1.Application of problem-based learning in dermatology and venereology teaching of clinical medicine

		                        			Objective To explore the application of PBL in dermatology and venereology teaching. Method According to random principle, 120 students were divided into experimental group and control group (60 students per group). The experimental group received PBL, and the control group received tradi-tional teaching. The teaching contents included systemic lupus erythematosus, pemphigus, psoriasis, syphilis. The course needed 16 periods totally . the traditional teaching group applied 8 classes extensive class teaching and 8 classes clinical trainee, and the PBL teaching group implemented case-based teaching 4 hours each time. Teaching satisfaction evaluation and test scores would be analyzed at the end of the semester. SPSS 17.0 was used for t test. Results The teaching satisfaction (contains professional values and ethics, communication skills, clinical skills, community health and health systems, information management, critical mind and research, etc.) of experimental group was higher than control group (P<0.05); The test scores of experimental group was higher than control group (P<0.05), especially in total score of theoretical examina-tion, comprehension and application. The difference mainly came from PBL teaching (P<0.05). Conclusion PBL is conductive not only to the comprehension and application of knowledge but to training the students' abilities of independent learning.
		                        		
		                        		
		                        		
		                        	
2.Application of PBL combined with RPT teaching in clinical practice teaching of dermatology and venereal disease

		                        			Objective To explore the application effect of based-learning problem (PBL) combined with role playing method(playing teaching role,RPT)in clinical practice teaching of skin diseases. Methods 112 clinical undergraduate interns were randomly divided into experimental group (PBL teaching combined with RPT teaching group, n=56) and the control group (PBL teaching group, n=56). After the internship, clinical probation theory examination, clinical skills assessment and questionnaire survey were used to evaluate the two teaching modes,using SPSS 13.0 software for statistical analysis of the data. Results The results showed that theory examination and clinical skills examination scores in the PBL+RPT group were significantly higher than the PBL group, i.e. (74.29±3.17) vs. (66.14±2.17); (16.66±1.11) vs. (14.50± 1.51), and there were statistically significant differences. The survey showed that the experimental group was better than the control group in learning interest, doctor-patient communication ability and humanistic concern. Conclusion The teaching mode of PBL+ RPT improved the students' enthusiasm for learning, cultivated and improved the humanistic quality, clinical thinking and practical ability of medical and ob-tained the satisfactory teaching effect,It will has a good application prospect.

		                        			The professional identity of medical postgraduates has great influence on their profes-sional learning, career development and realization of self-value. The effect factors of the professional iden-tity are as following:the conflict between personal needs and professional requirements, professional employ-ment prospect, the social recognition of medical profession and individuality. OBJECTIVEPsoriasis is an autoimmune-related chronic inflammatory skin disease strongly associated with the dysfunction of Th17 cells. Retinoic acid-related orphan nuclear receptor γt (RORγt) plays a critical role in the differentiation and maturation of Th17 cells and in cell-derived immunologic derangement. We conducted this study to investigate potential mechanism by which the derivative of digoxin selectively antagonizes RORγt transcriptional activity.
METHODUsing molecular docking in combination with molecular electrostatic potential (MEP), we detected the interaction between the derivative of digoxin (Dhd) and ROR transcription factor (RORα,RORβ and RORγt), and the results were further confirmed by bioluminescent assay.
RESULTMolecular docking demonstrated that Dhd could exclusively inhibit the conformation of RORγt; bioluminescent assay further indicated that RORγt was selectively antagonized by Dhd in a dose- and time-dependent manner.
CONCLUSIONDhd can selectively suppress RORγt transcriptional activity.

5.The effect of TGP assisted treatment of SLE on the expression of CD4+CD25+T in patient peripheral blood

		                        			Objective to investigate the effect of TGP assisted treatment of SLE on the expression of CD4+CD25+ T in patient peripheral blood .Methods flow cytometry was used to detect the peripheral blood CD4+ CD25+ T cells in healthy group ,routine group and TGP group .Results The expression rate of CD4+CD25+ T cells in SLE patients was (6 .15 ± 1 .21)% ,and that of the healthy controls was (12 .30 ± 1 .78)% .The expression rate of CD4+CD25+ T cells in active SLE patients and healthy controls were significantly different (t=22 .03 ,P<0 .05) .In the routine group ,the expression rate of peripheral blood CD4+CD25+ T cells before and after treatment were significantly different (t= 12 .30 ,P<0 .05);in the TGP group ,the expression rate of peripheral blood CD4+ CD25+ T cells before and after treatment were significantly different ,too (t=16 .68 ,P<0 .05) .The expression rate of periph‐eral blood CD4+CD25+ T cells in routine group and TGP group were (9 .34 ± 1 .37)% and (11 .49 ± 1 .14)% respectively ,and the difference was statistically significant (t=6 .46 ,P<0 .05) .Conclusion The expression rate of CD4+ CD25+ T cells in active SLE patients was significantly lower than that of healthy controls ;the expression rate of CD4+CD25+ T cells in SLE patients increased significantly after treatment .The TGP treatment may work on CD4+CD25+ T cells .

8.Autologous peripheral blood hematopoietic stem cell transplantation in the treatment of three cases of severe pemphigus

		                        			ObjectiveTo explore the clinical efficacy and safety of autologous peripheral blood stem cell transplantation(APBSCT) in the treatment of pemphigus.MethodsTotally,3 patients with pemphigus vulgaris who responsed poorly to 6-month treatment with glucocorticoids or immunosuppressants or experienced aggrevation of disease and developed treatment-related complications,received APBSCT and were followed up for more than 5 years.There were 1 male and 2 females with an average age of 27.3(21-39) years.The mobilization program included cyclophosphamide (CTX) 4 g/m2,recombinant human granulocyte colonystimulating factors(G-CSF) and Rituximab 375 mg/m2,and the preconditioning regimen included intravenous CTX (50 mg/kg per day on days -6,-5,-4,-3),antithymocyte globulin at 2.5 mg/kg per day(on days -3,-2,-1 and 0) and Rituximab (600 mg/d on days 0 and 7).ResultsAll the 3.patients were successfully engrafted.The mean time for peripheral reconstruction:white blood cells 13.3 days (from day 11 to 16),platelet 16.3 days (from day 16 to 17).Monitoring of immunity indices and related antibodies showed no abnormality and the immune system was well reconstructed.No serious complications occurred during the follow up,and the patients' quality of life was obviously improved.ConclusionAPBSCT may be an effective and safe option for the treatment of pemphigus.
		                        		
		                        		
		                        		
		                        	
9.Influence of Stratum Corneum on the Percutaneous Absorption of Fluorescein Sodium Liposome in Rat Skin

		                        			Objective To study the effects of stratum corneum on the percutaneous absorption of fluorescein sodium (NaFl) liposome in rat skin. Methods Stratum corneum was stripped off by Scotch adhesive tape, and the NaFl content in the stratum corneum and the skin with no stratum corneum were detected by spectrofluorimetry at regular intervals. The dynamic permeated amount of different preparations of NaFl in the skin at different intervals in Franz diffuse cell was determined and the distribution of NaFl at 4 h after stripping off the stratum corneum was observed by fluorescence microscope. Results The concentrations of NaFl in the stratum corneum and the skin with no stratum corneum after applying liposome NaFl preparation were significantly higher than those of NaFl solution and gel preparations (P  0.05). Conclusions It is suggested that liposome can increase and change the penetration of NaFl into the stratum corneum and the skin with no stratum corneum in vitro. 10.Detection of autoimmune antibody in patients with SLE after autologous hematopoietic stem cell transplantation

		                        			Objective To study changes of the autoimmune antibody level in patients after autologous hematopoietic stem cell transplantation(CD_(34)~+).Methods Twelve patients with SLE received autologous hematopoietic stem cell transplantation.All they survived and their immune system were recoverd after a period of time.The serum autoimmune antibody levels were measured before and after the transplantation,Results The antibody levels became normal 6 months after transplantation.Conclusion Autologous hematopoietic stem cell transplantation can effectively reduces the level of autoimmune antibody in patients with SLE.
