1.Application of methylation detection in gynecologic tumors

The occurrence and development of human malignancies is often accompanied by epigenetic changes, and DNA methylation is one of the most common epigenetic modification. Methylation events are often discovered in the early stage of gynecologic neoplasia. The methylation detection protocol based on liquid biopsy technology has made important progress in the screening and prognosis assessment of gynecologic tumors. The reliability of methylation detection in micro-noninvasive samples from multiple sources is similar to or better than that of traditional protocols and strategies. This article reviews the progress of methylation detection in screening, diagnosis and monitoring of gynecologic tumors, and discusses its value in the present and future diagnosis and treatment of gynecologic tumors.

3.Analysis of disease burden of welder's pneumoconiosis in Jiangsu Province

Objective:To investigate the current status of disease burden and its influencing factors among welder's pneumoconiosis patients, and provide scientific basis for taking targeted intervention measures.Methods:From June 2022 to June 2023, the patients with welder's pneumoconiosis in Jiangsu Province were selected from 1956 to 2020 as the research objects, and disability adjusted life years (DALY) were used as the comprehensive index to study the disease burden. The direct and indirect economic losses caused by the diseases were calculated, and the factors affecting the disease burden were discussed by multiple linear regression method.Results:A total of 974 cases of welder's pneumoconiosis were reported in Jiangsu Province, the cumulative loss of DALY was 6300.73 person-years, and the per capita loss was 6.47 person-years. Among them, the healthy life years lost due to disability (YLD) was 6156.50 person-years (97.71%) , and the healthy life years lost due to premature death (YLL) was 144.23 person-years (2.29%) . Multiple linear regression analysis showed that the main factors affecting DALY were disability grade, diagnostic age, pneumoconiosis grade and length of dust exposure ( P<0.05) . The total economic loss caused by 974 welder's pneumoconiosis patients was 1831838160.18 yuan, and the per capita loss was 1880737.33 yuan. Among them, the direct economic loss was 970917563.75 yuan (53.00%) , and the indirect economic loss was 860920596.43 yuan (47.00%) . Conclusion:Welder's pneumoconiosis causes serious disease burden to patients, and at the same time causes huge economic losses to individuals and society, which seriously hinders the development of society. Taking effective control measures to prevent the incidence of welder's pneumoconiosis is the key to reduce the disease burden.
4.Two-year follow-up of the outcomes of endoscope-assisted minimally invasive nonsurgical periodontal therapy for deep intrabony defects

Objective To explore the clinical efficacy and imaging changes of minimally invasive nonsurgical peri-odontal therapy(MINST)assisted by endoscopy for deep intrabony defects and to compare its effectiveness with that of traditional scaling and root planning(SRP)to therefore provide a reference for clinical periodontal treatment.Methods Patients with deep intrabony defects ≥ 4 mm in size were selected and divided into two groups:the MINST(MINST,20 cases,81 sites)group and the classic scaling and root planing(SRP,20 cases,80 sites)group.Before treatment and 12 and 24 months after treatment,probing depth(PD)and clinical attachment loss(CAL)were examined.Moreover,changes in the depth and angle of the intrabony defects were analyzed.Follow-up examination and maintenance treat-ment should be conducted every 3 months for 12 months after the initial treatment and every 6 months thereafter until 24 months.Results The PD and CAL of patients in both groups continued to decrease(P<0.001),and imaging examinations revealed a decrease in defect depth and an increase in intrabony defect angle(P<0.001).The changes in the first 12 months were significantly greater than those in the last 12 months in both groups(P<0.001).The de-creases in PD,CAL,and depth of intrabony defects and increase in angle in the MINST group were significantly greater than those in the SRP group(P<0.001).At 12 and 24 months after treatment,the PD and CAL in the MINST group were lower than those in the SRP group(P<0.001).The defect height of the MINST group decreased more than that of the SRP group(P<0.001),and the defect angle of the MINST group increased more than that of the SRP group(P<0.001).Conclusion Minimally invasive nonsurgical periodontal therapy can significantly promote the healing of deep intrabony defects and the regeneration of alveolar bone.Imaging reflects that alveolar bone healing is rapid at first and then slows.Compared with traditional SRP,endoscopically assisted MINST can yield better clinical indicators and imag-ing changes in intrabony defects.
5.Development of negative pressure cabin environment detector for aviation

In order to detect the environmental data of isolated cabin under various environmental conditions,a negative pressure cabin environment detector for aviation was developed,which was composed of a shell,a differential pressure transmitter,a pressure sensor,a carbon dioxide concentration sensor,an oxygen concentration sensor,a temperature and humidity sensor,a data processing module,a liquid crystal display(LCD)screen and USB data interface.It could environmental data such as carbon dioxide concentration,oxygen concentration,temperature,humidity,pressure difference between cabin and outside the cabin and air pressure outside the cabin in real time,the data processing module collected and processed the data,the data and data change curves was displayed in real time by the LCD screen,and the detection data was extracted through the USB data interface.When used in aviation environment,the detector could work continuously without fault for no less than 300 hours,and the average fault repair time was about 30 minutes,with good performance and high detection accuracy,which can provide convenience for the environmental data detection of negative pressure cabins,and is worthy of popularization.
6.Observation of the clinical efficacy of external application of piyan formula in treating EGFR-TKIs-related rash

Objective To observe the clinical efficacy and safety of external application of Piyan Formula in treating epidermal growth factor receptor tyrosine kinase inhibitor(EGFR-TKIs)-related rashes.Methods Sixty cases of EGFR-TKIs-related rash patients were randomly allocated into either a treatment group or a control group.The treatment group received external application of Piyan Formula to the rash area twice daily for 14 days.The control group received external application of fucidic acid cream to the rash area twice daily for 14 days.Changes in rash grading,itching grading,quality of life scores and adverse event were observed and recorded in both groups.At the same time,levels of hypersensitive C-reactive protein,interleukin(IL)-6,and IL-1β were measured before treatment and 24 hours after treatment.Results After treatment,the rash severity,itching severity,and quality of life scores were notably lower in the treatment group compared to the control group(P<0.05).The levels of hypersensitive C-reactive protein,IL-6,and IL-1β exhibited a significant decrease compared to their pre-treatment values.(P<0.05).Compared with the control group,the levels of hypersensitive C-reactive protein,IL-6,and IL-1β decreased in the treatment group,with statistically significant differences(P<0.05).No adverse events related to Piyan Formula or fucidic acid cream occurred during the treatment process.Conclusion External application of Piyan Formula in treating EGFR-TKIs-related rashes shows significant clinical efficacy,can effectively reduce the levels of inflammatory factors,and has high safety,thus warranting clinical promotion.

8.Beam trajectory correction and dose distribution in the presence of fringe fields in magnetic resonance imaging-guided proton therapy

Objective To explore the correction of beam trajectories in the presence of fringe fields in magnetic resonance imaging-guided proton therapy and dose changes in the body before and after correction.Methods The open-source treatment planning software matRad was used to design plans for brain tumor,liver tumor,and prostate cancer cases,and simulation studies were conducted in the Monte Carlo simulation toolkit TOPAS to calculate proton dose distribution.A proton beam trajectory correction model suitable for three-dimensional magnetic fields was established,and a beam trajectory correction algorithm was developed.The deflection of the proton Bragg peak in the presence of fringe fields was analyzed.Furthermore,3 treatment plans were simulated and dose correction was carried out when the fringe field existed.Gamma analysis method is used to evaluate the correction effect;and the dose changes in the target area and organs-at-risk after correction were quantitatively analyzed.Results The perturbation of the magnetic field would cause lateral deflection of the proton beam trajectory,and the presence of fringe fields would significantly increase this effect,which increased with the increasing of beam energy.When the fringe field existed,the treatment plans for brain tumor,liver tumor,and prostate cancer were corrected.Under the 3%/3 mm criterion,the gamma passing rates for target area were 94.844%,92.054%,and 97.863%,respectively,and after correction,the total dose in the body was increased by 2.8%,2.5%,and 1.5%,respectively.The increased dose was mainly contributed by incident protons.Conclusion In magnetic resonance imaging-guided proton therapy,the effects of fringe fields should be considered.The increase in incident proton beam energy after correction will lead to an increase in the total dose in the body.Since the beam trajectory still has curvature,the dose changes differently in different organs-at-risk.
9.Subacute combined degeneration of spinal cord with autoimmune disease in children: a case report

The clinical manifestations of subacute combined degeneration of spinal cord (SCD) in children are complex and vary greatly. Due to the fact that some patients with SCD may be complicated with autoimmune diseases, the high early misdiagnosis and missed diagnosis rates are observed. One case of 13-year old female with severe anemia, multiple joint swelling and pain in left limbs and paralysis of bilateral lower limbs with the extremely low level of serum vitamin B12 and poly-glandular involvement as well as a variety of positive auto-antibodies (anti-intrinsic factor antibody, anti-parietal cell antibody, thyroid peroxidase antibody, thyroid globulin antibody and perinuclear anti-neutrophil cytoplasmic antibody) was retrospectively analyzed. The patient was diagnosed as SCD with autoimmune disease (undifferentiated connective tissue disease and autoimmune polyglandular syndrome). The patient′s condition gradually alleviated after high-dose intravenous methylprednisolone, immunoglobulin, naproxen (then changed to hydroxychloroquine 1 month later), vitamin B12 and levothyroxine sodium tablets supplementation, blood transfusion and rehabilitation. SCD with autoimmune diseases is rare in children, and the clinical manifestations vary greatly. Early recognition and early treatment can improve the prognosis of SCD. The clinical data of this child were retrospectively analyzed, so as to improve the understanding of the disease by clinicians.
10.Treatment of paclitaxel and doxorubicin changes the immune microenvironment of breast cancer and inhibits the growth of tumor cells in mice.

Objective To investigate the effects of paclitaxel and doxorubicin on the immune microenvironment of breast cancer in mice. Methods The CTR-DB database, a database for analysis of gene expression profiles and drug resistance characteristics related to tumor drug response, was used to analyze the effect of chemotherapeutic drugs on the immune microenvironment of breast cancer. Mouse models with breast cancer were established by in situ injection with 4T1 cells, a triple-negative breast cancer (TNBC) cells. Then they were treated with doxorubicin and paclitaxel, respectively. The sizes of tumor were recorded and analyzed by growth curve. The number of different types of immune cells was analyzed using flow cytometry. The expressions of Ki67, S100 calcium binding protein A9 (S100A9) and matrix metalloproteinase 9 (MMP9) were detected by immunohistochemistry. The cell cycles of 4T1 cells in paclitaxel group and doxorubicin group were analyzed by flow cytometry. Results The results of CTR_Microarray_75 analysis showed that the immune scores, and the number of cytotoxic lymphocytes, B lineages, CD8+ T cells, dendritic cells (DCs), monocytic lineages and natural killer (NK) cells in chemotherapy-sensitive breast cancer were higher than those in chemotherapy-insensitive breast cancer. Through growth curve analysis in mice with breast cancer, we found that both paclitaxel and doxorubicin could inhibit the increase of the tumor sizes, and the paclitaxel showed a higher inhibitory effect. The results of cytometry displayed that both paclitaxel and doxorubicin could restrain the expression of Ki67 and increase the number of breast cancer cells in G2/M phase, and in the paclitaxel group, the expression of Ki67 was lower and the number of breast cancer cells in G2/M phase was larger. Paclitaxel and doxorubicin enhanced the infiltration of CD45+ immune cells but decreased the infiltration of neutrophils. Additionally, paclitaxel promoted the infiltration of CD3+CD4+ T helper cells, CD3+CD8+ cytotoxic T cells and CD45+CD19+B cells, while doxorubicin increased the infiltration of CD4+CD25+ regulatory T cells (Tregs). The results of immunohistochemistry displayed that the paclitaxel significantly inhibited the expression of S100A9, while the doxorubicin significantly restrained the expression of MMP9. Conclusion Paclitaxel and doxorubicin can effectively inhibit the growth of breast cancer cells and change immune microenvironment of TNBC by regulating the different patterns of cell infiltration and the expression of different extracellular matrix components.
