1.Application of robots in rehabilitation of children with autism spectrum disorder from 2004 to 2024: a bibliometrics analysis

ObjectiveTo analyze the current research status, hotspots and trends of robot applications in the rehabilitation of children with autism spectrum disorder (ASD). MethodsRelevant literature from the Web of Science Core Collection database from 2004 to 2024 was retrieved, and was analyzed with CiteSpace 6.3.R1. ResultsA total of 902 articles were included, with a fluctuating upward trend in publication volume. The United States was the country with the most publications, while the England with the highest centrality. The most prolific author was Ichiro Yoshikawa, and the institution with the most publications was the University of Hertfordshire. High-frequency keywords and bursting words in the field included rehabilitation robots, imitation ability, human-computer interaction, joint attention and humanoid robots. ConclusionResearch on robots in the rehabilitation of children with ASD is on the rise. Future research should focus on improving robot intelligence, implementing multimodal data collection and analysis, integrating brain science to uncover neural mechanisms during interactions, and developing personalized rehabilitation plans tailored to the specific needs of children.
2.Low diastolic blood pressure and adverse outcomes in inpatients with acute exacerbation of chronic obstructive pulmonary disease: A multicenter cohort study.

BACKGROUND:
Although intensively studied in patients with cardiovascular diseases (CVDs), the prognostic value of diastolic blood pressure (DBP) has little been elucidated in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). This study aimed to reveal the prognostic value of DBP in AECOPD patients.
METHODS:
Inpatients with AECOPD were prospectively enrolled from 10 medical centers in China between September 2017 and July 2021. DBP was measured on admission. The primary outcome was all-cause in-hospital mortality; invasive mechanical ventilation and intensive care unit (ICU) admission were secondary outcomes. Least absolute shrinkage and selection operator (LASSO) and multivariable Cox regressions were used to identify independent prognostic factors and calculate the hazard ratio (HR) and 95% confidence interval (CI) for adverse outcomes.
RESULTS:
Among 13,633 included patients with AECOPD, 197 (1.45%) died during their hospital stay. Multivariable Cox regression analysis showed that low DBP on admission (<70 mmHg) was associated with increased risk of in-hospital mortality (HR = 2.16, 95% CI: 1.53-3.05, Z = 4.37, P <0.01), invasive mechanical ventilation (HR = 1.65, 95% CI: 1.32-2.05, Z = 19.67, P <0.01), and ICU admission (HR = 1.45, 95% CI: 1.24-1.69, Z = 22.08, P <0.01) in the overall cohort. Similar findings were observed in subgroups with or without CVDs, except for invasive mechanical ventilation in the subgroup with CVDs. When DBP was further categorized in 5-mmHg increments from <50 mmHg to ≥100 mmHg, and 75 to <80 mmHg was taken as reference, HRs for in-hospital mortality increased almost linearly with decreased DBP in the overall cohort and subgroups of patients with CVDs; higher DBP was not associated with the risk of in-hospital mortality.
CONCLUSION:
Low on-admission DBP, particularly <70 mmHg, was associated with an increased risk of adverse outcomes among inpatients with AECOPD, with or without CVDs, which may serve as a convenient predictor of poor prognosis in these patients.

3.Transluminal repair of iatrogenic bladder fistula: a report of 7 cases

【Objective】 To explore the technical methods and clinical efficacy of transvaginal or transrectal repair in the treatment of iatrogenic bladder fistula. 【Methods】 The clinical data of 7 cases of iatrogenic bladder fistula patients treated during 2016 and 2019 were retrospectively analyzed, including 6 cases of vesicovaginal fistula (VVF) and 1 case of vesicorectal fistula (VRF). The operation was conducted 3 to 10 months after the diagnosis of urinary fistula, and the vagina or rectum was fully cleaned before operation. Modified Latzko technique was employed to separate the gap between the bladder wall and vaginal or rectal wall along the fistula, the fistula scar was sharply removed, and the fistula, bladder wall, vaginal or intestinal wall, and vaginal or intestinal mucosa were sutured in layers. The urinary catheter was indwelled for 4 weeks. 【Results】 All 7 cases were successfully repaired at one procedure. No urine leakage was found after the urinary catheter was removed. There was no recurrence after 6 to 12 months of follow-up. 【Conclusion】 Selective application of the modified Latzko technique to repair iatrogenic urinary fistula through the natural lumen is an advantageous treatment scheme, which simplifies the operation and reduces trauma.
4.Analysis of the clinicopathological characteristics of thymoma patients and the influencing factors for prognosis

Objective To analyze the clinicopathological characteristics of thymoma patients and the influencing factors for prognosis. Methods Thymoma patients who received treatment in Sichuan Cancer Hospital from March 2015 to March 2021 were collected. Clinical data of the patients were analyzed using Kaplan-Meier and Cox regression analyses. Results A total of 177 patients were included. There were 89 males and 88 females aged 17-88 (52.3±13.0) years, including 160 surgical patients and 17 non-surgical patients. There were 160 patients survived, 17 died of thymoma, and 5 had recurrence and metastasis. Overall, the 1-year, 3-year and 5-year progression-free survival rates were 94.4%, 88.7%, 88.1%, respectively; the 1-year, 3-year and 5-year overall survival rates were 94.9%, 91.5%, 91.0%, respectively. The Kaplan-Meier analysis showed that World Health Organization classification, clinical symptoms, Masaoka-Koga staging, treatment methods and surgery were statistically associated with progression-free survival; clinical symptoms, age, treatment methods and surgery were statistically associated with overall survival (P<0.05). Patients with younger age (P=0.018), without clinical symptoms (P=0.039), and with surgical treatment (P=0.004) had higher overall survival rates; those patients undergoing surgery had a higher progression-free survival rate (P=0.002). Conclusion Age, clinical symptoms and surgical treatment are independent factors influencing the prognosis of patients with thymoma.
5.Bibliometric Analysis of Off-label Drug Use in Dynamic Themes Evolution

OBJECTIVE To study of the dynamic evolution process of the themes of off-label drug use from a macroscopic perspective, perspective is needed to reveal the overall development vein of the field, and discover research hot spots and development trends of the field. METHODS A total of 3 876 literatures published in Web of Science from 2012 to 2022 on the topic of off-label drug use were used to obtain three visual maps of overlapping map, systematic evolution map and strategic map by using SciMAT analysis, and these maps of each period were analyzed. RESULTS Over the past ten years, there had been an increasing in the number of publications, themes, and keywords on the off-label drug use, with research themes from the former period having a large impact on the latter period, with complex evolutionary pathways and increasing evolutionary themes. Roughly, 14 evolutionary paths were formed in 4 research directions:adolescents and children, treatment, off label use, and drug RCT. CONCLUSION The content and scope of research in the field of off-label drug use will be diversified and developed, with greater focus on pediatric drug use, psychiatric drug use, COVID-19, and guidelines will be paid more attention.

7.Relationship between mental health status and coping styles among village poverty alleviation cadres in Leshan City

ObjectiveTo analyze the relationship between mental health status and coping styles among village poverty alleviation cadres in Leshan City, and to provide references for the improvement of mental health and coping styles of them. MethodsA cluster random sampling method was used to select 6 of the 11 districts and counties in Leshan City, and all the village poverty alleviation cadres in the selected areas were evaluated through the psychological cloud CT system. A total of 700 cadres participated in the questionnaire evaluation, and were assessed using Symptom Checklist 90 (SCL-90) and Coping Style Questionnaire (CSQ). Results①A total of 207 (35.9%) village poverty alleviation cadres factor scored above 2, and 92 (15.9%) cadres scored above 200. ②There was a statistically significant difference in SCL-90 obsessive-compulsive symptom factor score and other factor score between different poverty alleviation time groups (P<0.05 or 0.01), and the factor score of SCL-90 obsessive-compulsive symptom showed significant difference among village poverty-alleviation cadres of different genders (P<0.05). ③The scores of problem-solving factor in CSQ showed significant differences among village poverty alleviation cadres of different genders and different ages (P<0.01). ④Correlation analysis showed that the scores of each dimension and total score of SCL-90 of village poverty alleviation cadres in Leshan City were positively correlated with the score of self-blame factor in CSQ (r=0.423~0.521, P<0.01). ConclusionThe village poverty alleviation cadres of female and with a working length of less than 90 days or more than 360 days are at high risk of obsessive-compulsive symptoms, while the cadres of male and aged over 41 years adopt mature coping styles. Furthermore, the immature coping style of cadres will lead to serious mental health problems.
8.Summary of best evidence for chronic pain management in the elderly

Objective:To summarize the best evidence of chronic pain management in the elderly, and to provide reference for the clinical practice of chronic pain management in the elderly.Methods:Focusing on the problem of chronic pain in the elderly, evidence-based nursing method was used to search the relevant literature in domestic and foreign databases from the establishment of the database to June 2, 2020. Two researchers evaluated the quality of various studies and level of evidence recommendation by the JBI evidence-based health care center's literature quality evaluation standard and evidence recommendation system, and extracted evidences from the literature meeting the inclusion criteria.Results:A total of 20 articles were included with 8 guides, 3 systematic reviews, 3 expert consensus, 1 best decision, 1 position statement and 4 evidence summaries. Finally, 20 pieces of best evidence were formed, including general principles of pain management, pain assessment, intervention strategies, self-management, education and training.Conclusions:This study summarizes the current literature on chronic pain in the elderly, which provides evidence-based basis for the management of chronic pain in the elderly. Clinicians should use the evidence according to the best clinical evidence and the actual medical conditions of the hospital, in order to improve the quality of nursing.
9.Analysis of Risk Factors of SkinAdverse Reactions by Intravenous Injection of Iodine ContrastAgent

OBJECTIVE:To investiga te main risk factors for adverse drug reactions (ADR)of skin by intravenous injection of iodine contrast agent. METHODS :From Jan. 2009 to Apr. 2020,the patients suffering from skin ADR after enhanced CT with iodine contrast agent were collected from our hospital. The basic information ,laboratory test results before using iodine contrast agent and ADR related information were collected through hospital information system (HIS). The use of iodine contrast agent ,main manifestations of skin ADR and drug combination were analyzed statistically. Taking the sex ,age,body mass index (BMI),the dosage of iodine contrast agent ,length of stay ,laboratory examination ,tumor history ,basic disease ,allergy history ,drinking history as independent variables ,the incidence of skin ADR related to iodine contrast agent was analyzed by single factor analysis ,and the variables with statistically significant were selected for multivariate Logistic stepwise regression analysis. RESULTS :There were 157 cases of skin ADR ,involving 79 males(50.3%)and 78 females(49.7%). The age ranged from 19 to 86 years old ,being(52.68± 18.73)years old in average. BMI was 14.6-40.7 kg/m2,being(22.5±3.7) kg/m2. 67 cases(42.68%)were treated with iprodione ,34 cases(21.66%)with iodixanol ,31 cases(19.74%)with iohexol and 25 cases(15.92%)with iopamidol ;the dose of iodine contrast agent were 50-100 mL,being(73.06±13.29)mL in average. There was no significant difference among different dosage of 4 kinds of iodine contrast agents (P≤0.05). Among 4 kinds of iodine contrast agents ,the incidence of skin ADR induced by iopromide was the highest(0.197%). The skin ADR related to iodine contrast agent was mainly acute (89.2%),the severity was mild (75.2%),and urticaria(38.9%)was the most common. After symptomatic treatment ,135 cases were cured ,13 cases were improved and 9 cases were not improved. Among the patients with iodine contrast agent related skin ADR ,the incidence of ADR induced by combined use of anti infective drugs was the highest (33.1%);however,the combined use of anti-tumor drugs was the main cause of severe skin ADR. The length of stay {11~20 d[OR=1.21,95%CI(1.07,1.20),P=0.042]、21~30 d[OR=1.39,95%CI(1.12,1.52),P=0.035]、31~40 d[OR=1.15,95%CI(1.03,1.37),P=0.008]、>40 d[OR=1.33,95%CI(1.28,1.53),P=0.003]},respitatory and circulatory system tumor history[OR =1.51,95%CI(1.35,1.61),P=0.037],injection allergy history[OR =1.50,95%CI(1.37,1.59),P=0.005] can significantly increase the incidence of iodine contrast agent related skin ADR. CONCLUSIONS :The main manifestation of skin ADR related to iodine contrast agent was urticaria. The main risk factors of skin ADR related to iodine contrast agent were length of stay (> 10 d),respiratory and circulatory system tumor history and injection allergy history.
10.Influence of different kinds of valve types on clinical benefits of elderly patients with valvular heart disease

Objective 聽 聽To investigate the influence of mechanical and biological valves on clinical benefits of elderly patients with valvular heart disease. Methods 聽 聽We retrospectively analyzed the clinical data of 280 elderly patients with valvular heart disease treated by valve replacement between 2008 and 2014 year. The patients were divided into two groups by tendency score matching including a group A with biological valves and a group B with mechanical valves. Finally, there were 96 patients in each group. There were 43 males and 53 females at age of 64.41卤6.52 years in the group A, 44 males and 52 females at age of 64.07卤6.20 years in the group B. Results 聽 聽The bleeding rate of skin and mucosa of the group B was significantly higher than that of the group A (P<0.05). There was no statistical difference in mortality within 30 days after operation, all-cause mortality, re-hospitalization rate, re-valve replacement rate, combined atrial flutter/atrial fibrillation ratio, drug use, incidence of cerebral infarction, cerebral hemorrhage, new peripheral vascular embolism and visceral hemorrhage, heart function (NYHA) classification, the cumulative survival rate of all the patients during follow-up (P=0.63), or the cumulative survival rate of the patients with no thrombus/hemorrhage (P=0.75) between the two groups (P>0.05). Conclusion 聽 聽Mechanical valve replacement and bioprosthetic valve replacement in the treatment of valvular heart disease in the elderly can achieve similar clinical benefits and both have clinical application value.
