 to 0.04,P=0.06) and HDL-C (MD=-0.09, 95%CI: -0.21 to 0.02, P=0.11) were not statistically significant. Hypoxic training can significantly improve body composition, glucose metabolism, and lipid metabolism indicators in obese individuals, with greater benefits observed under moderate hypoxia (FiO>14%). 4.A Survey on the Perceived Experience and Acceptance of Intrapartum Ultrasound as a Novel Method for Labor Progress Assessment

		                        			ObjectiveTo investigate the perceived experience and acceptance of intrapartum ultrasound (IPUS) as a novel method for labor progress assessment among pregnant women. MethodsFrom February 2023 to December 2024, a total of 180 pregnant women admitted to the Labor Ward of Lingnan Hospital, the Third Affiliated Hospital of Sun Yat-sen University, who were planned for vaginal trial of labor , were accessed for labor progress using IPUS and vaginal examination (VE) after the onset of labor and prior to the initiation n of labor analgesia. A self-designed questionnaire was used to investigate the women's perceived experiences with both examination methods and their acceptance of IPUS. The pain intensity associated with the examinations was evaluated using the visual analogue pain scale (VAS). Differences in the women's experiences and pain intensity between the two labor progress assessment methods were compared. ResultsThe acceptance rate of IPUS was 96.67% (174/180), with the remaining 6 cases undecided. Over 60% of the pregnant women reported IPUS assessment as comfortable and none of them felt discomfort, whereas 32.8% felt uncomfortable with VE (χ2=196.02, P<0.001). Nearly two-thirds of the pregnant women believed that VE would cause psychological distress, while none reported such effect with IPUS (χ2=261.52, P<0.001). Approximately 77.78% (140/180) of the pregnant women believed that IPUS could reduce their fear of vaginal delivery and enhance their confidence if it replaced VE. The VAS score for IPUS [0 (0, 2)] was significantly lower than that for VE [4 (4, 6)] (Z=-14.62, P<0.001). Further stratified analysis showed that over 90% (164/180) of the pregnant women found IPUS painless, with no moderate or severe pain reported, compared to 43.33% (78/180) experienced moderate or severe pain with VE (P<0.001). ConclusionAs a novel approach for labor progress assessment, IPUS not only alleviates the pain and discomfort associated with traditional VE and reduces the fear of childbirth but also enhances women's confidence in delivery, thereby achieving a high level of acceptance among parturient women in China. 
		                        		
		                        		
		                        		
		                        	
5.Identification of chemical components and determination of vitexin in the raw powder of Tongluo Shenggu capsule

		                        			The present study employed UPLC-MS/MS to analyze and identify compounds in the raw powder of Tongluo Shenggu capsules. An HPLC method for the determination of vitexin content was established. The analysis of this drug was performed on a 30 ℃ thermostatic Acquity UPLC® BEH C18 (2.1 mm×100 mm,1.7 μm) column, with the mobile phase comprising 0.2% formic acid-methanol flowing at 0.3 mL /min in a gradient elution manner. Mass spectrometry was detected by ESI sources in both positive and negative ion modes for qualitative identification of chemical constituents. 12 flavonoid and 3 stilbenes compounds in the raw powder of Tongluo Shenggu capsules were successfully identified. Additionally, an HPLC method for the determination of vitexin content was established using a XBridge C18 column (4.6 mm × 250 mm, 5 µm) with a mobile phase of 0.05% glacial acetic acid in methanol for gradient elution, at a column temperature of 30 °C, a flow rate of 1.0 mL/min, and an injection volume of 20 μL. The method demonstrated good linearity in the concentration range of 10 µg/mL to 40 µg/mL (R=1.000) with an average recovery rate of 96.7%. 6.Effect of Baduanjin on mood and sleep quality in patients with mild to moderate Parkinson disease

		                        			Objective To investigate the effect of Baduanjin on mood and sleep quality in patients with mild to moderate Parkinson disease (PD) and related mechanisms. Methods A total of 110 patients with Hoehn-Yahr stage 1-3 stable PD were randomly divided into Baduanjin group and control group, with 55 patients in each group. The patients in the Baduanjin group received Baduanjin exercise for 30 minutes each time, 5 days a week for 12 weeks, and those in the control group did not do any exercise. Motor function, anxiety and depression mood, and sleep quality were assessed before exercise and after exercise for 12 weeks. Results Compared with the control group, the Baduanjin group had significant improvements in UPDRS-Ⅲ score and 6-minute walk test results. There was a significant difference in Berg Balance Scale in terms of the interaction between time and intervention. There were significant differences between the two groups in HAMA14,HAMD24,and PSQI scores. 7.Safety and efficacy of puncture cyanoacrylate selective seal under endoscopic ultrasound versus traditional endoscopy in treatment of gastroesophageal varices: A randomized controlled trial

		                        			ObjectiveTo investigate the safety and efficacy of puncture cyanoacrylate selective seal (PCSS) under endoscopic ultrasound in the treatment of gastroesophageal varices (GOV). MethodsA total of 100 patients with liver cirrhosis who underwent endoscopic therapy for the secondary prevention of GOV bleeding in Beijing Ditan Hospital, Capital Medical University, from March 1 to December 31, 2023 were enrolled and randomly divided into PCSS group and traditional endoscopy group. The patients were followed up for 6 months after surgery, and the two groups were compared in terms of clinical outcome and complications. The primary outcome measure was the rate of alleviation or disappearance of GOV, and the secondary outcome measure was variceal rebleeding and death. The independent-samples t test was used for comparison of normally distributed or approximately normally distributed quantitative data between two groups, and the Wilcoxon non-parametric test was used for comparison of non-normally distributed quantitative data between two groups; the chi-square test or the Fisher’s exact test was used for comparison of qualitative data between two groups. ResultsThere were 50 patients in the PCSS group, among whom 1 patient was lost to follow-up, and there were 50 patients in the traditional endoscopy group, among whom 3 patients were lost to follow-up. There were no significant differences between the two groups in baseline data such as age, sex, Child-Pugh class, varices grade, and GOV typing (all P>0.05). Compared with the traditional endoscopy group, the PCSS group had significantly better results of the number of endoscopic treatment sessions (t=-15.671, P=0.001), the total amount of tissue adhesive used (t=-2.830, P=0.006), and the rate of alleviation or eradication of varices sclerosis (χ2=7.078, P=0.029). Both groups had low rates of postoperative rebleeding, adverse reactions, and complications, and there were no significant differences between the two groups (all P>0.05). ConclusionCompared with traditional endoscopy, PCSS can significantly enhance treatment outcome while maintaining safety standards. 
		                        		
		                        		
		                        		
		                        	
8.Fast Object Perception in The Subcortical Pathway: a Commentary on Wang et al.’s Paper in Human Brain Mapping (2023)

		                        			The subcortical visual pathway is generally thought to be involved in dangerous information processing, such as fear processing and defensive behavior. A recent study, published in Human Brain Mapping, shows a new function of the subcortical pathway involved in the fast processing of non-emotional object perception. Rapid object processing is a critical function of visual system. Topological perception theory proposes that the initial perception of objects begins with the extraction of topological property (TP). However, the mechanism of rapid TP processing remains unclear. The researchers investigated the subcortical mechanism of TP processing with transcranial magnetic stimulation (TMS). They find that a subcortical magnocellular pathway is responsible for the early processing of TP, and this subcortical processing of TP accelerates object recognition. Based on their findings, we propose a novel training approach called subcortical magnocellular pathway training (SMPT), aimed at improving the efficiency of the subcortical M pathway to restore visual and attentional functions in disorders associated with subcortical pathway dysfunction. 
		                        		
		                        		
		                        		
		                        	
9.Effects and mechanism of Tianma xiongling zhixuan tablet on autophagy of vascular endothelial cells

		                        			OBJECTIVE To explore the effects of Tianma xiongling zhixuan tablet on autophagy in vascular endothelial cells of rats and its potential mechanism. METHODS The rat aortic endothelial cells (RAECs) were divided into normal group, model group, blank serum group, traditional Chinese medicine (TCM) medicated serum group, autophagy blocker group, autophagy agonist group, and TCM combined with autophagy agonist group. Except for normal group, other groups were given 10 μg/mL lipopolysaccharide for 24 hours to induce RAECs inflammation injury model. Blank serum group was treated with 10% blank serum; TCM medicated serum group received 10% medicated serum derived from Tianma xiongling zhixuan tablet; autophagy blocker group was treated with 20 μmol/L of PD98059; autophagy agonist group was administered 50 μmol/L Honokiol. Lastly, the TCM combined with autophagy agonist group was given both 10% medicated serum derived from Tianma xiongling zhixuan tablet and 50 μmol/L Honokiol. The morphological characteristics of RAECs in each group were observed. The cell viability of each group, the contents of endothelin-1 (ET-1) and nitric oxide (NO), mitochondrial reactive oxygen species, mitochondrial membrane potential, and the expression levels of PTEN-induced kinase 1 (PINK1), Parkin, ubiquitin-binding protein (p62), and microtubule-associated protein 1 light chain 3 (LC3) were detected. RESULTS Compared with model group, the levels of ET-1, mitochondrial reactive oxygen species, and the relative expressions of PINK1, Parkin, and LC3 proteins in the autophagy blocker group and TCM medicated serum group were decreased or down-regulated significantly (P<0.05 or P<0.01); the cell viability rate (only autophagy blocker group), NO level, mitochondrial membrane potential, and the E-mail:46164660@qq.com relative expression level of p62 protein were increased or up-regulated significantly (P<0.05 or P<0.01); the pathological damage of RAECs was significantly improved, the number of cells increased significantly, and the typical paving stone-like characteristics were restored. The levels of ET-1, mitochondrial reactive oxygen species, and the relative expression levels of Parkin and LC3 proteins in the autophagy agonist group were increased or up-regulated significantly (P<0.05 or P<0.01), while cell viability rate was decreased significantly (P<0.05), the damage of RAECs was aggravated. Compared with the autophagy agonist group, the cell viability rate and the relative expression level of p62 protein in TCM combined autophagy agonist group were increased or up-regulated significantly (P<0.05 or P<0.01), while the levels of ET-1, the relative expression levels of PINK1, Parkin, and LC3 proteins were down-regulated significantly (P< 0.01), the damage of RAECs was reversed to a certain extent. CONCLUSIONS Tianma xiongling zhixuan tablet protects vascular endothelial function by regulating mitochondrial autophagy, the mechanism of which may be associated with the regulation of PINK1/Parkin signaling pathway and the inhibition of mitochondrial autophagy.
		                        		
		                        		
		                        		
		                        	
10.Treatment of Psoriasis from ''Fuyu'' Theory

		                        			Psoriasis is a chronic inflammatory skin disease with a polygenic genetic background. Its etiology remains unclear, and its pathogenesis is complex and refractory, collectively posing significant challenges in its treatment and greatly affecting the physical and mental health of patients. With the advantages of multi-target and multi-pathway treatment, traditional Chinese medicine has shown unique efficacy and value in the diagnosis and treatment of psoriasis. Modern doctors have a lot of discussion on psoriasis, and most of them tend to treat the disease by solving disorders of the blood system. They think that the disease is closely related to the "heat in the blood". Combining the clinical characteristics and accompanying symptoms of psoriasis, this article traced the causes of "heat in the blood" in psoriasis and believed that multiple internal and external factors have prevented the smooth circulation of Qi. Yang Qi Fuyu (stagnation) and transformation into heat and toxicity is the source of "heat in the blood" in psoriasis. Furthermore, it was proposed that "Fuyu" is the core pathogenesis of psoriasis. The etiology of "Fuyu" is complex, such as external wind and cold pathogens, emotional injuries, internal accumulation of dampness, and deficiency of healthy Qi, all of which can disrupt the ascending and descending movement of Qi, impede the circulation of Qi and fluids, close the pores and skin texture, and subsequently lead to stagnation. Based on the above understanding, "resolving stagnation" is crucial for treating the disease. Many doctors have explored the treatment ideas of psoriasis from the perspectives of dispelling wind, warming cold, regulating Qi, eliminating dampness, tonifying deficiency, and external treatment, aiming to remove the causes, promote the circulation of Qi and fluids, and resolve stagnation and heat. Clinical studies have shown that the therapies can relieve clinical symptoms, reduce recurrence rate, and improve quality of life, which also have good safety in the treatment of psoriasis. This article discussed the treatment of psoriasis from the perspective of "Fuyu", enriching the understanding of TCM regarding the etiology and pathogenesis of psoriasis.
